Optimal therapeutic regimen of famotidine based on plasma concentrations in patients with chronic renal failure.

An oral therapeutic regimen of famotidine, a histamine H2-receptor antagonist, was evaluated in patients with chronic renal failure who were receiving dialysis three times a week. Measurements of the half-life, bioavailability, and dialysis elimination rate were performed. The half-life of famotidine after oral administration in patients with renal failure was 22.5 hours as opposed to 2.5 hours in control subjects: after intravenous injection, it was 14.6 hours as compared with 2.2 hours. The bioavailability after oral administration was 49.4% in patients with renal failure and 55.2% in control subjects. The dialysis elimination rate was stable and independent of its plasma level, with an average of 41.9%. The oral administration of famotidine (20 mg) immediately after dialysis (60 mg/week) was found optimal and safe. There were no significant side effects with this regimen.

[A case of adrenal myelolipoma].

A case of adrenal myelolipoma is reported. The patient was a 63-year-old female on whom liver dysfunction was diagnosed and a tumor was detected accidentally in the right adrenal region by ultrasound abdominal test. This patient was the 26th patient from whom this tumor was extirpated in Japan. Reports on similar cases are on the increase owing to the advances in diagnostic imaging. A half of such cases were asymptomatic, and 85% of them were right unilateral cases.

Resection of metastatic lung tumor: the evaluation of histologic appearance in the lung.

The analysis of the data resulting from 58 operations for metastatic lung tumors is presented. The surgical procedures performed were unilateral lung tumor resection in 27 cases and bilateral lung tumor resection in 13 cases. The total cumulative five year survival rate was 41 per cent. The resected tumors were divided into two types according to the histologic appearance of the tumor margin: infiltrative and non-infiltrative (pseudo-capsulated). The cumulative five year survival rates were 14.7 per cent and 53.2 per cent, respectively. Lymph node metastasis was found in four patients with the infiltrative type of metastatic tumor.

Radiosensitivity of late recurrences following radiotherapy of murine fibrosarcomas.

Radiosensitivity of late recurrent tumors which emerged after radiotherapy was investigated. Tumors observed were fibrosarcomas. Recurrences emerged in the irradiated area approximately 200 days after a 50% tumor control dose of radiation of 60Co gamma rays or mixed irradiation with fast neutrons and gamma rays. The recurrent and radiation-induced tumors were differentiated by karyotype analysis. Once transplanted into fresh mice, the recurrent tumors grew more slowly than the original tumor. Tumorigenicity of the late recurrences was lower than that of the original tumor. Radiosensitivity of the late recurrences, which was examined using methods to assess control, tumor growth delay, and colony forming assays, was significantly higher than that of the original tumor. D0 values of hypoxic tumor cells were significantly smaller in two of the three recurrences compared to the original tumor. Oxic cells, when irradiated in vitro, also showed smaller D0 values for the recurrent tumors than the original tumor. Hypoxic cell fractions were between 0 and 14% in the late recurrences and 10% in the original tumor. These results are consistent with the hypothesis that radiotherapy causes mutation of tumor cells which results in increased radiosensitivity of surviving tumor cells.

[Effect of combined use of radiation and twice intratumor administration of OK-432 at one week intervals against a murine fibrosarcoma (NFSA)].

Effect of combination use of radiation and multiple administration of OK-432 was studied using a radio-resistant and weakly immunogenic murine fibrosarcoma (NFSa) originated spontaneously in a C3H female mouse. Mice implanted the tumor in the leg were locally irradiated with 40 Gy of gamma rays and locally administered total 8 KE of OK-432. The first group was given 1 KE daily for 8 days, 2 KE every other day in the 2nd group, 4 KE at 1 week interval in the 3rd group and 8 KE immediately after irradiation in the 4th group. The tumor volume at day 19 after irradiation was compared. The tumor growth inhibitory effect was observed in the 3rd group (p less than 0.01) and 4th group (p less than 0.05) compared to the group given radiation alone. This effect was seen at even more smaller dose of OK-432 as 0.5 KE by 2 times administration at 1 week interval. The effect of route of OK-432 administration was studied comparing the tumor volume at day 22. OK-432 was given 2 times at 1 week interval. A significant difference was seen in the group given OK-432 locally compared to the control group given radiation alone (p less than 0.001). There were no significant differences between the groups given OK-432 by the routes of i.p., s.c. i.v. and the control group. TCD50 value was examined giving OK-432 4 KE on the day and 2 KE on day 7 after irradiation. TCD50 value in the group treated with irradiation alone was 83.5 (79.6-87.4) Gy and the group given combination therapy with OK-432 was 60.7 (55.9-65.4) Gy. Two times OK-432 administration at 1 week interval appears to potentiate the radiation response by about 23 Gy.

[Behavioral pharmacology investigation of the wriggle mouse Sagami].

To elucidate the ataxic mechanism of Wriggle mouse Sagami (WMS), a behavioural pharmacological investigation was carried out by open-field study. The dysfunction of DA receptor, alpha-receptor, beta-receptor, 5-HT receptor and Ach-receptor appeared in WMS. Further examination will be necessary to investigate WMS with receptor dysfunction.

Fundamental tumor perfusion analysis with nuclear magnetic resonance imaging using gadolinium-diethylene triamine pentaacetic acid.

The measurement of blood perfusion is an important factor for both the diagnosis and follow-up of tumor lesions. However, noninvasive detection of this local blood flow factor is very difficult. To accomplish this, we tried to calculate tissue blood perfusion indexes with nuclear magnetic resonance imaging using gadolinium-diethylene triamine pentaacetic acid (Gd-DTPA). We experimentally applied this method to C3H mouse's tumor NFSa (fibrosarcoma) and its recurrence tumor R1137, whose experimental hypoxic cell fraction is different; that is, R1137 is more oxic than NFSa. Imaging pulse sequence was T1 (TR = 1,000 ms, TI = 300 ms), and longitudinal relaxation rate (R1 = 1/T1) values were calculated. Injected dose of Gd-DTPA was 0.1 mmol/kg. The T1 images produced are from both the preinjection period from every 5 min postinjection for 30 min. Using two-exponential analysis and compartment analysis we calculated two fundamental parameters: uptake index and flow index as blood perfusion factors. We found that this method has the possibility of differentiating the tissue's hypoxic cell fraction and is effective both for follow-up study after radiation therapy and for tissue characterization.

Two human cases of gnathostomiasis and discovery of a second intermediate host of Gnathostoma nipponicum in Japan.

Two human cases of gnathostomiasis from ingestion of raw native Japanese loaches, Misgurnus anguillicaudatus, are reported. Seven early third-stage larval Gnathostoma nipponicum were recovered from 3,098 loaches in the same district in which 2 human patients had obtained and eaten raw loaches. Encapsulated G. nipponicum larvae were also recovered from loaches infected under laboratory conditions. All 6 weasels captured in the same district in which the naturally infected loaches were found and where the humans had become infected were infected with adult worms of the same species. This is the first report of M. anguillicaudatus serving as a second intermediate host of G. nipponicum.

Purification of Sarcophaga (fleshfly) lectin and detection of sarcotoxins in the culture medium of NIH-Sape-4, an embryonic cell line of Sarcophaga peregrina.

An established cell line originating from a Sarcophaga peregrina (fleshfly) embryo, NIH-Sape-4, was found to synthesize mRNAs for Sarcophaga lectin and sarcotoxin IA, but not those for storage protein or 25 kDa protein. These four proteins are known to be synthesized in the fat-body of third-instar larvae, and the two former in particular are known to participate in the defence mechanism of this insect and to be induced in response to injury of the body wall. Thus the embryonic cell line NIH-Sape-4 synthesizes certain defence proteins constitutively. This cell line will be useful for large-scale purification of Sarcophaga lectin, since 50 micrograms of purified Sarcophaga lectin could be obtained from about 400 ml of culture medium.

Lack of change in indoleamine metabolism in spinal cord of patients with amyotrophic lateral sclerosis.

Indoleamine metabolism was determined by high-performance liquid chromatography with electrochemical detection in the cervical spinal cord of patients with amyotrophic lateral sclerosis (ALS) and controls with non-neurological diseases. The concentrations of 5-hydroxytryptamine (5-HT) and 5-hydroxyindoleacetic acid (5-HIAA) were the highest in the ventral horn compared with other regions of the spinal cord both in ALS and controls. There was no significant change in the concentration of 5-HT in any region of the spinal cord between ALS and controls. In addition, the concentration of 5-HIAA in ALS was lower than that in controls, however, no statistical significance was observed.

Development of host-dependent high-grade tumor-specific immunity through a novel mechanism triggered by the Lyt-2+ tumor-specific T cell clone (K7L) that induces temporal growth of L1210 leukemia-K7L-variant.

When a murine leukemia L1210-specific Lyt-2+ T cell clone, K7L, was injected i.p. into CD2F1 mice together with L1210, the normal growth of L1210 in the peritoneal cavity of the mice at the early stage (days 0 to 5) was strongly inhibited, but L1210 grew progressively at the middle-stage (days 5 to 10), and then was rejected at the late stage (days 10 to 20). The mice thus survived for long times (more than 60 days), whereas the normal control injected with L1210 alone died within 14 days. The L1210 that grew at the middle stage in mice initially inoculated with L1210 together with K7L was a K7L-insensitive (K7L-) variant. All of eight tumor clones established from L1210-K7L- by limiting dilution was insensitive to the antitumor activity of K7L, and this property of tumor clones was stable after repeated in vitro passage. The initial depression of the L1210 growth by K7L followed by growth and rejection of the variant L1210-K7L- by the host T cell activity was then found to prepare a strong, long-lasting (more than 3 mo) immunity to protect mice against the high-dose (10(7) cells per mouse) challenge of original L1210. Corresponding to this result, definite tumor (L1210)-specific cytotoxic T lymphocyte (CTL) activity against both variant and original L1210 targets was developed by antigen (L1210) restimulation in the culture of spleen cells from these mice, but was not increased to a detectable level before L1210-K7L- variant started to grow. It was suggested that the 1210-K7L- variant and the original L1210 should have the common tumor-specific antigen that was independent of the K7L-reactive antigen, and that original L1210, whose growth was retarded by K7L, primed the host with the common antigen to be enormously boosted by the subsequently growing L1210-K7L- variant.
