111In monoclonal antimyosin antibody imaging: imaging of myocardial infarction and myocarditis.

A new scintigraphic method to detect myocardial necrosis has been developed using antimyosin monoclonal antibody Fab labeled with indium-111. Using this method, we studied 35 patients with myocardial infarction, 5 patients with myocarditis and 3 patients with angina pectoris. 111In antimyosin Fab was administered intravenously and antimyosin images were recorded by planar and single photon emission computed tomography (SPECT) 48 hours after injection. Planar images showed discrete localization of 111In antimyosin in 25 of 26 patients within 14 days after the onset of acute myocardial infarction. In 14 of these patients creatine kinase, glutamic oxaloacetic transaminase and lactic dehydrogenase had already normalized. Positive scans were also obtained in 6 of 12 patients between the third week to the ninth year after the onset of the disease. Three patients with acute myocarditis had positive scans 2 and 4 weeks after the onset of the disease. Thus, 111In antimyosin imaging may be a useful noninvasive method for the diagnosis of coronary diseases and myocarditis. Although the mechanism of persistent positive antimyosin images in the chronic stage remains to be clarified, 111In antimyosin scintigraphy holds potential promise as a non-invasive method for the detection of myocardial injury in the subacute to chronic stage as well as in the acute stage.

Comparison of the intestinal bacteria in specific pathogen free mice from different breeders.

Specific pathogen free BALB/c mice from 3 commercial laboratory animal breeders in Japan were compared on the composition of caecal flora revealed by selective and nonselective cultivation as well as direct microscopical observation on smears, and relative caecal weight. Large differences were detected in viable counts of total bacteria and almost all bacterial groups, while direct microscopical counts which consisted mainly of fusiform bacteria were almost equal, resulting in diverse recovery rates among 3 breeders. Eubacterium and spiral shaped organisms were not detected from all breeders. There also existed variations within breeders, especially those in the number of Enterobacteriaceae. Relative caecal weight also differed among breeders, suggesting the participation of variety of caecal bacteria in determining this parameter. As these variations in bacteriological status of animals can influence experimental results, standardization of intestinal microbial flora is required.

Construction of an immunoradiometric assay for ovarian cancer associated antigen CA125 recognizing different antigenic determinant.

We generated five murine monoclonal antibodies reactive with ovarian cancer-associated antigen CA125. These monoclonal antibodies seemed to bind to separate epitopes from OC125 antibody, known to recognize CA125. A series of immunoradiometric assays for measuring serum CA125 values rapidly and sensitively were devised using these monoclonal antibodies. The antigenic determinant of a new immunoradiometric assay was different from that of a currently used CA125 kit employing OC125 both as a catcher and a tracer. However, serum antigen levels were closely correlated to each other and were elevated not only in patients with ovarian cancer, but also in patients with endometriosis and in some normal females during menstruation. These results suggest that CA125 has at least two antigenic determinants close to each other and this new rapid assay is useful, although not specific for ovarian cancer, in patients with gynecological disorders.

[Cine MRI of the ascending aorta in the elderly with respect to the flow signal void and aortic valve morphology].

Cine flow MRI was performed on a 1.5 Tesla system to observe signal intensity of blood flow within the ascending aorta in the elderly who had no aortic stenosis and to determine frequency of the flow signal void. Coronal and sagittal imaging planes of the ascending aorta were obtained in 27 aged patients with no known cardiac diseases (14 men and 13 women, mean age of 76) and 7 young volunteers (7 men, mean age of 24), utilizing ECG-gating. GRASS (gradient-recalled acquisition in steady state), and a flow compensation sequence. The young volunteers presented little or no signal void within the ascending aorta. In 26 (96%) of the 27 aged patients, on the other hand, signal void was demonstrated in the blood flow distal to the aortic valve during systole. The maximum length of the signal void that was measured at 318-632 msec after the R wave of ECG ranged from 33 to 97 mm. Conventional and Doppler echocardiography was used to evaluate motion and morphology of the aortic valve in 19 of the 27 aged patients. Eighteen of these 19 subjects had aortic signal void on cine MRI. Echocardiography showed sclerotic changes of the aortic valve (i.e., increased echogenicity of the cusps and/or commissure fusion) in 10 (53%) of the 19 subjects. The mean maximum length of the signal void in the 10 patients with aortic valve sclerosis was significantly greater than that in the 9 patients with echocardiographically normal valve (68 vs. 45 mm, p less than 0.01).(ABSTRACT TRUNCATED AT 250 WORDS)

[The removal efficiency of beta 2-microglobulin, alpha 1-microglobulin and alpha 1-acid glycoprotein by using dialyzers in the bloodstream and transference of endotoxin through membranes].

Retention of beta 2-microglobulin (beta 2-MG) has been indicated as one of the causes of hemodialysis-associated amyloidosis. Membranes with higher permeability (high-performance membrane) have recently been developed accordingly. Several dialyzers were tested by us in an attempt to study their removal efficiency of beta 2-MG and higher molecular substances of alpha 1-microglobulin (alpha 1-MG) and alpha 1-acid glycoprotein (alpha 1-AG), as well as transferring of endotoxin (ET) through membranes into bloodstream. The membranes subjected to our study were PMMA, CTA, PS, EVAL-C and CA. Removal efficiency of beta 2-MG through EVAL-C and CA was lower as compared with that through other membranes (P less than 0.05), while the EVAL-C showed relatively higher removal efficiency for alpha 1-MG and alpha 1-AG. Permeability for ET was not observed with all membranes studied herein.

[Toxic activities of the Ga-Sn alloy (Adlloy-OH) on nutritional condition and dental caries development in rats].

The toxic activities of Ga-Sn alloy (Adlloy-OH) in experimental rats, its relations to the nutritional condition and dental caries development, were studied for three months. Adlloy, 0.3 g or 3 g per body weight (kg), was fed orally with the basal diet consisting of casein, sucrose, bean oil, mineral, and vitamin mixtures. Biochemical assays of serum was carried out for total protein, albumin, calcium, inorganic phosphate, glucose, urea, creatine, alkaline phosphatase, GOT, and GPT. There was no convincing evidence of toxic effects on growth and biochemical data by the oral feeding of Addloy-OH.

Indium-111-antimyosin antibody imaging for detecting different stages of myocardial infarction: comparison with technetium-99m-pyrophosphate imaging.

The diagnostic value of 111In-antimyosin (AM) imaging for identifying myocardial infarction was evaluated in comparison with 99mTc-pyrophosphate (PPi) imaging. Twenty-four patients with various stages of myocardial infarction, ranging from three days to nine months after the onset of infarction, underwent both AM and PPi scans. Of 26 infarct lesions AM scan identified 22 (85%), while PPi scans detected 10 (38%) (p less than 0.01). When less than a week had passed since the onset both scans demonstrated all infarct lesions. For seven subacute lesions studied within one to two weeks of onset, AM scans detected (100%), while PPi scans identified only 2 (29%). Furthermore, AM scans showed discrete myocardial uptake in 7 (64%) of those studied more than two weeks after onset. The intensity of AM uptake in the infarcts studied more than seven days after onset was less than that in acute infarcts studied within seven days of onset (p less than 0.05). These preliminary data indicate that the abnormal myocardial uptake of AM persists beyond the first two weeks when PPi no longer accumulates. Thus, AM scans can be considered to provide a sensitive diagnosis of subacute as well as acute myocardial necrosis.

Scintigraphic evaluation of aggressive fibromatosis.

Despite its benign microscopic appearance, aggressive fibromatosis has potential to recur and infiltrate neighboring tissues. Therefore, it is necessary to determine the exact extent before therapy. In the present study, 11 cases of aggressive fibromatosis were examined scintigraphically using [99mTc(V)]dimercaptosuccinic acid (11 cases) and 67Ga-citrate (7 cases). Technetium-99m-(V)-dimercaptosuccinic acid demonstrated all lesions, while 67Ga-citrate detected 57% of the cases.

Comparative studies of DU-PAN-2, carcinoembryonic antigen, and CA19-9 in the serum and bile of patients with pancreatic and biliary tract diseases: evaluation of the influence of obstructive jaundice.

The levels of DU-PAN-2 antigen, carcinoembryonic antigen, and CA19-9 in serum and bile of patients with pancreatic and biliary tract diseases were measured. The sensitivities (true positive) of DU-PAN-2 in serum to pancreatic carcinoma (64%) and to biliary tract carcinoma (62%) were similar to those of CA19-9 in serum (69% and 72%, respectively). Nine of 18 (50%) patients with CA19-9-negative pancreatic carcinoma tested positive for DU-PAN-2. The sensitivities of CEA to pancreatic carcinoma (56%) and to biliary tract carcinoma (52%) were lowest. The measurement of these antigens in bile seemed to be of little diagnostic value in differentiating between malignant and benign diseases. False positives of these three assays occurred frequently in patients with benign pancreatic or biliary tract disease coupled with obstructive jaundice. After percutaneous transhepatic biliary drainage, serum DU-PAN-2 and CA19-9 levels returned to normal ranges in patients with benign diseases, but not in patients with carcinoma of the pancreas or of the biliary tract. Serum CA19-9 and DU-PAN-2 antigens are useful tumor markers for pancreatic and biliary tract carcinomas. Longitudinal assays of these antigens may be useful for the differential diagnosis of patients with obstructive jaundice.

[Computerized analysis of EEG background activity in mono-therapeutic patients with epilepsy--relationships between antiepileptic drugs and types of seizures].

EEG background activity influenced by antiepileptic drugs (AED) was studied in 109 monotherapy and drug-free epileptic patients using t-Statistical Significance Probability Mappings (t-SPMs). Patients taking phenobarbital (PB) had an increase in alpha 1 and a decrease in alpha 2 activity in comparison with drug-free epileptics. Patients taking PB for generalized seizures with tonic-clonic convulsion only (GTC) also had a significant increase in alpha 1 and a decrease in alpha 2, whereas those with partial seizures (PS) had an increase in theta and beta 1 and a decrease in alpha 2 activity. Patients taking valproic acid (VPA) had a decrease in only beta 1 activity. Patients taking VPA for GTC showed an increase in delta activity, but those with PS did not show any changes. Patients taking carbamazepine (CBZ) for PS exhibited marked slowing with an increase in theta and alpha 1 and a decrease in alpha 2 activity. These results mean that changes in EEG due to AEDs differ depending on the type of seizures. More interestingly, discrepancy between EEG background activity and effects of AEDs was found: In PS type of seizures, the most effective CBZ exhibited striking slowing, PB was next, and VPA was last. In GTC, VPA resulted in greater slowing than PB.

[Indium-111 antimyosin monoclonal antibody uptake in patients with cardiomyopathy and myocarditis].

Prognostic significance of myocardial uptake of indium-111 antimyosin antibody was evaluated in 17 patients with idiopathic cardiomyopathy; 10 patients with dilated cardiomyopathy and 7 patients with hypertrophic cardiomyopathy. Seven of 10 patients with dilated cardiomyopathy showed positive images. Three of these 7 patients with strongly positive scans died after scintigraphic examination. Six of 7 patients with hypertrophic cardiomyopathy showed positive images. Three of the patients with dilated left ventricle had prominent positive scans and higher heart to lung ratio. The heart to lung ratio of antimyosin uptake in total patients was correlated with left ventricular end-diastolic dimension and ejection fraction measured by echocardiography. In patients with myocarditis, all three patients showed positive scintigrams within 4 weeks after the onset of the disease and 1 of 6 patients was positive thereafter, who had dilated ventricle and decreased cardiac function. Thus, indium-111 antimyosin antibody imaging may be useful to evaluate prognosis of patients with cardiomyopathy and myocarditis.

[111In-antimyosin Fab scintigraphy in cardiovascular diseases: (multicenter clinical trial)].

In a multicenter study, a total of 380 patients with myocardial infarction, myocarditis and cardiomyopathy underwent 111In-Antimyosin Fab myocardial imaging. 111In-Antimyosin Fab was administered intravenously and myocardial images were obtained 48 hours later. Only 3 patients developed mild adverse effects. Human antimouse antibodies were detected in 7 patients. Positive scans in patients with myocardial infarction were seen in 92/119 (77%) within 2 weeks after the onset of myocardial infarction, in 58/71 (82%) at 3-4 weeks, in 20/22 (91%) at 4-8 weeks and 17/31 (55%) thereafter. The location of myocardial damage delineated by 111In-Antimyosin Fab imaging was concordant with the infarct location by ECG and coronary angiography. In patients with myocarditis, 111In-Antimyosin Fab uptake was positive in 7/12 (58%) within 8 weeks and 6/17 (35%) thereafter. Positive 111In-Antimyosin Fab scans were seen in 12/36 (33%) in dilated cardiomyopathy and in 17/19 (89%) in hypertrophic cardiomyopathy. Although the mechanism of persistently positive 111In-Antimyosin Fab images in the subacute to chronic stage of myocardial infarction and myocarditis remains to be clarified, 111In-Antimyosin Fab may be useful for the detection of the diseases and in evaluating the prognosis of patients with cardiomyopathy.

[Bronchopleural fistula after pulmonary resection of lung cancer].

From 1977 to 1988, 390 cases of lung cancer were resected in our hospital. In 12 patients (3.1%) a postoperative bronchopleural fistula developed. This complication occurred more commonly after right pulmonary resection than left and most common after right pneumonectomy. Moreover, that had occurred following right pneumonectomy was most fatal. After right middle and lower lobectomy, this complication occurred at the same rate as after pneumonectomy and also caused a fatal result. On the other hand, after the resection of single lobe, the rate of the occurrence of a bronchopleural fistula was lower. And, even if it had developed, the fistula sometimes could be healed by a conservative treatment only. In one of our cases the fistula was successfully treated by intra-bronchial gluing through a bronchofiberscope.

In vitro and in vivo properties of human/mouse chimeric monoclonal antibody specific for common acute lymphocytic leukemia antigen.

A human/mouse chimeric monoclonal antibody specific for a common acute lymphocytic leukemia antigen was efficiently obtained by ligating human heavy-chain enhancer element to the chimeric heavy- and light-chain genes. Cell binding and competitive inhibition assays of both radioiodine and indium-111- (111In) labeled chimeric antibodies demonstrated in vitro immunoreactivity identical with that of the parental murine monoclonal antibodies. The biodistribution of the radiolabeled chimeric antibody in tumor-bearing nude mice was similar to that of the parental murine antibody. Tumor accumulation of radioiodinated parental and chimeric antibodies was lower than that of 111In-labeled antibodies, probably because of dehalogenation of the radioiodinated antibodies. Indium-111-labeled chimeric antibody clearly visualized xenografted tumor. These results suggest that a human/mouse chimeric antibody can be labeled with 111In and radioiodine without the loss of its immunoreactivity, and that chimeric antibody localizes in vivo in the same way as the parental murine antibody.

[Clinical trial of 111In-antimyosin antibody imaging: (4). Effect of reperfusion in acute myocardial infarction].

Effects of reperfusion by intra-coronary thrombolysis (ICT) or percutaneous transluminal coronary angioplasty (PTCA) on the myocardial imaging using 111In-labeled antimyosin monoclonal antibody Fab (In-AM) were studied. Reperfusion by ICT or PTCA was done in 16 patients (reperfusion group) and recanalization was seen in 14. Positive images were obtained in 28 of 30 patients (93%) with acute myocardial infarction (onset to imaging: 9.9 +/- 9.8 days). Among the reperfusion group, one patient in whom PTCA was done early after the onset of chest pain and CPK did not elevate showed no significant uptake of radioactivity at cardiac region, while other 15 patients with elevation of CPK demonstrated positive images. Planar images (anterior, left anterior oblique 45 degrees, left lateral) were divided into 15 segments and infarct size (antimyosin-segment) was determined by sum of positive segments on each image. There were no significance between the infarct size in the reperfusion group (7.2 +/- 2.9) and that in the non-reperfusion group (6.9 +/- 3.4). Intensity of the accumulation of radioactivity in each image was classified into 5 grades by comparison with uptake of the liver (antimyosin-score). Reperfusion group demonstrated high intensity compared with non-reperfusion group (2.6 +/- 0.7 vs. 2.0 +/- 0.4; p less than 0.05). Thus, In-AM imaging may be influenced by coronary blood flow, which should be taken into consideration in the interpretation of In-AM imaging.
