[Therapeutic aspects of endoscopic polypectomy].

466 endoscopic polypectomies of the upper and lower gastrointestinal tract were analysed according to their curative value. In 254 removed polyps of the upper gastrointestinal tract the result of the histological examination was twice a proof of a carcinoma. One it had to be after-resected, once a carcinoid, five times bleeding polyps were removed. Four times prolapsing gastric polyps were removed in the bulbus duodeni. In these cases intermittent disturbances of the passage could be repaired. Thus the endoscopic polypectomy in the upper gastrointestinal tract was of therapeutic value in 4.3%. In the lower intestinal tract in 204 polypectomies 16 focal carcinomas or invasive carcinomas removed in the healthy tissue, 1 carcinoid and 98 bleeding adenomas were cut away. Thus the coloscopic polypectomy was connected with a therapeutic use in 56.3%. 81.8% of the polyps was tubular, papillary or villous adenomas. Taking into consideration the "adenoma-cancer-sequence", the coloscopic polypectomy must thus be regarded as a prophylactic and curative method, while in the polypectomy in the upper gastrointestinal tract the diagnostic value is in the first place.

Confirmation of cancer diagnosis--methods and problems.

Histo- und cytopathological proof of the diagnosis of cancer is crucial for the choice of an appropriate treatment. In the last decade effective methods for obtaining material from various organs of the body within operation have been developed, especially by new procedures in endoscopy, radiology, sonography and fine needle biopsying allowing further progress in cancer management.

[The CEA-concentration in plasma of patients with colorectal carcinoma and polyps has been determined by radioimmunoassay (RIA, Z-gel-method)].

82% of the cancer patients and 52% of the patients with polyps showed CEA levels above the upper normal limit of 2,5 microgram/1. CEA-values of more than 10 microgram/1 could only be detected in patients with carcinoma. Complete removal of the tumour by surgery has been followed by a marked decrease of prior increased plasma-CEA to values in the normal range, which could not be observed in the cases with tumour residues or metastases. In the patient's follow-up recurrence or metastases could be recognized several months before clinical detection by a renewed increased of the CEA-level in plasma.

[Experiences with coloscopy].

The introduction of fully flexible coloscopes has considerably contributed to the improvement of the diagnostics of the large intestine. According to the hitherto got experiences is to be expected that this method will contribute to the possibility of diagnose malignant and benign colon processes earlier and more exactly. In the paper the methodology of the coloscopy is described and it is referred to its difficulties and complications. The indications and contraindications for the coloscopy are described. It is reported on first results after a one-year experience with coloscopy. The conclusion is that in patients with radiologically and rectoscopically not clarifiable, referring to a severe disease of the colon, symptoms a coloscopy should be carried out.
