[Serum bilirubin subfractionation by high-performance liquid chromatography in patients with fulminant hepatic failure].

Serum bilirubin subfractionation, using high-performance liquid chromatography (HPLC), was carried out and clinically evaluated in 9 patients with fulminant hepatic failure (FHF). Serum unconjugated bilirubin (UCB), C-8 bilirubin mono-conjugate (MBC), C-12BMC and bilirubin di-conjugate (BDC) were quantified by the alkaline methanolysis-HPLC method described by Blanckaert. Similar studies were performed in 10 patients with acute hepatitis (AH) and 6 patients with obstructive jaundice and the results were compared. In patients with rising serum bilirubin, the serum C-12BMC/C-8BMC ratio was calculated to be as follows: 2.10 +/- 0.21 for FHF, 1.05 +/- 0.34 for AH and 0.81 +/- 0.08 for obstructive jaundice, respectively. A significant differences were observed between FHF and AH (P less than 0.01). As there was almost no overlapping, it was considered that this determination is useful for the differential diagnosis of jaundiced patients, especially, in those patients with AH whose sera show a prolonged prothrombin time lower than 40%, and there is a concern that they may develop FHF. The calculation of C-12BMC/C-8BMC allows us to make a differential diagnosis between AH and FHF at an early period in the development of the disease, and it was proved that serum C-12BMC/C-8BMC ratio can reflect the magnitude of hepatocyte damage as well as bilirubin metabolism disturbance.

[Antitumor activity of a new platinum complex, (R)-(-)-2-aminomethylpyrrolidine (1, 1-cyclobutanedicarboxylato) platinum (II), against cisdiamminedichloroplatinum (II)-resistant murine leukemia cell line].

Antitumor activity of a new platinum complex, (R)-(-)-2-aminomethylpyrrolidine (1, 1-cyclobutanedicarboxylato) platinum (II) (DWA 2114 R) against cisdiamminedichloroplatinum (II) (CDDP)-resistant tumor was examined in in vitro and in vivo experiments. CDDP-resistant line was established from L 1210 mouse leukemia cells by continuous exposure to CDDP in dose-escalation manner. Six clones were isolated from parental resistant line and one of these clones, clone f, which was found to be highly resistant (30-40 fold) to CDDP, was used in the following experiments. Clone f showed 4-7 fold cross-resistance to DWA 2114 R and 11-19 fold to cisdiammine-1, 1-cyclobutanedicarboxylatoplatinum (II) (CBDCA) in in vitro growth inhibition assay. DWA 2114 R showed the most effective antitumor activity against mice transplanted with the resistant cells in the increase of life span (ILS%). About 100% of ILS and cured mice were observed in the treatment with DWA 2114 R. On the other hand, CDDP or CBDCA showed a little increase in the survival time (less than 40% of ILS) and all mice died. These results suggest that DWA 2114 R seemed to be more effective against CDDP-resistant tumors clinically than CDDP and CBDCA.

Alleviation of gallbladder complications by treatment of hepatic arterial embolization with caerulein.

Transcatheter arterial embolization (TAE) with the concurrent use of caerulein was assessed for the purpose of preventing gallbladder complications often seen after TAE of hepatic carcinoma. Ninety-six cases with primary hepatic carcinoma, who had undergone TAE in the right hepatic arterial region over the past 4 years, were divided into three groups: 22 cases for which embolization was possible on a selective basis by passing the catheter to the peripheral side beyond the bifurcated region of the cystic artery; 40 cases who had undergone TAE in which caerulein was not administered, from the central side of the bifurcated region of the cystic artery; and 34 cases given 20 micrograms caerulein 15-30 min before TAE. A comparison was made using the abdominal pain, pyrexia, rate of leukocytosis and the US findings of the gallbladder as the indices of the gallbladder complications. As a result, it became evident that it was possible to prevent or alleviate gallbladder complications if caerulein were administered before TAE in cases where the embolizing substances were infused in the right hepatic artery from the central side of the bifurcated region of the cystic artery. It was conclusively shown that the gallbladder blood flow decreases if the organ is contracted by caerulein, which in turn causes a decrease in the inflow of the embolizing substances whereby complications are alleviated.

[Antitumor activity of a new platinum complex (R)-(-)-2-aminomethylpyrrolidine (1,1-cyclobutane dicarboxylato) platinum (II) (DWA2114R) by its serial administration].

Antitumor activity of a newly synthesized platinum complex, DWA2114R, by the serial administration was examined and compared with that of cis-diammine-1,1-cyclobutane dicarboxylato platinum (II) (CBDCA). In mice transplanted s.c. with tumor, the serial i.p. administration resulted in the increases of both maximal tolerated dose (MTD) and growth inhibitory ratio (GIR) of DWA2114R than single administration. Such increases in MTD and GIR were also shown by CBDCA, but the degree of these increases, such as the ratio of MTD or GIR by the serial administration compared to that at the single administration, was higher in DWA2114R than CBDCA. GIR of DWA2114R by the serial administration was higher than that of CBDCA at the doses to induce the same toxicity which was estimated by body weight loss. In addition, in the experiment using ascites tumor-bearing mice, better antitumor activity of DWA2114R was shown by the elongation of survival time. These results indicate that the cumulative toxicity of DWA2114R is lower than that of CBDCA, which causes the therapeutic advantages of DWA2114R in the serial administration.

Antithrombin III concentrate in the treatment of fulminant hepatic failure.

Twenty-six patients with fulminant hepatic failure were treated with daily infusions of antithrombin III concentrate until recovery of consciousness or death. Seven patients were alive (group A), 7 survived 17 to 47 days after treatment (group B), and 12 died within 9 days (group C). Decreased plasma antithrombin III levels increased on the day after treatment, irrespective of the pretreatment levels in all patients. Continuous or temporary normalization was seen in all patients in groups A and B, but in only 5 in group C patients whose bleeding was extensive (p less than 0.05). An abrupt drop in peripheral platelet counts occurred when plasma antithrombin III levels were below normal. General bleeding accompanied this drop. These results suggest that maintained normal plasma antithrombin III levels are beneficial for prolonged survival time in fulminant hepatic failure, probably through controlling intravascular coagulation, and that antithrombin III infusion may be useful for such treatment.

Effects of prolonged oral cimetidine, ranitidine, and famotidine therapy on antipyrine elimination.

The effects of three histamine H2-receptor antagonists on the antipyrine metabolizing capacity of the liver were studied in 32 patients with gastrointestinal disorders and in three healthy volunteers. The drugs studied were 800 mg and 400 mg of cimetidine, 300 mg of ranitidine, and 40 mg of famotidine daily. Only cimetidine at 800 mg/day inhibited hepatic antipyrine metabolism, and the inhibitory effects disappeared rapidly with no accumulation.

[Usefulness of caerulein in suppressing post-TAE complications of the gallbladder].

While transcatheter hepatic arterial embolization (TAE) has been extensively performed as a form of treatment for nonresectable malignant hepatic tumors, complications, such as abdominal pain, fever or leukocytosis due to gallbladder infarction by embolic materials frequently occur and have not yet been overcome. We devised a new procedure for reducing the incidence of gallbladder infarction by administering caerulein prior to TAE. Between 1984 and 1986, 63 patients with hepatocellular carcinoma were treated by TAE with the use of Gelfoam. These patients were divided into 3 groups. Fourteen patients underwent TAE in which the tip of the catheter was placed in the right hepatic artery distal to the origin of the cystic artery (group A). In the other patients the tip of the catheter was placed proximal to the origin of the cystic artery; 40 patients were not treated by caerulein (group B); 9 patients were administered caerulein 20 micrograms intramuscularly 15 to 30 minutes prior to TAE. The incidence of complications after TAE, such as abdominal pain, fever over 38 degrees C, leukocytosis and ultrasonographical abnormalities of the gallbladder was compared in these 3 groups. The results showed that in group C (TAE after administration of caerulein), the incidence of complications was significantly decreased compared with group B(TAE without caerulein). The authors suggest that post-TAE infarction of the gallbladder is effectively diminished by contracting it with caerulein.

Distribution of estradiol and percentage of free testosterone in sera of Japanese women: preoperative breast cancer patients and normal controls.

For the purpose of investigating a possible correlation between the genesis of breast cancer and the serum contents of free (non-protein-bound) estradiol (E2) and free testosterone (T) in Japanese women, the distributions of free and total E2 and T and the binding capacity of sex hormone-binding globulin (SHBG) in serum were compared for 39 patients with preoperative breast cancer and 40 normal women (controls). The patients were found to have significantly higher free E2 and free T (in terms of the percentage of the total) than did controls. Conversely, percentages of SHBG-bound E2 and albumin-bound E2 were not significantly different for patients and controls. The SHBG binding capacity was significantly smaller in postmenopausal, but not in premenopausal, patients. Free E2 and free T were found to be negatively correlated with the SHBG binding capacity in both patients and controls. A significant negative correlation also was noted between the percentage of free E2 or free T and the age of controls. The percentage of free T was positively correlated with body weight for both premenopausal and postmenopausal patients. These results suggest a possibility that the elevation of free E2 in the serum may be one of the potential risk factors for genesis of breast cancer in Japanese women.

Inhibitory activity of the serum from patients with fulminant hepatitis against liver regeneration.

The effect of sera from 8 patients with fulminant hepatitis, including 2 survival cases, on DNA and protein synthesis in primary cultured rat hepatocytes was studied. The serum from patients at an early stage or within 10 days after onset tended to intensify DNA synthesis in isolated hepatocytes, whereas the serum from patients with a history of over 50 days distinctly inhibited synthesis. When the serum was fractionated by gel filtration or free-flow electrophoresis, only the albumin fraction inhibited DNA synthesis in cultured hepatocytes. The suppressive effect of the albumin fraction was demonstrated even in patients suffering for only a short period of time. The inhibitory activity against DNA and protein synthesis in cultured hepatocytes was demonstrated in a substance extracted with a chloroform and methanol mixture from the albumin fraction of patients with fulminant hepatitis. The extract from the patients' sera also inhibited acceleration of DNA synthesis by epidermal growth factor (EGF) in the same cells.

Increased asialo-alpha-fetoprotein in patients with alpha-fetoprotein-producing tumors: demonstration by affinity electrophoresis with erythroagglutinating phytohemagglutinin of Phaseolus vulgaris lectin.

Affinity electrophoresis of human alpha-fetoprotein (AFP) with the erythroagglutinating phytohemagglutinin of Phaseolus vulgaris (E-PHA) gave up to five resolved bands (y, h, i, l and a; given in the order of decreasing affinity for E-PHA); band a having no affinity and bands y and h representing asialo-AFP's. The proportion of band y increased in extrahepatic tumors producing AFP, including yolk sac tumor, and of band h, in addition, in hepatocellular carcinoma. The proportion of either band y or h (or y + h) increased, over the means plus 2 standard deviations of the respective bands of cord serum AFP, in 20 out of 25 cases (80%) of hepatocellular carcinoma, including cell lines, and in all the patients with extrahepatic malignancy. Band i was detected in more than half the cases with malignancy, although the extent of its increases was much less. Band a appeared only in limited cases. None of the hepatitis and cirrhotic patients showed increased proportions of band y or h (or y + h), indicating the usefulness of the determination of asialo-AFP for the discrimination between benign and malignant liver diseases.
