A case of congenital esophagobronchial fistula in a 46-year-old woman was described. The patient had presented with repeated attacks of pulmonary infection from her childhood and had been diagnosed pneumonia at least five or six times after the age of 18 years. Preoperative examinations including esophagogram by barium swallow revealed the existence of a fistula between middle thoracic esophagus and left B6 bronchus. The fistula was recognized as Type II of Braimbridge and Keith's classification, and successfully resected with left lower pulmonary lobe. Sixty-eight cases with congenital esophagobronchial fistula in adult including our own case have been reported in Japanese literature and about 70 cases in the literature of western countries. From the analysis of these cases, diagnostic basis on congenital origin of the fistula, useful diagnostic methods, types of the fistula and appropriate surgical procedures were discussed.
Recent remarkable development of the non-invasive diagnostic apparatus such as two dimensional echocardiography, computed tomography and RI angiography improves early diagnosis for secondary cardiac tumors and promotes surgical treatments for them. Five cases of secondary intracardiac tumors underwent open heart surgery using cardiopulmonary bypass with successful removal. Although a case of direct invasion of a malignant thymoma into the pericardium, RA and SVC was lost at ICU after surgery because of massive hemorrhage followed by LOS, another 4 cases, i.e., a 27 year-old female with implantation of LA myxoma into the RA and RV, a case with intracavitary extension of leiomyosarcoma of the IVC, a case of intravenous extension of the uterine leiomyomatosis through the IVC to the pulmonary artery and a case of intravenous extension of chondrosarcoma of the left fibula to the heart are alive actively. Open heart surgery has become acceptable treatment for cases with secondary intracavitary cardiac tumors in order to rescue emergency cases with circulatory obstruction by the tumor mass, to make sure a pathological diagnosis of unknown origin and to promote further chemotherapy and/or radiation therapy after surgery. Our surgical results may support these active treatments for secondary cardiac tumors.

The purpose of present study was to assess the relationship between anaerobic threshold (AT) and performances in three different distance races (i.e., 5 km, 10 km, and 10 mile). AT, VO2 max, and related parameters for 17 young endurance runners aged 16--18 years tested on a treadmill with a discontinuous method. The determination of AT was based upon both gas exchange and blood lactate methods. Performances in the distance races were measured within nearly the same month as the time of experiment. Mean AT-VO2 was 51.0 ml . kg-1 . min-1 (2.837 l . min-1), while VO2 max averaged 64.1 ml . kg-1 . min-1 (3.568 l . min-1). AT-HR and %AT (AT-VO2/VO2 max) were 174.7 beats . min-1 and 79.6%, respectively. The correlations between VO2 max (ml . kg-1 . min-1) and performances in the three distance races were not high (r = -0.645, r = -0.674, r = -0.574), while those between AT-VO2 and performances was r = -0.945, r = -0.839, and r = -0.835, respectively. The latter results indicate that AT-VO2 alone would account for 83.9%, 70.4%, and 69.7% of the variance in the 5 km, 10 km, and 10 mile performances, respectively. Since r = -0.945 (5 km versus AT-VO2) is significantly different from r = -0.645 (5 km versus VO2 max), the 5 km performance appears to be more related to AT-VO2 than VO2 max. It is concluded that individual variance in the middle and long distance races (particularly the 5 km race) is better accounted for by the variance in AT-VO2 expressed as milliliters of oxygen per kilogram of body weight than by differences in VO2 max.
Right bundle branch block (RBBB) has been the most frequent conduction disturbance in postoperative patients with a large ventricular septal defect. It has long been considered as an insignificant postoperative sequela. However, recent documentation of its possible deleterious effects on cardiac performance led us to modify the standard technique in an attempt to avoid the postoperative RBBB. In addition to approaching the defect through the right atriotomy, special care was taken for suture placement at the inferior margin and the antero-inferior corner of the defect of defect of perimembranous type. With this technical modification, the frequency of surgical RBBB in patients with a large perimembranous defect has decreased from 31.3 to 8.3 per cent.
A rare case of upbeating nystagmus, which occurred under the condition of a loss of visual fixation, due to anticonvulsant intoxication is reported. This patient, a 29 year old male, suffered from grand mal epilepsy for about 12 years, and he had been treated with anticonvulsive drugs. Although a neurological evaluation by a neurosurgeon could suggest the presence of a vague cerebellar sign, neuro-otological evaluation disclosed diverse abnormal findings which clearly demonstrated the presence of dysfunction in the cerebellar system, especially of the verminal and flocculonodular lobe lesion. This may indicate that the anticonvulsive drug affected the vermis and flocculonodular lobe much more than the hemisphere of cerebellum, or that the cerebellar system sign in verminal and flocculonodular lobe lesion and its connection is detected more easily than the hemispheric lesion with the aid of a neuro-otological examination. It was presumed that the upbeating nystagmus in this case was elicited from the lesion of the anterior vermis of the cerebellum and/or its connections. Usefulness and sensitivity of the neuro-otological examination in the field of dysequilibrium of central disorder as well as peripheral vestibular disorder is emphasized.
By careful and aggressive managements throughout the follow-up period, the actuarial survival rate of the patients who underwent valve replacement was improved to 88.5% after 5 years and 83% after 10 years. Hereafter, it is expected that the over-all survival rate will be improved much more by the progress of the treatment in the early stage following the operation.
Response rates and survival times were studied in 47 patients who had multiple myeloma and who were being treated with Prednisolone and sequential Melphalan and Ifosfamide (MIP therapy). The clinical response was determined by objective parameters such as the reduction of M-protein level, tumor volume and healing of bone destruction. Twenty-eight of the patients (59.6%) responded to the MIP therapy. The 50% survival time as followed from the initiation of treatment to death was 19 months. Of the prognostic factors, the age (greater than or equal to 70 years), clinical stage III of Durie and Salmon, hypercalcemia, extensive bone lesions, and the patho-morphological type IV of Brücher were associated with a decreased life-span. Therefore, MIP therapy was more effective in poor risk (high tumor mass group) than in good risk (low or intermediate tumor mass group) patients, but the survival of patients on MIP therapy was shorter in the poor risk group than in the good risk one. In addition, the group which responded rapidly (i.e. within 2-5 weeks) had longer remission and longer survival than the group which improved slowly (i.e. after 6-16 weeks).
