Medical liability claims and lawsuits filed against the University of Texas System involving adult psychiatric patients.

A survey was conducted to investigate medical liability claims filed against the University of Texas System (the U.T. System), physicians, and/or institutions concerning care of adult psychiatric patients. There were 34 such claims filed between 1978 and 1991 that had closed by December 1991. In many of these cases, medical record documentation was determined to be inadequate. Of the 34 claims filed, only seven were settled on behalf of U.T. personnel or U.T. institutions. Of these seven settlements, the mean monetary amount paid per case on behalf of U.T. System and non-U.T. System parties was $98,678.57. The two categories with the largest number of claims were 1) cases involving suicides, and 2) cases involving adverse reactions to medication. The largest award was $350,000, paid on behalf of U.T. physicians, a private physician, and non-U.T. institutions for alleged negligent failure to diagnose Cushing's disease. Of this amount, only $150,000 was paid on behalf of U.T. physicians. Risk management strategies should be implemented to address areas of liability in psychiatry residency programs.

Adult

Senior medical student clinical and research electives in dermatologic surgery.

Five senior medical students enrolled in the 4 week clinical elective in dermatologic surgery and cutaneous oncology (DSCO); two of these students also enrolled in a 4 week research elective in DSCO. During the elective(s) the students made significant improvement in the six areas of clinical diagnosis, histopathologic diagnosis, clinical management, DSCO procedures, research skills, and general information about the subspecialty. Following their elective(s), these students submitted a total of six abstracts, one letter, and seven manuscripts for presentation or publication.

Optical illusions in clinical dermatology: the Mach band phenomenon and afterimages.

A survey of dermatologists was conducted to determine whether the perceptions of afterimages and Mach bands impacted on clinical dermatology practice. 26.5% (13/49) of respondents indicated that they perceived one or both of these optical illusions. No false-positive potassium hydroxide interpretations of skin scrapings for hyphae were reported due to perceived afterimages, and no skin biopsies were reportedly performed as a result of Mach bands.

Parents' use of sunscreen on beach-going children. The burnt child dreads the fire.

BACKGROUND: Little is known about why some parents elect to use sunscreen on their children while others do not. A survey was distributed to beach-going childrens' parents who used (n = 42) or did not use (n = 40) sunscreen on their children to determine differences between these two groups. RESULTS: Parents who applied sunscreen to their children had children who previously had experienced significantly more painful sunburns (chi 2 = 6.40; df = 1; P less than .05). CONCLUSIONS: Sunscreen use by beach-going children is motivated by parental attempts to prevent acute painful sunburn. Parental attitudes and behaviors about their childrens' sun exposure need modification if sun protection strategies are to reduce the future epidemic of skin cancer successfully. Sun protection education during well-child visits by pediatricians and family physicians is recommended.

Fear of AIDS related to development of obsessive-compulsive disorder in a child.

Fear of acquired immune deficiency syndrome (AIDS) has been associated with a variety of psychiatric disorders in adults and in some adolescents. A case is reported of a child with an unrealistic fear of AIDS who developed an obsessive-compulsive disorder. This case illustrates the importance of providing adequate AIDS information to children.

The necessity for treatment of childhood port-wine stains.

The pulsed tunable dye laser has recently become available for the treatment of children with port-wine stains. Parental reactions and psychosocial implications of untreated port-wine stains in children are reviewed. It is recommended that children with port-wine stains be treated as early as possible to prevent adverse sequelae to their psychological development.

[Incidence, differential diagnosis and follow-up of respiratory affective seizures (rage seizures) in children].

The anamneses, course, general and specific findings in 99 cases of respiratory affective spasm in children between 1960 and 1983 were carefully studied. The clinical records, which sometimes lacked details, were augmented by a comprehensive questionnaire which also contained catamnestic questions. The author gives diagnostic and therapeutic recommendations and instructions for talks with the parents. Correctly diagnozed respiratory affective spasm is an absolutely harmless disease of limited duration with a good prognosis for the child's development.

[Pregnancy in adolescence--psychosocial development of young mothers and their children].

Hitherto existing concepts on circumstances favouring pregnancies in adolescents have mainly focussed attention on physically precocious maturity, on social risks and on the type of personality of the pregnant adolescents. Based on our material this opinion seems to be in need of correction. Changing of the validity of these factors may be suggested due to psychosocial conditions in connection with both the acceleration of maturity and a new estimation of sexuality including heterosexual development at earlier stages of life. Children from 100 juvenile mothers were clinically and psychologically investigated at the age of 7 years. Neither serious social problems in the children nor considerable deviations of mean average of the physical, mental or social age specific standards could be observed.

Adolescent

[The prognosis of encephalitis in childhood].

Follow-up examination of 95 former patients who had been suffering from encephalitis in infancy were performed. The coincidence of disturbance of consciousness, pathological-EEG and neurological irritation- and deficiency symptoms proved to be an unfavourable prognosis. These patients are especially handicapped in their concentration and motorial performances. Only 50% of the group with a severe course of the disease achieved a complete vocational training.
