From May to September 1987, 221 women of over 45 years of age were examined by transvaginal ultrasound at the Department of Gynaecology (Gynaecological Hospital) of the University of Marburg. Transvaginal ultrasound proved to be significantly superior to clinical examination, since palpation had resulted in "NAD" findings in all cases. The average diameter and the volume of postmenopausal ovaries, determined by transabdominal sonographic studies, were easily confirmed by the new method. In 6% of the 221 patients, pathological findings of the ovaries were clinically "NAD". Since the usefulness of transabdominal ultrasound in screening for ovarian cancer has already been confirmed by other authors, this pilot study aims at providing an opportunity to test transvaginal ultrasound also in a large ovarian cancer screening programme by furnishing the essential basic research data. Age-adapted average ovarian sizes and significant differences in size between the right and the left ovary were used to differentiate between normal and pathological findings as well as changes in the ovarian parenchymal patterns.

In a 28-year-old patient with a large uterine leiomyoma, complete tumor regression was achieved by the LHRH agonist Zoladex (ICI 118.630) when administered three times. During the follow-up period of three months, new myoma growth did not occur. On the basis of this case study, it is necessary to discuss a new therapy concept for the differentiated treatment of uterine leiomyomata in young women with a desire for pregnancy.
Cervical smears from 516 women were investigated cytologically and for the presence of papilloma virus (HPV) type 16/18 DNA sequences. From the cytologically normal smears (Pap I, II, IIw) 57/424 (13%) 16/18 were found HPV positive and from the pathological ones (Pap III, IIID, IVa, IVb, V), 30/92 (33%). The age prevalence of the HPV infection--provided a cytologically normal smear--appears compatible with the period of sexual activity, but persistence of the HPV infection has to be considered as a complicating factor. Our investigations on successive smears nevertheless permit the hypothesis that an HPV infection may disappear. The use of biotin-labeled nucleic acid probes yields results at least as reliable as those obtained with radioactive probes. The test for HPV positivity appears at present to be of limited diagnostic and prognostic benefit, particularly for the individual case. Investigations on fundamental oncogenic mechanisms are a different matter.
In the present study we used the well known antiviral properties of interferon alpha 2b in condylomata acuminata. Firstly we investigated 3 different application forms. We found that 1. the local INF application in gel was completely ineffective, 2. the injection of INF into the base of the warts was effective but too painful for routine therapy, 3. the systemic treatment by s.c.-injection had an excellent compliance and a high efficacy. In the second part of the study we treated 16 patients with our own s.c.-regimen of 3 x 5 mill. U and 6 x 2 mill. U INF alpha 2b within 5 weeks. 60% showed a complete response, 30% a partial response; these were treated surgical afterwards. In none of the patients a relapse occurred at a median follow-up of 11 months. The observed side-effects were poor. We consider the systemic treatment with interferon for condylomata acuminata to be highly effective and well tolerated. The relapse rate is very poor.
A urinary chorionic gonadotrophin (hCG) preparation, mitogenic for ovarian carcinoma cells, was analysed by gel filtration through Sephadex G-100 Superfine. The resulting fractions were tested for hCG and for properties of the epidermal growth factor (EGF) by radioimmunoassays (RIA) in comparison with their ability to stimulate the growth of EFO-27nu ovarian carcinoma cells. The elution profile of the RIA activities for hCG corresponded to molecular weights of 12 and 71 kDa, whereas the mitogenic activity was found in peak fractions eluting at 7, 11 and 52 kDa, indicating the presence of mitogenic substances distinct from hCG or its beta-subunit. In comparison experiments, radiolabelled recombinant human EGF eluted at 7 kDa from the column. The profile of EGF immunoreactivity determined in the eluant fractions of hCG preparation A correlated with the mitogenic potential. Eluant fractions with growth-promoting activity competed with 125I-labelled EGF in binding to EFO-27nu cells; the inhibition of EGF binding was correlated with the mitogenic potential and the EGF immunoreactivity. We assume that the 7 kDa component of the gel filtration eluate corresponds to monomeric EGF; the high molecular weight mitogens may represent EGF precursor protein fragments of various molecular size classes.
The proliferation of three mammary carcinoma cell lines was explored for the effectiveness of dihydrotestosterone (DHT) and the antiandrogenic substances cyproterone acetate (CPA) or hydroxyflutamide. The cell growth, determined in multiple experimental cultures of the estrogen-sensitive lines MCF-7 and EFM-19, was stimulated by 10(-9) M to 10(-6) M DHT, whereas estrogen-resistant MFM-21 cells were unresponsive to the hormonal factors applied. Growth-promoting effects of 10(-8) M to 10(-6) M CPA were detected in cultures of those cell lines which were sensitive to estrogen and androgen. Competition experiments with DHT and the antiandrogens suggested involvement of the androgen receptor in the stimulation of cell growth by CPA. Participation of the estrogen receptor was excluded by lack of competition between CPA and the enhancement of proliferation by estradiol-17 beta. At the receptor level the antiandrogens were able to compete with androgen binding. The results of the study demonstrate androgenic properties of CPA in regard to the growth of human mammary carcinoma cells.
Benign and malignant breast diseases were histochemically investigated with regard to their secretory function. Thereby, the labelled lectin from peanut (peanut agglutinin = PNA) which possesses a high affinity for D-galactosyl-(1-3)-N-acetyl-D-galactosamine, showed a binding pattern partly indicating disturbed secretory cell mechanisms. Further studies with a labelled antibody directed against a milk fat globule membrane glycoprotein, which was isolated by PNA affinity chromatography from human milk, confirmed the PNA receptor as a marker of a milk protein in breast carcinomas. Additionally, two other monoclonal antibodies which were raised against other components of the milk fat globule membrane were used for a more detailed characterization of the functional situation of mammary carcinomas. These immuno- and lectin-histochemical investigations revealed that tumors with a high secretory activity responded in about 80% to endocrine treatment, whereas breast carcinomas that exhibited no secretory function failed to respond to endocrine therapy. Thereby, the hormone dependence of tumor tissue showed an inverse correlation with the proliferative activity, as determined by the mitosis index.
Lectin from the peanut (peanut agglutinin = PNA) possesses a high affinity for D-galactosyl-(1-3)-N-acetyl-D-galactosamine, used for histochemical investigations on formalin-fixed tissue sections of normal mammary gland as well as of benign and malignant breast diseases. Thereby, a lectin-binding pattern was found that partly exhibited secretory malfunction. Additional studies with a labeled antibody directed against a milk fat globule membrane glycoprotein, which was isolated by PNA affinity chromatography from human milk, confirmed the PNA receptor as a marker of a milk protein in breast carcinomas. The most important clinicopathological finding, however, was that in accordance with the secretory activity about 77% of PNA-positive tumors responded to endocrine treatment, whereas PNA-negative mammary carcinomas usually failed to show any response to hormonal therapy. These results also correlated with the hormone receptor content of tumor tissue.

During the last decade many new experimental and clinical data were published demonstrating the inclusion of estrogens, progestagens, androgens, corticosteroids, prolactin, growth hormone and of insulin in the endocrine control of normal mammary tissue. Different hormonal disorders, such as lowered progesterone-estrogen-index (P.E.I.), anovulation, hyperprolactinemia, or increased prolactin-sensitivity of mammary tissue, are often responsible for the induction and/or maintenance of benign breast disease. therefore the endocrine treatment possesses deciding importance for the improvement or removal of breast symptoms. The present paper evaluates the role of progestagens, prolactin-inhibitors or anti-gonadotropins in the differentiated therapy of mastodynia, mastopathia, mammary secretion, mastitis, mammary hyperplasia, premenstrual syndrome and other disturbances. Dose levels and side effects of the various drugs are reviewed. The relation to other therapeutic procedures, as non-hormonal preparations or operative methods, is discussed.
