Biologic and immunologic in vivo and in vitro studies with biosynthetic human insulin.

A preliminary study was performed to estimate the effect of biosynthetic human insulin (BHI) produced by the Eli Lilly Company in comparison with purified pork insulin (PPI) from the same manufacturer. Immunologic studies were done using an intradermal skin test with both insulins and in vitro studies binding with sera containing preformed antibodies against bovine and pork insulin. The studies were undertaken in eight healthy volunteers aged 24-31 yr. Insulin was given subcutaneously in an amount of 0.1 U/kg and 3 days later intravenously in an amount of 0.03 U/kg injected slowly over 1 min. Blood glucose, insulin, C-peptide, and growth hormone were measured at different times between 0 and 360 min. It was shown that BHI and PPI behaved similarly in lowering blood glucose, suppressing C-peptide, and increasing growth hormone via hypoglycemic action. After s.c. injection, BHI was found to reach higher serum concentrations after 60 min than PPI. Correspondingly, the blood glucose curve was significantly lower after BHI than after PPI. This difference was not observed after the i.v. application of both forms of insulin.

[Polyclonal activated circulating B-lymphocytes in patients with rheumatoid arthritis].

Peripheral B lymphocytes were studied to investigate the role of polyclonal B-cell activation in the pathogenesis of rheumatoid arthritis (RA). Proliferative responses to anti-mu antibodies coupled to sepharose used as a B-cell-specific mitogen were reduced as compared to healthy controls. When cell surface antigens were studied by flow cytometry employing monoclonal antibodies, B cells from patients with RA showed enhanced expression of Interleukin-2 (IL-2) receptor. In-vivo-activated, in-vitro spontaneously proliferating Epstein-Barr virus transformed B cells positive for immunoglobulin M, G, and A were obtained from patients with RA. In summary, our findings suggest the presence of polyclonal activated B cells in the peripheral blood of patients with RA, and a close relationship of polyclonal activated B cells and the activity of the disease process.

[Circadian rhythm of cortisol and corticotropin (ACTH) in patients with rheumatoid arthritis in relation to inflammatory activity].

In patients with rheumatoid arthritis, corticosteroids are usually given as a single morning dose to minimize adrenal suppression, because of the well-known circadian rhythm of the secretion of cortisol and corticotropin. However, it has not been so far examined whether there really is a normal secretion pattern of those hormones in all patients with rheumatoid arthritis, as there is in healthy subjects. We therefore studied twelve patients with rheumatoid arthritis who had never been treated before with corticosteroids and disease-modifying drugs such as gold, D-penicillamine or immunosuppressive drugs. The study was designed to examine adrenal secretory activity by measuring the cortisol and corticotropin levels at 2 h intervals throughout a 24 h cycle. The circadian secretion patterns of cortisol and corticotropin were disturbed in most patients. High inflammatory activity of the disease was accompanied by a severe disturbance of the circadian rhythm, whereas patients with low inflammatory activity showed a nearly normal secretion pattern. Besides other causes, the influence of mediators of inflammation on hypothalamic centers, analogous to endogenous pyrogen in fever, must be discussed.

[Ankylosing spondylitis and urogenital infection: diagnosis of urologic infection and correlation with rheumatologic findings].

We examined 134 male patients with confirmed ankylosing spondylitis. The study protocol included a medical-rheumatological examination and thorough exploration for infections of the urinogential tract. An urethroadnexitis was found in 37 of 134 patients (27.6%): Two patients suffered from balanitis, 17 patients from urethritis, 18 patients from prostatitis, and two patients from epididymitis. Only four patients gave a history of urethritis and eight patients of prostatitis. The microorganisms isolated most frequently from patients with urogenital infection were Chlamydia trachomatis and, in few cases, Ureaplasma urealyticum. By comparing the urethroadnexitis group and the non-infected group with regard to other clinical parameters, we found a significantly increased C-reactive protein in the infected group. Other clinical parameters like involvement of the free spinal column, inflammatory involvement of the joints, and HLA-B27 correlation did not differ significantly.
