The present experiments on cats were performed to explore the process of survival and take of transplanted homografts of tympanic membrane with malleus, the homografts being respectively preserved in 4% and 0.5% formalin (pH 7.0) solutions, and to explore the reaction of middle ear mucosa of the recipient. The present study also included gross and histopathological examinations of the eardrum and observation of fibers of the lamina propria of the transplanted tympanic membrane by means of polarizing microscopy and of vascularization by intravascular India ink injection. The experiments conducted using a total of 96 cats yielded results leading to the following conclusions. 1) After six months the transplanted tympanic membrane showed nearly normal histologic features, with a fibrous layer between the lining epithelial and thin mucosal layers. The middle ear mucosa also became thinner and was covered with ciliated epithelium at this stage. 2) As regards vascular distribution, most vessels entered from the peripheral regions to become distributed all over the eardrum with mutual anastomoses. 3) Fibrous tissues of the lamina propria of the pars tensa of the transplanted eardrum remained intact over the six month period. 4) The transplanted malleus was neither destroyed not absorbed, and proved to survive well with active vascularization. 5) The microscopic changes observed in the middle ear mucous membrane of the recipient showed non-specific reactions to Gelfoam. The grafts were not subjected to immunological rejection. Homograft thus offer the best grafting material, but seemed to show poor resistance to infection.
Involvement of bacteria in serous otitis media was studied in man and by animal experimentation (using rabbits). Bacteria from the middle ear fluid, the skin of the external ear canal and the epipharynx of 100 human ears were cultured. Bacteria were also cultured from 50 ears of rabbits with serous otitis media induced experimentally by tubal stenosis, in the same way as those taken from human ears. Bacteria were detected in the middle ear fluid of 61 human ears (61%), and in 48 human ears (48%) except for those with contamination. The bacteria detected in the middle ear fluid were the same as those detected in the epipharynx in 40 of the 50 rabbit ears during the period from the start of treatment to the 30th day. In ears which were treated for more than 30 days, the bacteria detected in the middle ear fluid tended not to be the same as those detected in the epipharynx. From these results it was inferred that inflammation of the epipharynx spread over the middle ear cavity through the auditory tube.

The responses to a supine rest, norepinephrine (NE) and angiotensin II (Ang II) were investigated in the absence and presence of calcium antagonist nifedipine or diltiazem in essential (genetic, arterial) hypertension and normotension in humans. A supine rest significantly decreased blood pressure (BP), heart rate (HR), stroke volume index (SI), and cardiac output index (CI). On the contrary, the rest increased total peripheral vascular resistance index (TPRI) in both normotensives and hypertensives. The decrease in BP was significantly greater in hypertensives than in normotensives. NE significantly increased BP and TPRI, whereas it decreased HR, SI, and CI. The increase in BP was greater in hypertensives than in normotensives. Nifedipine and diltiazem inhibited the NE-induced increases in BP and TPRI. Ang II increased BP and TPRI, but it decreased HR, SI, and CI. Diltiazem did not inhibit the Ang-II-induced increases in both BP and TPRI. The increased responses to a rest and NE were observed in the early stage of essential hypertension. The increased responses may contribute to both the increase in BP and the induction of high blood pressure in essential hypertension. The calcium antagonists inhibited the NE-induced increases in BP and TPR. The results suggest that the antagonists inhibit the NE-dependent calcium influx and calcium release in the arterial smooth muscle. The observed responses to Ang II suggest that the antagonists may not inhibit Ang-II-dependent calcium-channel activity in the smooth muscle.
Correlation between blood pressure (BP) and total peripheral vascular resistance (TPR), hypotensive mechanisms of calcium antagonists, and cardiovascular responses to norepinephrine with and without administration of calcium antagonists were investigated in normotensive and genetic essential hypertensive humans. Supine resting decreased BP, heart rate (HR), stroke volume (SV), and cardiac output (CO), and, in contrast, it increased TPR. After 1 h supine rest, BP was positively correlated with TPR (r = 0.710; number = 45, p less than 0.001), but it was not correlated with CO. Intravenous infusion of the calcium antagonist diltiazem lowered BP and TPR, without apparently affecting HR, SV, and CO. In contrast, the calcium antagonist nifedipine diminished BP and TPR while increasing HR, SV, and CO. Norepinephrine elevated BP and TPR and decreased HR, SV, and CO. Prior administration of nifedipine inhibited elevation of TPR after treatment with norepinephrine. In contrast, prior administration of propranolol did not inhibit norepinephrine-induced BP and TPR elevation. From the results it may be concluded that elevation of BP is dependent on alteration of TPR, but not CO, in essential hypertensive humans. The arterial vasodilating effects of calcium antagonists induce a fall in both TPR and BP and inhibit norepinephrine-induced TPR increase. This suggests that abnormal contraction and relaxation of systemic arterial smooth muscle is a primary cause of the development and persistence of high BP in genetic (main gene) essential hypertensive humans.

A simple, sensitive enzyme linked fluorescence immunoassay has been developed to detect IgM antibodies against Chlamydia trachomatis. Reticulate bodies and elementary bodies from C trachomatis L2/434 Bu strain were isolated and used as antigens in the assay. Of 113 serum samples obtained from infants with pneumonia, 27 (23.9%) had IgM antibodies to C trachomatis L2 reticulate bodies and nine (8.0%) had IgM antibodies to C trachomatis L2 elementary bodies (titre greater than or equal to 1/500). Specific IgM antibodies were not detected in 20 control serum samples obtained from healthy adults and children. The possible use of enzyme linked fluorescence assay to determine IgM antibodies in the serodiagnosis of C trachomatis infection is discussed.
Relations between occupational history and the development of cancer of the lower urinary tract ("bladder cancer") were evaluated in Boston, Massachusetts, USA, Manchester, UK, and Nagoya, Japan. Population-based series of incident cases and controls were identified and interviewed in each area. The present analysis was limited to men and was based on 430 cases and 397 controls in Boston, 399 cases and 493 controls in Manchester, and 226 cases and 443 controls in Nagoya. In Boston, elevated risk of bladder cancer was associated with employment related to dyes (relative risk = 2 X 1; 90% confidence interval, 0 X 9-5 X 1), leather (1 X 7; 1 X 1-2 X 6), paint (1 X 5; 0 X 9-2 X 4), or organic chemicals (1 X 6; 1 X 1-2 X 5). In Manchester, elevated risk was associated with 0 X 9-3 X 6). No clear association was observed between occupation and risk in Nagoya. Elevations in risk related to occupation generally were stronger in men under 65 years of age compared to older men. Differences from place to place in associations between occupation and risk may be the result of differences in the exposures to hazardous agents that jobs with the same title involve.

A method for rapid identification of Chlamydia trachomatis (C. trachomatis) from clinical specimens using fluorescein-conjugated monoclonal antibodies was evaluated. Attempts to isolate C. trachomatis by McCoy cell cultures were simultaneously performed. C. trachomatis was isolated from 8 of 62 newborns with neonatal conjunctivitis in tissue culture as compared with 12 positives using monoclonal antibodies. C. trachomatis was isolated from 2 of 7 infants with pneumonia in tissue cultures as compared with 3 positives using monoclonal antibodies. In order to identify either elementary bodies (EB) or reticulate bodies (RB) in clinical specimens, EB and RB of C. trachomatis L2/434 strain were isolated, purified and reacted with monoclonal antibodies. Majority of fluorescence observed in the clinical specimens was considered to be EB.

We conducted a population-based case-control study of patients with bladder cancer and of controls drawn randomly from the general population of Metropolitan Nagoya and interviewed both groups. The incidence rates of bladder cancer were 2.42 and 7.05/100,000 for females and males, respectively. The analysis, based on 293 patients and 589 controls who were frequency matched for age, sex, and residence, provided the following major findings. Age-adjusted relative risks of 1.89 (1.15-3.10) and 3.53 (1.71-7.27) were found in male and female cigarette smokers, respectively. Significant relative risk was also found in males who drank cocoa. Elevated risk with a dose-response relationship was observed among women who used hair dye and who smoke, but this risk was insignificant, with the disappearance of a dose-response relationship, when it was adjusted for smoking. Age- and smoking-adjusted relative risk of coffee drinking was insignificant with no dose-response relationship. Relative risk of artificial sweetener use was below 1 with adjustment for age and smoking. Intake of alcoholic beverages and cola was insignificantly associated. Reduced risk of significance was suggested for the intake of black tea and matcha (powdered green tea) in females and of fruit juice in males.
