The present study was conducted histologically to elucidate the influence of vagotomy on the gastric mucosa. Bioptical tissues were obtained from 10 control cases and from 26 cases of duodenal ulcer using an endoscope. Of the 26 cases of duodenal ulcer, 20 were subjected to truncal vagotomy with pyloroplasty and 6 to truncal vagotomy with hemigastrectomy. In the cases of duodenal ulcer, tissues were obtained at the preoperative stage, and at 1, 3, 6 and 12 months on the time-course basis after the operation. The parietal cells were counted in a unit area (0.0176 mm2) between the middle and lower 1/3 part of the fundic gland in H.-E. stained paraffin sections under microscopy. The length of the fundic gland was also measured. The results are summarized as follows: (1) There were a greater number of parietal cells and the length of the fundic gland was longer in the preoperative cases than in the controls. (2) Postoperative observations revealed that the number of parietal cells and the length of the fundic gland significantly decreased up to 3 months after the operation by both surgical techniques. Thereafter, the number of parietal cells and the length of the fundic gland significantly increased in the cases subjected to vagotomy with pyloroplasty. (3) The difference between the two surgical techniques was made clear when the mean values for the absolute number of parietal cells were compared at 3 months after the operation, the absolute number of parietal cells being smaller in hemigastrectomy than in pyloroplasty.
Changes of FHR long term variability (LTV) were studied in four groups of patients under different types of analgesia. The LTV was evaluated according to our visual criteria in each episode of more than one minute of duration on baseline fetal heart rate level between contractions. In the balanced anesthesia group incidence of the episodes with LTV decreased significantly after the administration of Diazepam and Methoxyflurane (64%-30%), further decreasing after the injection of Pethilorfan, but it increased to some degree within 20 min. before delivery. In the epidural group the change of LTV was mild, but the LTV decreased after each administration of 0.25% Bupivacaine (58%-44%-36%). It is also showed mild increase within 20 min. before delivery. Loss of variability was persistently noted until the end of labor from 20-40 min. after the administration of Diazepam and Pentazocine in the modified NLA group. The change in paracervical block group was quite different from the other groups. The incidence of episodes with LTV was increased after the injection of 1% Lidocaine Hydrochloride (35%-41%). There was no significant different of blood gas and acid-base balance among four types of analgesia.
M-mode and two-dimensional echocardiography were performed to investigate the motion of the interventricular septum (IVS) and posterior wall (PW) in cases with pure mitral (MI) or aortic (AI) insufficiency. Subjects were classified into four groups; 15 cases with chorda rupture or floppy mitral valve (acute MI), four with AI of acute onset (acute AI), 17 with chronic MI including rheumatic MI, mitral valve prolapse syndrome, and 11 with chronic AI. There wee no differences of left ventricular dimension (LVDI), stroke volume (SV), ejection fraction (EF) and cardiac output (CI) between acute and chronic MI, and between acute and chronic AI, although LVDI and CI were increased in all groups. The ratio of the amplitude of IVS to PW excursion was significantly increased in cases with LV dilation due to acute MI or AI, compared with that in chronic MI or AI. This ratio had a linear relationship with LVDI in acute volume overload, but in chronic volume overload, it was constantly 1.0 or less. The mechanism of the increased septal motion in acute MI or AI is still unknown, but it appears to reflect the intact and contractile septum which adapts to volume overload more readily than the posterior wall. It is probably because the posterior wall is prevented from distension by the pericardium. On the other hand, in chronic MI or AI, vigorous septal motion would not be observed because of the occasional presence of relative tricuspid insufficiency, the effects of the long standing burden on the septal myocardium, and compensatory distension of the posterior wall and pericardium.

Since Locher suggested the possibility of neutron capture therapy, a number of investigators have been making efforts to establish clinical application of this treatment. For success of this therapy one of the keys is to develop a proper boron compound with selective accumulation and high concentration in the tumor. So far, sodium mercaptoundecahydrododecaborate, Na2B12H11SH is the best candidate for this purpose with its high concentration and selectivity shown by chemical analysis. Nevertheless, histological study of boron uptake to know fine distribution in tissue is inevitable for evidence of effectiveness of clinical therapy. Cytological effects in neutron capture therapy largely depend on close association of B-10 with the tumor cell. B-10, being non-radioactive isotope, can not be used as a tracer for autoradiographical study of boron distribution in tissue. Instead, alpha particles produced by (n, alpha) reaction can be used for autoradiography which demonstrates not only location of boron in tissue but tracks by alpha particles expected to occur in actual neutron irradiation. Dielectric organic polymer has been calling attention for its ability to register heavy particles which was utilized for alpha autoradiography. Autoradiography by use of plastic film without nuclear emulsion has advantages; (1) simple procedures, (2) no darkroom needed, (3) a lower background obtainable, (4) little influence of temperature and moisture, (5) long standing material and so on. The disadvantage of this procedure is that tissue sections are easily damaged by strong alkalinity used for chemical etching. To solve this problem an overlapping histologic technique was worked out, permitting one to determine the relation of the heavy particle tracks to the intra and extracellular structures. Na2B12H11SH, a boron compound already in practical use, has been restudied by using this new technique. Autoradiograms showed favorable results with abundant alpha tracks over the tumor tissue in close relation to the cells and with very few tracks over the normal brain parenchyma. This technique is a useful means not only for further development of boron compounds but for other alpha emitters.
The purpose of the present echocardiographic study is to determine the timing and order of both mitral and tricuspid valve openings in normal subjects and to provide the basic data for evaluating diastolic hemodynamics in cardiac patients. The subjects consisted of three groups: (i) 30 normal young cases in which the opening of both valves was recorded, (ii) 8 cases with idiopathic atrial fibrillation in which the relationship between preceding R-R and early diastolic intervals was determined, and (iii) 10 cases in which the effect of the direction of the ultrasonic beam on mitral valve echograms was evaluated. By applying dual echocardiography or phonoechocardiography, the interval from the aortic component of the second heart sound (IIA) to the point of maximal anterior opening (E), the isovolumic relaxation period from IIA to the onset of opening (D') and opening slope of the anterior leaflet were measured in each valve echogram. Careful recording was mandatory for measurements because even slight angulation of the transducer toward the mitral ring induced the timing of D' point and opening slope to vary. Respiratory variation was observed on the movements of both valves. During inspiration mitral valve opening occurred further from IIA or did not change in timing, but the tricuspid valve opened prematurely. During expiratory phase, the mitral valve opened nearly simultaneously with the tricuspid valve. The measurements were: IIA-E interval = 99.8 +/- 13.1 (mean +/- SD), 104.5 +/0 20.6 msec; IIA-D' interval = 47.2 +/- 9.9, 48.3 +/- 13.1 msec; opening slope = 301 +/- 76, 264 +/- 59 mm/sec, in mitral and tricuspid valve echograms, respectively. In cases with atrial fibrillation, these measurements were nearly constant regardless of varied preceding R-R intervals in 8 mitral and in one tricuspid valve echograms. It was expected from observing the effect of R-R intervals on valve opening in a case of valvular disease that diastolic hemodynamics could be evaluated by analysing the timing and order of both valvular openings, especially in relation to the preceding R-R intervals in atrial fibrillation.

A 37-year-old male with adrenocortical tumor presenting marked gynecomastia and impotence as initial symptoms was experienced. Abnormal elevations in urinary 17-KS, 17-OHCS and blood estrogen levels were noted. Total surgical removal of the tumor weighing 800g was performed. Postoperatively the gynecomastia disappeared and he regained libido with no signs of recurrence one year later.
Two cases of cervical dumb-bell tumour were operated on through the eroded and enlarged intervertebral foramina after the exact position of the tumours had been revealed by CT scan. This anterolateral approach was probably the best route for minimizing any operative damage to the spinal cord in these very advanced cases.

Immunoreactive beta-endorphin in the third ventricular fluid was measured in response to electrical stimulation of the periaqueductal gray matter in 8 patients with intractable pain during rostral mesencephalic reticulotomy for pain relief. In all patients, marked increase of immunoreactive beta-endorphin was observed. On the other hand, in cases of electrical stimulation of the zona incerta performed during stereoencephalotomy, in 5 patients with involuntary movement, immunoreactive beta-endorphin in the third ventricular fluid did not show any significant change. The authors conclude that the increase of immunoreactive beta-endorphin on electrical stimulation of the periaqueductal gray matter is not a nonspecific response to brain stimulation but a specific response in regard to cerebral localization of endorphins. Direct correlation between pain relief and periaqueductal gray stimulation is also questioned.
The authors report a case of cystic meningioma in a 10-month-old boy diagnosed by metrizamide ventriculography and computerized tomography. Intracranial meningioma in infants under 1 year old is extremely rare. This case is only the 16th case reported in the world literature; the other 15 cases are reviewed.
This report is concerned with a 55-day-old female hospitalized because of intraventricular hemorrhage from a tumor in the pineal region. Examination of serum alpha fetoprotein showed 7400 ng/ml. The patient expired because of frequent intraventricular hemorrhage and progressive hydrocephalus. Autopsy demonstrated the tumor was ependymoblastoma. No teratomatous component was noticed in the histological examination. High serum alpha fetoprotein in relation to tumor histology was briefly discussed.
