[Serum endotoxin level in the course of open peritonitis treatment].

In 27 patients with severe diffuse purulent or fecal peritonitis planned relaparotomies with peritoneal lavage or continuous dorsoventral lavage with open abdomen were performed after surgical treatment of the primary infection. During the course of the lavage treatment serum endotoxin was measured daily. The endotoxin-induced liberation of lysosomal proteases was studied by determining the elastase from polymorphonuclear leucocytes. 16 surviving patients showed decreasing endotoxin levels and decreasing elastase concentrations during the course of abdominal lavages. Planned peritoneal lavage and continuous dorso-ventral lavage seem to have the same potency in eliminating endotoxin from the infected peritoneal cavity. In letal courses endotoxinemia either persisted at high levels or even progressed inspite of lavage treatment.

[Specificity of lymphocytic reaction to tumor antigens--approaches to a cancer diagnosis in vitro?].

In this study soluble tumor extracts as antigens were used in the electrophoretic mobility test. 115 cancer-patients, 15 patients with precancerous large bowel lesions and 54 healthy controls were tested. Lymphocytes were incubated with tumor extracts of various organ types and cell migration inhibition was measured. 105 out of the 115 cancer-patients (91%) had positive test results with extracts from identical tumor types; lymphocytic reactions to extracts from tumors of different histiogenesis were weaker or negative. 10 out of 15 patients with colorectal adenomas showed positive results with extracts from colorectal cancer. When 54 healthy controls were tested against the same tumor extracts out of 158 tests 139 were truly negative (90.3%). The differentiating lymphocytic reactions may be indicative for the organ site of an unknown primary tumor.

[Effect of postoperative parenteral feeding on protein metabolism in heart surgery patients. A comparative study].

To investigate the importance of amino acid infusions in immediate postoperative parenteral nutrition, cardiac patients were randomly allocated into two groups. Applicating identical carbohydrate calories (2000 kcal/day) group 1 received only essential amino acids while in group 2 a combined pattern of essential and non essential amino acids was infused. In addition to routine laboratory data several parameters of protein metabolism including nitrogen balance were evaluated. Although nitrogen balance was positive only in group 2 the differences between the two groups concerning other parameters measured were minimal. The different infusion regimes are discussed revealing the significance of additional parenteral nutrition in these patients.

[Immunoreactivity in colorectal cancer. Approaches for an immunologic diagnosis?].

An attempt was made to demonstrate a specific immunoreaction against antigens of colorectal carcinoma and its precursors in vitro. Lymphocytes of patients with colorectal carcinoma or adenoma were incubated with tumor antigens from various human carcinomas. The preparation of the tumor antigens was performed with 3 m KCl extraction from cancer tissue. The cellular immunoreactivity was measured by the electrophoretic mobility test (EMT). 51 of 57 patients with colorectal cancer had specific lymphocytic reactions to extracts isolated from colorectal cancer; 8 showed cross-reactivity to antigens from adenocarcinoma of the stomach, while there was a negative lymphocytic reaction to extracts from cancer of the breast or lung. In 10 of 15 patients with colorectal adenoma, values in the tumor range were found with extracts of colorectal carcinoma. In 16 healthy controls using identical tumor extracts, 158 measurements revealed 139 negative responses (90%).

Primary pulmonary neurilemoma.

Solitary primary nerve cell tumors of the lung are a rarity among benign neoplasms. These tumors arise from the cells of the Schwann sheath enwrapping the nerve bundles of the autonomic nervous system. Their content of collagenous fibers varies. X-ray examination of a 32-year-old woman revealed a round focus in the ventral paracardial lower area of the right lung. Bronchoscopy and computerized tomography gave no indication of malignancy. The tumor, which originated in the middle lobe, was removed from healthy pulmonary parenchyma and histologically proven to be a pulmonary neurilemoma. Since its first description in 1940, only some 53 cases of pulmonary neurilemoma have been reported in the literature.

KCl carcinoma extracts as tumor antigens in the electrophoretic mobility test: clinical evaluation.

Specificity of cellular immune reactivity was tested in 182 cancer patients, 89 patients with inflammatory diseases, and 61 healthy controls. The in vitro method of electrophoretic mobility testing permitted evaluation of cellular immune reactivity. Soluble potassium chloride extracts from various human carcinomas were used as antigens. Out of 182 cancer patients 164 (90.1%) showed positive test results using extracts from tumors of the same type as the patient's tumor. Using the same tumor extracts in 61 healthy control persons, a total of 182 tests showed negative results in 156 cases (85.7%). Lymphocytic reactions to various tumor extracts varied in a typical pattern: The most pronounced migration inhibition was observed when tumor patients were tested against extracts of carcinomas of the same histogenesis. Tumor extracts deriving from different organs had a weaker or negative reaction. For each tumor a typical lymphocytic reaction pattern was noted. The method seems also to detect precancerous lesions; out of 19 patients with colorectal adenomas, 12 showed positive reactions to extracts from colorectal cancer. Inflammatory diseases of the various organs reduced the reliability of the method. Demonstrated specificity of sensitized lymphocytes could prove to be valuable aid in immunological tumor recognition.

[Immune reactivity in colorectal cancer--clinical value from a surgical viewpoint].

Specificity of cellular immunity was verified by the cell migration inhibition test in 36 patients with colorectal cancer and 15 with colorectal adenomas. The electrophoretic mobility test is considered as an indirect method for demonstrating cellular immunity against malignant tumors. Soluble potassium chloride tumor extracts from various human carcinomas were used as antigens. In 31 of 36 tumor patients, a specific lymphocytic sensitization against antigens of colorectal carcinomas could be shown. Lymphocytic reactions to antigens from histologically different tumors (cross-reactions) were weaker or negative. This reaction, verified for carcinomas, was also noted in cases with precancerous lesions. Measurements in 54 healthy control persons with the same antigenic substances showed negative results in 48 cases. Acute inflammatory lesions of the colon reduce the reliability of the test.

[Immunologic localization of primary tumors].

Specificity of cellular immune reactivity was tested in 115 cancer patients. The electrophoretic mobility test was used as in vitro test for cellular immunity, KCl extracts from various human carcinomas were used as antigens. For each tumor a typical lymphocytic reaction was noted: the most pronounced migration inhibition in cancer patients was caused by corresponding tumor extracts. Lymphocytic reactions to extracts from tumors of different histogenesis were weaker or negative. A total of 158 tests in 54 healthy control persons with the same antigenic substances showed negative results in 139 cases. Demonstrated specificity of sensitized lymphocytes could prove to be a valuable aid in immunological tumor recognition.

[KCl-tumor extracts in the electrophoretic mobility test [EMT]. - Clinical relevance to diagnosis of gastric cancer (author's transl)].

Using KCl-tumor extracts from various human carcinomas we tried to improve the reliability of the electrophoretic mobility test (EMT). 25 of 26 patients with histologically proven gastric cancer showed a correct positive reaction if tested with gastric cancer tumor extracts. 44 of 50 healthy controls showed negative test results when tested with the same tumor extracts. Migration inhibition in the tumor patients differed significantly from that of the healthy controls (p less than or equal to 0,05). Specificity was verified by testing gastric cancer patients with extracts from tumors of the same organ as well as different carcinomas: the most pronounced migration inhibition was observed, when gastric carcinoma extracts were used. Tumor extracts of different histogenesis showed weaker or negative reactions. Inflammatory lesions reduce the reliability of the test system: of 17 patients with ulcers or erosive gastritis 9 had false positive reactions.
