Lobular capillary hemangioma of the esophagus. A case report and review of the literature.

A case of lobular capillary hemangioma in the lower portion of the esophagus in a 49-year-old woman was investigated. Lobular capillary hemangioma of the esophagus must be a rare lesion. We have been unable to find a single case in the literature. The excised lesion consisted macroscopically of a single strip of red tissue ulcerated and covered with a white exudate, 7 mm in length, with an attached vascular pedunculated lesion no more than 4 mm in diameter. Microscopically, the pedunculated lesion had the features of a capillary hemangioma with a circumscribed aggregate of capillaries arranged in one or more lobules. Similar cases from the oral and nasal mucous membranes were recorded in 1980 by Mills et al.

Esophageal Neoplasms↗

Seven-year follow-up study after the replacement of the esophagus with an artificial esophagus in the dog.

An artificial esophagus was made of silicone rubber (Phycon) tube covered with a Dacron mesh. A segment of thoracic esophagus of 16 dogs was replaced with this graft using three different types of anastomosis--overlayer end-to-end anastomosis with flanged tube, two-layer end-to-end anastomosis with flanged tube, and monolayer end-to-end anastomosis with no flange tube. Seven of 16 dogs (44%) survived more than 12 months without complications, four of them surviving more than 6 years. In six of seven of the prolonged survivors, extrusion of the graft was recognized in the 3 to 6 months after operation. Esophageal stricture increased slightly up to 6 months after extrusion of the graft, but it did not further advance until sacrifice. In these dogs, mucosal regeneration of the neoesophagus was complete with muscle layers and mucous glands in the submucosa recognized microscopically. Proximal esophagus from the replaced portion was apparently dilatated more than that of the distal portion. There was no definite difference between the anastomotic techniques with regard to complication or prognosis. These results suggest that this artificial esophagus should be considered as a possibility for clinical trials in the future.

[Factors affecting the serum thyroid hormone concentration during fasting in rats--with special reference to the relation with temperature].

The present studies examine the effect of starvation together with cold or hot exposure on thyroid hormone levels in rats. At 23 degrees C starved for 5 days, serum thyroid hormone levels decreased significantly compared with fed rats, averaging 3.6 +/- 0.5 micrograms/dl of thyroxine (T4), 47 +/- 11 ng/dl of triiodothyronine (T3), 1.4 +/- 0.3 ng/dl of free T4 and 39.6 +/- 5.1 pg/ml of reverse T3, respectively. At 15 degrees C rats starved for 5 days, serum free T4 level significantly more increased than that of 23 degrees C starved rats, while serum T4 level and T3 did not increase significantly. At 30 degrees C rats whether concomitant starvation or not, serum thyroid hormone levels of both group markedly more decreased than control rats. These experiment provide additional evidence that thyroid gland and the peripheral metabolism of thyroid hormone respond to variety situations such as cold or hot exposure together with starvation or not.

Influence of cigarette smoke on antipyrine metabolite formation in rats.

We examined the effect of cigarette smoke in rats on antipyrine (AP) disposition in blood and AP metabolites in urine and compared these results with those after pretreatment with tar, 3-methylcholanthrene (3-MC), and phenobarbital (PB) in the same species. In vitro hepatic monooxygenase activities were also examined in smoke-exposed and tar-pretreated rats and compared to saline-pretreated or control rats. Both the smoke-exposed and tar-pretreated rats showed a significant (p less than 0.05) increase in AP clearance as compared to the control rats. The increases in AP clearance of PB and 3-MC groups were more remarkable (p less than 0.01), being more than three and eight-fold greater than the control group, respectively. The urinary amount of 4-hydroxyantipyrine (4OHA) was significantly (p less than 0.05) greater and that of norantipyrine (NORA) and AP less (p less than 0.05) in the smoke-exposed rats than in the controls. This trend was not observed in the tar-pretreated rats. In the 3-MC-treated rats a significant (p less than 0.05) increase in the urinary amount of 4OHA was found, while that of 3HMA was significantly (p less than 0.01) diminished without a change in AP or NORA in the urine. In the PB-pretreated rats the trend was similar to the 3-MC group except that the amount of unchanged AP was significantly (p less than 0.05) decreased. In the study of in vitro hepatic monooxygenase activities 7-ethoxycoumarin O-deethylase was significantly (p less than 0.01) increased in the smoke-exposed rats but not in the tar-treated rats. There was no significant increase in the content of cytochrome P-450 or aminopyrine N-demethylase in the two groups examined in the study. These results suggest that the metabolic pathways of AP, a multimetabolized drug, would be affected differently with different experimental inducers and that smoke and tar act somewhat differently on the hepatic monooxygenase system resulting in the different profiles of AP metabolites in rat urine.

[Clinical studies on micronomicin treatment for surgical infections].

The clinical effects of micronomicin (MCR, Sagamicin) treatment for 10 patients with surgical infections were investigated and following results were obtained. 1. Clinical effectiveness Of 10 patients treated with MCR, the results were good in 7 cases, poor in 3 cases. 2. Side effects of MCR No significant side effects were observed in 10 patients treated with MCR.

Automated high-performance liqiud chromatographic method for the determination of antipyrine and its metabolites in urine. Some preliminary results obtained from smokers and non-smokers.

A simple, accurate and fully automated high-performance liquid-chromatographic method was developed for the simultaneous determination of antipyrine (AP), 3-hydroxymethylantipyrine (3HMA), 4-hydroxyantipyrine (4OHA) and norantipyrine (NORA) in urine. This method requires no extraction step and only one chromatographic run with the use of a reversed-phase system. The coefficient of variation (%) (n = 8 each) was: 4.14 for AP, 2.31 for 3HMA, 3.48 for 4OHA, and 2.71 for NORA. The method was applied to studies on AP metabolism in three smokers and three non-smokers who received an oral 10 mg/kg dose of AP. These preliminary results suggest that smokers appear to excrete more 4OHA and NORA in the urine than non-smokers.
