[Follow-up study of precancerous lesions of the breast].

A follow-up study of 105 patients with microdochectomy for nipple discharge during the ten year period from 1965 to 1974 revealed that cancer developed in seven of the cases. The histology of the seven cases consisted of atypical hyperplasia in the terminal duct or lobular neoplasia. Accordingly, patients with either of the above histological findings were regarded as high risks for the development of breast cancer. Twenty such high risk patients were selected from 1850 patients biopsied for benign breast disease during the eleven year period from 1975 to 1985. This follow-up study revealed that five of the 13 patients (38%) with atypical hyperplasia in the terminal duct and two of the seven patients (28%) with lobular neoplasia developed breast cancer. Microphotocytometry using TV image analyzed system showed that the mean +/- standard deviation of the nuclear area and the absorbance were, respectively, 46.79 +/- 2.84 and 0.42 +/- 0.01 in fibroadenoma, 62.93 +/- 12.15 and 0.32 +/- 0.02 in precancerous lesion and 86.16 +/- 14.38 and 0.44 +/- 0.07 in breast cancer.

[Surgical experiences of Cor triatriatum in infancy].

Three operative cases with Cor triatriatum, age ranged from 30 days to 16 months, were presented. All types were IB1 of Lucas-Schmidt's classification and one patient was associated with right upper PAPVC. One patient died of low output syndrome due to preoperative shock. Communication of abnormal diaphragm in left atrium varied from small and multiple to 5 mm in diameter. Their preoperative diagnoses were established by two-dimensional echocardiogram prior to angiocardiogram and the intracardiac communications were well evaluated by color doppler echocardiogram which was superior to angiocardiogram in this evaluation. Postoperatively, no abnormal diaphragm were detected in two survivors. The diagnosis and operative procedure for this anomaly were discussed on.

[Evaluation of the effect of treatment in patients with liver tumors using 18F-fluorodeoxyglucose PET].

To evaluate the glucose metabolism in the liver tumor, 18FDG PET (Positron Emission Tomography) was performed in 9 patients with primary and metastatic liver tumors. And usefulness of 18FDG PET in clarifying the effect of treatment on liver tumor was examined. Glucose metabolism of tumor was showed in terms of the coefficient 'C' calculated by Patlak's plot. Each tumor's coefficient 'C' was 0.016-0.054 and it decreased after treatment. Compared with pathological findings, the coefficient 'C' of necrosis part of tumor was 0.004 and fibrous change after irradiation was 0.007. It suggested that coefficient 'C' reflected activity of tumor and it was a useful index to know viability of tumors in the treatment for patients with liver tumors.

[A case of pseudotumor cerebri associated with colon cancer].

A case of pseudotumor cerebri associated with iron deficiency anemia due to colon cancer is reported in a 37-year-old woman. Her initial symptoms were vomiting and severe headache. On physical examination, no lymph nodes and abdominal mass were palpable but marked anemia was noted in her skin and conjunctiva . Neurological examination revealed papilledema in her both eyes and stiff neck. There was no abnormal findings on CT scan on admission. Spinal puncture revealed CSF pressure as high as 620 mmH2O with normal cells, protein, sugar and chloride levels. Hematological examination revealed iron deficiency anemia and thrombocytosis. Angiography at third day revealed no sinus occlusion, but retention of contrast media was seen on the cortical vein of parietal lobe and right transverse sinus. Brain scintigram at sixth day revealed mild accumulation in left parietal lobe, so small venous infarction was suggested. There were two circumscribed stenotic lesions of right ascending colon in the barium enema, and right hemicolectomy was achieved. The pathological diagnosis was adenocarcinoma. The symptoms of pseudotumor cerebri was completely disappeared soon after the surgery together with resolution of anemia. She lives with no deficits now 1 year 3 months after surgery. In conclusion much attention is necessary to a patient of pseudotumor cerebri with iron deficiency anemia for the presence of cancer, because not only this central nervous system lesion is reversible and curable but also the cancer itself may be curable by surgery.

Post- and presynaptic effects of vesamicol (AH5183) on the frog neuromuscular junction.

The effects of vesamicol (AH5183), a blocker of acetylcholine transport, on the voltage-clamped neuromuscular junction of the frog were studied. Vesamicol (15-30 microM) reduced the peak height of the ionophoretically applied acetylcholine-induced current. The amplitude of the evoked endplate current was also decreased in the presence of vesamicol (30 microM). The endplate current was reduced further when the nerve was tetanically stimulated. The reduction of the endplate current after tetanic stimulation in the presence of vesamicol was due to a decrease in the mean quantal content. The decay time constants of the evoked endplate current and the miniature endplate current were also decreased by vesamicol. It was concluded that vesamicol acts as a postsynaptic blocker at the endplate. This neurotoxin could also decrease the immediately available quanta in the presynaptic nerve terminal, but the mechanism of action of vesamicol on transmitter release remains obscure.

Kinetic analyses of transmitter release in neuromuscular transmission.

A majority of transmitter quanta in the nerve terminal is in a large pool of store (S), which can be utilized for release only after transformation into activated quanta (n) via two intermediate states called available quanta (A) and releasable quanta (N). Mobilization is a collective term applicable for aggregates of S----A, A----N and N----n conversions. In the present article, various electrophysiological procedures for kinetic analyses of the transmitter release in neuromuscular transmission were discussed to elucidate this not well understood process of mobilization. Especially, frequency augmentation, tetanic rundown and depression, and recovery from depression experiments were proposed to be very useful tools in identifying the drug effect on the mobilization process. Since d-tubocurarine, 2-(4-phenylpiperidino)cyclohexanol (AH5183) and Ba ions satisfied the above three criteria of drug action on mobilization, these were concluded to affect the mobilization of transmitter in neuromuscular transmission.

Mode of action of estra-1,3,5(10)-triene-3,17 beta-diol 3-benzoate 17-[4-(4-bis(2-chloroethyl)amino)phenyl)-1-oxobutoxy)acet ate) on human breast carcinoma xenografts in nude mice.

To elucidate the mode of action of busramustine (KM2210), 17 beta- and alpha-busramustine, estradiol and chlorambucil were used for experimental chemo- and endocrino-therapy against hormone-dependent (T-61) and independent (MX-1) human breast carcinomas serially transplanted into BALB/cA female nude mice. Busramustine was administered po daily for 3 weeks at doses of 12.5-300 mg/kg for the beta-isomer and 25-300 mg/kg for the alpha-isomer. Five to 50 mg of estradiol per kg was administered im once, and 3 to 6 mg of chlorambucil per kg was administered po daily for 3 weeks. All of the compounds were effective against estrogen receptor-positive T-61 with a clear dose-response relationship, while estrogen receptor-negative MX-1 was sensitive to all of the agents except estradiol. Since the alpha-isomer of busramustine was effective against both tumor lines, the mode of action of 17 beta-busramustine may not be related to estrogenic action by estradiol released from the maternal compound. However, 17 beta-busramustine generated the estrogen receptor system of T-61 tumor and resulted in the endometrial hyperplasia of tumor-bearing nude mice, suggesting that this compound also has estrogenic action on transplanted human breast carcinoma and tumor-bearing host mice, besides non-estrogenic antitumor activity on human breast carcinoma xenografts.

[A fatal case of skin cancer 18 years after cadaveric donor renal transplantation].

A 39 years old man was admitted with chronic renal failure, and in June, 1968, he received renal transplantation from a cadaveric donor, 30 years old male, died from cerebral contusion. The graft had been well functioning for 18 years, and he had been engaged in his occupation. At the age of fifty-seven skin cancer (squamous cell carcinoma) occurred in his face, and surgical treatment was carried out. But it metastasized to left parotid, bilateral lungs and other areas of skin, and he died in October 1986, complicated by cerebral infarction. Some reports mention that the incidence of malignancies in posttransplant patients is greater than in general population. Our study reveals that immunological surveillance is weakened in posttransplant patients because of decrease of NK activity, but that the incidence of malignancies (male 0.78%, female 0.45%) is not so great as that reported. There is need of further investigation in this problem.

[A case of multiple myeloma (IgG, k) associated with hypertetraploid chromosomal abnormality].

A patient with multiple myeloma (IgG, k) associated with hypertetraploid chromosomal abnormality is reported. The patient was a 62-year-old female. She was admitted to our clinic in October 6, 1982 because of lumbago and a supraorbital subcutaneous tumor. Cellulose acetate membrane electrophoresis of her serum disclosed a M-protein in the gamma-globulin, and immunoelectrophoresis showed monoclonal IgG, k type. A marrow aspirate of her sternum contained 15% myeloma cells, some of which were large, while others were atypical. A chromosomal analysis of her bone marrow revealed 34 hypertetraploid cells of 35 metaphase cells. Chemotherapy was administered without success and she died of DIC and uremia on November 11, 1982. Histologically, pleomorphic myeloma cells that contained large cells with atypical nuclei proliferated in her bone marrow.

[Clinical evaluation of adriamycin in advanced and recurrent breast cancer (No. 4)--Joint study by 30 institutes on the duration of remission using various maintenance therapies in patients treated with CAF. Clinical Study Group of Adriamycin for Breast Cancer in Japan].

In order to further extend the duration of remission induced by high-dose CAF therapy, maintenance therapies based on long-term oral administration of cyclophosphamide and fluorouracil (CF) were applied for patients with advanced and recurrent breast cancer. The duration of remission was compared by the envelope method among the basic therapy (Arm I), Arm I plus intermittent administration of adriamycin (Arm II) and Arm I plus tamoxifen from the start of high-dose CAF therapy (Arm III). The response rate of 163 evaluable patients in 30 institutes was 42.1% (27/57) and 44.4% (25/54) for Arm I and Arm II (CAF therapy). The rate for Arm III (CAF therapy) was 61.5% (32/52), a little higher than that for the other two. As side effects due to the induction therapy, alopecia, fatigue, anorexia, nausea and vomiting, and leukopenia were observed in a relatively high proportion of patients. Mean values of CR duration, PR duration and the whole duration of remission in responding patients were 14.0 approximately 21.5 weeks, 24.0 approximately 32.2 weeks and 28.0 approximately 42.6 weeks, respectively, without any significant difference among the three groups given different maintenance therapies. The mean values of survival duration in responding patients were 27.1, 22.0 and 17.2 months, respectively for Arm I, Arm II and Arm III no significant difference being recognized in this point, either.

[Hormone independent murine carcinoma BX-1 spontaneously arising from nude mouse bearing Br-10, a hormone dependent human breast carcinoma strain].

BX-1, an adenocarcinoma spontaneously arising from nude mouse bearing Br-10, a human breast carcinoma strain was characterized. And the purification of a hormone independent murine carcinoma strain, BX-1 was found in August 1986 in three institutes where a hormone dependent transplantable human breast carcinoma cell line Br-10 has been serially passaged. The histological features of BX-1 were different from any other strains which were maintained in these three institutes. Estrogen receptor of BX-1 was negative and no estrogen dependency was observed while Br-10 was the receptor positive and the growth of Br-10 was dependent on estrogen. Although the graft of BX-1 into the thymus intact littermates was rejected, the chromosomal analysis revealed only murine chromosomes for BX-1, while both of human and murine chromosomes were detected in Br-10 tumor. By incubating Br-10 tumor in untreated female nude mice for 2 months and stimulating the growth of the tumor by exogenous estradiol, the purification of Br-10 from BX-1 could be achieved. Whereas the stability of human tumor xenografts in nude mice is confirmed, the spontaneously arising murine tumor from nude mice bearing human tumor xenografts should be considered for the experiments.

Mechanically induced electrical responses in murine mammary epithelial cells in primary culture.

In mouse mammary epithelial cells in primary culture, mechanical stimulation of a cell induced in other cells within the same colony a short depolarization of less than 15 mV with a duration of 1-8 s and a subsequent, prominent hyperpolarization of 6 mV lasting 10-40 s. Epidermal growth factor induces a spontaneous hyperpolarizing response in cultured mammary cells, and in cells treated with EGF mechanical stimulation produced a greater hyperpolarization, while the amplitude of the depolarizing response was not affected. The amplitude of the mechanically induced hyperpolarization was markedly reduced by quinine and tetraethylammonium, blockers of the Ca2+ -dependent K+ channel. The results suggest that the Ca2+ -dependent K+ channel was involved in the hyperpolarization.

Effects of synthetic omega-conotoxin, a new type Ca2+ antagonist, on frog and mouse neuromuscular transmission.

A new type Ca2+ antagonist, synthetic omega-conotoxin (omega-CgTX) decreased the peak height of the endplate potential (EPP) in frog muscle but had no effect on the mouse neuromuscular junction. The reduction of endplate potential in frogs was due to a decrease in transmitter release, since the mean quantal content estimated by variance of EPPs (m) and from the peak heights of EPPs and miniature EPPs (m1) was reduced by omega-CgTX, but the postsynaptic sensitivity to ACh was unaltered. The decrease of mean quantal content caused by omega-CgTX was reversed by 4-aminopyridine, guanidine and Bay K 8644. Also, the effect of omega-CgTX was weakened in the presence of 10 mM Ca2+ or 12 mM Mg2+. Statistical analysis revealed that omega-CgTX decreased the number of quanta available (n) whereas the probability of release (p) remained unaffected.

Augmentation of circulation of pedicled transverse rectus abdominis musculocutaneous flaps by microvascular surgery.

The biggest problem of the TRAM flap for breast reconstruction is distal necrosis or fat lysis due to poor circulation. In order to utilise the entire TRAM flap tissue in extensive tissue defects, the contralateral rectus muscle is used as a pedicled carrier and the ipsilateral superficial or inferior epigastric vessels are anastomosed with appropriate recipient vessels in the axilla. This procedure has been performed in three cases with no necrosis, fat lysis or hardening of the flap tissue, proving adequate circulation in the flap.
