[Closure of traumatic internal carotid-cavernous fistula with an improved type of captive embolus (author's transl)].

A 29-year-old male was treated for a traumatic internal carotid-cavernous fistula by embolization with an improved type of captive polyurethane foam embolus which was inserted through the common carotid artery. Neither the intracranial nor cervical carotid artery was ligated. The angiogram of right carotid taken one month after the surgery revealed satisfactory closure of the fistula and a perfect blood flow from the carotid artery involved. Soon after the operation, the chemosis, exophthalmos and disturbance of the right ocular movements began to diminish. The intracranial noise and bruit disappeared completely. There have been several opinions concerning the best kind of embolizing material and operative procedure. In our case, we devised an improved type of captive mooring embolus. A cylindrical piece of polyurethane foam was used because of its elasticity, plasticity and unabsorbability. To make our embolus we tied one end of a cylindrical embolus with a mooring suture of Nylon mono-filament so that its tip would be small enough to pass easily into the fistula, threaded discoid piece under the other end of the embolus for hook holding action, and fixed a silver clip marker at the tip of the embolus for locating with X-ray. We kept the mooring Nylon thread straignt without curling till operative use. The incised common carotid artery was closed by Iwabuchi's interlacing vascular suture method with excellent results.

Clinical study of chronic arachnoiditis in the posterior fossa.

A series of 12 cases of chronic arachnoiditis in the posterior fossa is reported. The diagnosis in six patients was established at suboccipital craniectomy whch brought about marked improvement in symptoms. The other six were diagnosed as having arachnoiditis in the posterior fossa from the facts that improvement in clinical symptoms was achieved by lumbar air insufflation, which has often been said to be effective in arachnoiditis, and no abnormal findings were revealed by pneumoencephalo-roulette tomography. By investigating the clinical features of these cases, we came to the following conclusions: many cases of this disease have had head trauma or chronic inflammatory disease in the past; the symptoms are apt to occur abruptly, and display repeated remission and aggravation; symptoms originating in the cranial nerves are diversified, and are often bilaterial; peripheral vestibular disturbances and retrocochlear hearing losses are observed on neuro-otological examination. In the caloric test especially the patients show canal paresis (CP) on the more impaired side, but none of them appeared to be nonresponsive to the test; lumbar air insufflation is often an effective treatment for this disease; pneumoencephalo-roulette tomography is very useful for diagnosing pathological changes in the posterior fossa.

[Primary arachnoid cyst in an adult (author's transl)].

Primary arachnoid cyst in adults seems to be relatively rare. Our case was a 42-year-old female, who complained of diplopia, dizziness, mild headache, and showed papilledema. Right carotid angiograms revealed the characteristic findings. Right fronto-parietal craniotomy was then performed. In the operation, a large, thin walled cyst was found on the surface of the right cerebral hemisphere. When the membrane was removed, essentially normal, but slightly atrophi gyri, could be seen. Aspiration of encapsulated fluid, and extirpation of the cyst wall resulted in an excellent improvement. Microscopically, the wall of the cyst was lined with normal arachnoid cells. The case was presumed to be a primary arachnoid cyst for the reasons that no definited etiological factor could be found in this case an no abnormal findings other than the cyst itself could be found in the operation and in the histological specimen. We discuss reported cases in our review, and about pathological findings, symptoms, diagnosis, and treatment of this disease.

Memory clip for intracranial aneurysm surgery. Technical note.

The authors have devised a malleable V-shaped removable clip with mechanical memory effect of shape memory. This clip is easily removed by heating part of it to 79 degrees to 107 degrees C, causing the closed clip to return to its original V shape.
