[Chemodectoma of the orbit. Apropos of a case and review of the literature].

The clinical, follow up and diagnostic features of a case of chemodectoma (paraganglioma) of the orbit are reviewed. 30 cases are reported in the literature. Chemodectoma may arise in any area of the body where non chromaffin paraganglion structures are situated. These are localized in the orbit of the chimpanzee but not in the orbit of man. Malignant behavior of chemodectoma has been a controversial subject, with most authors considering them to be benign tumors. Some authors have diagnosed locally recurrent and infiltrative tumors as malignant. The authors report one case of chemodectoma of the orbit. A 5 year old child has had right exophthalmos. She was treated by tumorectomy. The follow-up is marked by recurrence of the tumor and infiltration of the right hemi-face.

[Histiocytoid hemangioma of the heart. Histological and immunohistochemical study of a case].

We present the fourth case report of an histiocytoid hemangioma of the heart, which was found incidentally, as in the other three reported cases, in a 50 year old man during surgery for congenital heart disease. A polypoid sessile reddish tumor with a smooth surface, which measured 2 cm in diameter, was found in the right atrium. No necrosis nor cystic areas were seen. Histologic evaluation revealed a proliferation of histiocytoid or epithelioid cells, whose arrangement varied a great deal within the infiltrate. Sometimes, the cells were isolated but in most places they were arranged in sheets or nests surrounded by connective tissue. Some of these nests were compact whereas in others a lumen formation, frequently occupied by erythrocytes, could be appreciated. A striking feature was the presence of vacuolated tumor cells sometimes assuming a signet-ring configuration. Some large vascular spaces were lined by histiocytoid or epithelioid endothelial cells, many of which protruded into the lumen. Multinucleated tumor cells were occasionally observed. Mitoses were rare (up to 1/10 HPF) and scattered inflammatory cells such as lymphocytes, macrophages, neutrophils and eosinophils were seen. Histologic appearance of our patient's tumor is identical to the histiocytoid hemangioma of Rosai et al., which is an ubiquitous endothelial benign neoplasm.

[Retrocervical cystic hygroma. Ultrasonic diagnosis. Apropos of 5 cases].

The authors report five ultrasonic imaging between 13 and 32 weeks of gestation. All of them have an hydrops fetalis and an aspect of like masses located in the nuchal region. A cystic retrocervical hygroma was confirmed at autopsy examination in three cases. The diagnosis of cystic hygroma remains inaccurate in the other two cases in the lack of the radiographic and anatomical verification. A review of the literature was made up to define the diagnostic aspects of the hygroma colli, their histological aspects and the mains etiological factors.

Detection of cross-reactive determinants shared by human monoclonal IgM reacting with myelin-associated glycoprotein.

Monoclonal IgM from patients with peripheral neuropathy frequently have anti-myelin-associated glycoprotein (MAG) activity. We investigated the idiotypes of 10 monoclonal anti-MAG by using rabbit polyclonal antisera. Three groups of anti-idiotypic antisera could be distinguished. Four sera reacted only with the immunizing protein. Two sera reacted with a single other anti-MAG IgM in addition to the immunizing one. Immunoenzymatic studies showed that these two couples of anti-MAG IgM reacted identically with 100% cross-inhibition, indicating that the whole set of idiotypes identified by the rabbit antiserum was present on both IgM antibodies. The four other anti-idiotypic sera reacted with the homologous IgM, as well as with most of the other anti-MAG IgM. In contrast to the previous antisera the binding of these four sera to the homologous IgM could not be inhibited by other cross-reacting anti-MAG IgM. However, when a heterologous IgM was used for coating, these antisera with one exception showed complete cross-inhibition. The absence of inhibition by purified MAG of the patient of the anti-idiotypic antisera sera suggests that these antibodies are most likely to be directed against framework determinants rather than against combining site epitopes.

Characterization of intermediate filaments expressed by Ewing tumor cell lines.

The histogenesis of Ewing sarcoma is still controversial; we therefore studied the expression of intermediate filaments (IF) in cell lines derived from Ewing tumors since identification of IF in tumor cells is considered a reliable marker of tissue origin and differentiation. All nine lines studied expressed vimentin IF; in addition, a small number of Ewing cells from three lines expressed keratin filaments. After treatment with phorbol esters, a high percentage of cells from these three lines synthesize keratin IF identified by immunoblotting as keratin 8 and 18 polypeptides, which are expressed by single epithelia and epithelial cells in early embryonic development. Furthermore cells from a fourth line synthesize keratins after transplantation in nude mice. These data indicate that, under certain conditions, undifferentiated Ewing cells may acquire an IF phenotype related to that of epithelial cells.

Heterogeneity of human anti-MAG IgM as revealed by their reactivity on avian embryonic tissues.

Monoclonal IgM from patients with peripheral neuropathies frequently have an antibody activity directed against myelin-associated glycoprotein (MAG). Reactivity of 19 of these sera with avian embryonic tissues was compared. Immunohistological studies allowed their classification into distinct groups according to the staining pattern. Both neural and non-neural cell lineages were recognized by these antibodies. The IgM specificities were also compared with those of the monoclonal antibodies HNK-1 and NC-1, which react with MAG and some cell adhesion molecules. Only one group displayed a pattern of staining similar to that observed with these monoclonals. The reactivity of these IgM paraproteins with two cell adhesion molecules, neural cell adhesion molecule and neuron-glia-cell adhesion molecule by immunoblotting confirmed the heterogeneity found in the histological study.

Vimentin gene: expression in human lymphocytes and in Burkitt's lymphoma cells.

We have isolated a human genomic clone for the intermediate filament subunit vimentin with a DNA probe encoding chicken vimentin. We show that the gene for this protein exists as a single copy in the haploid human genome and is transcribed into one mature RNA species of 2 kb. In vitro translation of poly(A)+ mRNA in a rabbit reticulocyte cell-free system showed that vimentin is a major product of RNA from normal lymphocytes but not of RNA extracted from Burkitt cells. 2-kb vimentin mRNA can be detected with a DNA probe in normal lymphocytes and in fibroblasts, but not in cell lines derived from Burkitt's lymphoma (JI, JBL2, BJAB, DAUDI). The abundance of vimentin mRNA is correlated with the quantity of vimentin present in the cells, suggesting that the level of expression is regulated by the abundance of mRNA. The half-lives of vimentin mRNA were found identical in both fibroblasts and lymphocytes and belong to the class of stable mRNA.

Phenotypic characterization of Ewing sarcoma cell lines with monoclonal antibodies.

The histogenesis of Ewing sarcoma, the second most frequent bone tumor in humans, remains controversial. Four Ewing cell lines were analyzed by immunological methods. A panel of antibodies directed to T, B, and myelomonocytic markers gave negative results. Surface antigens recognized on Ewing cells were found to be related to the neuroectoderm lineage. Ganglioside GD2, a marker of neuroectodermal tissues and tumors, was present on all lines. These were also stained by the mouse monoclonal antibody HNK-1, which detects a carbohydrate epitope present on several glycoconjugates of the nervous system, including two glycoproteins, the myelin-associated glycoprotein and the neural cell-adhesion molecule (N-CAM), and an acidic glycolipid of the peripheral nervous system. The P61 monoclonal antibody, which reacts with a peptide moiety of N-CAM, and a rabbit antiserum, raised to purified mouse N-CAM and not recognizing the HNK-1-defined epitope, were also reactive. By contrast, all antibodies specific for hematopoietic cell surface antigens were totally negative. Besides these antigenic features, Ewing sarcoma cells are characterized by a specific t(11;22)(q24;q12) translocation also observed in neuroepithelioma, a neuroectodermal tumor, suggesting a possible evolutionary related origin. The recent finding that the human N-CAM gene is located at the vicinity of the breakpoint on chromosome 11 indicates that it might be involved in genetic rearrangements occurring in this region.

[Role of the digestive tract immune system in the control of bacterial translocation in gnotoxenic mice].

The aim of this work was to study the role of gut associated lymphoid tissue in the control of bacterial translocation. Two strains of Escherichia coli were orally inoculated to 71 axenic mice. Ten days after, the 2 initial strains and 2 others, resulting from plasmidic exchanges, were present in the digestive tract of the mice which were divided in two groups: the first group (n = 41) received one intraperitoneal injection of cyclophosphamide 100 mg/kg; the second control group (n = 30) received isotonic saline. The following parameters were studied 3, 5 and 9 days after the injection: the population level of the 4 strains in the caecum, their translocation to mesenteric lymph nodes, liver, spleen and circulating blood, the density per unit surface of lamina propria plasma cells and intraepithelial lymphocytes in duodenal and caecal mucosae. The population in each strain found in the caecum was different from the 3 others but similar within the two groups of animals and remained unchanged with time. In the control group, bacterial translocation to the mesenteric lymph nodes decreased (p less than 0.01), while the density of plasma cells increased (p less than 0.01) from the 3rd to the 9th days. In the cyclophosphamide treated group, translocation to the mesenteric lymph nodes increased (p less than 0.01), while the density of intestinal plasma cells decreased (p less than 0.05) from the 3rd to the 9th days. Density of intraepithelial lymphocytes did not vary with time in each group and from one group to another. Bacterial translocation to liver, spleen and systemic blood was weak and did not increase in the treated group.(ABSTRACT TRUNCATED AT 250 WORDS)

[Papillary carcinoma developing in thyroglossal duct cyst. Apropos of 2 cases].

The authors report two cases of papillary carcinoma of a thyreoglossal duct cyst. In both cases, the thyroïd gland was normal. Clinical manifestations are not specific and the diagnosis is usually made by histopathologic examination which reveals in the majority of cases a papillary carcinoma. When loco-regional or metastatic extension are absent surgical excision of the cyst is sufficient.
