[Simultaneous operations--the nomenclature and the definition].

304 operations with simultaneous herniotomy were performed on 282 patients for different associated diseases. Most frequent combinations of the diseases coexistent with ventral hernias, specific features and immediate results of the operations are described. The opinions of different scientists on the name, definition and essence of simultaneous operations are analyzed. The authors propose their personal definition of the simultaneous operations.

[Effect of endolymphatic antibiotic therapy on the status of cellular immunity in peritonitis].

The immune status in 68 patients with diffuse and generalized peritonitis was studied. It was established, that a degree of the impairment in immune response depended on peritonitis spreading. Endolymphatic antibiotic therapy had immunostimulating effect in patients with diffuse peritonitis, in generalized peritonitis, this was not observed. In patients with generalized peritonitis, it is necessary to include into the complex of treatment the other methods of immunologic correction.

[Variants of reconstructive and reparative surgery in high strictures and injuries of the biliary system].

The authors' views of the problem of reconstructive and restorative surgery of the biliary tract are set forth and original methods are suggested for external and through drainage after formation of hepatohepatic- and hepaticojejunal-anastomosis (via the inferior surface of the right lobe of the liver in the region of the gallbladder bed and through the round ligament of the liver). Some problems of operative techniques in establishment of the anastomoses are discussed. A total of 39 operations were carried out: 25 reconstructive and 14 restorative. The postoperative mortality was 28.2% (11 patients). Preference is given to restorative operations because they are technically simpler.

[Drainage variants in reconstructive and restorative operations for high strictures and injuries of the biliary tract].

The article deals with the author's views on certain aspects of the problem of reconstructive and restorative surgery of the biliary tract. Original methods are suggested for external drainage (through the inferior surface of the right hepatic lobe in the region of the gallbladder seat and through the round ligament of the liver) in formation of ++hepato-hepatico- and hepaticojejunoanastomoses. Problems of operative techniques in formation of the anastomoses are discussed. Thirty-nine operations have been carried out in the clinic in the recent decade in high strictures and traumas of the biliary tract, 25 were reconstructive and 14 restorative. Postoperative mortality was 28.2% (11 patients). Intoxication and hepatargia associated with cholangiolytic abscesses of the liver were the main causes of death.

[Pancreatic enzymatic activity in the blood and urine in tumors of varying localization].

Under study was the activity of proteinases and their inhibitors, amylase and lipase of the blood serum and urine in 15 healthy subjects and 40 patients with tumors of different localization, and in 8 patients subjected to surgical interventions. There was found a reliable increase of total antitryptic activity in tumors of different localization and extensive operative procedures, while an increase of proteinase activity of blood serum in gastric cancer.

[Suppurative-necrotic pancreatitis and its surgical treatment].

Purulent-necrotic processes in the pancreas may arise against the background of different forms of acute pancreatitis. Biological tamponade of the pancreas with the greater omentum and hermical closure of the abdomen result in a reduced number of cases of the gland suppuration. The treatment of purulent pancreatitis should be complex, comprising an early surgical intervention with subsequent rational conservative therapy.

[Experimental morphologic findings serving as a basis for use of xenogenic dura mater for plastic surgery of blood vessels].

Heterotransplants of the human dura mater were used to replace circular and lateral defects of the thoracic and abdominal aortas, carotid and iliac arteries in experiment. Operations were performed in 106 dogs. Observations of permeability of the vessels, morphological and histochemical studies were carried out at different periods after operation (from 1 day to 2 years). The xenogenous dura mater has well pronounced plastic properties--it is solid, elastic, characterized by low antigenicity. During transplantation there appear no hard inflammatory--destructive changes in reconstructed vessels and the surrounding tissues. The transplant is gradually substituted by recipients tissues characterized by high specialization. They are rich in elastic and smooth muscle fibres, have good blood supply and innervation. The dura mater is a good material for plastics of vascular wall defects.
