[A case of inclusion body myositis (IBM) associated with lipid-storage vacuoles].

A 62-year-old woman, who has had slowly progressive muscle weakness and wasting of the four limbs for past 10 years, was evaluated by muscle biopsies; i.e. the deltoid, a moderately atrophic muscle, and the quadriceps femoris, a muscle that was only mildly involved. The specimen from the deltoid disclosed the basic muscle structures having been collapsed, containing many fibers undergoing degeneration. There were rimmed vacuoles and infiltration of macrophages and round cells into the muscle fibers and perimysial space. Inclusion bodies containing fibrillary structures were identified in sarcolemmal nuclei by electron microscopy. When the quadriceps femoris muscle was examined, however, a large number of vacuoles were interspersed between myofibrils that contained numerous lipid droplets. There were no pathological changes suggestive of IBM in this specimen. The case described here is unique in that the muscle biopsies from two separate muscle revealed a distinctly different pathology. Although we were unable to determine the cause of lipid-storage in this case, it is our impression that inclusion body myositis is not a uniform disorder but the characteristic pathology to this particular myositis might be an ultimate feature of several diverse degenerative and/or inflammatory muscle disorders.

[Domestic infection of neurocysticercosis in a Japanese woman, who had not travelled overseas].

A 52-year-old Japanese woman was found to have a single parenchymal cystic lesion in the operculum of the left frontal lobe. Although she had not travelled overseas, neuroradiological and serological studies supported the diagnosis of neurocysticercosis. Three months after a trial with praziquantel, although the cystic lesion decreased in size on MR imagings, she suddenly developed motor dominant aphasia as a manifestation of epileptic attack. This appears to have followed the death of the cysticercus, releasing the cysticercus contents into the surrounding brain and to have stimulated an intense reactive inflammation. Neurocysticercosis, although rare in Japan, may be positively identified by neuroimaging and serological studies and can be treated conservatively with praziquantel.

[A construction of a PACS and reporting system linked with the hospital information system in Gunma University Hospital].

A PACS (Picture Archiving and Communication System) and reporting system were introduced into Gunma University Hospital. These systems were linked with Hospital Information System (HIS), enabling us to refer to images, such as those of CR, CT, MRI and scintigraphy, at wards and conference rooms within a reasonable times. Bone scintigraphy and a report on it are illustrated as an example. A standardized protocol for the interface between PACS and the imaging format must be established without delay for the progress of PACS.

[Measurement of serum TSH receptor antibodies in thyroid patients using TRAb Dade kits].

TRAb Dade kit is a radioreceptor assay that detects autoantibodies in serum of patients with thyroid diseases which interfere with the binding of 125I labeled TSH to its receptor. In this paper we examined the basic and clinical usefulness of newly developed TRAb assay kits. The intra and inter assay coefficients of variation were 7.9% and 8.4%, respectively. There was no significant effect of temperature and incubation time on TRAb values. The influence of conjugated, unconjugated bilirubin, hemoglobin and turbidity was not so significant on TRAb values. TRAb values in serum of 68 untreated Graves' disease were above the normal range, but most (78%) treated Graves' patients who became euthyroid after receiving antithyroid drug, showed the normal range. In patients with chronic renal failure TRAb values were also within the normal range. A significant correlation (y = 1.0x-0.88, r = 0.95) was observed between this TRAb kit and TRAb Cosmic values in various thyroid conditions. This new assay for TRAb was simple, rapid and reproducible. The measurement of serum TRAb values was useful for the evaluation of physiological and pathological function and seemed to be helpful in the management of patients with thyroid disease.

Semiquantitative SPECT tumor uptake of technetium-99m-labeled anti-CEA monoclonal antibody in colorectal tumor.

UNLABELLED: Technetium-99m-BW431/26 images were compared to images of resected tumor specimens to evaluate the efficacy and accuracy of SPECT imaging of colorectal carcinoma. METHODS: Immunoscintigraphy with 99mTc-BW431/26 was performed on seven patients with colorectal carcinoma and one patient with a benign colorectal tumor. Accumulation of 99mTc-BW431/26 in the tumor was expressed as the tumor-to-normal tissue count ratio (SPECT T/N ratio) calculated by a region of interest method on the SPECT images obtained 24 hr after administration of 99mTc-BW431/26. All patients underwent research of the tumor immediately after 24-hr imaging, and the radioactivity in tumor specimen and normal tissue was measured to calculate the tissue T/N ratio. RESULTS: SPECT demonstrated definite increased tracer uptake by the tumor in all colorectal cancer patients. The benign lesion showed tracer uptake to a lesser extent. SPECT, however, failed to visualize a 10-mm lesion in a patient with familial adenomatous polyposis despite a tissue T/N ratio of 4.8, while autoradiography showed radioactivity uptake in the polyps. CONCLUSION: Although SPECT has limitations in detecting small lesions because of its limited spatial resolution, T/N ratios could be measured exactly by SPECT if the lesion is of a certain volume. SPECT imaging with 99mTc-BW431/26 can precisely evaluate tracer uptake in tumors and predict the efficacy of radioimmunotherapy in patients with colorectal cancer.

[Solitary plasmacytoma of the rib--a case report and review of Japanese literatures].

A 72-year-old woman with solitary plasmacytoma of the right fifth rib was surgically treated. She underwent radical resection of the bony chest wall including the right fifth rib, the ribs above and below the involved rib, the intercostal muscle, and the parietal pleura. Histological finding of the tumor was plasmacytoma of the rib. The type of monoclonal protein was IgG and lamda. She is doing well one year and eleven months after surgery without any signs of recurrence. Solitary plasmacytoma is rare as compared with multiple myeloma. Patients with solitary plasmacytoma originating in the rib have a feasibility of operative indication, and radical treatment is expected to be by adequate surgical resection. Seventeen cases of solitary chest wall plasmacytoma described in the Japanese literature are reviewed.

[Usefulness of 123I-MIBG and 123I-BMIPP myocardial scintigraphy for detecting coronary artery disease and for evaluating left ventricular function].

We evaluated the diagnostic value of 123I-metaiodobenzylguanidine (MIBG) and 123I-labeled beta-methyliodophenyl pentadecanoic acid (BMIPP) myocardial SPECTs for evaluating coronary artery disease and left ventricular function, in comparison with the diagnostic value of 201Tl (Tl) SPECT. For forty-nine patients with coronary artery disease, resting MIBG and BMIPP SPECTs were performed to detect coronary artery stenosis, compared with the diagnostic value of exercise Tl. Left ventricular ejection fraction and regional wall motion were compared with the total US (TUS) and regional US (RUS) of resting MIBG and BMIPP SPECTs, and in turn, compared with resting Tl SPECT. The sensitivity of resting BMIPP SPECT for detecting coronary artery stenosis was lower, and the specificity of resting MIBG SPECT was lower than the other two methods. The accuracy of resting MIBG SPECT for evaluating coronary lesions was nearly the same as the accuracy of exercise Tl, but higher than that of BMIPP SPECT. Left ventricular ejection fraction was well correlated with TUS of resting MIBG SPECT (r = 0.80), resting BMIPP SPECT (r = 0.77), and resting Tl SPECT (r = 0.68). Regional wall motion was most correlated with RUS of resting BMIPP SPECT, compared with that of resting Tl and MIBG SPECTs. These data suggest that resting MIBG SPECT is useful for detecting coronary artery disease and that resting BMIPP SPECT is valuable in evaluating regional left ventricular function.

Detecting recurrent or residual lung cancer with FDG-PET.

UNLABELLED: We investigated the diagnostic accuracy of FDG-PET in the detection of recurrent lung cancer. METHODS: Thirty-nine lesions in 38 patients with clinically suspected recurrent or residual lung cancer were studied with PET. All PET images were visually interpreted in conjunction with thoracic CT or MRI. Semiquantitative analysis using standardized uptake values (SUVs) was also performed in 25 lesions. FDG-PET diagnoses were correlated with pathological diagnoses and clinical outcome. RESULTS: The sensitivity and specificity of FDG-PET for detecting recurrent tumors were 100% (26/26) and 61.5% (8/13), respectively. The difference in mean SUV between recurrent tumors and noncancerous lesions was statistically significant [11.2 +/- 5.7 (n = 16) vs. 3.5 +/- 1.8 (n = 9), p < 0.0001]. False-positive results showed relatively lower SUVs than true-positives and also demonstrated increased uptake in a curvilinear rather than nodular shape. CONCLUSION: FDG-PET is useful for detecting recurrent lung cancer after treatment. False-positive diagnoses might be reduced by analysis of uptake shape and serial changes in SUV, but further study is needed.

Primary lung cancer SPECT imaging with pentavalent technetium-99m-DMSA.

UNLABELLED: To assess the clinical role of 99mTc(V)-DMSA in primary lung cancers, SPECT imaging was performed on 31 patients with suspected lung cancer. METHODS: Planar and SPECT images were obtained at 3 to 4 hr after intravenous injection of approximately 555 MBq 99mTc(V)-DMSA. Two uptake ratios (the maximum counts/pixel in the lesion to the average counts in normal tissue) were calculated. RESULTS: Various types of primary lung cancers (adenocarcinoma, squamous-cell carcinoma, small-cell carcinoma, large-cell carcinoma and bronchial carcinoid tumor) were imaged by 99mTc(V)-DMSA SPECT. Approximately 90% of the lung carcinomas showed increased uptake and were clearly demonstrated by SPECT images. Four cases incidentally revealed osseous metastatic lesion. Three benign lesions did not show increased uptake. Three cases were false-negative and there were no false-positive cases for the primary lesions. CONCLUSION: Technetium-99m(V)-DMSA SPECT images demonstrated approximately 90% of the primary lung cancers. Uptake ratios were higher in squamous-cell carcinomas than adenocarcinomas. Evaluation of mediastinal tumor extension and nodal metastatic lesion was very difficult by high blood-pool activity in the major cardiovascular structures due to slow blood-pool clearance. However, 99mTc(V)-DMSA SPECT imaging was very useful for detecting primary lung cancers and metastatic lesions to the osseous structures.

Presurgical diagnosis of a primary carcinoid tumor of the thymus with MIBG.

A primary carcinoid tumor of the thymus showing ectopic ACTH syndrome was evaluated scintigraphically with four radiopharmaceuticals and a fluorescence method. Iodine-123-MIBG and 201Tl-Cl scintigraphy clearly demonstrated the tumor. Gallium-67-citrate and 99mTc(V)-DMSA showed no tumor uptake. The fluorescence method confirmed numerous storage granules of norepinephrine. Iodine-123-MIBG scintigraphy could be useful in the presurgical diagnosis of carcinoid tumors of the thymus.

[Medical image transmission via communication satellite: evaluation of bone scintigraphy].

As compared with terrestrial circuits, the communication satellite possesses superior characteristics such as wide area coverage, broadcasting, high capacity, and robustness to disasters. Utilizing the narrow band channel (64 kbps) of the geostationary satellite JCSAT1 located at the altitude of 36,000 km above the equator, the authors investigated satellite-relayed medical images by video signals, with bone scintigraphy as a model. Each bone scintigraphy was taken by a handy-video camera, digitalized and transmitted from faculty of technology located at 25 kilometers apart from our department. Clear bone scintigraphy was obtained via satellite, as seen on the view box. Eight nuclear physicians evaluated 20 cases of bone scintigraphy. ROC (Receiver Operating Characteristic) analysis was performed between the scintigraphies on view box and via satellite by the rating method. The area under the ROC curve was 91.6 +/- 2.6% via satellite, and 93.2 +/- 2.4% on the view box and there was no significant difference between them. These results suggest that the satellite communication is very useful and effective system to send nuclear imagings to distant institutes.

[Current status of nuclear oncology in Japan].

The most commonly used radionuclides for cancer patients in Japan have been still 67Ga and 201T1 chloride. In addition to the diagnosis of lung cancer and thyroid tumor, 201T1 is recently applied to patients with brain tumor, bone and soft tissue tumor and parathynoid adenoma. Comparing to Nuclear Cardiology and Brain Nuclear Medicine, where many new radiopharmaceuticals have been developed, there are few new drugs in Nuclear Oncology. In other words, new radiopharmaceuticals are expected to be developed for the diagnosis and/or therapy of cancer. In addition to 131I for thyroid cancer, new radiopharmaceuticals such as 111In-octreotide and 99mTc(V)-DMSA have been clinically employed. In spite of strong expectation, radiolabeled monoclonal antibodies have not been clinically used in Japan. However, the technique of humanized antibodies has been established and in U.S.A., 131I-labeled antibodies are reported to be effective for the treatment of malignant lymphoma. 89Sr is useful for the relief of bone pain caused by the bone metastasis. New findings that SPECT of 18F-FDG, a positron emitter, has been revealed to have a great potential in the management of cancer patients, will give a great impact on Nuclear Oncology.

[Unilateral effective renal plasma flow measurement using one-compartment analysis of 99mTc-MAG3 and gamma-camera renography].

The aim of this study was to establish a simple and accurate procedure to calculate the unilateral effective renal plasma flow (ERPF) using 99mTc-MAG3 gamma-camera renographyl. Eleven patients with urological disorders were studied with 99mTc-MAG3 to calculate the renal uptake ratio (RUR), which was defined as the ratio of absorption-corrected renal counts within 1-2 min after intravenous injection to injection radioactivity measured with a gamma-camera. We assumed that 99mTc-MAG3 was distributed in the circulation, moved from the circulation to the kidneys, and was excreted solely from the kidneys. We thus adopted an one-compartment model to calculate 99mTc-MAG3 clearance (CLMAG) using RUR. Sequential p-aminohippurate (PAH) clearance study was performed as a gold standard in all the patients to compare CLMAG and PAH clearance. Obtained CLMAG correlated well with PAH clearance, and this one-compartment model seemed appropriate for describing CLMAG. Unilateral ERPF was accurately measured within two minutes after the intravenous injection of 99mTc-MAG3 in conjunction with the renal imaging study. This procedure is simple and reliable, and requires no blood or urine sampling.

[Phase 3 multicenter clinical study of 111In-DTPA-D-octreotide (MP-1727) in patients with gastrointestinal hormone producing tumors].

A phase 3 multicenter clinical study was performed to investigate the efficacy, safety and usefulness of MP-1727, a novel tumor imaging agent which binds to somatostatin receptors, in 23 patients with gastrointestinal hormone producing tumor. The efficacy was graded effective or very effective in 16 cases (76.2%) out of 21 cases who could be evaluated. Positive predictive accuracy was 77.8% (7/9) for carcinoids, 100% (5/5) for gastrinomas and 40.0% (2/5) for insulinomas. In three cases, tumor lesions which had been unknown were detected by MP-1727 scintigraphy. The consistent ratio between scintigraphically positive sites and preinjection localizations was 82.0% (41/50 sites) in total. Although adverse drug reactions were observed in two cases, the safety was graded 'not problematic at all' in all of 23 cases. These results show that MP-1727 is a very useful radiopharmaceutical for detection and localization of gastrointestinal hormone producing tumors.

POEMS syndrome with central nervous system involvement: a case report.

POEMS (polyneuropathy, organomegaly, endocrinopathy, monoclonal gammopathy and skin changes) syndrome was found to be associated with a disorder of the central nervous system (CNS) in a 56-year-old woman. Elevated intracranial pressure, neuroimaging evidence of diffuse white matter edema and papilledema point to a disease process affecting the CNS. Brainstem auditory (BAEPs) and visual evoked potentials (VEPs) also provided evidence of reduced central conduction velocities without, however, apparent clinical deficits attributable to CNS involvement. The intracranial hypertension is thought to be a part of a generalized tendency toward edema characteristic of this particular syndrome. We speculate that POEMS syndrome is an autoantibody-mediated immunological disorder in which cerebral vascular endothelium is one of the targets of the immunopathogenetic process and undergoes subsequent enhancement of vascular permeability.

[Response-oriented salvage chemotherapy with mitoxantrone, etoposide and enocitabine for previously treated acute myeloid leukemia. Tohoku Leukemia Study Group].

Twenty-two patients with previously treated acute myeloid leukemia (AML) received salvage chemotherapy with mitoxantrone, 5 mg/m2/d on days 1 to 3, etoposide, 70 mg/m2/d on days 1 to 5, and enocitabine, 170 mg/m2/d on days 1 to 7 (BHAC-ME). Additional mitoxantrone, etoposide (both for up to 2 days) and enocitabine (for up to 7 days) were given if the bone marrow obtained on day 8 was not severely hypoplastic. Mitoxantrone and etoposide had been previously administered in 14 and 15 patients, respectively. Seven patients had primary resistance; 14 patients had first relapse (4 early and 10 late): and 2 patients had second relapse. Overall, seven patients (31%) achieved a complete remission; 7/14 with first relapse, and 0/8 with primary resistance or second relapse. Four patients, 3 with primary resistance and 1 with first relapse, died of infectious complication in aplasia. First relapse patients who had been previously treated both with mitoxantrone and etoposide, had a lower CR rate than the other first relapse patients [2/8 (25%) vs 5/6 (86%)], although patient characteristics such as duration of first CR, initial karyotype, and performance status, were similar between the two groups. We conclude that response-oriented BHAC-ME regimen is still active in first relapse AML patients unless they have received both mitoxantrone and etoposide previously.

[Biological features determining the chemosensitivity of gastric cancer].

We analysed the relationship between several biological properties of gastric cancers and their chemosensitivity determined by MTT assay. Higher chemosensitivity was associated with poor differentiation, aneuploidy, and higher proliferative activity. Lymph node metastasis was more chemosensitive than primary lesion, while liver metastasis was less. Gastric cancer expressing multidrug-resistance associated protein (MRP) showed lower sensitivity to several anticancer drugs, including adriamycin and etoposide. p53 status and susceptibility to apoptosis were also associated with chemosensitivity. Thus, chemosensitivity of clinical gastric cancer might be increased if these characters can be modified by some new biologic therapy.
