[Percutaneous nephrostomy].

Sixty three cases (male 32, female 31) of supravesical obstruction were treated with ultrasonically guided subcutaneous nephrostomy at our hospitals from October 1980 to October 1984. Their age ranges from 2 months to 84 years old, and median age was 52 years old. The major causes of ureteral obstruction were extended pelvic malignancies, 45 cases (71%) (gynecological 18, urological 16, gastrocolorectal 7 and others 4), and benign disease 18 cases (29%). Objectives of nephrostomy were classified in 4 categories (drainage, manipulation, irrigation and evaluation), and results of nephrostomy were discussed for each category. No major complications were observed. Only prolonged macrohematuria (over 48 hours) and fever (over 38 degrees C) were observed in 5 cases respectively. Success rate of nephrostomy placement was 97.5% of all cases. With ultrasonography and angiographic technique, percutaneous nephrostomy is an easy, safe and reliable method.

[Effect of UFT on urinary bladder tumors in rats induced by N-butyl-N-(4-hydroxybutyl)nitrosamine].

The effect of combined tegafur and uracil (UFT) on the development of rat urinary bladder tumors induced by N-butyl-N-(4-hydroxybutyl)nitrosamine (BBN) was studied. Two hundred F344 male rats were divided into 10 groups. Groups 1 to 5 were given 0.05% BBN in drinking water for the initial 8 weeks of the experiment, and Groups 6 to 10 were the controls of the prior 5 groups treated with BBN. UFT in commercial diet was administered at daily doses of 100mg (30.9 mg as FT-207) per kg of body weight in Groups 2 and 4 and 200 mg (61.7 mg as FT-207) per kg of body weight in Groups 3 and 5. Groups 2 and 3 received UFT throughout the period of the experiment, and Groups 4 and 5 for 12 weeks after 8 week treatment with BBN. All animals were sacrificed at 20 weeks, and studied histopathologically. In Groups 1 to 5, urinary bladder tumors developed in 20 of 20, 13 of 20, 6 of 20, 14 of 20 and 6 of 20, respectively. Incidences of tumors in the 4 groups treated with UFT were significantly lower than that in Group 1 treated with BBN alone. This result shows that UFT inhibits the development of urinary bladder tumors in rats induced by BBN.

[Fundamental studies on intravesical instillation of cis-diamminedichloroplatinum for treatment of urinary bladder tumors. I: On the effects of intravesical instillation of cis-diamminedichloroplatinum in normal beagle dogs].

Cis-Diamminedichloroplatinum (II) (DDP) is one of the most effective agents in the treatments of advanced bladder cancer, but little is known of its local and systemic toxicity when it is instilled intravesically. The objectives of this experiment were to study the local and systemic toxicity of intravesical DDP in order to know whether intravesical DDP is applicable for prophylaxis and/or definitive treatment of superficial bladder cancers. Six female Beagle dogs were used for the 48-hour retention study and 2 mongrel dogs were used for the 3-hour study. Plasma and bladder platinum concentration were measured following intravesical DDP, and also histopathological examination, urinalysis, complete blood count and blood chemistry were performed in order to know the toxicity of intravesical DDP. Plasma platinum concentration was not elevated significantly and no remarkable changes were observed in blood examinations after intravesical DDP. Bladder tissue platinum concentration was significantly elevated up to 16.96 micrograms/g . wet after 3 hours DDP retention and 4.02 +/- 2.8 micrograms/g . wet after 48 hours retention in dogs. At present, intravesical instillation of DDP seems to have minimum local toxicity and no systemic toxicity, but further study is necessary for long term topical instillation of DDP.

[Corticosteroid as an anti-emetic agent in cancer chemotherapy with cis-diammine-dichloroplatinum (II)].

The anti-emetic effect of corticosteroid during cancer chemotherapy with cis-diamminedichloroplatinum (II) (CDDP) is analyzed retrospectively. From November 1981 to April 1984, 23 of 50 patients receiving CDDP were given corticosteroid as an anti-emetic. Corticosteroids, methylprednisolone (80-500 mg/day) or hydrocortisone (200-480 mg/day), were administered intravenously for 1 to 3 doses. Including 4 patients who had no symptoms, 60% of patients were free from nausea and vomiting. The incidence of gastrointestinal symptoms was significantly lower than that in patients who were not given corticosteroid. Corticosteroid reduced gastrointestinal symptoms in 2 of 3 patients who had prior CDDP treatment. There were no remarkable adverse reactions except acne in 2 patients. A further controlled study is needed to determine the appropriate dose and modality of administration of corticosteroid.

[Fundamental studies on intravesical instillation of cis-diamminedichloroplatinum for treatment of urinary bladder tumors. II. On the effects of intravesical instillation of cis-diamminedichloroplatinum in bladder cancer patients].

This study was performed to determine whether intravesical Cis-Diamminedichloroplatinum (II) (DDP), one of the most effective agents in the treatments of advanced bladder cancer, is applicable for prophylaxis and/or definitive treatment of superficial bladder cancers. Six patients were instilled 10 mg/20 ml of DDP immediately after TUR-Bt, but no remarkable elevation of plasma platinum concentration was observed. In one patient planned to have total cystectomy, 20 mg/40 ml of DDP was instilled daily for 10 days. No remarkable changes in symptom and blood examination was found during the instillation periods. Transitional cell carcinoma of grade 2 was necrosed extensively, but minimum cellular injury was observed in normal mucosa. Tissue platinum concentration was 3.19 micrograms/g. wet in carcinoma and 0.28 microgram/g. wet in normal mucosa. Longterm prophylactic instillation of 20 mg/40 ml of DDP was performed in 16 patients starting immediately after TUR-Bt. As the side effects of DDP instillation, contracted bladder was observed in one patient and allergic reactions, such as flush of the skin and dyspnea, were observed in 4 cases. These allergic reactions were happened suddenly after 15 approximately 19 times of DDP instillation without any prelude. At present, intravesical instillation of DDP seems to cause minimal irritation of bladder and absorption through the bladder wall, but clinical trials of intravesical DDP should be taken under careful observation until the allergic reaction by intravesical DDP are clarified.

[A case of congenital mesoblastic nephroma growing with a polypoid pattern in the renal pelvis and ureter].

Congenital mesoblastic nephroma (CMN) is an unusual renal tumor, similar in some respects to Wilms' tumor and reported only in infants. We report a case of CMN in a 2-year-old boy. He was referred to us with a right abdominal mass. Excretory urogram, arteriogram and computed tomogram revealed a large malignant renal tumor on the right side. With a clinical diagnosis of Wilms' tumor of the right kidney, he received radiation and chemotherapy. After radiation and chemotherapy, tumor size was decreased. Right nephrectomy was performed on April 22, 1982. The kidney and tumor weighed 270 g and measured 12 X 7 X 5 cm. The tumor extended to the renal pelvis and the ureter with polypoid growth pattern. Microscopically, the tumor was composed of compact interlacing bundles of elongated fibromyomatous cells. Islands of entrapped glomeruli and tubules were also detected within the tumor. He was well and had no signs of recurrence of tumor at the age of 3 years.

[Experience of renal autotransplantation].

We present 2 cases with renovascular hypertension treated by renal autotransplantation and 3 cases with extensive renal calculi treated by renal bench surgery and autotransplantation. The cases with renovascular hypertension were due to fibromuscular dysplasia in the trunk of renal artery, and return to normotension and improvement of renal function were obtained within a few days after the operation. In 2 of the cases with extensive renal calculi, the contralateral kidney was contracted. In all cases, the stones were removed completely and urinary tract infection persisting before operation was eradicated. The postoperative renal function was well preserved and no recurrence of renal calculi was observed throughout the follow up period. The indication of renal bench surgery and autotransplantation was discussed both for renal calculi and renovascular hypertension.

Rapid induction of carcinoma in situ in dog urinary bladder by sequential treatment with N-methyl-N'-nitrosourea and N-butyl-N-(4-hydroxybutyl)-nitrosamine.

A highly atypical intraepithelial proliferation, which was interpreted as carcinoma in situ, developed in the urinary bladder of 2 beagle dogs after repeated intravesical instillations of N-methyl-N'-nitrosourea followed by oral administration of N-butyl-N-(4-hydroxybutyl)nitrosamine. The latent period was 37 weeks. The technique may provide a useful model to study the natural history of bladder cancer, particularly carcinoma in situ.

A minimal dose of N-methyl-N-nitrosourea carcinogenic to heterotopically transplanted rat urinary bladder.

The present investigation was conducted to determine a single dose of the carcinogen N-methyl-N-nitrosourea (MNU) which remains subcarcinogenic in a heterotopically transplanted rat urinary bladder with a communicating reservoir (HTB) either in a urine-free or urine-existing environment. A single dose of MNU, either 0.1 mg or 0.025 mg, was instilled directly into the HTB, and was followed by weekly instillation of normal rat urine or 2.1% NaCl solution (equiosmolar to the urine) for up to 52 weeks. Bladder tumors observed were divided into 2 categories depending upon their location. Those arising at the hyperplastic foci which were induced by mechanical irritation by the connector tip were referred to as inflammatory polyp (IP)-related tumors and those arising in the region free from the IP changes were referred to as IP-unrelated tumors. The results indicated that both carcinogen levels fail to induce IP-related or IP-unrelated tumors in the urine-free environment. In the presence of urine, however, the high MNU dose (0.1 mg) was tumorigenic at both IP-related and IP-unrelated sites, whereas, the low MNU dose exhibited tumorigenicity only at the IP-related site. Physical irritation by the connector enhanced MNU-initiated tumorigenesis at the connector tip only in the presence of urine in bladder lumen. Physical irritation per se induced tumors at the connector tip even without prior carcinogen treatment provided urine was present in the HTBs.

[Fundamental studies on intravesical instillation of carboquone for treatment of urinary bladder tumors--on the effects of intravesical instillation of carboquone in normal beagle dogs].

The effects of intravesical instillation of Carboquone at the clinically used doses of 5 and 10 mg on the normal mucosa of female beagle dogs was compared with that of 10 mg Mitomycin C used as the control drug. Intravesical instillation for 48 hours of 10 mg carboquone/20 ml phosphate buffer solution (PBS) after bilateral cutaneous uretrostomies produced severe inflammatory changes in all layers of the bladder wall. However, no secondary effects were observed in blood laboratory examinations or histological examinations of the whole organ after autopsy. Phosphate buffer solution produced no remarkable secondary effects in animals. Five milligrams carboquone per 20 ml PBS was instilled intravesically once a week for 3 weeks in normal animals. Cystoscopically , the bladder mucosa recovered normally. Blood laboratory examinations showed no abnormal results, but the bladder epithelium had regenerative epithelial hyperplasia and slightly inflammatory changes in the submucosal layers. Two of the three control animals given instillation of 10 mg of Mitomycin C/20 ml PBS had slight leucopenia at 7 days after the last intravesical instillation, but leucocyte count was normal at the end of the experiment. Cystoscopic and histological examination of the epithelium of the urinary bladder revealed severe inflammatory changes in 2 of the 3 animals.

[Clinical study of urothelial tumors of the upper urinary tract. 1: Primary renal pelvic tumors].

Forty primary renal pelvic tumors treated at our University between 1963 and 1981, were reviewed retrospectively. The conclusions of this study are as follows. Sex and age distribution of the patients were 30 males and 10 females (3: 1), and average age was 60.5 years old. The major symptoms were hematuria and flank pain; however, palpable mass was rare. The majority of patients were admitted to our clinic within 6 months from manifestation of symptoms. The major findings of IVP were non-functioning kidney and filling defect. The positive rate of urinary cytology was 46.7%. Total nephroureterectomy with bladder cuff was performed in 20 out of 32 cases. Histologically, 29 cases were transitional cell carcinoma and 4 cases were squamous cell carcinoma with renal calculi. Simultaneous urothelial tumors were seen in 10 cases, 3 in the ureter and 7 in the bladder. A subsequent ureteral tumor was found in one out of 12 cases in which ureters were resected incompletely, and 7 subsequent bladder tumors were found out of 32 cases receiving surgical treatment in the follow-up period. The 5-year survival rate by the actuarial method was 75.9%. Among several factors, grade and stage of the tumor were the most influencing factors for prognosis. An effective method of post-operative treatment could not be established.

[Clinical study of urothelial tumors of the upper urinary tract. 2: Primary ureteral tumors].

Twenty-five primary ureteral tumors treated at our University between 1963 and 1981, were reviewed retrospectively. The conclusions of this study are as follows. Sex and age distribution of the patients were 18 males and 7 females (2.6: 1), and average age was 63.04 years old. The major symptom was hematuria. The majority of the patients were admitted to our clinic within 6 months from manifestation of symptoms. The major finding of IVP was non-functioning kidney. The positive rate of urinary cytology was 63.2%. Total nephroureterectomy with bladder cuff was performed in 21 out of 23 cases. Histologically, 22 cases were transitional cell carcinoma and one case was squamous cell carcinoma. Simultaneous urothelial tumor was found in 13 cases in the bladder. Most of the ureteral tumors (63.6%) were found in the lower third segment of the ureter. Subsequent urothelial tumors were seen in 3 bladders and one urethra out of 22 cases receiving surgical treatment in the follow-up period. The 5-year survival rate by actuarial method was 39.4%. Among several factors, grade and stage of tumor were the most influencing factors for prognosis. An effective method of post-operative treatment could not be established.

[Effect of FT-207 on bladder cancer in rats induced by BBN. II: The effect of oral administration of FT-207 during oncogenesis of bladder cancer].

The effect of Tegafur (FT-207) by oral administration on the development of urinary bladder tumors in Wistar strain male rats induced by N-butyl-N-(4-hydroxybutyl) nitrosamine (BBN) was studied. Urinary bladder tumors were induced in 18 of 20 rats (90.0%) when rats were given 0.05% BBN in the drinking water for 8 weeks and then given water without BBN for 12 weeks. When FT-207 100mg/kg B. W./day was given in their diet after treatment with 0.05% BBN for 8 weeks, tumors developed in the urinary bladder with low incidence (9 of 16 rats: 56.3%). Hematotoxicity was not observed in all animals treated with FT-207. These results shows that FT-207 also inhibited the development of urinary bladder tumors treated with BBN in rats by oral administration, which were similar those our previous results showed by intraperitoneal administration of FT-207.

[Effect of carboquone (esquinone) on the development of urinary bladder tumors in rats induced by N-butyl-N-(4-hydroxybutyl) nitrosamine].

The effect of carboquone (CQ) by intraperitoneal administration on the development of urinary bladder tumors in Wistar strain male rats induced by N-butyl-N-(4-hydroxybutyl) nitrosamine (BBN) was studied. Urinary bladder tumors were induced in 9 of 11 rats (81.8%) when they were given 0.05% BBN in drinking water for 8 weeks and then given water without BBN for 12 weeks. When CQ 0.25 mg/kg/day was administered intraperitoneally for 7 days after treatment with 0.05% BBN for 8 weeks, incidence of urinary bladder tumors was 19 of 26 rats (73.1%). When CQ 0.25 mg/kg/weekly was given for 12 weeks after treatment with BBN, incidence of urinary bladder tumors was 17 of 27 rats (63.0%). When CQ 0.5mg/kg. B.W./day was given for 7 days after treatment with BBN, tumors developed in the urinary bladder with low incidence as in 8 of 20 rats (40.0%) (P 0.1). When CQ 0.5mg/kg/weekly was given for 12 weeks after treatment of BBN, urinary bladder tumors were induced in 17 of 22 rats (77.3%). Hematotoxity was not observed in any animals treated with CQ. These results showed that CQ inhibited the development of urinary bladder tumors induced by BBN in rats.

Carcinogenesis in heterotopically-transplanted urinary bladder by N-methyl-N-nitrosourea: a model for initiation and promotion.

The present investigation was conducted to establish a working model to evaluate the role of potential modifiers (promoters and inhibitors) of urinary bladder carcinogenesis. A single dose (0.25 mg) of the carcinogen N-methyl-N-nitrosourea (MNU) when instilled directly into heterotopically transplanted urinary bladder (HTB) induced a 9% (2 of 22) incidence of urinary bladder tumors by 30 weeks in the urine free HTB environment, whereas repeated instillation of urine into the HTB after MNU treatment resulted in a 61% (14 of 23) incidence of these tumors. Three doses of MNU (0.25 mg/dose once a week for 3 weeks) induced 33% (6 of 18) and 89% (16 of 18) incidence of tumors, respectively, in the absence and presence of urine in the HTB. These data confirm our previous observation that normal urine enhances urinary bladder carcinogenesis, and are consistent with the notion that normal urine exerts a tumor promoting effect.
