Secretin: A new role for an old hormone.

Plasma-secretin levels were measured by radioimmunoassay during a 72-hour starvation period in nine healthy subjects. Immunoreactive secretin (I.R.S.) levels rose dramatically during starvation. The rises were several orders of magnitude greater than those after intraduodenal acid. The classic concept that the major function of secretin is the control of the exocrine pancreas is challenged by these results. The findings suggest that secretin is a hormone of importance during starvation, and its role in starvation may be related to its lipolytic properties.

Maturity onset diabetes mellitus: response to intensive dietary management.

Analysis of the first six months of intensive dietary management of 57 maturity onset diabetics showed that a large proportion of such patients could be satisfactorily controlled without the need of either oral hypoglycaemic agents or insulin. A dietitian's assessment of the patient's adherence to the prescribed diet allowed groups of good and poor dieters to be selected. Among the poor dieters the plasma insulin and triglyceride levels were significantly increased though plasma glucose levels were not significantly higher. Dietary adherence may thus be an prognostic risk factor in this group of diabetic patients.

Immunoreactive gastrin and gestation.

Serum concentration of radioimmunoreactive gastrin reflects closely the known physiology of gastric acid secretion during pregnancy. In particular, marked rises of immunoreactive gastrin occur in late pregnancy, at parturition, and in the early puerperium.

Neonatal secretion of secretin.

The plasma levels of secretin have been measured in mothers after labour, and in their babies at birth and on day 4 of life. The mean cord venous level was higher than the maternal level, and there was a significant correlation between the individual maternal and cord values. The level had again increased by day 4, and at this time the secretin level was inversely proportional to the blood glucose level.

Plasma gastrin in congenitial hypertrophic pyloric stenosis. A hypothesis disproved.

Fasting plasma gastrin levels were measured in babies with pyloric stenosis and in normal babies of similar age. There was no difference in gastrin levels either before or after operation between the babies with pyloric stenosis and normal babies. Similarly, neither the fasting blood glucose nor the fasting gastric pH of the babies with pyloric stenosis differed significantly from the values obtained in normal babies. Our findings do not support the hypothesis that gastrin in the fasting baby has an aetiological role in the development of hypertrophic pyloric stenosis of infancy. An alternative hypothesis is suggested.

A duodenal role in gastrin release.

Observations on the plasma gastrin response to feeding protein in patients with the dumping syndrome, patients with pyloric stenosis, and patients asymptomatic after gastric surgery suggested that the duodenum might be important in the mechanism of gastrin release. This duodenal role was confirmed by the finding that when the stimulus was placed directly in the duodenum peak gastrin levels occurred earlier than when the stimulus was placed directly in the stomach.
