Hypercoagulability in cancer.

The association of cancer with a hypercoagulable state is documented by numerous clinical, biochemical, pathologic, and pharmacologic studies. This association is manifested clinically by an increased incidence of intravascular thrombotic events in cancer patients and by fibrin deposition in and around tumor beds. Thromboembolic disease is a major cause of morbidity and mortality in patients with malignancy. This article discusses the complex pathogenesis of this problem and the associated laboratory and clinical syndromes with recommendations on diagnosis and treatment.

Breast cancer in young women: effect of chemotherapy on ovarian function, fertility, and birth defects.

A comprehensive review of the literature was done to assess the effect of adjuvant chemotherapy for operable breast cancer on ovarian function, fertility, and birth defects. Data were limited. Cyclophosphamide, an alkylating agent, is the major cause of amenorrhea, which is due to primary ovarian failure. Ovarian dysfunction is related to age, dose, and duration of treatment. In women less than 35, pregnancy following adjuvant chemotherapy is possible. However, data are limited regarding the impact of subsequent pregnancy on the results of breast cancer. There appears to be no increased risk of teratogenesis in offspring exposed to chemotherapy after the first trimester of pregnancy. Prospective data on women who have subsequent pregnancies and on their offspring are very limited. Formation of a registry for long-term follow-up of young women detailing reproductive potential and follow-up of offspring is needed.

Effects of chloroquine on release of mediators from mast cells.

This study was designed to investigate the effect of chloroquine (CQ) on the release of preformed and generated mediators from rat serosal mast cells (greater than 90% pure). CQ alone (10-1,000 micrograms/ml) induced the release of preformed mediators, while stimulation with calcium ionophore A23187 of CQ-treated cells resulted in suppressed releases of both preformed and generated (particularly prostaglandin D2) mediators. These effects were calcium-independent and reversible. Whether these results partly explain the efficacy of CQ in the treatment of mast cell-mediated dermatoses remains to be further investigated.

Effects of aircraft noise on hearing ability of school-age children.

Two hundred one cases with permanent bilateral high-frequency hearing loss and 208 controls with normal hearing were identified through the New York City hearing screening program. Aircraft noise exposure was estimated for the residences of cases and controls from Noise Exposure Forecast contour maps of the New York City airports. The noise exposure estimates had a correlation on 0.89 with noise level measurement made in the area. Birth certificates and questionnaires were used to provide additional information on other sources of noise exposure and potential confounders. The results showed a positive, but not statistically significant association between aircraft noise exposure and the risk of high-frequency hearing loss.

Effects of aircraft noise on reading ability of school-age children.

The percent of students reading below grade level from 1972 to 1976 was regressed on racial, socioeconomic, educational, and noise level variables for all elementary schools in Brooklyn and Queens, New York. Schools were assigned noise exposure scores based on Noise Exposure Forecast contours for New York City airports. The correlations between these noise scores and a variety of noise level metrics ranged from 0.74 to 0.97. The regression coefficients adjusted for confounding factors, indicated that an additional 3.6% of the students in the noisiest schools read at least 1 yr below grade level with 95% confidence limits from 1.5 to 5.8%. The dose-response relationship indicated that the percent reading below grade level increased as noise level increased.
