Variations in the vascular pedicle of the rat gracilis muscle flap.

The rat gracilis muscle is a reliable model for microsurgical research. Exploration of 364 inguinal regions and preparation of 80 free gracilis muscle flaps in Sprague-Dawley and Lewis rats revealed six distinct origins of the dominant arterial pedicle and four variations of the venous pedicle. Popliteal, external iliac, and truncus pudendoepigastricus associations are described for the first time in this study. The total incidence of atypical variation of arterial pedicle origin is 20.7 percent. The most common origin of muscular branch arteries are the superficial epigastric artery (53.3 percent), femoral artery (22.0 percent), popliteal artery (14.6 percent), and the external iliac arteries (5.5 percent). Muscular branch veins drain most frequently into the femoral vein (79.1 percent). Gracilis muscle flaps prepared from rats weighing 329 +/- 45 g weighed an average of 0.64 +/- 0.13 g, and their volume averaged 0.87 +/- 0.12 ml. Muscular branch artery and vein diameters (external) averaged 0.21 +/- 0.05 mm and 0.54 +/- 0.06 mm, respectively. This new anatomic information is essential for reproducible microsurgery research using the rat gracilis muscle.

Modified split-level lid resection for blepharoptosis.

Mustardé's split-level lid resection surgery was popular through the 1980s for the correction of blepharoptosis with 7 mm or more of levator function. Although the aesthetic results gained with this technique were good, last line deformity and eyelid margin irregularities, such as central peaking at forward and upward gaze, were experienced. A simple modification of the tarsal resection pattern of Mustardés operation has solved these complications. In the treatment of 24 ptotic eyelids (12 unilateral, 6 bilateral with a modified split-level lid resection procedure), symmetrical appearance and level eyelids were obtained without lid margin peaking at 5 years' follow-up. The only persistent complication was lid lag at down-gaze in 10 eyelids.

Vulva reconstruction with a tissue expander.

A patient with exstrophy of the bladder in whom we performed vulva reconstruction using a tissue-expansion technique and local flaps was presented. The ultimate result was in concert with the anatomic characteristics of normal external genitalia with regard to type of skin, pigmentation, hair pattern, quality of circulation, and sensory supply.

Facial resurfacing in xeroderma pigmentosum with monoblock full-thickness skin graft.

A case of xeroderma pigmentosum with multiple skin tumors on the face that was treated with total excision and replacement of face skin except the eyelids with a monoblock full-thickness abdominal skin graft is reported. The quality and tumor-free features of the monoblock full-thickness skin graft in xeroderma pigmentosum are indicated. Despite the increased morbidity of the donor region, the radical surgical approach advocated here has improved the prognosis in the case reported.

The treatment of burn scar hypopigmentation and surface irregularity by dermabrasion and thin skin grafting.

Scar tissue and leukoderma-type discoloration of the skin due to deep burns are treated by dermabrasion and thin split-thickness skin-graft application. This method was applied to 18 patients on whom the treated lesion sites were located as follows: 8 in the facial area, 9 on the extremities, and 1 on the neck. Adequate repigmentation and flat surfaces were obtained in all patients at the end of 6 months, and results persist at the end of a considerable follow-up period (1 to 4 years). The technique, advantages, disadvantages, and results are discussed.

Congenital lateral cleft palate: a new anomaly?

A case of atypical cleft palate abnormality that had not been identified before in a 9-year-old girl is presented. The cleft was localized laterally and in an oblique position at the soft palate. The patient had cleft palate repair. Finally, she had acceptable soft palate movements and speech.
