Transcutaneous oxygen tension (TcPo2) of the lower extremities was investigated in 20 patients of peripheral arterial occlusive disease. TcPo2 was monitored from bilateral calf and dorsum of the foot. Femoral compression method meant that the tourniquet was set at femoral region and the tourniquet was inflated to a suprasystolic pressure for 4 minutes, after cuff was deflated, TcPo2 returned to the initial level. T(min) was the time it took until initial TcPo2 level. T1/2(min) was the time it took until 50 percent of the initial level. The following results were obtained. T1/2 showed significant relation to Ankle Pressure Index. T1/2 of dorsum of foot showed blood flow of distal side of calf region and T1/2 of calf showed blood flow of femoral region. T1/2 was significant difference between ASO and TAO, among different sites of peripheral vascular occlusion. T1/2 measurement was a much more useful indicator of limb flow after operation than TcPo2 measurement.

Preceding infrainguinal artery bypass surgery, a complete arteriographic delineation of the recipient artery is not always feasible. For 13 out of 63 patients with occlusion of the infrainguinal arteries, distinct delineation could not be accomplished with preoperative angiography. However, prebypass operative arteriography was successful in 8 of the 13. In 5, we performed bypass surgery on vessels not visualized on the preoperative angiography. In the other 3, bypass was performed on vessels in which the status of the distal circulation had not been well defined, although the popliteal and infrapopliteal arteries were partially visualized on the preoperative angiography. Intraoperative arteriograms--a technique requiring operative exploration of the target vessel--can significantly improve the visualization of the recipient artery and the distal circulation. Therefore, prebypass operative arteriography may become another indicator for arterial bypass surgery.
Out of sixty patients with thromboangitis obliterans from January 1966 to December 1981, 50 cases underwent such surgery as sympathetic ganglionectomy, thromboendarterectomy and arterial bypass. The remaining 10 cases received conservative therapies such as intravenous infusion of Prostaglandin E1 (60 micrograms in 5% glucose 500 ml) and epidural sympathetic ganglion block using 1% xylocaine. Good results were obtained in 29 operative cases and in 5 cases with non-surgical therapies. Among the operative cases, the subjective symptoms after discharge improved in forty-one patients (82%) unchanged in three (6%) and remained poor in six (12%). While, those of non-surgical group improved in eight and unchanged in two. In twelve of twenty-nine operative patients whose subjective symptoms improved immediately after discharge, their subjective distress turned worse again. However, the subjective symptoms of 5 patients in non-surgical group remained well in follow-up studies. Many patients who were diagnosed unchanged during follow-up period, had underwent the lumbar and/or thoracic sympathetic gangloinectomy. It is thus concluded that the effects of the sympathetic ganglionectomy are not permanent. The arterial reconstructive surgery is very effective for relief of peripheral ischemia in the follow-up studies if good function of the grafts was obtained in the early postoperative period.
Transcutaneous oxygen tension (tcPO2) of superficial layer of the foot at 44 degrees C was studied preoperatively in 33 patients (38 legs) with arteriosclerosis (ASO) or thromboangiitis obliterans (TAO). The cases were divided into two groups. The first group consisted of 3 legs, amputated. The second group consisted of 35 legs, not amputated but receiving such treatments as arterial bypass, lumbar sympathectomy or conservative therapy. The tcPO2 index was calculated by dividing the tcPO2 of the chest wall by the tcPO2 of the foot. The value of the tcPO2 of the chest decreased in proportion to the advance of age. There was no significant differences in the tcPO2 indices between ASO and TAO, nor among different sites of peripheral vascular occlusion. On the other hand, as the grade of Fontaine classification increased, the tcPO2 index reduced, with significant difference between grade II and IV. The tcPO2 index of the amputated group was 0.08 +/- 0.04. In contrast, the tcPO2 indices of the lumbar sympathectomy, arterial bypass and conservative therapy groups were 0.74 +/- 0.18, 0.84 +/- 0.12 and 0.74 +/- 0.13, respectively. There was significant difference between the amputated and non-amputated groups. From our results, it is suggested that oxygen tension measured transcutaneously is a useful indicator for the decision of operative indication and surgical procedure.

A 36-year-old woman with a history of left nephrectomy for renovascular hypertension secondary to arterial occlusive lesion of Takayasu's arteritis was re-admitted to our hospital with complaints of postprandial abdominal pain in the sixth post-operative month. On the 14th hospital day, the developing abdominal distension and generalized tenderness suggested a mesenteric vascular occlusion. Following abdominal aortography, emergency surgery was performed. The entire small bowel was edematous and markedly cyanotic with spotted, dark colored areas and the mesentery was pulseless. The patient was successfully treated by thromboendarterectomy at the origin of the celiac and superior mesenteric arteries and the necrotic loop of intestine was then resected 7 days later. Takayasu's arteritis was diagnosed by histological examination of the resected specimens. Although the occurrence of mesenteric infarction secondary to Takayasu's arteritis is rare, the possibility of mesenteric vascular occlusion should be given consideration in the follow-up of patients with Takayasu's arteritis.

Human umbilical cord veins treated with various concentrations of glutaraldehyde (GA) and ethanol (ET) were used to replace short segments of the infrarenal abdominal artery in 40 dogs. The patency rates were 62.5% (5/8) in Group I; tanned with 1% GA for 24 hours and preserved in the same solution 87.5% (7/8) in Group II; tanned with 1% GA for 24 hours and preserved in 50% ethanol, 100% (8/8) in Group III; tanned in the same manner as Group II, but preserved in 70% ET; 87.5% (7/8) in Group IV; tanned with 0.5% GA for 24 hours and preserved in 50% ET, and 87.5% (7/8) in Group V; tanned as in Group IV but preserved in 70% ET when examined 14 days to 6 months after implantation. Angiographically, most of the anastomotic lines gradually contracted with the lapse of time because of excessive proliferation of connective tissue into the intraluminal surfaces, although the structural integrity of the graft was well preserved. Microscopically, the irregular fibrin membrane covering the entire inner surface of the graft became smoother and thinner with the lapse of time. The pannus increased gradually both in thickness and width. Scanning electron-micrography of the graft revealed that no endothelial cells were found more than 5 mm from the suture lines even 6 months after implantation. Antigenicity of the graft using a complement-dependent cytotoxicity test was effectively suppressed with glutaraldehyde regardless of whether its concentration was 0.5% or 1.0%. Although human umbilical cord vein tanned with 0.5% glutaraldehyde and preserved in 50% ethanol until used was considered to be useful as an arterial substitute, final conclusion could not be drawn since six grafts followed for six months, the longest follow-up, were stenotic.
Twenty vascular grafts of expanded polytetrafluoroethylene with a 1.5 mm. internal diameter and a 4.0 cm. length, were placed in the femoral arteries of dogs. The animals, divided into two groups, with and without pluronic F 68, were sacrificed two weeks after surgery. The patency rate was 33.3% (2/6) and 21.5% (3/14), which was not statistically significant. One factor contributing to the occlusion of the grafts were excessive proliferation of granulation tissue due to early formation of the thrombus. Histological findings showed that the pseudointima was almost complete, although it was only about 0.114 mm. thick at the anastomotic line.

A 2-year-old boy with Type I glycogen storage disease received an end-to-side anastomosis of the mesenteric vein to the inferior vena cava with marked improvement in his biochemical factors and nutritional status and with shrinkage of the liver. The efficacy and simplicity of this technique and its lack of complications, as evidenced by follow-up studies over a 4 year period, suggest that its further use is warranted.
One hundred and forty one patients with mammary cancer underwent the extended radical mastectomy with parasternal lymph nodes dissection between January, 1966 and December, 1974. From the basis of the present report involvement of parasternaly lymph node chain was evaluated retrospectively with respect to the stage, location, size, histological type of cancer, metastasis to axillary and subclavicular lymph nodes, and the five-year survival rate. The parasternal as well as subclavicular and axillary lymph node involvements were not found in non-infiltrating cancer. The more the stage of cancer advanced, the more frequently the parasternal lymph nodes were involved regardless of the location of cancer in the breast. The parasternal lymph node chain alone was rarely involved, but frequently affected along with the axillary lymph nodes. When the parasternal lymph nodes were involved, the five-year survival rate was extremely poor, even after their surgical removal. Subsequently, addition of parasternal lymph node dissection does not seem to be beneficial.
The efficiency of femoral and carotid artery perfusion was compared in 10 dogs. The purpose of our study was to determine under what conditions the femoral artery can be used to perfuse the upper body satisfactorily during prolonged perfusions.
