[A case of localized pleural mesothelioma].

A 68-year-old male with localized pleural mesothelioma was described. The tumor was removed surgically in association with left upper pulmonary lobe and regional lymph nodes, since low grade malignancy had been suggested by preoperative needle biopsy. The operative and macroscopic findings revealed that the tumor was well encapsulated and arise from visceral pleura of S3 segment of left upper lobe, projecting into pleural cavity as a pedunculated mass. The tumor measured 9 X 6 X 4 cm in diameter. Based on electron microscopic and immunohistochemical findings of the tumor, the origin of tumor cells was discussed.

[Two cases of inflammatory pseudotumor of the lung].

Two cases of inflammatory pseudotumor of the lung were reported. Case 1. A 68 year-old man was referred to our hospital because of an abnormal shadow on chest X-ray. He had a history of pneumonia in the right upper lobe five months before. The chest X-ray film revealed a coin lesion in the right upper lung field (S1), the same segment as the previous pneumonia. Although RPF and INH were administered for three months, the shadow did not change, and cough and sputum continued. In order to confirm the diagnosis, open thoracotomy was performed and microscopic findings of the resected tumor showed inflammatory pseudotumor; proliferation of fibrous tissue with infiltration by inflammatory cells (plasma cells, lymphocytes and a few neutrophils). Case 2. A 35 year-old man was admitted to our hospital because of an abnormal shadow on chest X-ray, i.e. a coin lesion with vascular indentation in the left lower lung field (S8). A wedge resection including the mass was performed, and histopathologic examination revealed inflammatory pseudotumor, or plasma cell granuloma; proliferation of fibrous connective tissue with infiltration by predominantly mature plasma cells. Both patients have been doing well after the operation. We also reviewed 46 cases reported in the Japanese literature and discussed various aspects of this disease. Of 10 patients who had a history of previous respiratory tract infection, 8 had histopathologic features with various inflammatory cells, including many lymphocytes, as in our Case 1. Although the common etiology of inflammatory pseudotumor is obscure, we suppose that in some cases the lesion may be a result of post-inflammatory repair process.

[Development of a system for ultrasonic three-dimensional reconstruction of fetus].

We have developed a system for ultrasonic three-dimensional (3-D) fetus reconstruction using computers. Either a real-time linear array probe or a convex array probe of an ultrasonic scanner was mounted on a position sensor arm of a manual compound scanner in order to detect the position of the probe. A microcomputer was used to convert the position information to what could be recorded on a video tape as an image. This image was superimposed on the ultrasonic tomographic image simultaneously with a superimposer and recorded on a video tape. Fetuses in utero were scanned in seven cases. More than forty ultrasonic section image on the video tape were fed into a minicomputer. The shape of the fetus was displayed three-dimensionally by means of computer graphics. The computer-generated display produced a 3-D image of the fetus and showed the usefulness and accuracy of this system. Since it took only a few seconds for data collection by ultrasonic inspection, fetal movement did not adversely affect the results. Data input took about ten minutes for 40 slices, and 3-D reconstruction and display took about two minutes. The system made it possible to observe and record the 3-D image of the fetus in utero non-invasively and therefore is expected to make it much easier to obtain a 3-D picture of the fetus in utero.

[A study of the proliferative activity of DNA polymerase alpha immunoreactivity in breast cancer].

DNA polymerase alpha activity is associated with cell proliferation, independently of the cell cycle phase, and a simple and reproducible method for the immunohistochemical demonstration of DNA polymerase alpha has been developed. In an analysis of a total of 76 human breast cancer tissues using this procedure, the clinico-pathological findings revealed that only the estrogen receptor (ER) significantly correlated with the DNA polymerase alpha activity. Therefore, this suggests that ER-negative tumors had a higher proliferative activity.

[Unilateral pulmonary vein atresia].

A case of unilateral pulmonary vein atresia in a 20-month-old girl is described in this report. A chest X-ray revealed a reticular shadow on the right side, while an electrocardiogram revealed right ventricular hypertrophy. After cardiac catheterization and angiography, a diagnosis of a ventricular septal defect, patent ductus arteriosus, severe pulmonary hypertension, and atresia of the right pulmonary vein was made. The patient underwent an operation for the ventricular septal defect and ligation of the patent ductus arteriosus. During the operation, the right pulmonary vein was found to be atretic, and unrepairable. Pneumonectomy was not performed because the pulmonary arterial pressure decreased to 41 mmHg after the surgical treatment of the intracardiac lesions. Pneumonectomy will be necessary if one or more complications such as persistent hemoptysis, repeated pulmonary infections or the development of pulmonary hypertension, may occur in future.

[A case of a small hepatoma with multicentric development in precirrhotic fibrosis].

Multicentricity of hepatocellular carcinoma (HCC) is considered, especially in patients with liver cirrhosis. This paper describes an operative case of a male patient with probable multicentric development of HCC in precirrhotic fibrosis. The main tumors inside the capsule were completely necrotic due to transarterial embolization (TAE). Histologic examination disclosed discrete tiny nodules of HCC that were not detected grossly. They showed highly differentiated trabecular arrangements: Edmondson I. A scirrhous type was noted in the center of the tumors. At the borders of the tumors the carcinoma cells exhibited replacing growth patterns and it was thought that they developed multicentrically. Partial resection must be performed as extensively as possible in a case such as the present one.

Adrenal renin: a possible local hormonal regulator of aldosterone production.

The complete renin-angiotensin system is present in the adrenal cortex: prorenin, renin, angiotensinogen, angiotensin I and II, and converting enzyme. Most of the renin found is probably synthesized there since the renin concentration increases after nephrectomy, and the mRNA for renin is present. The renin-angiotensin system has the highest activity in the zona glomerulosa cells, the site of aldosterone formation. A low-sodium diet or a high-potassium diet, or nephrectomy markedly increases the adrenal renin concentration in the zona glomerulosa cells without any effect on the fasciculata-medullary cells. There is a close correlation between adrenal renin and aldosterone production. The adrenal renin angiotensin system may be a local regulator of aldosterone production.

Detection of antibodies against porcine parvovirus in swine sera by enzyme-linked immunosorbent assay.

An enzyme-linked immunosorbent assay (ELISA) was developed for detection of antibody against porcine parvovirus in swine sera. The antigen used for the assay was partially-purified virus treated with fluorocarbon and shown to contain 7 proteins by sodium dodecylsulfate-polyacrylamide gel electrophoresis. Of these proteins 83-, 64- and 60-K proteins reacted in Western immunoblotting with swine serum after infection with porcine parvovirus. Antibody responses were demonstrated by ELISA in pigs subcutaneously-infected with porcine parvovirus as by hemagglutination-inhibition (HI) test and Western immunoblotting reaction with the 83-, 64- and 60-K viral proteins. The results of ELISA on random swine-serum samples were well-correlated with those of the HI test. These findings indicate the usefulness of the ELISA as a serological tool for porcine parvovirus infection.

The dissociation constant of verapamil estimated from its effect on Ca concentration-tension curves in guinea-pig tracheal muscle.

Concentration-tension curves for calcium ions (Ca2+) were studied in indomethacin-treated guinea-pig tracheal muscle in the presence of different concentrations of carbachol in media containing 5.9 mM K+ or 40 mM K+. The effect of verapamil was investigated taking into account the steepness (the Hill coefficient) of the Ca2+ curve. When carbachol (1 microM) was added to 40 mM K+ solution, the Ca2+ concentration to produce half maximum tension (EC50) was reduced from 0.2 mM to 0.08 mM and the Hill coefficient was increased from 1.4 to 2.0, respectively. In the presence of carbachol (1 microM), the Ca2+ concentration-tension curve was not much influenced by increasing the K+ concentration from 5.9 to 40 mM K+. Verapamil (0.5 microM) shifted the Ca2+ concentration-tension curve to the right in a parallel manner under all experimental conditions, the shift being greater with curves having a smaller Hill coefficient. The dissociation constant of verapamil was not altered by carbachol when estimated from the shift of the curve if the Hill coefficient is taken into consideration. It is concluded that the relatively low susceptibility of carbachol-induced contractions to verapamil in the presence of 40 mM K+, compared with these produced by K+ alone, is not due to a decreased verapamil affinity but to improved Ca(2+)-response coupling.
