[Curative limb saving operation (curative wide resection)].

The recent results of surgical treatment of soft tissue sarcomas have dramatically improved. The result of the group study of soft tissue sarcomas, which was supported by the Ministry of Health and Welfare (1962-1976) showed that the local recurrence rate was 50%, and the metastatic rate was 60%. In contrast with this result, the recent results of our curative wide resection showed the local recurrence rate of 10%, and the metastatic rate of 30%. These procedures are based on the clinico-pathological findings which we studied. Curative wide resection is a method of radical wide resection, similar to radical local resection (reported by Enneking) and radical local excision (reported by Stenar). Our procedures are the least aggressive of the three radical wide resections, but our results are the same as the other wide resection methods. To obtain good results with these procedures, accurate preoperative diagnosis and skillful surgery are necessary.

[Nutritional support in surgery: with special reference to fat emulsion and carnitine].

Energy metabolism of fat emulsion has been studied in rats with various conditions including postoperative catabolism, chronic and acute hepatic dysfunction, diabetes and sepsis. The results indicate that fat emulsion is rapidly eliminated from the blood stream and metabolized readily even in such conditions. Based on these findings, we have actively employed fat emulsion clinically as energy source. In septic rats, it was demonstrated that levels of carnitine decreased and that this decrease was based upon a decrease of synthesis. When carnitine was administrated together with fat emulsion, the energy metabolism returned to approximately normal level. This report also describes the tissue concentrations of carnitine in diabetic rats and the blood and urine concentrations of carnitine in postoperative patients.

[Spongiform encephalopathy in mice inoculated with scrapie material of sheep origin].

The authors report spongy degeneration in experimental scrapie (second passage) in mice. The scrapie agent was originally isolated from Suffolk sheep imported from Canada and diagnosed histopathologically to be infected with scrapie by intracerebral inoculation into JCL/ICR mice. Ten female SIc/ICR mice, 4 weeks of age, were injected intracerebrally in the right frontal lobus with 20 microliter of 10(-1) or 10(-4) dilution of JCR/ICR mice brain homogenate involving scrapie agent. All animals showed signs of the advanced stages of the disease, clinically manifested by lassitude, arched backs, lethargy and paresis of hind quarters. They were sacrificed five to six months post inoculation, and sections of the brain and spinal cord were examined by light and electron microscopy. Focal symmetrical spongiform lesions were seen light microscopically in the cerebral mantle, thalamus, hypothalamus, midbrain, medulla oblongata, cerebellum and cervical mark. There was evidence that these lesions tended to be more intense in the mice inoculated a higher concentration of scrapie agent. Astrocytic proliferation was present in the deep layer of cerebral gray matter, white matter, corpus callosum, dorsal part of hippocampus and thalamus. No leukocytic infiltration was observed. Electron microscopically, the spongiform lesions were shown to be caused by vacuolation or swelling within the neuropil, and vacuolation and focal swelling in the neuronal perikaryon. The changes in the neuronal perikaryon were caused by enlargement of endoplasmic reticulum and cisterns of the Golgi apparatus, accompanied by spherical swelling of a part of the cytoplasm. The vacuolation near or within the neuron produced deformation of the cell contours and displacement of the nucleus.(ABSTRACT TRUNCATED AT 250 WORDS)

A case of Behçet's disease associated with membranous nephropathy.

A 35-year-old female patient with Behçet's disease associated with nephrotic syndrome is described. Renal biopsy specimens revealed typical features of membranous nephropathy in light microscopical, electron microscopical and immunofluorescent microscopical examinations. Circulating immune complexes (IC) phagocytized by polymorphonuclear leukocytes (PMN) in this patient were significantly increased compared with those in healthy adults. Recurrent oral aphthous ulcers had persisted for 14 years. In the course of Behçet's disease, nephrotic syndrome due to membranous nephropathy was observed. Depositions of IgA, IgM, C3 and C4 were observed in the subcutaneous arteriolar walls by immunofluorescence. It is suggested that the pathogenesis of Behçet's disease and membranous nephropathy might be due to a deposition of circulating immune complexes in various types of vascular vessels in this patient.

Effect of neomycin on azoxymethane-induced colon carcinogenesis in F344 rats.

The effect of oral administration of neomycin (100 and 200 micrograms/ml in drinking water) on colon tumors induced by azoxymethane [(AOM); CAS: 25843-45-2] was studied in female F344 rats. Five-week-old rats were fed NIH-07 diet and given daily in drinking water 0, 100, and 200 micrograms neomycin/ml (0, 100, and 200 ppm). At 7 weeks of age, all animals except vehicle-treated groups received weekly sc injections of 8 mg AOM/kg body weight for 8 weeks. The AOM- or vehicle-treated groups were necropsied 30 weeks after the last injection of AOM. The combined incidence of adenomas and adenocarcinomas of the colon did not differ significantly among the 3 groups. The animals in the groups given 100 and 200 micrograms neomycin had a higher incidence of colon adenocarcinomas than did those in the control group. Colonic and cecal bacterial beta-glucuronidase activity was significantly lower in the group given 200 micrograms neomycin than it was in the control group. The excretion of fecal cholesterol, total bile acids, and deoxycholic acid was increased significantly in animals given 100 and 200 micrograms neomycin as compared to animals given no neomycin. These results suggest that long-term oral administration of neomycin increases the incidence of colon adenocarcinomas.

Blockade of synaptic transmission in the squid giant synapse by a spider toxin (JSTX).

We studied the effect of spider toxin (JSTX)--specific blocker of glutamate receptor--on giant synapse of the squid stellate ganglion. JSTX irreversibly blocked the excitatory postsynaptic potential (EPSP) without affecting the presynaptic action potential or the antidromic action potential. L-glutamate depolarized the postsynaptic membrane and suppressed EPSP which may be due to desensitization. The action of glutamate was completely blocked in the presence of JSTX. The results suggest that glutamate is the transmitter at the giant synapse of squid.

Ultrastructural changes in differentiating neuroblastoma x glioma hybrid cells.

The ultrastructure of differentiating neuroblastoma x glioma hybrid cells NG108-15 was observed. Cells cultured in growth medium showed undifferentiated features, while cells treated with dBcAMP became round and large, and extended thick long neurites. After 1 week in culture, cells showed features similar to those of normal neurons. The dense cored vesicles with diameters ranging from 60 to 170 nm were observed in differentiated NG108-15 cells, but clear vesicles were usually rare. However, in the case of co-culture with striated myotubes, clusters of clear vesicles appeared in the neurites and terminals. The timecourse of the differentiation process was correlated with results obtained by the electrophysiology and freeze-fracture.

An enhancing effect of the antihistaminic drug methapyrilene on rat liver carcinogenesis by previously administered N-2-fluorenylacetamide.

The effect on hepatocarcinogenesis of sequential administration of the antihistaminic drug methapyrilene (MP) or the typical liver tumor promoter, phenobarbital (PB), each given after the genotoxic liver carcinogen, N-2-fluorenylacetamide (FAA) was studied. An initial exposure to 0.02% of FAA in the diet for 8 weeks was used to produce hepatocellular altered foci. Rats maintained for an additional 24 weeks on basal diet developed a low incidence of liver neoplasms. MP at 500 and 1000 ppm in the diet for 24 weeks after FAA increased the frequency of altered foci at early stages and liver neoplasms later, as did PB. Neither MP nor PB alone produced neoplasms, but MP alone produced a significant incidence of altered foci. Therefore, the results provide evidence for a promoting action of MP, but additional effects may be involved in its carcinogenicity.

Electrostatic and hydrophobic effects in chromatography of rabbit IgG immunoglobulins on aminohexyl sepharose substituted with bis-(p-chlorophenyl)- acetic acid.

The binding interactions of rabbit IgG immunoglobulins with bis-(p-chlorophenyl)-acetic acid (DDA) substituted aminohexyl Sepharose (AHS) (DDA-AHS) at low ionic strength are under the influence of pH, temp, ionic strength and their antibody specificity. The IgG molecules held on a DDA-AHS column in the presence of 0.01 M phosphate buffer (pH 6.8) at 4 degrees C could be stepwise eluted by the addition of 0.01 M acetate buffers (pH 5.5, 5.0 and 4.5) followed by 3 M NaClO4, a chaotropic reagent. The adsorbability of IgG molecules by the DDA-AHS column was reinforced at temps higher than 4 degrees C and at increasing NaCl concns ranging from 0.05 to 0.2 M. The antibody specificity and, perhaps, binding affinity greatly affected this binding system. On the basis of these results, it was evident that both the DDA ligand and charged groups (omega-amino and isourea groups) took part in the binding interactions between IgG molecules and the agarose derivative at low ionic strength. Thus the binding interactions of rabbit IgG immunoglobulins with DDA-AHS at low ionic strength are assumed to be due to hydrophobic plus electrostatic interactions. It was also observed that the binding site on the surface of IgG molecules at low ionic strength was located only in the Fab region.

Developmental time courses of Na and Ca spikes in neuroblastoma X glioma hybrid cells.

The developmental time course of membrane excitability in neuroblastoma X glioma hybrid cells (NG108-15) was analyzed by measuring the electrical potential at each culture day after dBcAMP treatment. The maximum rate of rises of Na and Ca spikes which reflect their inward currents were measured in appropriate medium respectively. The Ca spike began to appear at 2-3 days in culture and reached its maximum on the fifth day. The Na spike began to appear at 3-4 days in culture and was completed on the seventh day. After then Ca and Na spikes remained constant and did not decrease even on the eighteenth day. Electrical excitability of the hybrid cells, as a whole, developed stepwise and was completed on the seventh day of culture.

A quantitative analysis of intramembranous particles during the development of neuroblastoma X glioma hybrid cells.

The process of neural differentiation of neuroblastoma X glioma hybrid cells (NG108-15 cells) was studied by freeze-fracture electron microscopy. The distribution of intramembranous particles (IMPs) was random and their density increased in both P-, and E-face of the plasmalemma when the membrane excitability developed. Undifferentiated cells grown in growth medium for 2-6 days had an average of 1300-1500 IMPs/micron2 in the P-face and 250-300/mu 2 in the E-face. After the cells were differentiated with dBcAMP for 1 week, the density of IMPs was 2600/micron2 in the P-face and 540/micron2 in the E-face. The analysis of size distribution showed the increase of the number of medium (7.5-10 nm) and large (greater than 10 nm) particles. The slight increase of large particles was observed in the case of differentiated N18TG2 (the parental neuroblastoma clone) cells. Treatment with dBcAMP did not affect the number and the size distribution of IMPs in the C6BU1 (the parental glioma clone) cells.

Congenital hydrocephalus revealed in the inbred rat, LEW/Jms.

We studied the development of congenital hydrocephalus found in a colony of an inbred strain of Wistar-Lewis rats (LEW/Jms) at various intervals after birth. The disorder was transmitted as a simple recessive mendelian character. Hydrocephalic neonates were recognized 2 days after birth by stretching of the skin over the head. Death usually occurred between 10 and 20 days of age. The findings suggested the possibility of a disturbance of cerebrospinal fluid circulation resulting from primary occlusion of the 3rd or lateral ventricles during embryological development. In later phases, the hydrocephalus was aggravated by obliteration of the subarachnoid space and by stenosis of the aqueduct occurring secondary to compression of these structures from increased pressure within the brain. In some animals, external hydrocephalus occurred as a result of rupture of the occipital pole and the establishment of a direct communication between the lateral ventricles and the subdural space. We looked for antibodies against viruses that have been known to produce hydrocephalus in experimental animals and obtained entirely negative results.

Enhanced survival and absence of progressive growth of transplanted rat liver altered eosinophilic foci and neoplastic nodules in phenobarbital-treated rats.

Hepatocellular altered eosinophilic cell foci and neoplastic nodules induced in inbred F344 male albino rats by the feeding of N-2-fluorenylacetamide were transplanted into the inguinal mammary fat pads of syngeneic animals to assess their ability to produce neoplasms. Treatment of one-half of the recipients with the neoplasm promoter phenobarbital sodium enhanced survival of both types of transplants. However, at the end of 6 or 12 months of maintenance of recipients of altered foci and 6 months of maintenance of recipients of early and late nodules, no neoplasms occurred, in spite of persistence of abnormal cells at the transplant sites. Therefore, the conclusion was that these carcinogen-induced liver lesions possess limited growth capacity in this ectopic site and that transition to carcinoma under these conditions did not occur in high frequency.
