Preparation of egg albumin microcapsules and microspheres.

Protein microspheres and microcapsules containing fine hydrophilic particles (spherical silica gel beads, iron hydrous oxidemodified nylon 12, or phenacetin) were prepared by using water-soluble egg albumin, which is heat-coagulable at 50-80 degrees. The size and size distribution of the microspheres and microcapsules formed and the microencapsulation percentage of fine particles were determined, and the factors affecting them were studied. Favorable conditions are suggested to obtain phenacetin-containing microcapsules in high yield.

Oral high-dose medroxyprogesterone acetate (MAP) in treatment of advanced breast cancer. A preliminary report of clinical and experimental studies.

Medroxyprogesterone acetate (MAP) in the treatment of advanced breast cancer has been regarded as a minor agent according to the previous reports when used at low doses (less than 500 mg/day). High doses of more than 500 mg which have come into use since 1973 give a response rate of over 40%, but sometimes cause gluteal abscess or induration because of daily intramuscular injections. In order to administer easily and to avoid the side effect, we have attempted to use oral administration in a daily dose of 1200 mg (400 mg X 3). Of those 20 patients treated with oral high-dose MAP, 1 showed complete response, 6 showed partial response, 7 no change, and 6 progressive disease. As for site of lesion, 4 out of 6 (67%) in skin and 4 out of 16 (25%) in bone responded. Neither severe side effects nor abnormal laboratory data were seen. Then, we examined the blood levels of MAP in vivo by RIA in 9 patients. The blood level of MAP reached 39-250 ng/ml in 3 days and was maintained at least over 50 ng/ml for 1 or 2 months of continuous administration. Subsequently, we examined the effects of MAP on binding to ER in vitro. The inhibition of binding of estradiol-17 beta to ER was about 60% at 10(-5) M MAP. The blood level of 50 ng/ml in vivo corresponds to about 1.3 X 10(-5) M.

Pharmacological effect of beta toxin of Clostridium perfringens type C on rats.

The biological effect of purified beta toxin of Clostridium perfringens type C in vivo was investigated. After intravenous injection of the purified beta toxin into rats, a rise in blood pressure and a simultaneous fall in heart rate were observed. After the blood pressure reached a maximum, the heart rate recovered gradually, and electrocardiographic and respiratory changes began. The rise in blood pressure induced by beta toxin tended to be proportional to the amount of toxin. The latent period between the injection of toxin and the onset of the increase, and also the time between the injection and the maximum pressure induced by the toxin decreased with increasing concentration of the toxin. A good correlation was found between the factor producing the rise in blood pressure and beta toxin. Alpha adrenergic and ganglionic blocking agents reduced blood pressure levels elevated by beta toxin. The data suggest that the toxin causes a release of catecholamines, and that the increase in blood pressure was induced by released catecholamines.

[Laboratory and clinical studies on cefsulodin (author's transl)].

Laboratory and clinical studies on cefsulodin (CFS), a new antipseudomonal cephalosporin antibiotic, were carried out and the following results were obtained. 1. The MIC of CFS against P. aeruginosa showed the peak of susceptibility at 3.13 mcg/ml in the inoculum size of 10(8) cells/ml and at 1.56 mcg/ml in the inoculum size of 10(8) cells/ml. It has a higher superiority as compared with gentamicin or piperacillin and gentamicin-resistant strains of P. aeruginosa were sensitive to CFS. 2. CFS was given to 3 patients with acute pneumonia and 5 patients with chronic cystitis which were all due to P. aeruginosa. Clinical effects were good in 2 patients and poor in a patient with acute pneumonia, and those were good in 3 patients and poor in 2 patients with chronic cystitis. Side effects were not recognized at all.

Plasma secretin level and exocrine pancreatic secretion in response to intraduodenal infusion of 1-phenyl-1-hydroxy-n-pentane and HCl by using a new four lumen tube.

We have designed a four lumen tube which has enabled us to aspirate duodenal juice while infusing endogenous stimuli into the lower part of duodenum. Exocrine pancreatic secretion and plasma immunoreactive secretin (IRS) levels have been simultaneously observed in response to intraduodenal infusion of 1-phenyl-1-hydroxy-n-pentane (PHP) and 0.1 N HCl in normal subjects, patients with diabetes mellitus and patients with chronic pancreatitis by use of this tube. (1) Pancreatic flow rate and bicarbonate concentration were found to increase to twice those of basal levels and plasma IRS levels rose significantly after infusion of PHP or HCl in normal subjects. The same tendency was observed in patients with diabetes mellitus. (2) Exocrine pancreatic secretion did not change after infusion of PHP or HCl in patients with chronic pancreatitis. Pancreatic flow rate, bicarbonate concentration and amylase output were significantly low in patients with diabetes mellitus. (3) Pancreatic flow rate and bicarbonate concentration incresed to about twice those of basal levels after both PHP and HCl infusion. On the other hand, although pancreatic amylase output increased significantly after infusion of HCl as compared with basal secretion, it did not change after infusion of PHP. It appeared reasonable to conclude that PHP is a morespecific substance for releasing secretin than HCl.
