Home peritoneal dialysis (two year's experience).

Twenty patients on home peritoneal dialysis were studied during two years. The patients dialysed 4--5 times a week, using an average of 78 l dialysis fluid. The dialysis equipment used was either a simplified delivery system or a fully automatic dialysis machine. Despite clinical improvement, serum urea and serum creatinine levels were the same after 12 months of therapy as before. The patients' weight and serum albumin levels remained constant. Only 1 patient developed hyperparathyroidism, otherwise serum calcium levels ranged from normal to subnormal. Fifteen patients did not require blood transfusions. Twenty episodes of peritonitis occurred, an incidence of 0.58%. All patients carried out dialysis themselves without assistance. Three were working full-time, and 5 were able to look after their homes. The rest were able to take care of themselves. Four patients died from causes unrelated to peritoneal dialysis. In selected patients this mode of treatment provides an acceptable alternative to haemodialysis.

Black and white human skin differences.

This review of black and white human skin differences emphasizes the alleged importance of factors other than the obvious, i.e., skin color. Physicochemical differences and differences in susceptibility to irritants and allergens suggest a more resistant black than white skin. Differences appear to exist in the frequency of which several skin diseases occur among blacks and whites. A striking feature in this literature is the disagreement between authors. Common for much of this information is difficulty of interpretation, because of socioeconomic influences and other environmental factors.

[Infection in man and dog with the mite, Cheyletiella yasguri Smiley].

The infection with the mite Cheyletiella yasguri Smiley in 3 dogs and 5 out of 6 persons exposed to infestation is reported. Two of the dogs appeared to have been infected during a stay in a kennel. The dogs presented with moderate itching and slight to massive floury squamation on the back. In the infected persons, excoriated papules were present on the body and the extremities. The itching, which had persisted for 4--12 months, disappeared in all cases a few days after the dogs had been removed from the house. In the dogs, three antiparasitic baths were sufficient to exterminate the mite population.

Out-patient gastroscopy risks.

Complications in out-patient gastroscopy were evaluated retrospectively in 995 examinations performed in 625 patients. At the examination seven complications were registered in the gastroscopy record. Two complications: perforation of the stomach and cardiac arrhythmia, required hospitalisation. None of the complications was lethal. Inquiries to the referring general practitioners revealed 5 cases where the gastroscopy had led to consultations because of minor complaints after the examination. Considering all complaints as complications the frequency of complications in out-patient gastroscopy was 1.2%.

An investigation of the possible immunological relationship between allergen extracts from birch pollen, hazelnut, potato and apple.

In a retrospective study on selected group of patients, the coincidence of birch pollen allergy and a clinically relevant positive prick test reaction to apples and potatoes was confirmed. Immunochemical comparison using the crossed line immunoelectrophoresis technique (CLIE) confirmed partial identity between birch pollen and hazelnut. By the same method no partial immunological identity between birch pollen and extracts and fresh peel from apples and potatoes was found. However, both apples and potatoes gave rise to non-immunological affinity precipitates. On this basis it is discussed, whether the active agents from apples and potatoes were extremely labile allergens, plant lectins, or low molecular weight substances with a direct or indirect histamine release effect.

Contact allergy to toothpaste flavors.

Toothpaste flavors are fragrance mixtures. Oil of peppermint and spearmint, carvone and anethole are ingredients with a low sensitizing potential, but they are used in almost every brand of toothpaste and caused seven cases of contact allergy in a 6-year period at Gentofte Hospital. Toothpaste reactions are rare due to several reasons; local factors in the mouth, the low sensitizing potential of the flavors generally used, and the lack of recognition. It is emphasized that the toothpaste battery for patch testing has to be relevant and changed according to the consumers' and manufacturers' taste and fashion.

The painful, non-indurated chancre.

After reviewing 20 consecutive male patients with primary syphilis and 23 textbooks on venereology, it can be emphasized that the syphilitic chancre may be painful, and not indurated, and that the textbooks in some cases give a misleading description of the disorder.
