[Intra-arterial infusion of cisplatinum in bone and soft tissue tumors].

Cisplatin was administered by intravenous injection, intra-arterial injection, and balloon-occluded arterial infusion. The total Pt, ultrafiltrated Pt, and free Pt levels were measured. Intra-arterial injection, in which ultra-filtrated Pt level on the normal side is the same as in intravenous injection, acts to increase the ultrafiltrated Pt level on the affected side. Thus, intra-arterial injection is as effective as preoperative chemotherapy. In two-route chemotherapy, free Pt level on the affected side is extremely high, so for only local control of tumors, two-route chemotherapy is very effective.

[The value of limb-saving surgery in the treatment of osteosarcoma].

Since 1981, 38 limb-saving operations for malignant bone tumors have been performed. Of these, 22 cases were osteosarcomas. The principle of limb-saving surgery for osteosarcoma is the same as for curative wide resection in cases of soft tissue sarcoma. Among the osteosarcoma cases for which limb-saving procedures were carried out, five complicated cases (two skip metastases and two infections) were noted. However, in the other cases, patients had better functional benefits and psychological acceptance compared with those subjected to radical ablative procedures. Indication for limb-saving surgery is usually decided by patient age, existence of skip metastasis and venous infiltration, effect of preoperative adjunctive therapy and the relationship of the tumor with the major artery necessary for preserving the affected limb. If the adjunctive therapy is effective, local control can be achieved by establishing a wide margin.

Changes in plasma oxytocin, prostaglandin E1, and 13,14-dihydro-15-keto-prostaglandin F2 alpha during labor induced by prostaglandin E2 or F2 alpha and spontaneous labor.

To observe the changes in endogenous oxytocics during spontaneous and induced labor, the plasma concentrations of oxytocin, prostaglandin E1 (PGE1) and 13,14-dihydro-15-keto-prostaglandin F2 alpha (PGFM) were measured during labor in 9 cases of spontaneous labor (group 1), 10 of PGE2-induced labor (group 2), and 7 of PGF2 alpha-induced labor (group 3). Unextracted samples were used for radioimmunoassay of oxytocin. PGE and PGF were extracted and separated for radioimmunoassays of PGE1 and PGFM. Although oxytocin levels in groups 1 and 3 did not change during labor or slightly increased toward delivery, those in group 2 decreased as labor progressed. The mean oxytocin in group 2 was significantly lower at the times of established labor (15.3 +/- 3.2 microU/ml, mean +/- SE) and crowing of the fetal head (10.8 +/- 2.0 microU/ml) than before labor (52.7 +/- 14.8 microU/ml). Plasma PGE1 levels in groups 1 and 3 were low and did not change during labor. Plasma PGFM levels in groups 1 and 2 gradually rose toward delivery. These results suggest that exogenous PGE2 suppresses oxytocin secretion during labor and stimulates endogenous PGF2 alpha production, that endogenous PGE1 may not play an important role in the progress of spontaneous and PGF2 alpha-induced labor, and that endogenous PGF2 alpha may participate in the promotion of all kinds of labor.

[Surgical treatment of malignant soft tissue tumors].

The ultimate survival of patients with soft tissue sarcoma is determined by a number of factors. Radical removal by adequate surgery is one of the most important factors together with early treatment and chemotherapy. We usually select curative wide resection, amputation, or resection after radiotherapy as forms of radical surgery for soft tissue sarcomas according to each clinical stage. The method of curative wide resection is based on biological barrier effects. In this report we discuss the operative results obtained in 148 cases of soft tissue sarcoma which we have treated over the past ten years, and also discuss the causes of recurrence after radical operation. Among 55 primary NoMo which were treated by the curative wide resection cases, the recurrence rate was 5.5%, the metastatic rate was 21.8%, and 5-year survival was 79.3%. These results were better than those for 30 recurrent and additional NoMo cases. Of cases involving the extremities, 81% were controlled by limb-saving operations.

[Combination chemotherapy including cis-platinum in Ewing's sarcoma].

A clinical study of cis-platinum was conducted on 5 patients with Ewing's sarcoma. Cis-platinum was preoperatively administered at doses of 70-100 mg/m2. Good response on imaging diagnosis was recorded in all cases. With regard to side effects, 2 of the 5 patients suffered from hearing loss, but only slight bone marrow suppression was observed. We concluded that cis-platinum was effective for the preoperative treatment of Ewing's sarcoma.

The effect of chronic ethanol consumption on the tumorigenicity of N-nitrosopyrrolidine in male Syrian golden hamsters.

The effect of oral administration of ethanol on the tumorigenicity of N-nitrosopyrrolidine (NPYR) in chow-fed male Syrian golden hamsters has been investigated. Groups of hamsters were given tap water, 7.4% ethanol or 18.5% ethanol-water mixtures 4 weeks prior to and throughout the carcinogen administration period. A total dose of 1 mmol NPYR was administered by intraperitoneal injection to both tap water and ethanol-consuming animals over a 25-week period. Twenty-four hours after the last injection, ethanol-consuming animals were returned to tap water. In control animals treated with NPYR we observed 3/26 animals with tracheal papillomas, and 6/26 animals with hepatic neoplastic nodules. In animals receiving 7.4% ethanol-water mixtures, we observed 6/26 tracheal papillomas, and 17/26 hepatic neoplastic nodules. Similar results were observed in animals receiving 18.5% ethanol. These data indicate that chronic administration of ethanol to chow-fed hamsters increases the incidence of hepatic neoplastic nodules although no differences in the two levels of ethanol were observed.

Effect of DNA synthesis on induction of preneoplastic and neoplastic lesions in rat liver by a single dose of methylazoxymethanol acetate.

A single intravenous injection of methylazoxymethanol (MAM) acetate in doses of either 20 or 35 mg/kg body weight to male Sprague-Dawley rats induced altered liver cell foci and later, liver neoplasms in a dose related manner. Sequential observations in the rats given 35 mg/kg and thereafter fed an iron-loading diet revealed that the number of iron-excluding foci/cm2 increased with time. Partial hepatectomy (PH) before the high dose of MAM acetate resulted in 100% lethality while hepatectomy before the low dose carcinogen exposure lead to a higher incidence of neoplasms than in rats that received carcinogen alone. PH after either high or low dose carcinogen exposure did not result in a greater occurrence of liver neoplasms.

Ovulatory disturbances. Causative factors among Japanese student nurses in a dormitory.

The incidence of ovulatory disturbances in student nurses living in a restricted circumstance was evaluated by observing a basal body temperature (BBT) chart recorded for three to nine months by all of the student nurses in our medical college. The students were 18-21 years of age and were all living in a school dormitory. One hundred of the 154 students (64.9%) were anovulators during the school term. However, 40 of the anovulators showed ovulatory cycles with biphasic BBT patterns during their spring and/or summer holidays. The subjects' living conditions were investigated by questionnaire. Age of menarche, sport activity, self-perceived physical and mental work load, and native region did not appear to be related to their ovulatory disturbances. Weight-height-derived indices indicated that the anovulators had a tendency to corpulence as compared to the regular ovulators. Serum luteinizing hormone, follicle stimulating hormone, and prolactin levels of the anovulators were not significantly different from the regular ovulators.

Pathogenesis of dimethylnitrosamine-induced hepatocellular cancer in hamster liver and lack of enhancement by phenobarbital.

The early stages of dimethylnitrosamine (CAS: 62-75-9)-induced liver cancer and the effect of administration of phenobarbital (CAS: 50-06-6) after dimethylnitrosamine were studied in Syrian golden hamsters. A single ip injection of 6 mg dimethylnitrosamine/kg (body wt) induced hepatocellular altered foci by 6 months. Most foci were composed mainly of large clear glycogen-containing cells, while smaller numbers were formed by cells with abnormally acidophilic or basophilic cytoplasms. Biliary cysts, but not biliary neoplasms, also occurred. A few neoplastic (hyperplastic) nodules and hepatocellular carcinomas developed by 12 months. gamma-Glutamyltranspeptidase activity was not present in any of the hepatocellular lesions, but foci of all types, neoplastic nodules, and carcinomas were characterized by a lack of iron accumulation. Phenobarbital at 0.05% in the diet for up to 12 months did not increase the number of lesions of any type, indicating a lack of promoting effect under these conditions.

Localization and kinetic properties of prostaglandin E1 receptor on the plasma membrane of rabbit myometrium in late pregnancy.

The distribution of prostaglandin E1 (PGE1) receptor in various subcellular fractions of pregnant rabbit myometrium was closely related with the distribution of [Na + K]-ATPase, 5'-nucleotidase, and Ca2+-ATPase activities, marker enzymes for plasma membrane, but not with that of NADPH-cytochrome c reductase activity, which is a marker enzyme for endoplasmic reticulum. The specificity of PGE1 receptor was significantly different from those of PGE2 and PGF2 alpha. The PGE1 receptor concentration in the plasma membrane fraction increased significantly from 89.0 to 111.1 fmol/mg protein between the 28th and 30th days of pregnancy. Scatchard analysis of PGE1 receptor revealed a nonlineal plot which indicated the existence of negative cooperativity in a kinetic study.

Genetic study on ossification of posterior longitudinal ligament.

Family studies and blood grouping tests were conducted in an attempt to elucidate the etiology and genetic mechanism of ossification of posterior longitudinal ligament of the spine (OPLL). Analysis of the collected date revealed the following results: Testing of inheritance pattern Fitness of OPLL to the hypotheses of simple recessive inheritance, simple dominant inheritance and multifactorial inheritance was tested, but fitness to any one of these hypotheses was statistically denied. Analyses of OPLL and genetic markers A comparative study was made between the OPLL patients and healthy donors residing in Tokyo on the phenotype frequency of the blood groups (eight systems), serum groups (six systems) and red cell enzyme groups (three systems). A significant association of OPLL with MN blood groups, Hp types and PGM1 types was observed. Examination of the correlation after combining MN and Hp showed the coefficient of contingency to be C = 0.6739 and that after the multiple combination of MN, Hp and PGM1 revealed the coefficient of contingency to be C = 0.8923, indicating a remarkably high correlation.

[Studies on the blood constituents of the neonates delivered by the induced delivery of the Defense Medical College method].

For the assessment of the influence of the Defense Medical College planned delivery method upon neonates, the measurement of the blood constituents of the umbilical cord was conducted. Statistically speaking, no marked differences were seen between the neonates delivered by the Defense Medical College Method and neonates delivered naturally. The blood constituents investigated were: Blood cells, activity of various enzymes, other blood constituents and electrolytes.

[Hypertrophic osteoarthropathy and hypophosphatemic osteomalacia associated with tumor].

Hypertrophic osteoarthropathy and hypophosphatemic osteomalacia are both associated with neoplasm and unusual clinical syndromes. Although the etiologies of these conditions are unknown, their clinical courses are interesting, so we are reporting two cases of these conditions separately. Case 1: A 20-year-old man had an osteogenic sarcoma originating in the 2nd thoracic vertebra which was developing in the mediastinal region. He had complained of numbness and swelling in the left arm and of clubbing of the fingers of both hands. A chest radiograph showed a billiard-ball-sized, round opacity in the left upper mediastinal region. Periosteal new bone formation was demonstrated symmetrically in both humeri, radii, ulnae, femurs, tibiae, fibulae and metacarpals. Case 2: A 30-year-old man had complained of lower back, hip, knee and ankle pain and muscle weakness of five years' duration and was admitted to the National Yokosuka Hospital. Surum phosphorus was 0.7 mg/dl, alkaline phosphatase was 24.9 K.A. and glucosuria was noted. He had a fibrous xanthoma on the right thigh, and after removal of the tumor, his symptoms improved dramatically and pertinent laboratory data returned to normal. However, ossification of the ligaments of the spine subsequently developed.

[Light and electron microscopic observations of sheep scrapie].

Two Suffolk sheep diagnosed as scrapie clinically and epidemiologically were investigated light and electron microscopically. They were female and four years four months of age. Spongiform lesions were found in the gray and white matter of midbrain, pons, medulla oblongata, spinal cord and the cerebellar white matter as well as the cerebral gray and white matter. Ultrastructurally, the spongiform lesions were shown to be caused by vacuolation in neuronal perikarya, vacuolation and/or swelling of neuropil, dilatation of the periaxonal space, and splitting of the myelin sheath followed by the intramyelinic vacuolation. Vacuole in neuronal perikaryon was associated with the enlarged endoplasmic reticulum. Adjacent to the vacuole, enlargement of the rough endoplasmic reticulum and cisterns of Golgi apparatus was observed. The cytoplasmic processes found in the intraneuronal vacuoles were formed by the separation of the cytoplasm caused by the continuous dilatation of the endoplasmic reticulum and cisterns of Golgi apparatus. This indicated that the degeneration of the neuronal perikaryon due to such separation contributed to the formation of the intraneuronal vacuole. Vacuolation and swelling with disappearance of the organelles were also found in astro- and oligodendroglial perikarya. Splitting of myelin sheath and the intramyelinic vacuolation were regarded as primary changes. The latter was supported by the degeneration of oligodendroglias found in this study.

Iron exclusion by proliferative foci and neoplastic lesions induced by 7,12-dimethylbenz(a)anthracene in the rat adrenal cortex.

Proliferative and neoplastic adrenal cortical lesions were produced in female Sprague-Dawley rats by 7,12-dimethylbenz(a)anthracene. These lesions were studied for their ability to store histochemically demonstrable cellular iron compared with normal adrenocortical cells following iron loading by subcutaneous injection of iron-dextran. Stainable iron was excluded, under conditions in which normal cortical cells were heavily loaded with iron, in three histological types of adrenal cortical lesions; eosinophilic and basophilic proliferative foci; adenomas and lesions transitional between foci and tumours. The property of iron exclusion may be useful in studying neoplastic development in the adrenal gland.
