[Ultrasound in the prenatal diagnosis of heart and lung abnormalities--Down's syndrome].

The authors describe a 21 year old gravida 2, para 2 patient in whom cardiac malformation (endocardial cushion defect) and secondary non-immune hydrops fetalis were detected by ultrasound in the 27th week of pregnancy. Because of the serious fetal demise premature delivery was induced. The on the basis of the prenatal ultrasound scan suspected Down-syndrome was proved by karyotyping from the amniotic fluid and fetal fibroblasts.

[Prenatal diagnosis of thoracopagus in the 19th week of pregnancy].

A pair of conjoined twins (thoracopagus) was detected in the 19th week of pregnancy, by ultrasound. Serumalphafetoprotein was normal. Since the fetuses had only one heart, the pregnancy was terminated. The embryopathological investigation confirmed the prenatal diagnosis. Authors recommend the ultrasound screening of all pregnant women between the 16-20th week of gestation. This case is a good example of progressive care in the field of prenatal genetics.

[Effect of local estriol therapy on the urethral pressure profile in the postmenopausal period].

Measurements of the pressure in the urethra were performed under standardized conditions with the aid of perfused catheters in 20 fertile and 20 postmenopausal women, from it 12 women after local estrogen therapy by means of Ostriol-Ovula. Hormone-cytological swabs from the urethra, vagina and from the urinary cells were done. Proliferation of the urogenital epithelialia under the estriol-therapy and an ascent of the MUP and MUCP in the urethra pressure profile under resting conditions could be achieved. The maximum of the pressure profile could be displaced proximaly. In the urethra pressure profile the distal pressure transmission increased. Estriol therapy was without secondary effects and is suitable for the conservative therapy of the stress incontinence type I to III.

[Aspiration of chorionic tissue: diagnostic potential in prenatal (fetal) diagnosis].

The authors have investigated the possibilities of obtaining chorionic villi from legal pregnancy termination patients between the 7th and 11th week and the method of direct chromosome preparation from the villi as well. According to their investigations under continuous "real time" ultrasound guidance and with the immediate microscopic checking of the obtained material it is possible to receive chorionic villi of necessary quantity in nearly 100% of the cases, and after 2 or 3 hours the evaluation in a great deal of mitosis of good quality can be carried out. The method may revolutionize the prenatal (fetal) diagnosis, but its routine application may not be proposed until the rate of fetal risk is exactly known.

Determination of the urethral stress profile with the aid of perfused catheters.

To make manometric studies of urethral function and obtain exact recordings of the so-called stress profile requires a pressure rise rate which exceeds that of the pressure complex due to coughing. The syringe perfusion pump commonly used in urodynamics limits the pressure rise rate. The rate ist also reduced by catheters with high compliance. A suitable recording accuracy necessitates values greater than 150 kPa/s. This can be achieved with the aid of an improved pneumatic-hydraulic infusion system and low-compliance catheters. We describe the improvement of a recording system and demonstrate the importance of a suitable pressure rise rate.

Dependence of nickel-induced coronary vasoconstriction on the activity of the electrogenic Na+, K+-pump.

The possible interactions between the vasoactive trace metal nickel ion (Ni2+) and membrane Na-K-ATPase in the isolated perfused rat heart and in the isolated canine coronary artery have been studied. The characteristic features of 1 microM Ni2+-induced contractile response in the canine coronary artery strip were similar to those evoked by the inhibition of Na-K-ATPase. Inhibition of the pump activity by ouabain (10(-4)M) or by K+-deficient Krebs solution prevented Ni2+-action both in the canine coronary artery strip and in the perfused rat heart, indicating that when Ni2+ causes coronary vasoconstriction the Na, K-exchange is influenced. Further studies are needed to clarify whether Ni2+ acts directly on the enzyme, or the vascular action of this trace metal depends on the ionic gradients maintained by the electrogenic Na-K-pump.

[Pulmonary fibrosis and pregnancy].

Complete pregnancy of a patient with progressive chronic interstitial pulmonary fibrosis (Hamman-Rich syndrome) is reported in this paper. One pregnancy was interrupted and the other intensively attended to up the 32nd week in which premature rupture of the membranes was followed by caesarean section and subsequent sterilisation. Reference is made to physiological and pathophysiological aspects of respiration in gravidity and in the presence of pulmonary fibrosis.

In utero allogeneic hematopoietic stem cell transplantation to induce tolerance.

We studied the efficacy of in utero hematopoietic stem cell transplantation and the ability of such transplantation to induce tolerance in a fetal normal mouse allogeneic model. In 9 of the 162 surviving recipients (5.6%), cells of donor origin were detected after birth. The highest engraftment rate was achieved by transplanting fetal liver cells in a relatively high dose (> 10(6) cells/fetal gram). Skin grafting was performed to determine the presence of prenatally induced tolerance. Only those mice which showed evidence of chimerism became tolerant to skin derived from the prenatal donor's strain while remaining competent to reject a skin transplant from a third strain. Tolerant mice could have significant chimerism reestablished by utilizing monoclonal antibody specific for the recipient H-2 antigen as conditioning for IV fetal stem cell retransplantation.
