[Value of lymphography in the preoperative diagnosis of gynecologic cancers].

In advanced tumors, the imaging techniques such as ultrasound, computer tomography or magnetic resonance imaging are at the forefront. In order to plan surgical procedure, lymphography is indicated as a supplementary diagnostic measure mainly in tumors of the corpus and the cervix uteri, especially in the early and middle stages. In tumors of the ovary, the vagina and the vulva, diagnostic assistance in deciding on therapy is to be expected only in occasional cases. The indication for lymphography should not only be established in relation to the tumor stage, but also tumor grading, since a very much higher local regional metastatic spreading is to be reckoned with in increasing grading, so that the choice of therapeutic procedure is crucially affected. In using preoperative diagnostic measures, the postoperative morphological follow-up should be considered: Examinability of the removed lymph nodes, especially in preoperative suspicion of metastases. Behavior of the lymph nodes in the post-therapeutic phase with consideration of recurrence diagnostics. Planning of radiotherapeutic target volume considering collateral circulations and atypical lymphatics.

Combined Modality Therapy↗

[Inflammatory changes in shoulder arthrography].

Arthrography of the shoulder joint in so-called periarthritis humeroscapularis often shows nodular contrast defects, irregular defects in the bursa and contrast-filling of irregular lymphatics; these are characteristic signs of a synovitis. Arthrography can provide an exact diagnosis, not only in suspected post-traumatic rotator cuff, tears, but also in other painful conditions that limit movement. It is superior to CT in demonstrating inflammatory changes.

[Combined low-dose radiation and adriblastin therapy of lung metastases].

Two patients with lung metastases are presented who were submitted to a combined low-dose radiotherapy and adriamycin monotherapy with good palliative results. The mode of action as well as the experimental examination results are discussed. The ineffectiveness of single low-dose chemotherapy is demonstrated by the non-response of lung metastases beyond the irradiation fields (heart block, diaphragm). Both patients were resistant to chemotherapy.

Adenocarcinoma↗

[Gorham-Stout syndrome--progressive osteolysis with viscerocranial involvement].

Two rather young patients with spontaneous massive mandible osteolysis are presented, i.e. a woman suffering from a rhinogenous liquor fistula and recurrent meningitides, and a young man. The authors discuss etiology, pathologic anatomy, clinical and radiological course, as well as the combined surgical and radiotherapeutical treatment. The pathologic process shall be stopped by the combination of both therapy methods. A radiation dose of 30 to 50 Gy is recommended.

Combination of 5-fluoro-uracil and Co60 irradiation in the treatment of carcinoma of the oro-facial region. A follow-up investigation of 30 patients.

During the years from 1974 to 1977, 30 patients were treated for carcinoma of the oro-facial region with a combination of 5-fluoro-uracil and telecobalt under the care of the Institute of Radiotherapy, Knappschafts-Krankenhaus, Recklinghausen. 13 of these patients were inoperable and had primary irradiation therapy, and the remaining 17 patients were irradiated postoperatively. The main effect that treatment showed was a rapid response in the inoperable tumors, while the 4- to 5-year survival rates of 1:13 of the inoperable cases and 6:17 of the operated patients are in accordance with generally accepted results. The possible radio-biologic relation to this is discussed.

[Diseases of the brachial plexus after surgery and irradiation of breast cancer (author's transl)].

The authors evaluated retrospectively the medical records of 1028 female and two male patients operated upon and post-irradiated because of a breast cancer. Thirty-nine among the female patients suffered from a plexus disease. When considering the reasons for the formation of this disease, one must not only suppose that the operation method and the influence of the dose is at its origin, but the hyperemization and hyperhydration which may develop in the early postoperative phase or due to hormonal action have to be taken into consideration, too. The following conclusions were drawn for patients submitted to radical operations: 1. The irradiations should not be started much earlier than twenty days after the operation. 2. The dose calculation and beginning of irradiations should be controlled very carefully for patients aged below 45 and/or patients presenting an oedema of the arm. 3. No maximum doses are administered to patients treated by hormonal and/or pharmacological therapy (not more than 42 to 45 Gy).

[Sympathicoblastoma in adults (author's transl)].

The article reports on 61-year old female patient with sympathicoblastoma. This disease is seen in only 0.5 - 6.5% adults. 60 - 70% of the patients are less than 5 years old, whereas about 20 - 30% are younger than 20 years of age. 75% of the tumors are metabolically active with increased excretion of catechol amine metabolites. Detailes x-ray examinations must be made besides laboratory tests. Treatment can be effected by surgery, radiotherapy and pharmacotherapy.
