Chondrosarcoma of the hyoid bone.

Chondrosarcoma of the head and neck is relatively rare. We report the sixth case known to us in which the chondrosarcoma had originated in the hyoid bone. The patient, a 66-year-old man, underwent surgery. The resected tumour was a 5 cm encapsulated lesion composed of lobulated, firm, gray-white tissue. Histologically it showed mature chondrocytes without capsular invasion. There was high cellularity and a few binucleated chondrocytes. Although the tumour grade was prognostic significance, our literature review of the five other cases suggests that the location of the primary lesion and the adequacy of its resection to be more important. Our patient continues to do well 15 months post-operatively. However, long-term follow-up is essential as recurrences have been reported even after eight or 10 years.

Moebius syndrome: continuous tachypnea verified by a polygraphic study.

Four polygraphic recordings were obtained in three cases of Moebius syndrome. The cases were a 4-month-old girl (Case 1), a 4-year-old boy (Case 2), and a 5-year-old girl (Case 3). The recordings revealed that in all three cases there was a lack of rapid eye movements to the lateral side, continuous low amplitude on chin EMG, and continuous tachypnea. In Case 3, polygrams were recorded at 2 and 5 years of age, both records showing the same pattern of tachypnea. Arterial blood gas analysis (AGA), PH, PaCO2 and PaO2 were found to be within normal limits. It has been assumed that this syndrome includes brainstem dysplasia, according to the chief symptoms, and autopsy and auditory brainstem response (ABR) findings. ABR examination showed low amplitude of waves, a prolonged wave I-V interval, and the absence of wave V. It is known that there is a close relationship between this syndrome and respiratory disorders. But there have only been a few reports on respiratory disorders in this syndrome. It is strongly believed that continuous tachypnea is an important symptom of Moebius syndrome.

Purification of the bovine nicotinic acetylcholine receptor alpha-subunit expressed in baculovirus-infected insect cells.

The bovine skeletal muscle nicotinic acetylcholine receptor alpha-subunit was produced in insect, Spodoptera frugiperda (Sf9), cells by infection with a recombinant baculovirus. The expressed alpha-subunit protein could not be solubilized efficiently with Triton X-100 or sodium cholate, but could be solubilized efficiently with Zwittergent 3-14, sodium dodecyl sulfate or tris(hydroxymethyl)aminomethane dodecylsulfate. After solubilization of the alpha-subunit protein with Zwittergent 3-14 from the Triton X-100-insoluble fraction, the alpha-subunit protein was purified by concanavalin A-Sepharose chromatography and DEAE ion-exchange chromatography. A milligram quantity of the alpha-subunit protein could be purified from 8 g (wet weight) of Sf9 cells infected with the recombinant baculovirus. Chromatographic analyses including hydroxyapatite chromatography, DEAE ion-exchange chromatography, gel filtration chromatography and chromatofocusing and sucrose density gradient centrifugation analysis suggest that the purified alpha-subunit protein is homogeneous. The purified alpha-subunit protein had a high affinity for 125I-alpha-bungarotoxin and was glycosylated with a high mannose-type N-linked oligosaccharide side chain. These results indicate that purification of ion channel proteins produced by the baculovirus expression system is a promising approach to structural analysis of ion channel proteins, which are extremely rare membrane proteins in native tissues.

Effects of sleep interruption on body temperature in human subjects.

We interrupted the nocturnal sleep of human subjects for an hour and investigated the effect on body temperature (BT). The interruption elicited sleep onset rapid eye movement period (SOREMP). We compared the BT patterns obtained during nights with SOREMP at the second sleep onset (SOREMP nights) and those obtained during nights without SOREMPs (non-SOREMP nights) with those obtained during baseline nights. BT decreased markedly after sleep interruption on both SOREMP and non-SOREMP nights. It remained lower for 2 hours after the second sleep onset when compared with the corresponding period on the baseline nights. Consequently, the minimum BT was lower and the timing of the minimum was advanced when compared with baseline. The fall of BT after sleep interruption was not accompanied by any increase in slow wave sleep (SWS, stage 3 + 4) when compared with the baseline nights. Thus, we suggest that no direct relationship exists between the decrease of BT and the amount of SWS. On SOREMP nights, the fall of BT from lights-out to the second sleep onset was larger than on non-SOREMP nights. This lower body temperature at the second sleep onset may reflect the physiological background necessary for the appearance of SOREMPs.

Pulmonary alveolar proteinosis. Xe-133 scintigraphic findings before and after bronchopulmonary lavage.

A 44-year-old woman with pulmonary alveolar proteinosis was treated by repeated unilateral bronchopulmonary lavages over 6.5 years. The effectiveness of the treatment was assessed by Xe-133 scintigraphy as well as lung function tests and chest roentgenograms. Xe-133 scintigraphy clearly demonstrated improvement of regional ventilation and perfusion, and equalization of the ventilation and perfusion ratio. Therefore, Xe-133 scintigraphy was found to be useful in the analysis of changes in regional aeration, ventilation, and perfusion before and after bronchopulmonary lavage in pulmonary alveolar proteinosis.

A hospital-based case-control study on hepatocellular carcinoma in Fukuoka and Saga Prefectures, northern Kyushu, Japan.

Three hundred and sixty-eight case-control sets (male 287 pairs; female 81 sets) were collected for a hospital-based case-control study of primary hepatocellular carcinoma (HCC) conducted in Northern Kyushu, Japan. All incident cases of HCC were collected weekly from the inpatients (aged 40-69) of the Second Department of Internal Medicine, Kurume University Hospital between April, 1986 and May, 1992. One control for a male case and 4 controls for a female case were sampled, being matched to a case on age (same 5-year age class), sex, residence (prefecture) and time of hospitalization (within 2 months after a case interview) from the inpatients of two general hospitals in Kurume. Information was collected by interview in person by a well-trained interviewer and from a review of hospital records by the authors. Multivariate analyses based on a conditional logistic regression model without an interaction term revealed that hepatitis B surface antigen (HBsAg) positive status (odds ratio (OR) = 8.67; 95% confidence interval (95%CI) = 2.54-29.57), history of blood transfusion over 10 years previously (2.40; 1.26-4.56), parental history of hepatic diseases (2.31; 1.11-4.80) and heavy alcohol drinking (60 < or = drink-years) by age 40 (3.23; 1.61-6.51) were statistically significant risk factors of male HCC. Univariate analysis for females also showed an elevated OR of HBsAg (7.58; 1.96-29.35). Although the sample size was limited, univariate analysis indicated that anti-hepatitis C virus antibody by c100-3 antigen positive status had a statistically significant OR for HCC in both sexes.

Further evaluation of a new penetration enhancer, HPE-101.

The penetration enhancer, 1-[2-(decylthio)ethyl]azacyclopentan-2-one (HPE-101), significantly enhanced the excretion of topically applied [14C]indomethacin when dissolved in dipropylene glycol, triethylene glycol, diethylene glycol, 1,3-butylene glycol, trimethylene glycol, glycerin, water, silicone or triethanolamine, but not when dissolved in ethanol, isopropyl alcohol, oleyl alcohol, olive oil, peppermint oil, isopropyl myristate or hexylene glycol. HPE-101 significantly enhanced the excrection of [14C]indomethacin, [14C]nicotinic acid, [14C]5-fluorouracil, [3H]oestradiol and [3H]triamcinolone acetonide, but not that of [3H]testosterone. HPE-101 also significantly enhanced the excretion of [14C]indomethacin applied to intact skin of rabbit, guinea-pig and rat, and to tape-stripped skin of guinea-pig, but did not enhance the excretion of [14C]indomethacin applied to tape-stripped skin of rat or rabbit.

Vertical transmission of hepatitis C virus (HCV) detected by HCV-RNA analysis.

The rate of transmission of hepatitis C virus (HCV) from patients with chronic hepatitis C to their children was studied. Of the 64 children with a parent with chronic hepatitis C, two (3%) had abnormal alanine aminotransferase (ALT) activities, six (9%) had anti-HCV detected by c100 ELISA, seven (11%) had anti-HCV detected by ELISA-II, and 21 (33%) had HCV-RNA by polymerase chain reaction (PCR). Anti-HCV detected by ELISA-II disappeared within six months in all six infants. Of the five children whose mothers were given a blood transfusion after the child's first birthday, none had anti-HCV or HCV-RNA. In the five families whose elder or eldest offspring had HCV-RNA, all of the younger offspring had HCV-RNA. The vertical transmission rate of HCV was low if judged by the presence of anti-HCV or abnormal ALT values, but the rate was high (33%) if judged by the presence of HCV-RNA.

Histamine H2 receptor mediates keratan sulfate secretion in rabbit chondrocytes: role of cAMP.

We obtained evidence for the presence of a single class of histamine H2 receptor on rabbit chondrocytes. Stimulation of these receptors with specific H2 agonists led to an inhibition of keratan sulfate secretion and rapid (15 min) accumulation of intracellular adenosine 3',5'-cyclic monophosphate (cAMP). Factors such as prostaglandin E2 and parathyroid hormone, which stimulate short-term increases in cAMP, also caused a reduction in keratan sulfate secretion. Conversely, cholera toxin and forskolin, which enhance cAMP accumulation over 48 and 4 h, respectively, as well as a continuous exposure to dibutyryl cAMP, stimulated keratan sulfate secretion. These data suggest that intracellular cAMP must be kept above a certain level for a prolonged period to stimulate keratan sulfate secretion. We conclude that inhibition of keratan sulfate secretion is coupled with activation of the H2 histamine receptor.

Transvaginal hysterosonography for differential diagnosis between submucous and intramural myoma.

Thirty-six patients with hypermenorrhea, dysmenorrhea, and/or anemia were examined by transvaginal sonography (TVS) and by transvaginal hysterosonography using an endometrial balloon catheter and saline (TVHS). Those patients who showed distinct intramural myomas by TVS were excluded from this study. Of 36 patients, 22 were diagnosed with submucous myomas, 10 with intramural myomas, and 4 with endometrial polyps by TVS, but 20 patients were diagnosed as having submucous myomas, 12 patients intramural myomas, and 4 patients endometrial polyps by TVHS. The correlation between ultrasound findings and surgery (hysterectomy or hysteroscopy) indicated that there were 13 misdiagnoses (36%) by TVS and only one misdiagnosis (2.8%) by TVHS.

Pathological extent of interventricular septal infarction in patients with acute anteroseptal myocardial infarction with and without right bundle branch block.

The purpose of the present study is to elucidate the difference in extent of myocardial infarction (MI) between those cases with and those without right bundle branch block (RBBB) occurring during the course of acute MI. We examined postmortem hearts from 20 patients with acute anteroseptal MI; 10 with (group A) and 10 without RBBB (group B). The extent of MI was studied pathologically in the interventricular septum (IVS) and reconstructed. The longitudinal extent of MI did not distinctly differ between groups A and B. In 5 hearts of group B, the anterior limit of the MI extended as high, or as close to the cardiac base, as in group A hearts. Transmural MI was relatively common and seen more frequently in group A than in group B. However, the MI did not always extend evenly to the left and right ventricular sides in the IVS. Left-sided predominance in extent of the MI was more frequently observed in group B than in group A. Right-sided predominance in the extent of septal MI was relatively rare and was seen in 5 cases at the portion where the right bundle branch came down. Four of these were group A patients. Thus, involvement of the right bundle branch might partly depend on the unevenness of the mural extent of the MI as well as the height of extension of the MI in the IVS.

Pleasantness-unpleasantness of environmental sounds and gender difference in evaluation.

The present experiment examined emotional evaluation of 40 environmental sounds which were presented to 388 college students in a lecture room. Students were required to rate pleasantness-unpleasantness, to identify the sounds, and to rate their confidence about their identifications. Analysis showed that the pleasant sounds were natural and musical sounds and that the unpleasant sounds were sounds of a belch, of a dentist's drill, and of scratching on a blackboard. It is interesting that for pleasant sounds confidence was always high, which suggested that emotional evaluation of the environmental sounds was closely related to the confidence of observers in their identifications of the sounds. Gender differences were noted on the ratings on pleasantness-unpleasantness of environmental sounds, that is, women evaluated the pleasant sounds as more pleasant than men, and men evaluated the unpleasant sounds as not so unpleasant as did women.

One explanatory basis for the discrepancy of reported prevalences of sleep paralysis among healthy respondents.

In a previous study, the author and coworkers found 39.8% of healthy young adults had experienced sleep paralysis. Some other studies reported prevalence as about the same or higher (i.e., 40.7% to 62.0%) than that previous estimate, while yet other studies, including Goode's work cited by ASDC and ASDA classifications, suggested much lower prevalences (i.e., 4.7% to 26.2%). The author tested the hypothesis that this discrepancy among the reported prevalences is partly due to the expression used in each questionnaire. University students who answered the questionnaire using the term 'transient paralysis' reported the lower prevalence (26.4%), while the second group of respondents who answered the questionnaire using the term kanashibari, the Japanese folklore expression for sleep paralysis, gave the higher prevalence (39.3%). The third group who answered the questionnaire with the term 'condition,' probably a rather neutral expression, marked the middle (31.0%) of these.

Carcinogenicity of benzotrichloride administered to mice by gastric intubation.

Epidemiological studies suggest that benzotrichloride (BTC) is a human carcinogen. In the present study, BTC was tested to evaluate its ability to induce lung tumors as a result of systemic exposure. Administration of BTC by gastric intubation, 2.0-0.0315 microliters/mouse (4 doses), twice a week for 25 weeks, in female ICR mice, produced forestomach tumors (squamous cell carcinoma and papilloma), lung tumors (adenocarcinoma and adenoma) and tumors of the hematopoietic system (thymic lymphosarcoma and lymphatic leukemia) with dose-related response by 18 months. The present and previous studies indicate that the target organs of BTC carcinogenesis in mice are the local tissue which is primarily exposed, and the lung and hematopoietic tissue when BTC is administered systematically.

Reduction of leaky lymphocyte clones producing immunoglobulins and thymic lymphocytic leukemia by selective inbreeding of SCID (severe combined immunodeficiency) mice.

Selective inbreeding of C.B17-scid/scid mouse pairs showing undetectable IgG and IgM has been carried out in order to reduce the mortality of mice by early occurrence of thymic lymphocytic leukemia and abnormal lymphocyte clones producing immunoglobulins, both of which inhibit the successful heterotransplantation of normal and neoplastic human tissues. Although the majority of C.B17-scid/scid mice showed undetectable (< 1 microgram/ml) or low level (< or = 25 micrograms/ml) of serum IgG and IgM, some produced abnormally high concentrations of IgG and IgM (> 25 micrograms/ml). The incidence of such mice showing higher levels of IgG was very high at F1 and F2 generation (10/55, 18.2%), but significantly low after the F3 generation (18/446, 4.0%, p << 0.001). Although leukemia incidence was very high at F4 to F5 generations (8/40, 20.0%), death from leukemia was not observed early in life (4-6 months after birth) at F7 to F10 generations (0/36, 0%, p < 0.01) and was very low during the age of 6-10 months after the F8 generation (11/66, 16.7% at F4 and F5 vs 4/93, 4.3% at F8-10), p < 0.01). Scid mice improved by the selective inbreeding will provide an invaluable experimental system for the heterotransplantation of normal and neoplastic human tissues.
