Sonographically guided core-needle biopsy in the diagnosis of superficial lymphadenopathy.

PURPOSE: This study was designed to evaluate the efficacy of ultrasound-guided percutaneous core-needle biopsy for establishing histopathologic diagnoses of palpable enlarged lymph nodes. METHODS: Thirty patients without a history of malignancy or recent infection underwent ultrasound-guided core-needle lymph node biopsies. Only patients with enlarged lymph nodes ranging from 1.5 to 3.5 cm in greatest diameter that had been present for more than 2 months were included in this study. Two cores from each lymph node were obtained by a freehand core-needle biopsy technique using a 7-MHz ultrasound transducer and an automatic spring-loaded biopsy gun with an 18-gauge cutting needle. RESULTS: The histologic diagnoses were conclusive in 24 cases (80%) and inconclusive in 6 cases (20%). In the 24 conclusive cases, the lymph node enlargement was due to benign causes in 12 cases and malignancy in 12 cases. All specimens in the conclusive cases were sufficient for histologic diagnosis, but there was insufficient material for a diagnosis in 3 of the inconclusive cases. No complications were seen in this series. CONCLUSIONS: Ultrasound-guided core-needle biopsy of enlarged lymph nodes is a safe, minimally invasive alternative to surgical biopsy, enabling a histologic diagnosis for treatment planning in most cases.

Computed tomographic evaluation of ocular trauma.

The CT scans of 10 traumatized patients with demonstrable ocular injuries were reviewed. The CT findings were correlated with the clinical findings in each case. CT manifestations of a variety of ocular and orbital injuries are reported and a systematic approach to the CT evaluation of ocular trauma is presented.

Angiolipoma of the breast with microcalcification. Mammographic, sonographic, and histologic appearances.

Angiolipoma of the breast is a rare benign fatty tumor that can be mammographically confused with breast carcinoma. We herein report such a case presenting as a partial obscured mass with microcalcification on mammogram. Sonogram revealed an encapsulated hyperechoic nodule with homogeneous internal echo. Microscopic evaluation revealed abundant adipose tissues with evidence of proliferating vessels and intravascular hyaline thrombi. To our knowledge, angiolipomas of the breast containing microcalcification secondary to intravascular thrombi had not been previously reported.

Sequence comparison of the EcoHK31I and EaeI restriction-modification systems suggests an intergenic transfer of genetic material.

The genes coding for the EcoHK31I and EaeI restriction-modification (R-M) systems from Escherichia coli strain HK31 and Enterobacter aerogenes, respectively, have been cloned and sequenced. Both ENases recognize and cleave Y/GGCCR leaving 4 nucleotide 5'-protruding ends, while the MTases modify the internal cytosine. The systems were isolated on a 2.3kb AseI fragment for EcoHK31I, and a 4.6 kb HindIII fragment for EaeI. The R and M genes of both systems converge and overlap by 14 nucleotides. Previously, we found that M.EcoHK31I consisted of two subunits, (alpha and beta), with the beta subunit being translated from an alternative open reading frame within the gene encoding the alpha subunit. Sequence comparison between the EcoHK31I and EaeI systems reveals striking similarity. The eaeIM gene also encodes alpha and beta polypeptides of 309 and 176 amino acids which share 96% and 97% identity, respectively, with those of ecoHK31IM. ecoHK31IR and eaeIR encode proteins of 318 and 315 aa, respectively, which share 92% identity but are otherwise unique in the GenBank database. The EaeI and the EcoHK31I R-M systems were found to be flanked by genes coding for integrases. It is possible that these integrases have facilitated the transfer of this system among different bacterial species.

Emergency laparoscopic unroofing of a ruptured hepatic cyst.

The use of laparoscopic techniques in the emergency treatment of liver cyst complications has not been reported in the literature. Laparoscopic surgery is a well-established treatment modality for giant hepatic cyst in the elective setting. We report a case of laparoscopic unroofing of a 15-cm ruptured hepatic cyst, which had recurred after previous percutaneous alcohol instillation therapy. Laparoscopic ultrasound was used to delineate the extent of the cyst while a Harmonic scalpel was used for dissection. The patient made an uneventful recovery and was discharged 4 days after the operation.

Evaluation of human sperm hyperactivated motility and its relationship with the zona-free hamster oocyte sperm penetration assay.

The authors studied hyperactivated motility of human spermatozoa as a method of evaluating capacitation by examining its relationship to results of zona-free hamster oocyte sperm penetration assays (SPA) of semen samples from 50 men attending the infertility clinic. Hyperactivated motility was assessed in the seminal plasma and after swim-up preparation of spermatozoa at 1, 3, and 24 hours of incubation in capacitation media using a computer-assisted semen analysis system equipped with a hyperactivation module. Hyperactivated motility reached a peak at 1 hour and plateaued at 3 hours. The percentage of spermatozoa in seminal plasma with star-spin hyperactivated motility was significantly lower in the group showing no penetration in the SPA. The hyperactivated motility characteristics did not differ in the groups with positive or negative penetration. Correlation analysis failed to show any significant relationship between the hyperactivated motility parameters and SPA score. When the hyperactivated motility characteristics were compared in samples with normal and abnormal semen analyses, the total percentage of spermatozoa with hyperactivated motility and the percentage with star-spin at 3 hours were significantly lower in the group with abnormal semen analysis. The data indicate that lower hyperactivated motility of spermatozoa was found in patients with a score of zero for SPA and in patients with abnormal semen analysis. It was concluded that although no direct correlations were found between the results of SPA and hyperactivated motility, evaluating hyperactivated motility may still be useful as an early indicator of capacitation abnormalities of human spermatozoa not measured by SPA.

1.5-T MR imaging of pituitary microadenomas: technical considerations and CT correlation.

Thirty-seven patients with suspected pituitary tumors were evaluated prospectively with MR imaging at 1.5 T. MR detected a microadenoma at its correct location in all eight patients who underwent transsphenoidal surgery, while CT showed a focal abnormality in the correct location in only four of the eight patients. In patients who were clinically and endocrinologically considered to harbor a microadenoma, MR detected a focal pituitary signal abnormality in 83% and CT demonstrated a focal density abnormality in 42%. Infundibular displacement, focal gland convexity, and sellar-floor abnormality were seen equally well with CT and MR. MR imaging protocol included sagittal T1-weighted spin-echo, coronal inversion-recovery, and coronal spin-echo or cardiac-gated spin-echo images. Although inversion-recovery images were superior in detecting focal pituitary lesions, some microadenomas were better seen on T2-weighted images. Cardiac-gated spin-echo images showed focal pituitary lesions better than ungated images did. Our technique demonstrates MR's superior sensitivity to CT in detecting a pituitary microadenoma.

Ischemic chiasma syndrome.

Thirty patients with chiasma syndrome attributed to ischemic processes of various origins are described. The initial diagnosis was made after an investigation based on polytome pneumoencephalography and angiography. This diagnosis was confirmed later with the use of high-resolution computed tomography (CT) and refined angiographic techniques. The ischemic chiasma syndrome may be classified into five pathogenetic categories: (1) mechanical compression of the chiasm secondary to ectatic tortuous arteries; (2) vascular occlusion secondary to atheromatous plaque formation; (3) arachnoiditis with fibrosis; (4) arteritis of various origins; and (5) postpartum necrosis. Sophisticated neurologic procedures including magnification angiography and high-resolution CT are essential in ruling out a mass lesion and correlating the clinical and radiologic findings of this syndrome.
