Effect of adult continuing wh-questions on conversational participation in children with developmental disabilities.

Children with developmental disabilities often converse less frequently than their developmentally matched peers. This low conversational participation can cause problems for the children's future language and discourse development. The purpose of this experimental study was to test the hypothesis that adult topic-continuing wh-questions would elicit topic continuations in children with relatively low language ability, but not in children with relatively high language ability. Twenty-three children with developmental delays interacted with an adult who conducted two play sessions. In each session, the adult used a different interaction style. The two styles differed in the adult's use of topic-continuing wh-questions. Results indicate that adult use of topic-continuing wh-questions supported the use of child continuations in children at all language levels. The type of continuations (single word versus multiword) that were elicited depended on the language level of the children. Clinical implications are discussed.

Adult interaction style effects on the language sampling and transcription process with children who have development disabilities.

A determination was made of whether an empirically derived interaction style improves the usefulness of the language sampling and transcription in 17 children with developmental disabilities in Brown's (1973) Stages I and II. Whether the interaction style affected the diversity and complexity of the language the children produced during the samples was also examined. All subjects experienced two 20-minute interaction sessions that differed according to whether topic-continuing wh-questions were used. Results indicated that, regardless of order of exposure to the styles, children talked more often and produced proportionally more transcribable utterances. In addition, they produced a larger sample of productive vocabulary in the style using topic-continuing wh-questions. There were no style effects on vocabulary diversity or length of utterance. The importance of using interaction styles that maximize the information available from language samples of young children with disabilities was discussed.

Case report: the use of indirect composite for the management of extensive erosion.

Erosive tooth surface loss due to acidic soft-drinks has been well documented. Unfortunately, the erosive danger, especially to young patients, of low calorie beverages seems less well appreciated. A clinical case is reported to highlight the problems that can occur with excessive exposure to such beverages. A method of restoring the damaged dentition using indirect composite is described. It is concluded that indirect composite may be particularly suited for the restoration of eroded dentitions, especially in young patients where aesthetics and repairability are important. Further clinical studies are necessary to assess the performance of these and other materials currently used in the management of erosive tooth wear.

Graded pelvic muscle exercise. Effect on stress urinary incontinence.

The purpose of this research was to determine the effect of pelvic muscle exercise (PME) on stress urinary incontinence in middle-aged and elderly women. The protocol required 16 weeks of exercise three times per week, for a total of 48 sessions. Measures were taken at baseline, after a 4-week control period and after 4, 8, 12 and 16 weeks of PME, corresponding to PME levels 1, 2, 3 and 4, respectively. Urine loss was evaluated using a urinary diary and 24-hour pad test at baseline, after the control period and after PME levels 2 and 4. Pelvic muscle function was evaluated every four weeks. Sixty-five parous women 35-75 years of age (mean, 51.3) with mild to moderate stress urinary incontinence were studied. No significant changes in the outcome variables were found after the control period. The results showed significant reductions in urine loss on the 24-hour pad test (t = -4.7, P < or = .0001), and episodes of urine loss decreased from 2.6 to 1.0 between the control period and PME level 4. In addition, a significant improvement occurred in pressures developed by the pelvic muscles in response to PME (t = 6.8, P < or = .0001). Significant correlations between changes in pelvic muscle pressure and in urine loss variables were not found. Women who completed the study experienced a significant reduction in the amount of urine loss as well as a significant increase in pelvic muscle characteristics.(ABSTRACT TRUNCATED AT 250 WORDS)

The reliability of physical examination in the evaluation of penetrating extremity trauma for vascular injury: results at one year.

All cases of penetrating extremity trauma (PET) seen at an urban trauma center were prospectively studied to determine the accuracy and safety of physical examination as the sole mode of evaluation for vascular injury. All patients with PET producing obvious or "hard" signs of vascular injury underwent immediate surgery. All asymptomatic proximity wounds were observed in hospital for 24 hours before discharge to outpatient followup. Patients with non-proximity wounds were discharged immediately. Patients with shotgun wounds and thoracic outlet injuries also underwent arteriography. Of 2,674 trauma patients evaluated during the 1-year study period, 310 (11.6%) had 366 penetrating extremity wounds, most (71%) occurring in the lower extremities. Gunshots caused most (82%) of the wounds, followed by stabs or lacerations (14.5%), and shotguns (3%). Clinically occult wounds in proximity to major limb vessels were the most common (78%) category of clinical presentation, followed by asymptomatic non-proximity wounds (16%), and wounds producing hard signs (6%). There were two missed vascular injuries, both in the asymptomatic proximity group (0.7% false negatives). Every patient taken immediately to surgery for hard signs had major arterial injury requiring repair, for a 100% positive predictive value for physical examination. No mortality or morbidity were related to protocol management. These results to date support prior reports of a negligible incidence of significant vascular injury following clinically occult proximity PET, and further suggest that the overall predictive value of physical examination of PET for vascular injury approaches 100%.

Intraperitoneal sensitization and the transplanted mouse heart.

Intradermal or skin-graft sensitization always results in accelerated rejection of subsequent MHC-disparate mouse heart transplants; intravenous or intrasplenic sensitization almost always results in prolonged survival. The survival time after intraperitoneal sensitization is unpredictable: hyperacute or accelerated rejection, as well as normal or prolonged survival, occurs suggestive of a delicate balance between suppressive and rejecting immune responses. We have observed a positive correlation between transplant survival and the distribution of 51Cr-labeled antigenic cells early after i.v., i.d., i.p., and intrasplenic injection. Label recovery from the viscera was already high 2 hr after i.v. injection, while after i.d. injection virtually all label was still confined to the carcass, even at the end of a 24-hr period. After i.p. injection label recovery varied significantly from mouse to mouse, from very low to a level approximating that seen after i.v. injection. This close correlation suggests that the fate of a subsequent transplant is decided within a few hours after antigen deposition, well before placement of the transplant itself.

Isolation and characterization of a new Cytophaga species implicated in a work-related lung disease.

A yellow-pigmented, gram-negative, gliding bacterium isolated from an industrial water spray air humidification system was implicated as a causative agent in several occurrences of lung disease with hypersensitivity pneumonitis-like symptoms. The bacterium, designated WF-164, lacked microcysts or fruiting bodies and had a DNA base composition of 34.8 mol% of guanine plus cytosine. Gliding, flexing, nonflagellated cells measuring 0.3 by 3.5 to 8.9 micron were observed by using light and electron microscopy. Tests to determine utilization of selected carbohydrates revealed an amylolitic, chitinoclastic, noncellulytic bacterium. A number of additional biochemical and physiological tests were performed. DNA homology studies detected a 77.8% similarity to Cytophaga aquatilis (ATCC 29551). Comparisons of cellular fatty acid and carbohydrate contents of isolate WF-164 with a Flexibacter sp., several Cytophaga spp., and Flavobacterium reference strains revealed similar patterns to that of C. aquatilis. On the basis of these characteristics, isolate WF-164 was identified as a new Cytophaga sp.

Bacterial endotoxin isolated from a water spray air humidification system as a putative agent of occupation-related lung disease.

Outbreaks of hypersensitivity pneumonitis or humidifier fever were attributed to the inhalation of organic material aerosolized by a chilled-water spray humidification system. The purpose of this study was to isolate and characterize the serologically detectable antigen(s) present in extracts obtained from the humidification system. By using bicarbonate or phenol-water extractions or both, the antigen was isolated and characterized, using colorimetry, gas-liquid chromatography, reverse-phase high-performance liquid chromatography, and X-ray fluorescence. Carbohydrates, hexosamines, phosphorus, and even-numbered saturated and unsaturated fatty acids were constituents of the serologically detectable antigen. When tested in in vivo and in vitro assays, the antigen had demonstrable endotoxin activity. All subjects with biopsy-proven lung disease and a majority of subjects suspected of having lung disease had antibodies directed toward the purified endotoxin. The data strongly suggest that an aerosolized bacterial endotoxin is a putative agent inducing lung disease.

A Cytophaga species endotoxin as a putative agent of occupation-related lung disease.

A previous study suggested that a biologically active bacterial endotoxin was a putative agent of lung disease in a textile-producing facility. The endotoxin was isolated from the biomass growing in a chilled-water spray air humidification system. The bacterial flora of the air humidification system were isolated and taxonomically identified to the genus level. By using indirect immunofluorescence assays, a serologically reactive Cytophaga species was identified. A serologically reactive, biologically active (Limulus assay) endotoxin was purified from phenol extracts of the Cytophaga species. The endotoxin contained sugars, hexosamines, and lipids identical to those found in the humidifier biomass endotoxin. All subjects with biopsy-proven and suspected lung disease had antibodies directed toward the purified Cytophaga endotoxin. The data suggest that the Cytophaga endotoxin is the putative agent of lung disease in the textile facility.

The burden of smoking-attributable mortality among African Americans--Indiana, 1990.

Recent trends in smoking prevalence in Indiana indicate a slow decline in smoking prevalence among adults aged 18 years of age and under. Despite this decline, the smoking prevalence has remained consistently higher among Blacks compared to Whites. The purpose of this investigation was to evaluate the extent of mortality due to cigarette smoking in 1990 among Blacks and Whites in the state of Indiana. Estimates of smoking-attributable mortality (i.e., smoking-attributable mortality [SAM] and years of potential life lost [YPLL] associated with smoking in 1990 were calculated using SAMMEC, a software program developed by the Centers for Disease Control and Prevention and designed to measure the disease impact associated with cigarette smoking. Although the SAM and YPLL for Whites accounted for the greatest percentage of total SAM and total YPLL, SAM rates and YPLL rates were higher among Blacks compared to Whites, for both men and women. The SAM rate for Black men was 4% higher than that of White men and 9% higher for Black women compared to White women. Similarly, the YPLL to life expectancy rate for Black men was 7% higher than the YPLL rate for White men and 29% higher for Black women compared to White women. The greater burden of tobacco-related mortality among Blacks is evidenced through higher SAM and YPLL rates compared to Whites. It is imperative that high-risk populations be targeted at national, state, and local levels through anti-tobacco campaigns.

Surveillance for posttransfusion hepatitis by a community hospital. Another view.

A critical appraisal of a recent AABB recommendation for hepatitis surveillance was undertaken. As part of the program, a formal, mailed inquiry was sent to physicians whose patients were transfused six months earlier. The replies did not provide an effective means of detecting posttransfusion hepatitis, and cost of the program was high. Although the method served as a useful reminder to physicians that transfusion-associated hepatitis is a reportable disease, caution is warranted in implementing such screening.
