Primary hepatic carcinoid tumor: a case report.

A case of primary hepatic carcinoid tumor is reported. A 57-year-old woman had no endocrine symptoms. Light microscopic findings revealed tumor cells forming trabecular or insular structures and demonstrating diffuse positive staining by the Grimelius method. Mucin stained with Periodic acid-Schiff (PAS) and alcian-blue was partially present in the tumor cells. Immunohistochemically, most tumor cells stained positive with chromogranin A and cytokeratin stains. Intensive and careful investigation before and after the operation revealed no other origin of the tumor. We present this rare case, describe the clinical features of hepatic carcinoid, and discuss its diagnosis.

Characterization of mixed-function oxidases and purified cytochrome P-450 of livers of house musk shrew, Suncus murinus.

Characteristics of mixed-function oxidases in the liver of house musk shrew, Suncus murinus, were studied. The basal level of cytochrome P-450 and the activity of drug-metabolizing enzyme in hepatic microsomes of Suncus murinus is relatively lower than that of rats, while the level of cytochrome b5 and NADPH-cytochrome c reductase is approximately the same as that of rats. The treatment of Suncus murinus with phenobarbital and 3-methylcholanthrene elevated the level of cytochrome P-450 and the activity of drug-metabolizing enzymes, but not the level of cytochrome b5. Two distinct forms of cytochrome P-450 have been purified from hepatic microsomes of 3-methylcholanthrene-treated Suncus murinus. These forms have their absorption maximum at 448.0 nm and 448.5 nm in CO-bound reduced form, and one is in the high-spin state and the other is in the low-spin state. They are different in their molecular weights (53,500 and 55,000) and in their spectral and catalytic properties. Characteristics of these forms were compared with those of the major forms of cytochrome P-450 purified from livers of rats treated with 3-methylcholanthrene.

Ultrasonographic measurement of tumor regression in radiation therapy.

Ultrasonography was evaluated as a measurement procedure for investigating the tumor regression of cervical lymph nodes. Preliminary results suggested that interobserver compatibility in the measurement of nodes deteriorates when the diameter of the node is less than 15 mm. There was no systematic disparity between ultrasonographic and manual measurements. The tumor regression line fit the exponential line with a correlation coefficient of more than 0.95 in seven of 16 nodes. During the sequential examination, a central low-echo area suddenly appeared in one node at 15 Gy/6 Fr.

Colorectal adenomatous polyps detected by immunochemical occult blood screening.

BACKGROUND/AIMS: This study aimed to evaluate the validity of immuno-chemical occult blood screening for colorectal adenomas and to clarify the characteristics of this disease which are detected. METHODOLOGY: We studied 140 colorectal adenomas and 140 healthy controls in a hospital based case control study, in which three-day testing was conducted to assess the accuracy of the immuno-chemical occult blood screening test (total 280 patients). A further 80 colorectal adenomas 1 cm or larger, which were detected by a screening program, were compared with matched cases detected by the outpatient clinics (total 160 patients). A comparison of characteristics including tumor location, shape and histology was made between the two groups. RESULTS: Sensitivity and specificity were calculated to be 46% (61% for tumors larger than 1 cm, 30% for those less than 1 cm), and 96% respectively, showing a significant difference in sensitivity between large and small adenomas (p < 0.01). The proportion of rectal adenomas was significantly higher in the outpatient clinics (41%) than in the screening program (21%) (p < 0.05), whereas no differences were noted in the other measures. CONCLUSIONS: These results suggest that immuno-chemical occult blood is inadequate as a screening test for detecting small adenomas, as t can not differentiate between the detection of rectal adenomas from large adenomatous polyps.
