Whole body hyperthermia: effects upon canine immune and hemostatic functions.

In this study, the effects of induced whole body hyperthermia (WBH; 42.3 degrees C for 90 min) on peripheral blood mononuclear cell (PBMC) phenotype distribution, in vitro blastogenic responsiveness and selected parameters of hemostasis were determined in dogs. Hyperthermia was induced by heating venous blood during extracorporeal circulation (EC); induction of WBH by peritoneal lavage (PL) and perfusion without heating (i.e. euthermic EC and euthermic PL) were used as controls. Whole body hyperthermia was associated with lymphopenia and thrombocytopenia that persisted throughout the eight-day post-treatment observation interval. The lymphopenia was selective in that CD5-positive T lymphocytes were more sensitive than were sIg-positive B cells and, within the T-cell compartment, suppressor (CD8-positive) cells were more sensitive to hyperthermic stress than helper (CD4 positive) lymphocytes. Lymphopenia was also observed in EC and PL euthermic controls, although that lymphopenia was transient and nonselective. Persistent suppression of T-cell phytomitogen-induced blastogenesis was induced by WBH in contrast to transient suppression in euthermic controls. For all treatment groups, lymphopenia and suppressed blastogenesis were correlated to elevated plasma cortisol levels. Induction of WBH by EC resulted in coagulopathy characterized by thrombocytopenia, increased plasma fibrin degradation products, prolonged clotting times, and evidence of spontaneous bleeding. These hemostatic alterations were correlated temporally to increased serum levels of liver enzymes. However, there was no evidence of hepatic injury when WBH was induced by PL, where transient thrombocytopenia was the only significant hemostatic alteration. These results indicate that 42.3 degrees C whole body hyperthermia can be well-tolerated and, although associated with suppression of general indexes of immunocompetence, is not associated with opportunistic infections.

Thyroid dysfunction in pregnancy.

Thyroid dysfunction is the second most common endocrine disorder among women of childbearing age. When thyroid dysfunction is well controlled, pregnancy outcomes are similar to those of women without thyroid disease. Perinatal nurses can support the woman affected by a thyroid dysfunction by being knowledgeable about the disorder and by providing support and education. The article provides an introduction to the physiology and pathophysiology of the thyroid, the effect of common thyroid disorders on pregnancy, and clinical management implications for perinatal nurses.

[Treatment of lumbar facet joint syndrome by CT-guided infiltration of the intervertebral joints].

INTRODUCTION: Lumbar facet joint syndrome is a common condition, involving about 79% of patients suffering from low back pain. Denervation by scalpel or electrofrequency is a well-known method of treating it. An alternative method for longer lasting pain relief is CT-guided intraarticular infiltration of facet joints with local anaesthetics and cortisone. METHODS: In 62 patients with facet syndrome a total of 205 joints were infiltrated, CT monitoring being used in all cases. Each facet joint was infiltrated with 0.3 ml bupivacaine and 0.8 ml methylprednisolone. The patients were divided into three groups. Group 1 consisted of 24 persons who had undergone lumbar disc surgery, group 2 of 23 patients with spondylarthrosis of the facet joints, and group 3 of 15 patients who had undergone lumbar disc surgery but had additional spondylarthrosis. Very good results were defined as pain relief for longer than 4 weeks, good results as pain relief for up to 4 weeks and poor results as brief pain relief or none at all. In 27 patients facet infiltration was performed for the purpose of diagnosis. RESULTS: Results were significantly in group 2 than in group 1 (Fisher's exact test P < 0.012). In group 3 results were better than in group 1, but worse than in group 2. These differences were not statistical by significant (Table 3). Seven patients in the diagnostic group had no pain relief, and facet syndrome was excluded. CONCLUSIONS: CT-guided facet joint infiltration is a good method for treatment and diagnosis of lumbar facet joint syndrome. It can be repeated and has no severe side effects. The best results were seen in patients with spondylarthrosis of the facet joints. After lumbar disc surgery the pain relief was shorter, though it also yielded good results. For patients with unspecific low back pain, facet joint injections are a very good diagnostic method, allowing definite exclusion of lumbar facet syndrome.

Feline gingivitis-stomatitis-pharyngitis.

Inflammatory conditions of the feline mouth are commonly encountered in small animal practice. Although the majority can be attributed to dental disease and a small percentage are due to autoimmune diseases, the eosinophilic granuloma complex, neoplasia, and other miscellaneous syndromes, many cases appear to be due to a gingivitis-stomatitis-pharyngitis complex, which is likely multifactorial in origin. Viruses, bacterial infection, diet, dental disease, oral conformation, genetic predisposition, hypersensitivities, immunoinsufficiencies, and other defects in oral defense mechanisms may all be contributory. The complexities of this syndrome have made it one of the most challenging diagnostic and therapeutic problems in feline medicine.

Establishment and characterization of a new Epstein-Barr virus transformed cell line from a human B cell lymphoma.

We have established a new cell line from a patient with centrocytic B cell lymphoma. Highly purified peripheral blood B cells from patient DUL (WBC counts 158,000/microliters) were infected in vitro with Epstein-Barr virus (EBV), and CD20+ B cells were cloned into 96 well culture plates with the aid of a cell sorter autoclone device. As shown by GTG-banding and Southern blot analysis, out-growing EBV-positive clones had the same chromosomal abnormalities and identical monoclonal IgH gene rearrangement as the original EBV-genome-negative leukemic B cell clone. Surface marker analysis with a panel of monoclonal antibodies revealed identical patterns on EBV-negative and -positive clones, with the exception of PCA1 (reactive with plasma cells) which was negative on freshly explanted leukemic B cells but positive on EBV-converted clones.

[Mumps orchitis--symptoms and treatment possibilities].

It is reported on 7 patients after mumps orchitis. In 7 out of 9 affected testes an atrophy was found. Because of this high atrophy rate the current therapeutic measures are discussed and a systemic immunoglobulin prophylaxis and corticosteroid therapy is emphasized. As an orchitis prophylaxis the application of mumps immunoglobulin in males aged from 12 to 20 years with mumps exposition is postulated.

[Aetiological treatment of benign goitre with l-thyroxine (author's transl)].

L-Thyroxine was given to 3956 patients with benign goitre between 1974 and 1977, 2314 of which were controlled over a period of 3 years at regular intervals. Benign goitre in children less than 14 years old and postoperative recurrences were not evaluated. Definite reduction in goitre size wss achieved in 77.2% of patients by continuous long-term treatment over 3 years. In large goitres and those with nodular transformation the status quo was maintained in all but exceptional cases.

[Stresses on osteosyntheses of femur and tibia in early mobilisation (author's transl)].

The knowledge of post surgery strain is of fundamental significance for dimension and location of application of osteosynthesis. In the following biomechanical research height and direction of moments of flexure will be determined that occur during sleep and/or anaesthesia without additional and at partially loaded early mobilisation with the muscle activity at course of doppel step at upright- and swing-leg position. Moments of flexure are calculated at frontal and sagittal level considering efficiency of weights of parts of the leg and their centres of gravity as well as bending efficiency of the active muscle groups. Thus, moments of flexure resulting important facts are found for dimenstion of osteosynthesis equipments.

Stresses and functional adaptation of alloarthroplasty of the proximal femur in dogs using a selfcuring rippled prosthesis. A comparative biomechanical and morphological study (author's transl).

A self-adhesive prosthesis for youth patients is developed by Mittelmeier that is implanted without bone cement. After an implantation term of 1 year in animal experiment (dog), an adapted femur is compared with the contra-lateral normal femur. Strain gauges are used to determine bone strain under load. Further is determined external and internal construction change after endoprosthesis. The construction changes found are in connection with the changed bone strength. A cancellous change of bone at entire area of stem of prosthesis is called a "metaphysation" of bone shaft.

[Mechanical and functional complications in endemic struma (author's transl)].

Of 1294 patients examined with struma of magnitude I to III, complications in the form of mechanical effects on the neighboring organs, disturbances of thyroid function and pathological anatomical changes in the struma were demonstrated in 1051 cases. The appearance of high-seated esophageal varices is described in 33 patients with primary struma of magnitude II and III. 72 patients had congenital or postnatal hypothyroidism, and 164 patients a subclinical one. The carcinoma ratio (2.9%) was in the lower range of the normal for the endemic areas. Because of the high complication rate, every struma should be clarified diagnostically and specifically treated.

[About the proposition of strength of animal-experimental results with hip endoprosthesis. A comparative analysis of the stress of the upper thigh of man and of four-leg (author's transl)].

Precise knowledge of the biomechanic of the hip of man and of animal is necessary to evaluate the procedures of adaption of the femur after endoprosthesis with animal experiment and the transfer into the endoprosthesis of the human medicine. After functional-morphological and biomechanical researches (Pauwels), the upper femoral end of the man is highly stressed by bending at direct level while lowly stressed at sagittal level. The present own biomechanical analysis of the hip of the dog reveal - under consideration of the gaits "pace", "trot" and "gallop" - that the coxal end of the femur of the four-leg compared with that of man is highly stressed by bending at sagittal level as well as at direct level. The observations are herewith clearly explained that intense reconstructions of the bone at artificial human thighs are found mainly at lateral and medial circumference of the bed of the prosthesis, at animal experiment however at dorsal, ventral as well as medial circumference.
