Computed angiotomography of unruptured cerebral aneurysms.

Twenty-seven unruptured cerebral aneurysms in 25 patients were detected by computed angiotomography. A comparison of the computed angiotomographic features in common aneurysm sites with plain CT and conventional arteriography was carried out. An isodense, round cisternal defect and a calcification or high-density mass in the basal cisterns on plain CT are important findings suggestive of unruptured aneurysms larger than 7 mm in diameter. The identification of the aneurysm, as well as of the afferent and efferent arteries on computed angiotomography is essential for the direct diagnosis of smaller unruptured aneurysms related to the circle of Willis. This is possible in a number of cases when the aneurysms are relatively large. It appears that the aneurysm size must be larger than 3 mm in diameter to permit its recognition on the basis of angiotomography and the avoidance of false-positive findings.

A method for mapping the distribution pattern of cariogenic streptococci within dental plaque in vivo.

This study was carried out to develop a method for mapping the distribution of cariogenic oral streptococci, Streptococcus mutans and Streptococcus sobrinus, from the outermost to the innermost plaque. Ten consenting subjects were asked to form plaque by abstaining from tooth brushing over 3 days within in situ plaque-generating devices, which were placed on the upper molars. The plaque formed in the devices was separated into 8-10 layered fractions (100 microm thick). Genomic DNA was extracted from each plaque fraction by a commercial DNA purification kit and used for the amplification of the 16S ribosomal RNA gene sequences by polymerase chain reaction (PCR) with universal primers. The products were then amplified by PCR with S. mutans- or S. sobrinus-specific nested primers. The final products were separated on agarose gels, stained and photographed to confirm the existence of S. mutans and S. sobrinus. The results showed that S. mutans was detected in the plaque obtained from all of the 10 subjects and S. sobrinus in the plaque of 7 subjects. However, the distribution patterns of fractions positive for S. mutans and S. sobrinus varied among the subjects, with a tendency for frequent detection of both species in the outer to middle layers of dental plaque. There were no plaque fractions in which only S. sobrinus was found. This method could be useful to map the distribution of cariogenic microorganisms and to estimate the bacterial ecology for oral biofilm.

Ultrastructural and fluorescence histochemical studies on the sympathetic innervation of the canine laryngeal glands.

The existence of the adrenergic terminals of the canine laryngeal glands has been revealed by electron microscopy and fluorescence histochemistry. Adrenergic fibres with fluorescent varicosities were observed around the base of th acini in the submucosa. In dogs treated with 5-OHDA, varicosities containing both small and large, dense-cored vesicles containing both small and large, dense-cored vesicles, believed to be adrenergic terminals, were found near blood vessels, gland cells and myoepithelial cells in the submucosal gland region. The role of these adrenergic terminals is briefly discussed.

Substance P nerve fibres in the canine larynx by PAP immunohistochemistry.

The distribution of the Substance P (SP) immunoreactive nerve fibres in the canine larynx and laryngeal nerves was studied by PAP immunohistochemistry. Many individual SP immunoreactive nerve fibres with varicosities were observed within the epithelial layer and in the connective tissue below the epithelium of the laryngeal mucosa. Small numbers of SP immunoreactive nerve fibres were also found in the submucosal gland region and some of them appeared to terminate in glandular cells. These findings are consistent with the view that SP might be involved in the laryngeal sensory innervation system and the laryngeal glandular secretion. No SP immunoreactive nerve fibres were found in any intrinsic laryngeal muscles. The recurrent laryngeal nerve and the superior laryngeal nerve contained SP immunoreactive nerve fibres and were considered to lie in the pathway of the SP nerve fibres to the larynx.

Radiological findings of vanishing tumor of the stomach.

Four lesions in three patients with so-called vanishing tumor of the stomach are described. Two patients had one lesion each and the third patient two lesions. The lesions were localized in the gastric cardia, angle, and antrum. Radiological findings of these tumors showed a round tumor with smooth surface and regular margins. The edges of the tumors were gently sloping, while the mucosa surrounding the tumor was edematous. The time interval until these tumors disappeared was five days, 20 months, 12 months and seven months, respectively. When the soft appearance of a gastric tumor is suspected in UGI examination, the patient should be re-examined after a month or more, to avoid unnecessary surgery.

Extended right hepatic lobectomy: combined resection of inferior vena cava and its reconstruction by EPTFE graft (Gore-Tex). Case report.

A 66-year-old man with hepatocellular carcinoma successfully underwent extended right hepatic lobectomy and combined resection of the right adrenal gland, right dome of the diaphragm and inferior vena cava including reconstruction with an expanded polytetrafluoroethylene (EPTFE) tube under a centrifugal pump driven veno-venous bypass without heparin. After 12 months, the graft was patent and there was no sign of recurrence.

Effects of angiotensin converting enzyme inhibitor (SA-446) on regional hemodynamics in renal and spontaneously hypertensive rats.

The effects of an angiotensin converting enzyme inhibitor, SA-446, on regional hemodynamics were examined in conscious renal and spontaneously hypertensive rats (RHR, SHR) using the radioactive microsphere method. We observed the differences in regional hemodynamics between hypertensive rats and normotensive rats, as well as the differences between SHR and RHR. An i.v. administration of SA-446 at a dose of 1 mg/kg induced a considerable hypotension in both types of hypertensive rats and increased the flow rates in most organs, along with an increase of cardiac output. The systemic and regional hemodynamic changes due to SA-446 in SHR and RHR were not qualitatively different, but the organs in RHR responded to SA-446 with greater magnitude. SA-446 increased the flow rates in organs which originally had low flow rates, more than it increased those of normotensive control rats.
