Effect of digital rectal examination on serum concentration of prostate-specific antigen.

Prostate-specific antigen (PSA) is used for screening and follow-up of patients with prostate cancer. The effect of certain diagnostic procedures on PSA serum levels is not well defined. Therefore, the effect of digital rectal examination (DRE) on PSA serum levels was investigated. No significant difference was observed in PSA values before and after DRE when blood samples were taken 1-3 min after palpation of the prostate. Kinetic studies demonstrated a significant increase in PSA values up to the factor 3.2 in 14 of 19 patients 2-6 h after DRE. In 9 of 14 cases, PSA was within the initial range 24 h after DRE. That means that a urologist is allowed to take blood samples for the determination of PSA at least within 3 min after palpating a suspicious prostate without getting false-positive results.

[Diagnosis of lung sarcoidosis in the hospital].

To confirm the diagnosis of sarcoidosis a characteristic X-ray and a positive histologic pattern is essential. In most cases one can get the histology by a rather simple bronchoscopy. In cases of negative bronchoscopic findings one should perform a mediastinoscopy, which can be done by an expert thoracic surgeon almost on outpatient basis. In rare cases the final diagnostic step is the open lung biopsy, preferably done on the right thoracic side, as the lung transplanting surgeons recommend for a possible lung transplant the left side.

Cell culture studies on neurofibromatosis (von Recklinghausen). VI. No increased stimulation of cell proliferation by sera from neurofibromatosis patients.

The growth-stimulating activity of sera from 10 patients with von Recklinghausen neurofibromatosis (NF-1) was compared with that of sera from 10 age- and sex-matched healthy donors. Cell cultures derived from peripheral neurofibromas and from skin biopsies of healthy persons were used as indicator cells. Stimulation of 3H-thymidine incorporation increased with the clotting time of the blood samples, reaching a plateau at about 35 h with no decline of activity during the following 48 h. There was no difference between the stimulating activity of sera from the NF-1 patients and that of sera from the healthy donors with either type of indicator cells. This held true when the indicator cells were stimulated during the exponential growth phase or when serum-starved confluent cells were exposed to the various sera. Three NF-1 strains used as indicator cells during this investigation showed a significantly less vigorous response to both types of human sera and to fetal calf serum than the respective control strains. All human sera collected after 42 h clotting time were superior to fetal calf serum with both types of indicator cells.

[Significance of 17-hydroxyprogesterone for the diagnosis of ovulation].

In 30 sterility patients with normal course of the cycle, 17 OHP already rises significantly one day before the LH-peak. It influences not only progesterone and E2 but also hypothalamic opiate activity and LHRH and LH secretion. 17 OHP is on the one hand discussed as a further trigger for the LH peak and is on the other hand regarded as an initiator for incipient luteinization.

[Interaction of plasma beta-endorphin and progesterone in the luteal phase].

Recent research indicates that opioid peptides may be participants in the coordination and control of the menstrual cycle. The peripheral ovarian steroids (progesterone and estradiol) seem to modulate endogenous opioid activity and thereby hypothalamic LH-RH-secretion. This mechanism works during the luteal-phase under the influence of progesterone. The purpose of this study is to investigate the interaction of endogenous opioid peptides and peripheral ovarian steroids in 24 women by comparing progesterone and beta-endorphin levels of unstimulated and HMG-stimulated cycles. The women with stimulated cycles showed significantly higher progesterone and beta-endorphin levels. The interaction of progesterone und beta-endorphin may partly be explained by the fact that progesterone acts on progesterone receptors in the central nervous system. This leads to activity of opioid neurons in the brain and finally influences LH-RH- and LH-release. Beta-endorphin production is also discussed to come from the ovary. Beta-endorphin is involved maintaining normal luteal phase and therefore plays an important role in reproduction.

[Determining the average posteruptive age of teeth from the results of cross-sectional studies (author's transl)].

If results of cross-sectional studies must be used as a basis for setting up models of development of caries in permanent teeth, then the average rather than individual posteruptive age of teeth has to be resorted to. Data available for this purpose had been obtained from a total of 54,000 children living in two G.D.R. cities, namely, Plauen and Karl-Marx-Stadt, as well as from two surveys made in 1959 and 1971, respectively. Calculation of the average posteruptive age of teeth assumes good knowledge of the distribution of dates of eruption in a population. Of the three types of distribution considered, namely, normal, lognormal, and log-logistic distribution, the log-logistic model proved to be superior to others because of better adaptation to the data available. The method of calculating the posteruptive age of teeth is described and illustrated by use of a number of examples. Since one of the two surveys (namely that which was made in Karl-Marx-Stadt in 1971) was made after twelve years' drinking water fluoridation, it was possible to study the effect of fluoridation upon the eruptive behavior of permanent teeth. The results of this study confirmed those obtained during previous investigations.

[Effect of sodium fluoride on the metabolism of the rat kidney cortex and medulla].

Tissue sections of cortex and outer and inner medulla of rat kidneys were examined in the Warburg apparatus with and without addition of NaF. It was shown that the O2 consumption was not significantly reduced in the presence of 0.05 mM F (=0.95 ppm F!) or smaller amounts. The respiratory quotient was not changed by NaF. Lactate formation in sections from the inner medulla was reduced to the same degree as O2 consumption. Maximal concentrations of plasma fluoride which are met in doses of fluoride used for the prophylaxis of caries, do not impair renal metabolism.

Distribution of caries among tooth types in fluoride and non-fluoride areas.

A method is described for estimating the preferential caries protective action of fluoride on certain morphological types of permanent teeth. The results of an analysis by this method, applied to data for 1164 Austrian school-children aged 6--14 years, are reported. There was no indication that fluoride would exert its caries inhibitory effect selectively on certain types of teeth. Tentative explanations are put forward for the only inconsistency within the general results observed in 11--14-year-old children having a total of 3 carious teeth.

[Indications for proton spin tomography in central nervous system diseases in childhood. A 4-year experience report on 164 cases].

1. Magnetic Resonance Tomography (MRT) is the method of choice for precise, comprehensive and reproducible imaging of congenital malformations and space occupying lesions. 2. MRT indicates inflammatory processes of the CNS earlier than CT by which extension and localization are visualized more precisely. MRT gains particular importance for follow-up studies because there is no radiation burden. In cases of disseminated inflammatory lesions or concerning early diagnosis of Herpes-encephalitis an exact and rapid diagnosis is possible. 3. In patients suffering from convulsions with or without focal lesions in EEG MRT instead of CT should be applied as MRT allows three-dimensional imaging not influenced by bone artifacts. 4. CT is superior to MRT if small calcifications or acute hemorrhage are looked for. MRT enables us to differentiate several stages of bleeding. 5. In patients with cerebral AV-malformations angiography can be replaced by MRT. However, angiography is indispensable for small aneurysms and AV-malformations in the spinal canal. 6. Due to high soft tissue contrast MRT is the imaging modality of choice in patients with metabolic and degenerative diseases and disturbances of brain maturation. 7. In infants sonography should be done first and supplemented by MRT and/or CT.

Adolescent

[Endocrine changes following progesterone substitution in early pregnancy].

250 mg hydroxy-progesteronecaproate was used for intramuscular administration in 25 patients, between 11 and 15 week's gestation, presenting either with a history of recurrent abortion or being admitted because of threatened abortion. Beta-HCG and progesterone concentrations were determined twice at intervals of 24 hours. Three women aborted without showing any increase in Beta-HCG levels nore in progesterone levels. In 20 patients Beta-HCG concentrations increased significantly after the medication, whereas only 5 patients showed a rise in progesterone levels. In 14 patients serum progesterone concentrations decreased making the use of progestational steroids in the management of threatened abortion questionable.
