Hypermyoglobinemia after successful arterial embolectomy.

Myoglobin concentrations in serum and urine were measured in eight patients who underwent successful arterial embolectomy in the femoral or iliac arteries. Median serum myoglobin levels significantly increased after revascularization to a maximum of 4741 micrograms/L (reference range: 0 to 80 micrograms/L) 2 hours postoperatively, with a concomitant and correlated increase in the urine myoglobin concentration. Three days after the operation, serum myoglobin concentrations were still substantially elevated in three patients. None of our patients suffered permanent renal damage, but transient renal impairment was noted in five patients, as evaluated from the serum and urine beta 2-microglobulin concentrations. We found an association between the concentrations of myoglobin in serum and urine (Spearman's rho: 0.66; p less than 0.001) and between the concentrations of myoglobin in urine and beta 2-microglobulin in urine (Spearman's rho: 0.65; p less than 0.001). Our results indicate a transient renal impairment associated with hypermyoglobinemia and myoglobinuria, even after successful arterial embolectomy.

A simple method for predicting pulmonary function after lung resection.

A simple method for preoperative prediction of lung function following pulmonary resection is described. The principle of the method is assessment of the ventilated part of the lung which possibly will have to be resected. It is based on careful evaluation of the bronchoscopic and radiologic studies always performed in this category of patients. The accuracy of the predicted postoperative change in FEV1 sec and VC is comparable to results obtained from standard radioisotope split lung function tests. The lung function seemed to improve between 1 and 6 months postoperatively.

Atypical lipomatous hamartoma of the lung.

An unusual lipomatous tumour discovered accidentally in the right middle lobe of a 34-year old woman is described. The tumour was associated with an intrapulmonary typical chondromatous hamartoma in the same lobe but separate from the first lesion. The lipomatous tumour was primarily an intrapulmonary lipoma but in a few of the numerous sections made minute islands of cartilage and bone were discovered along with a few epithelial-lined clefts. These justify the diagnosis of a lipomatous hamartoma. Dispersed among the mature fat cells were a few immature cells with atypical nuclei. Cellular atypia in predominantly lipomatous hamartomas has not previously been reported. As the occurrence of atypical lipoblast-like cells might lead to an erroneous diagnosis of liposarcoma, this case is thought worthy of reporting.

Sarcoma of the pulmonary artery. A light and electron microscopic study.

Approximately 60 cases of primary sarcoma of the pulmonary trunk with or without involvement of the pulmonary valve and pulmonary arteries have been reported. Only four cases have been studied by electron microscopy, and all four studies have been based on autopsy material. We have received surgically removed material for electron microscopy in a new case of sarcoma of the left pulmonary artery. The tumor showed an extensive intravascular growth, especially in the branches of the pulmonary artery in the left lower lobe. At the light microscopic level the tumor was predominantly composed of spheroidal cells arranged in a disorganized pattern. The tumor cells had indistinct cell borders and centrally placed, vesiculated nuclei, often with prominent nucleoli. Many tumor cells gave a strong positive reaction for alpha 1-antichymotrypsin. Ultrastructurally the tumor was predominantly composed by light and dark fibroblast-like cells and histiocyte-like cells with ruffled cell borders containing numerous electron dense bodies of lysosomal character. A few undifferentiated mesenchymal cells and intermediate forms were also noticed. The multitude of histological composition of the sarcoma of the pulmonary trunk and arteries reported in the literature favours a derivation from an undifferentiated myointimal mesenchymal cell.

Malignant fibrous histiocytoma of the lung.

A case of malignant fibrous histiocytoma of the lung is reported. The tumour margin was well circumscribed, showing an expanding border and no capsule. The main part of the tumour was composed of spindle-shaped fibroblast-like cells arranged in broad fascicles with a partially storiform pattern. Other parts of the tumour were arranged in a haphazard pattern, containing many mononucleated and multinucleated giant cells. Ultrastructurally six different cell types were encountered. The dominant type was a fibroblast-like cell; also present were many giant cells and some histiocyte-like cells, together with their intermediate forms, and few undifferentiated mesenchymal cells. We consider this tumour to have developed from the peribronchial connective tissue; it has the same cellular composition as the malignant counterpart arising in soft tissues.

Sleeve resection of the left main bronchus. A new technique.

Bronchial sleeve resection, as an isolated procedure or combined with lobectomy, is today a widely accepted treatment of endobronchial adenoma. Among the inherent technical problems is adequate ventilation of the contralateral lung after opening of the bronchus on the affected side. This problem is easily solved by using the Carlens double-lumen tracheal tube when a sleeve resection is planned on the right side or on the left side distally to the bifurcation of the main bronchus.

Continuous measurement of the oxygen tension in blood. A comparative study in the dog.

An equipment constructed by the International Biophysics Corporation, the Differential Oxygen Analyzer, which was designed for continuous recording of the oxygen tension in the blood, particularly during extracorporeal circulation, was tested. It proved to react rather slowly to changes in the PO2 and the magnitude of the error of the single measurements is far too high at the present stage of development.
