Retrieval of the resilient element in an osseointegrated implant system.

A fractured apical fragment of the resilient intramobile element within the IMZ dental implant cylinder is difficult to remove. The continued usefulness of the implant will be compromised if the fractured intramobile element is not removed. Two methods of retrieving the apical fragment without causing damage to the internal threads of the implant cylinder are described.

Soft-tissue cephalometric norms in Chinese adults with esthetic facial profiles.

Using a double selection process comprised of professional and lay judges, the cephalometric tracings on a final sample of 48 Chinese adults with esthetically pleasing profiles were analyzed. The soft-tissue cephalometric norms and standard deviations of two widely accepted soft-tissue analyses, the Legan and Burstone analysis and the Holdaway analysis, were determined. In comparison with white norms, the Chinese nose was less prominent (P < .01), the nasolabial angle was less obtuse (P < .01), both the upper and lower lips were more protrusive (P < .05), the upper lip curvature was greater (P < .01), and the soft-tissue chin thickness was less (P < .05). This variance between racial types emphasizes the need to recognize that soft-tissue lateral cephalometric norms are specific for the racial group and cannot always be applied across different racial types.

Pituitary surgery--a report of 46 cases.

Resection of pituitary adenomas, by the transcranial, or more commonly, by the transsphenoidal approach is an established operation in Singapore. The authors review the results of pituitary surgery in 46 patients over a 7 year period. Forty patients had transsphenoidal surgery. Six patients had transcranial resection of their tumours. There was only one operative mortality and low morbidity. Total macroscopic resection was achieved in all but 7 patients. Eighteen of 21 patients with non-secretory adenomas had radiological evidence of cure. Normalisation of growth hormone levels was achieved in 6 out of 9 acromegalics. Three out of 6 patients with Cushing's disease had normal ACTH following surgery. Only 1 out of 6 patients was cured of her prolactinoma. Vision improved in 16 out of 25 patients with decreased vision preoperatively.

Reduction of post-operative swelling by a placebo effect.

A placebo effect on post-operative swelling was investigated as a possible model for studying psychological influences on recovery from surgery. 79 patients undergoing removal of impacted third-molars received one of five different procedures shortly after emerging from general anaesthetic. These included dentist-administered or placebo ultrasound (the latter given in two different ways to control for massage effects), untreated controls and a group instructed to apply facial massage to themselves. Pre- and post-operative measurements included trait and state-anxiety, coping style, emotional state, pain, plasma cortisol and facial swelling. Cortisol levels correlated with anxiety and avoidant coping. Post-operative anxiety was negatively correlated with pre-operative arousal. Neither coping nor emotional state was affected by the treatments, but swelling was reduced by a placebo effect of ultrasound. Cortisol levels also responded, apparently to an effect of massage. The coping and emotional factors which we measured here cannot, therefore, explain the effects of this psychological procedure on post-operative recovery.

Combined CT scanning and sialography in the management of parotid tumors.

In the diagnosis of parotid gland tumors, three major questions need to be answered. First, is the mass intra glandular or extraglandular? Second, is it benign or malignant? Third, does it lie deep to or superficial to the facial nerve? Computerized tomography is a relatively new diagnostic technique and when used in conjunction with sialography can provide very useful preoperative information about parotid lesions. A case is presented in which computerized tomography sialography was performed preoperatively in a patient with a parotid gland swelling and greatly assisted in the diagnosis and operative treatment. The technique is described in detail and previous reports of the procedure are reviewed.

Adenolymphoma

Mandibular ameloblastoma in Singapore--a 10 year review.

This paper reviews 30 cases of mandibular ameloblastoma seen and managed in the Singapore General Hospital over a 10 year period from 1976 to 1985 inclusive. Data on age and sex distribution, mode of presentation, presenting symptoms, size of tumour at time of presentation, radiological features at time of diagnosis, method of treatment and post-operative complications are discussed. There was no case of local recurrence in the 28 cases that were surgically managed. The importance of adequate local excision is emphasised. This paper also compares our local data with that of equivalent studies made in Thailand (Far East), Sweden (Europe) and Nigeria (Africa). An interesting rare case of malignant mandibular ameloblastoma is included in this study.
