Congenital complete atrioventricular block--two cases report.

We encountered two cases of congenital complete atrioventricular block out of 23,400 deliveries during the past 3 years. Two-dimensional and M-mode fetal echocardiography revealed complete atrioventricular block without structural abnormality. Anti-Ro (SS-A) antibodies were present in both the mother and infant of these two cases. The ventricular rates of these two infants were 31 and 80 beats/min respectively. Pacemaker implantation was performed for the first infant.

The obstetric management in very-low-birth weight infants.

Eighty-three very-low-birth weight (VLBW) infants weighing between 500 g and 1,500 g were studied. The neonatal morbidity and mortality were 86.7% and 18.1%, respectively. Sixty-six point seven percent (8/12) of the infants weighing under 1,000 g suffered from a periventricular-intraventricular or intracranial hemorrhage (PVH/IVH) and their mortality rate was 50%. The incidence of PVH/IVH was 52.1%, and the mortality rate was 12.7% when the birth weight was between 1,000 g and 1,500 g. Forty-nine percent of the PVH/IVH took place on the first day of neonatal life, and 93% developed within 5 days after delivery. Cesarean section neither reduced the incidence of PVH/IVH nor increased the survival rate whether in cephalic or breech presentation. The incidence of PVH/IVH was not affected by type of presentation or the interval between the rupture of membranes and the delivery. Respiratory distress syndrome (RDS) and the requiring of a ventilator for ventilation significantly increased the incidence of PVH/IVH, while a low Apgar score (less than 3) at 5 minutes did not.

Pelvic actinomycosis associated with intrauterine contraceptive devices--a review of 6 cases.

We analysed the clinicopathologic aspects of 6 cases of pelvic actinomycosis associated with intrauterine contraceptive devices seen and treated at Mackay Memorial Hospital between 1987 and 1989. All of them had IUD for a period from 5 to 20 years. The incidence of pelvic actinomycosis among all our gynecological admission number during the same period was 0.08%. The most frequent presenting symptoms were lower abdominal pain and abnormal vaginal bleeding. An abdominal total hysterectomy with a bilateral or unilateral salpingoophorectomy was performed on all 6 cases due to a tubo-ovarian abscess or a pelvic mass. We do not find any relation between the presence of pelvic actinomycosis and the type of IUD. However, the study number was too small to make a definitive statement, but the length of time of IUD insertion is of greater significance.

Amniotic fluid index measurement with the four-quadrant technique during pregnancy.

Eleven hundred thirty-four pregnant women with reliable dates underwent ultrasound evaluation for amniotic fluid index (AFI) measurement with the modified four-quadrant technique. We found that from 13 weeks' gestation the AFI rose progressively until 26 weeks. From then to 38 weeks the AFI measurements demonstrated little variation. After 38 weeks the AFI appeared to decline gradually. In the third trimester the mean AFI was 16.0 +/- 4.8 cm; 5% of cases had an AFI less than or equal to 8 cm, and 5% had an AFI greater than or equal to 24 cm. These results confirm the reproducibility of the semiquantitative technique and suggest that in estimating amniotic fluid volume, AFI measurement may be a more appropriate means than measurement of the single largest pocket or subjective assessment. Serial AFI measurement would be an effective way of assessing fetal status throughout pregnancy.

Cell-mediated immune responses to poliovirus. I. Conditions for induction, characterization of effector cells, and cross-reactivity between serotypes for delayed hypersensitivity and T cell proliferative responses.

Human polioviruses are categorized into three distinct serotypes (types 1, 2, and 3) based upon their reactivity with specific antibodies. Although a great deal of information has been amassed about the induction and characterization of poliovirus antibody responses, little is known about cell-mediated immunity to poliovirus and its role in protection. Here, we show that intracutaneous injection of ultraviolet light-inactivated poliovirus into the tailbase of BALB/c mice induces delayed hypersensitivity (DTH) and T-cell proliferative (Tprlf) responses. Both DTH and Tprlf responses to poliovirus are mediated by Ly-1high2-, L3T4-bearing T cells. Moreover, known serologic cross-reactivity (i.e., antibody-mediated) of poliovirus serotypes is not predictive of cross-reactivity between the cell-mediated immune responses.

Radical hysterectomy with pelvic lymph node dissection for treatment of cervical cancer: a clinical review of 954 cases.

Nine hundred fifty-four patients, who received a radical hysterectomy and a bilateral pelvic lymphadenectomy, were included in this study which ran from January 1971 to December 1986. The subjects, by in large, ranged in age from 41 to 60. Of these cases, 62.5% were in clinical stage IB, while 26.2% were in stage IIA. Urinary tract fistula formations occurred in 2.4% of the cases and substantially decreased from 6.1% during 1971-1978 to 0.9% in the subsequent period from 1979 to 1986. The operative mortality rate was 0.4%. The follow-up included 438 cases that came from 1971 to 1981, and each was evaluated over a period of 5 years. Of these cases, 28 patients were lost during the follow-up period and were presumed to have died from cancer. The absolute 5-year survival rates for the patients in stage IB, stage IIA, and IIB were 86.1, 71.7, and 60.1%, respectively. The respective 5-year survival rates for patients with stage IB LN(-) and LN(+), i.e., without or with lymph node metastasis, were 87.7 and 73.1%. The survival rates for patients with stage IIA LN(-) and LN(+) were 79.8 and 40.9%, respectively. In retrospect, the 5-year survival rates seem to be related to age, parametrial involvement, and histological type. This study shows that because of improving surgical techniques and anesthesia, acceptable operative morbidity and mortality, as well as favorable 5-year survival rates, radical surgery seems to be the treatment of choice for patients with early invasive uterine cervical, and especially for younger women.

Routine pretreatment evaluation in cervical carcinoma.

A review of 573 previously untreated patients with invasive cervical carcinoma was undertaken to determine the significance of the routine pretreatment performance of chest x-ray, intravenous pyelography (IVP), cystoscopy and sigmoidoscopy. Chest x-ray was performed in 570 patients (99.5%), IVP in 514 (89.7%), cystoscopy, 502 (87.6%) and sigmoidoscopy, 496 (86.6%). The overall yield of tumor-related abnormalities demonstrated in chest x-ray, IVP, cystoscopy and sigmoidoscopy was 1.8%, 9.1%. 6.4% and 1.2% respectively. The chest x-ray findings changed the initial clinical stage in 1.3% of cases, IVP in 4.2%, cystoscopy in 4.0% and sigmoidoscopy in 0.6%. There were 5 instances of bladder involvement and 5 of bullous edema of the bladder found in patients originally classified as having stage I or stage II disease. Thus, to omit cystoscopy may not be so safe in patients with stage I and stage II diseases. We suggest that chest x-ray, IVP and cystoscopy should be performed as part of staging in all patients with cervical carcinoma, however, sigmoidoscopy can be reserved for those initially diagnosed to have advanced disease or symptomatic patients.

The residual ovary syndrome.

An analysis of 35 cases of the residual ovary syndrome was performed retrospectively. In sixty percent of the patients, the residual ovary syndrome occurred within 5 years after the hysterectomy. The most common symptom of these patients was chronic pelvic pain. Extensive pelvic adhesions were the typical operative findings, while follicular cysts and corpus luteum hematoma were the common pathological findings of the residual ovaries. However, malignant changes were found in 4 cases. In this study, the probable incidence of the residual ovary syndrome was 2.3% which is compatible with other reports and the probable incidence of malignant change among patients with the preserved ovaries was 0.26% which is about the same as the general population. Because the conservation of normal ovarian function has great importance to the premenstrual woman, we suggest that the grossly normal ovaries should be conserved on premenstrual woman but should always be removed on postmenopausal woman when hysterectomy is done.
