Who in America is trying to lose weight?

OBJECTIVES: To describe the characteristics and weight-loss methods of persons who are trying to lose weight; to compare the knowledges and practices regarding weight loss between those persons trying to lose weight and those not trying to lose weight; and to evaluate trends in these knowledges and practices between 1985 and 1990. DESIGN: Large (approximately 120,000 persons per year), nationally representative random sample of the U.S. population. SETTING: The 1985 and 1990 Health Promotion Disease Prevention Current Health Topics Supplements to the National Health Interview Survey. PARTICIPANTS: Random sample of the U.S. population. MEASUREMENTS: Proportions of the NHIS sample with characteristics of interest weighted to be representative of the U.S. population. RESULTS: Approximately 44 million persons 25 years or older were trying to lose weight in 1990; 61.8% of men and 59.6% of women were doing so by increasing their physical activity. Both proportions are significantly increased compared to the 1985 proportions of 56.9% and 56.2%, respectively. Twenty-seven percent of those persons who saw themselves as overweight were not trying to lose weight. CONCLUSIONS: More than one third of Americans see themselves as overweight, but fewer than two thirds of these persons are trying to lose weight. About 4% of self-perceived underweight persons and 11.4% of persons who think their weight was about right are also trying to lose weight. Most persons who are trying to lose weight are doing so by eating less, by increasing their physical activity, or by a combination of these methods.

Deficiency of plasma plasminogen activator inhibitor 1 results in hyperfibrinolytic bleeding.

A 63-year-old man was evaluated for a lifelong history of bleeding commencing with frequent epistaxis as a child; all previous routine coagulation parameters were within the normal range. The patient's hemorrhagic disorder is characterized predominantly by delayed bleeding at surgical sites. In the resting state, there was no clinical or laboratory evidence of excessive fibrin(ogen)olysis. Bleeding was not caused by disseminated intravascular coagulation, factor XIII deficiency, alpha 2-antiplasmin deficiency, or dysfibrinogenemia. It was found that the patient was deficient in plasma PAI-1 antigen and activity but with approximately half normal antigen and normal activity of platelet PAI-1. The low concentration of plasma PAI-1 was insufficient to neutralize circulating t-PA, resulting in high t-PA activity with normal antigen and causing the hyperfibrinolytic activity observed. Studies on seven family members of the proband indicated autosomal inheritance of plasma PAI-1 deficiency. Studies on this patient emphasize a clear correlation between decreased plasma PAI-1 activity and hyperfibrinolytic bleeding and also emphasize the unique role of plasma PAI-1 in the balance between the coagulation and fibrinolytic mechanisms.

Rational screening of oligonucleotide combinatorial libraries for drug discovery.

Combinatorial strategies offer the potential to generate and screen extremely large numbers of compounds and to identify individual molecules with a desired binding specificity or pharmacological activity. We describe a combinatorial strategy for oligonucleotides in which the library is generated and screened without using enzymes. Freedom from enzymes enables the use of oligonucleotide analogues. This dramatically extends the scope of both the compounds and the targets that may be screened. We demonstrate the utility of the method by screening 2'-O-Methyl and phosphorothioate oligonucleotide analogue libraries. Compounds have been identified that bind to the activated H-ras mRNA and that have potent antiviral activity against the human herpes simplex virus.

Laparoscopic gonadectomy in a case of testicular feminization.

Laparoscopy provides a minimally invasive technique for the accurate diagnosis of intersex problems and may also provide the opportunity for therapeutic management of these patients. Herein, we report our management of a patient with complete testicular feminization, by laparoscopic bilateral gonadectomy of fully developed intra-abdominal testes.

Inner-city pharmacies: can they meet the OBRA '90 mandates?

Visits were made to 21 pharmacies in two poor Chicago neighborhoods as one component of the University of Illinois at Chicago's commitment to help improve health care delivery, ensure that pharmacy services are not overlooked in local health reform efforts, and understand the implications of the Omnibus Budget Reconciliation Act of 1990 (OBRA '90). Pharmacists report that Medicaid reimbursement policies result in: (1) prescribing patterns that have no consistent therapeutic or economic rationale; (2) dispensing of expensive drugs, some of which are then sold on the street or to other pharmacies; and (3) dispensing of unnecessary and often expensive legend drugs when other legend drugs and generics or over-the-counter products would perform just as well. On the positive side, 76% of the pharmacies possess computerized patient-profile systems, and 72% maintain background information on their prescribers. These results suggest that OBRA '90's mandates should contribute toward improving the Medicaid program, but that implementation is likely to be difficult and uneven.

Effects of perchlorate on the molecules of excitation-contraction coupling of skeletal and cardiac muscle.

To understand the nature of the transmission process of excitation-contraction (EC) coupling, the effects of the anion perchlorate were investigated on the voltage sensor (dihydropyridine receptor, DHPR) and the Ca release channel (ryanodine receptor, RyR) of the sarcoplasmic reticulum (SR). The molecules, from rabbit skeletal muscle, were either separated in membrane vesicular fractions or biochemically purified so that the normal EC coupling interaction was prevented. Additionally, the effect of ClO4- was investigated on L-type Ca2+ channel gating currents of guinea pig ventricular myocytes, as a native DHPR not in the physiological interaction of skeletal muscle. At 20 mM, ClO4- had minor effects on the activation of ionic currents through Ca channels from skeletal muscle transverse tubular (T) membranes fused with planar bilayers: a +7-mV shift in the midpoint voltage, V, with no change in kinetics of activation or deactivation. This is in contrast with the larger, negative shift that ClO4- causes on the distribution of intramembrane charge movement of skeletal muscle. At up to 100 mM it did not affect the binding of the DHP [3H]PN200-110 to triad-enriched membrane fractions (TR). At 8 mM it did not affect the kinetics or the voltage distribution of gating currents of Ca channels in heart myocytes. These negative results were in contrast to the effects of ClO4- on the release channel. At 20 mM it increased several-fold the open probability of channels from purified RyR incorporated in planar bilayers and conducting Ba2+, an effect seen on channels first closed by chelation of Ca2+ or by the presence of Mg2+. It significantly increased the initial rate of efflux of 45Ca2+ from TR vesicles (by a factor of 1.75 at 20 mM and 4.5 at 100 mM). ClO4- also increased the binding of [3H]ryanodine to TR fractions. The relative increase in binding was 50-fold at the lowest [Ca2+] used (1 microM) and then decayed to much lower values as [Ca2+] was increased. The increase was due entirely to an increase in the association rate constant of ryanodine binding. The chaotropic ions SCN- and I- increased the association rate constant to a similar extent. The binding of ryanodine to purified RyR protein reconstituted into liposomes had a greater affinity than to TR fractions but was similarly enhanced by ClO4-. The reducing agent dithiothreitol (5 mM) did not reduce the effect of ClO4-, and 5% polyethylene glycol, with an osmolarity equivalent to 20 mM ClO4-, did not change ryanodine binding.(ABSTRACT TRUNCATED AT 400 WORDS)

Laparoscopic wedge resection of a renal tumor: initial experience.

Advances in laparoscopic surgery have resulted in the development of reliable techniques for performing laparoscopic simple, radical, and partial nephrectomy. Herein, we present a case of a small, low-stage renal mass laparoscopically explored and excised by a wedge resection. Although this minimally invasive technique allowed the patient a rapid recovery and achieved local tumor excision, the clinical appropriateness remains controversial.

Comparison of midazolam with propofol for sedation in outpatient bronchoscopy.

We have compared sedation for fibreoptic bronchoscopy provided by incremental doses of midazolam with that provided by a computer-controlled infusion of propofol. These two methods were compared in terms of operator and patient acceptability, anxiolysis, effects on systolic arterial pressure and oxygen saturation. Tests were made also of memory and motor reactions, before and 60 min after the end of the procedure. Acceptability to operators and patients was high in both groups. There were no significant differences between the groups in systemic arterial pressure or anxiolysis. Oxygen saturation decreased in both groups (propofol group median 83% (range 69-95%); midazolam group median 86% (range 77-95%)) (ns). The median recovery time was 5 min (range 5-10 min) in the propofol group and 10 min (range 5-40 min) in the midazolam group (P < 0.01). Memory and motor reaction times 60 min after the end of the procedure did not differ from baseline in the propofol group, but were significantly impaired in the midazolam group.

Angiosarcoma, porphyria cutanea tarda, and probable chloracne in a worker exposed to waste oil contaminated with 2,3,7,8-tetrachlorodibenzo-p-dioxin.

A worker developed angiosarcoma, porphyria cutanea tarda, and skin lesions characteristic of mild chloracne. About 10 years earlier he had been employed at a truck terminal in Saint Louis, Missouri, at a time when it was sprayed with waste oil contaminated with 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD). The occurrence of these three rare conditions in a single exposed worker supports the aetiological relation between environmental exposure to TCDD and the subsequent development of soft tissue sarcoma and porphyria cutanea tarda.

Discriminant analysis of symptom pattern and serum antibody titres in humidifier related disease.

BACKGROUND: The heterogeneous patterns of symptoms among factory workers exposed to aerosols from contaminated air humidifiers were analysed to assess the association between specific symptoms and the serum IgG antibody response to the humidifier water contaminants, and to test the ability of specific symptoms to predict this antibody response. METHODS: Symptoms from 88 factory workers were surveyed by a doctor administered questionnaire and compared with their serum IgG antibody titres to humidifier water contaminants quantified by enzyme immunoassay. RESULTS: The strength of association between individual symptoms and antibody showed that fever, shivering or chills, influenza-like symptoms, or headache were individually significantly associated with the presence and higher titres of antibody. This was also true for those subjects whose symptoms were most pronounced during the first day of the working week. Within each subject's full symptom profile there were significant associations between the description of chest tightness, breathlessness, and wheeze; between headache and influenza like symptoms; between fever and shivering or chills; and between intermittent onset and general tiredness. Discriminant analysis of the full symptom profiles showed that there was maximum information content in five independent parameters, namely, the descriptions of fever, headache, and chest tightness, the timing of their onset, and the readiness to describe miscellaneous symptoms in addition to those in the questionnaire. On the basis of these criteria 72% of subjects could be classified according to their antibody state. Cluster analysis with these five independent parameters described four symptom clusters: one associated with high median antibody levels, one with low, and two with zero median levels. These were, respectively: (1) fever with headache and chest tightness; (2) either no or few symptoms; (3) chest tightness and headache with intermittent onset; (4) headache and miscellaneous symptoms with intermittent onset. CONCLUSIONS: The association between serum antibody titres and specific symptom patterns may identify different categories of disease which constitute the spectrum known as humidifier related disease, and strengthens the hypothesis that antibody may be involved in the pathogenesis of some components of the disease.

Brain-derived neurotrophic factor (BDNF) prevents the degeneration of medial septal cholinergic neurons following fimbria transection.

Brain-derived neurotrophic factor (BDNF), a member of the neurotrophin family, supports the survival of developing basal forebrain cholinergic neurons in vitro and is retrogradely transported by cholinergic neurons of the medial septum and diagonal band following intrahippocampal injections in vivo. To substantiate a potential role for BDNF in the maintenance of forebrain cholinergic neurons in the adult brain, we assessed the ability of BDNF to sustain the phenotype of medial septal cholinergic neurons following a unilateral transection of the fimbria. BDNF, NGF, or vehicle solutions were infused continuously in adult female rats either into the lateral ventricle (intracerebroventricularly) or directly into the septum for 2 weeks beginning at the time of the transection. In vehicle-infused animals, only 28% of the ChAT-immunoreactive neurons remained on the side ipsilateral to the lesion compared to the contralateral intact side. When infused intracerebroventricularly, both BDNF and NGF reduced the extent of the phenotypic loss, in that 44% and 68%, respectively, of the ChAT-immunopositive neurons remained on the lesioned side. Intraseptal infusion proved even more effective, in that following BDNF and NGF treatment 60% and 86%, respectively, of the normal complement of ChAT-immunopositive neurons were apparent on the side ipsilateral to the lesion. Similar results were obtained when an antibody to the low-affinity NGF receptor was used to identify the cholinergic neurons. To determine if the apparent greater efficacy of NGF compared to BDNF might be related to differences in delivery, we examined the patterns of distribution of radiolabeled BDNF and NGF injected into the lateral ventricle. 125I-BDNF showed only very little diffusion from the ventricles into the adjacent neural tissue and negligible retrograde labeling of the neurons within the basal forebrain. 125I-NGF, however, diffused readily into the brain, resulting in widespread retrograde labeling of basal forebrain neurons. A similarly limited distribution pattern was observed where BDNF was detected immunohistochemically in animals infused intracerebroventricularly (12 micrograms/d) for 2 weeks. In contrast, when delivered intraseptally, the same dose of BDNF exhibited a widespread diffusion within the surrounding neuropil and retrograde labeling of neurons in the medial septum and the vertical limb of the diagonal band. Thus, when delivered effectively, BDNF has a substantial capacity to rescue axotomized cholinergic neurons.
