Decrease in cerebral blood flow with blood pressure reductions in patients with chronic stroke.

BACKGROUND AND PURPOSE: Possible effects of changes in blood pressure on the cerebral circulation were studied in patients with chronic stroke and age-matched nonstroke control subjects at 28 +/- 10 months (mean +/- SD) (range, 18 to 54 months) and 27 +/- 6 months (range, 19 to 44 months), respectively, after the first measurement. METHODS: Cerebral blood flow was measured by the 133Xe inhalation method in 55 patients (mean +/- SD age, 62 +/- 11 years; 39 with brain infarction and 16 with hemorrhage) and 10 control subjects (mean +/- SD age, 61 +/- 9 years). Correlations between changes in cerebral blood flow and blood pressure were evaluated. RESULTS: Among brain infarctions, average cerebral blood flow did not change from the first study; however, changes in cerebral blood flow in each individual were closely related to changes in systolic and mean arterial blood pressures (P < .01 and P < .05, respectively). Of these, in 10 patients with cerebral blood flow decreased more than 15% from the initial levels, systolic and mean arterial blood pressures decreased by 25 +/- 32 mm Hg and 16 +/- 14 mm Hg (P < .05 and P < .005, respectively). In contrast, in 29 patients with unchanged or increased cerebral blood flow, changes in systolic (0 +/- 19 or -2 +/- 12 mm Hg, respectively) and mean arterial blood pressures (3 +/- 22 or -1 +/- 11 mm Hg, respectively) were not significant, and their systolic blood pressure levels were maintained above 110 mm Hg. CONCLUSIONS: Blood pressure importantly correlates with cerebral circulatory changes among patients with chronic brain infarction. Early detection of cerebral hemodynamic changes should be useful for determining the most favorable levels of blood pressure and for selecting appropriate therapy.

Aneurysm of the posterior cerebral artery: report of eleven cases--surgical approaches and procedures.

Eleven cases of an aneurysm of the posterior cerebral artery are reported. All 11 aneurysms were saccular, and 3 were either giant or large. The aneurysms arose from the P1 segment in three patients, the P1-P2 junction in three patients, the P2 segment in three patients, and from the P3 segment in two patients. In all, 10 patients underwent surgery. All P1 and P1-P2 junction aneurysms were treated with the pterional approach. Three P2 and two P3 aneurysms were managed by the subtemporal approach. Two small aneurysms in the series were treated by coating the aneurysmal dome, two by clipping the afferent artery, and all other saccular type aneurysms were treated by clipping the aneurysmal neck. Seven patients had either an excellent or good outcome; two had poor results; and one patient died. The operative approaches and procedures are also discussed in relation to the anatomy of posterior cerebral artery aneurysms.

Physiological localization of the facial colliculus during direct surgery on an intrinsic brain stem lesion.

We designed a method for localizing facial colliculus intraoperatively by means of weak stimulation of the 4th ventricular floor and recording the electromyographic response of the facial muscle. This method is applicable clinically to prevent 6th and 7th cranial nerve complications during direct surgery on an intrinsic lesion of the brain stem, in which the normal anatomy of the rhomboid fossa is often distorted.

[Age-related changes in intracranial artery velocity measured by transcranial Doppler sonography in normotensive, hypertensive and diabetic patients].

We measured mean blood flow velocity (MFV) and Fourier pulsatility index (PI) of the M1 portion of the middle cerebral artery (MCA) by transcranial Doppler mapping technique (2 MHz, Trans-scan, EME Co., Ltd.). The correlations between these parameters and arteriosclerotic risk factors such as aging, hypertension and diabetes mellitus were examined. Healthy volunteers as well as patients suffering from hypertension and diabetes mellitus were studied. A total of 59 persons (85 MCAs) consisting of 30 males (mean age 54.4, 24-81 years) and 29 females (mean age 54.8, 20-75 years) were enrolled in this study. Some hypertensive or diabetic subjects had previous cerebrovascular disease, therefore cases who had only minor stroke or asymptomatic infarction without significant lesions in carotid artery system were included. After excluding brain embolism, these subjects were divided into three groups as normotensive/non diabetic group (NT/non DM), hypertensive group (HT) and diabetic group (DM). In each group, MFV decreased with aging, being more evident in the HT and DM groups than in the NT/non DM group. Fourier PI also showed a significant increase with aging, and the correlation was stronger than that between MFV and age. Moreover, these trends of age-Fourier PI relationship were more highly significant in HT and DM groups than NT/non DM group. In cases with HT or DM, age-related arterial changes appeared to be more severe than in those without. Fourier PI seemed valuable for detecting arterial lesions with aging, HT and DM.

Tumor induction in mice administered neonatally with N-bis(2-hydroxypropyl)nitrosamine (BHPN).

Two subcutaneous injections of N-bis(2-hydroxypropyl)nitrosamine (BHPN) to mice on day 1 and 8 induced tumors of the lung, liver, nasopharynx and pancreas, when animals were killed at the end of one year. The tumor incidences of lung were 90-100%; those of liver 60-92%; those of nasopharynx 3-13%; those of pancreas 2-3%, respectively. The average tumor nodules of lung and those of liver increased in a dose-dependent manner. Two doses of 50 mg/g body weight of BHPN induced one duct cell carcinoma and one acinar cell adenoma of pancreas in a mouse, respectively.

Micro-PIXE (particle induced X-ray emission) analysis of aluminum in rat-liver using MeV heavy ion microprobes.

Heavy ion microprobes (HIM) such as 3 MeV Si2+ and 3 MeV p2+ have been applied to the elemental analysis by PIXE (proton-induced X-ray emission). It was found that silicon and phosphorus microprobes have several times higher sensitivity for aluminum K alpha X-rays than 2 MeV proton microprobes, and detection limits were more favorable in a phosphorus microprobe. Using a 3 MeV P2+ microprobe, the liver of a rat, which had been injected with aluminum-lactate, was investigated and it was found that aluminum segregates in areas with a dimension of about 10 microns. These areas could hardly be observed with 2 MeV proton microprobes.

[The relation of vertebral artery blood flow velocity to mean arterial pressure during anesthesia with enflurane or isoflurane].

The relation of vertebral artery blood flow velocity (VABFV) to mean arterial pressure (MAP) during anesthesia with enflurane or isoflurane was examined. The subjects were 17 patients on the knee-chest position. Anesthesia was induced with thiamylal and fentanyl. The patients were placed on the knee-chest position after tracheal intubation. Ventilation was adjusted to maintain a constant level of PaCO2 (30-40 mmHg). The patients were divided into two groups. Anesthesia was maintained with enflurane and 67% N2O in group E (8 patients), or with isoflurane and 67% N2O in group I (9 patients). VABFV was measured with a transcranial Doppler sonography (TCD), and MAP was measured through a radial artery catheter. The mean VABFV and MAP were recorded at five-minute intervals. VABFV was found to change with the change in the MAP for the same degree in both groups. The results suggest that cerebral autoregulation is disturbed during either isoflurane-N2O or enflurane-N2O anesthesia.

[Akin moyamoya disease in children].

Moyamoya disease is characterized by bilateral involvement of the internal carotid arteries. The etiology of this involvement is unknown. However, we previously reported two pediatric cases of moyamoya disease that progressed from unilateral to bilateral involvement. Some cases of unilateral occlusion in the carotid fork seem to have occurred at an early stage of definite moyamoya disease. In the present study, we examined five pediatric patients showing bilateral and/or unilateral occlusion of the internal carotid artery. In each case, the etiology was known. They included Apert syndrome, radiation therapy for pilocytic astrocytoma, systemic lupus erythematosis, von Recklinghausen disease and Down syndrome. The clinical manifestations, EEG, CT, MRI, PET, and angiographic findings in these patients were presented in comparison with those of definite moyamoya disease. This comparison led us to suggest that definite moyamoya disease might be included in these cases, and we emphasize the importance of precise examination in each case.

Lack of blood pressure increase with age in long-term hospitalised patients with the sequelae of acute carbon monoxide poisoning.

We investigated retrospectively age-related changes in BP and body composition in 44 men who had been hospitalised for up to 26 yrs as a result of carbon monoxide poisoning. The average age of the patients was 42.5 +/- 9.1 (SD) yrs on the first admission. The average period of hospitalisation was 19.5 +/- 5.9 yrs. The patients received mild salt restriction and daily exercise in the hospital. During the study period, nine patients developed hypertension and received antihypertensive treatment. The prevalence of hypertension in our study population was 205 per 1,000 and the incidence of hypertension (DBP > 95 mmHg) was 9.1 per 100 person years. Both prevalence and incidence appear to be lower than those in the general population. In the other 35 patients BP significantly decreased from 127.5 +/- 10.0 mmHg (SBP)/79.8 +/- 8.1 mmHg (DBP) at the time of the first examination to 120.8 +/- 16.3 mmHg/71.9 +/- 10.3 mmHg on the last examination. Body mass index did not change significantly during the study period. Hypertensive cardiovascular death was not observed throughout the study period. In conclusion, controlled diet, salt intake, physical exercise and isolation from the social stimuli achieved by a long-term hospitalisation may prevent increased BP and cardiovascular death.

[Arterial blood ketone body ratio and systemic hemodynamics in patients with acute hepatic blood flow occlusion].

This study was undertaken in order to clarify the influence of acute hepatic blood flow occlusion on arterial blood ketone body ratio (AKBR) and systemic hemodynamics. Ten patients for hepatectomy were divided into two groups. Group I was composed of five patients who had clamping of total hepatic blood flow (Pringle's method), and Group II was composed also of five patients who had clamping of the right hepatic artery and portal vein during operation. Ten minutes after clamping blood flow, AKBR decreased significantly in both groups, and a marked reduction in AKBR was observed in group I. During occlusion, cardiac output was reduced significantly, and immediately after declamping, mean arterial blood pressure and systemic vascular resistance index (SVRI) decreased significantly. These changes in hemodynamics in group I were larger than those of the other group. A larger reduction in AKBR was observed during occlusion, and the greater change in SVRI appeared after declamping blood flow. These findings suggest that in the cases with hepatic blood flow occlusion, especially total occlusion, a special attention should be paid to mitochondrial liver dysfunction and hemodynamic changes associated with acute hepatic blood occlusion.

5-Methyl-4H-3,1-benzoxazin-4-one derivatives: specific inhibitors of human leukocyte elastase.

Inhibitors of human leukocyte elastase (HLE) may exert potent therapeutic effects on pulmonary emphysema, adult respiratory distress syndrome and other diseases involving tissue degradation. 7-(4-Chlorophenylsulfonyl-L-glutanyl)amino-5-methyl-2-isopro pylamino-4H-3,1- benzoxazin-4-one (TEI-5624) and 7-(4-chlorophenylsulfonyl-L-lysyl)amino-5-methyl-2- isopropylamino-4H-3,1-benzoxazin-4-one (TEI-6344), two derivatives of 5-methyl-4H-3,1-benzoxazin-4-one, showed strong and highly specific inhibition of human sputum elastase (HSE), which is equivalent to HLE, with Ki values of 6.91 and 16.3 nM, respectively. The selectivity of TEI-5624 for HSE vs. several proteinases ranged from 300-fold to 45,000-fold in favor of HSE. TEI-5624 and TEI-6344 also efficiently prevented degradation of insoluble elastin by stimulated polymorphonuclear leukocytes. The elastase inhibitory capacity of these compounds was not affected by treatment with stimulated polymorphonuclear leukocytes or Pseudomonas aeruginosa-origin elastase. When administered intratracheally to hamsters. TEI-5624 and TEI-6344 were eliminated from the lung with half-times of 85 and 240 min, respectively. In acute injury induced by intratracheal administration of HSE in hamsters, these compounds significantly suppressed pulmonary hemorrhage when administered intratracheally (1 mg/kg) either 30 or 240 min before challenge with HSE (1 mg/kg). HSE-induced emphysema in hamsters was also prevented by TEI-5624 (1 mg/kg) administered intratracheally 7 hr after HSE administration (1 mg/kg). These results suggest that TEI-5624 and TEI-6344 may be useful therapeutic agents for the treatment of HLE-mediated diseases.

[MR imaging of thyroid masses].

Magnetic resonance (MR) imaging was performed in 35 patients with histologically proved thyroid masses. Gadopentetate dimeglumine was used in 25 of these cases. The thyroid tumors were analyzed by MR imaging as to shape, margin, pseudocapsulation, homogeneity, and enhanced pattern. Gd-DTPA was helpful in identifying pseudocapsules and showing the extent of the tumor. Papillary carcinoma tended to display unclear margins, heterogeneous intensity, and inhomogeneously enhanced patterns. In follicular adenoma, the tumor margin was smooth and pseudocapsules were clearly detected. Follicular adenoma was enhanced homogeneously, but it was difficult to distinguish follicular adenoma from follicular carcinoma by MR imaging. Adenomatous goiter was delineated as multiple nodules with smooth margins, but without pseudocapsules. These nodules were enhanced in various ways after Gd-DTPA administration.

New approach for detection of amplification in cancer DNA using restriction landmark genomic scanning.

We developed a new approach for detecting the gene amplification of cancer DNAs with restriction landmark genomic scanning (RLGS). In cancer research, much effort has been made to find the amplified loci of cancer DNAs, because many lines of evidence indicate association between oncogene amplification and carcinogenesis. Conventionally, such gene amplification has been detected by using Southern hybridization with DNA probes. However, only the information of one locus can be obtained by one hybridization procedure, and analysis of many loci throughout the genome is too laborious and time consuming, even if only several candidate genes are investigated. On the other hand, the "in-gel renaturation method" was reported as another alternative for detection of amplified regions. However, even though this method is much improved, it is difficult to detect less than 7-fold amplification, which is often higher than the amplification of many cancer cases. To overcome these limitations and, in addition, to locate the amplified DNA two dimensionally, we applied RLGS for analysis of DNA amplification in cancer tissues, such as breast cancer (infiltrative tubuloadenocarcinoma), neuroblastoma, meningioma (endotheliomatous meningioma), and thyroid cancer (papillary adenocarcinoma). In some cases of breast cancer, several amplified spots located on the same amplicon were detected. In thyroid cancer, in which no amplification has yet been reported, low-grade amplification was also detected. In this report, we demonstrated that RLGS allows us to screen 2000-3000 restriction landmarks distributed on the genome simultaneously, and even low-grade amplification could be detected effectively. Thus, RLGS has proven to be a very useful method in detecting DNA amplification.

Unresolved rearrangement of thiazoline form of glutathione.

The thiazoline form of glutathione was investigated with regard to its unresolved instability under neutral conditions. A simple method was developed for production of the thiazoline in stable solid form, thereby facilitating preparation of its neutral solution without using excess base and enabling isolation of the reaction product in quantity by ion-exchange chromatography. Analysis of the product by HPLC, infrared and ultraviolet absorption spectroscopy, mass spectrometry and proton magnetic resonance led to the identification of a cyclic amidine form of glutathione. The instability of the thiazoline is, therefore, due to an intramolecular rearrangement reaction, rather than hydrolysis. Once formed, the amidine is stable at pH 5-7 and in concentrated HCl, showing no tendency to rearrange back to either the original thiazoline or glutathione under these conditions.

Cardiac free wall rupture in acute myocardial infarction: ameliorative effect of coronary reperfusion.

To investigate the pathophysiology of cardiac free wall rupture (cardiac rupture) following acute myocardial infarction (AMI), and to clarify whether reperfusion therapy prevents cardiac rupture, 1,329 cases of AMI (conventional therapy group: 807 cases and reperfusion therapy group: 533 cases) were studied retrospectively. The overall incidence of cardiac rupture was 2.3% (2.7% in the conventional therapy group vs. 1.7% in the reperfusion therapy group). Patients with cardiac rupture were divided into two subgroups according to the time interval from the onset of AMI to cardiac rupture (early rupture less than or equal to 72 h and late rupture greater than or equal to 4 days). The indices of initial evolution of AMI was a significant risk of early cardiac rupture. The reperfusion therapy group showed significantly lower incidence of late rupture (0.4 vs. 1.5% in conventional therapy group; p less than 0.05). The incidence of cardiac rupture in the unsuccessful reperfusion therapy group was higher than that of the successful group (5.9% of 118 cases vs. 0.5% of 404 cases; p less than 0.05). It is concluded that the etiology of cardiac rupture following AMI cannot be explained by any single factor. Early rupture depends on the initial evolution of AMI, and early reperfusion and collateral flow prevent the late onset cardiac rupture.
