[Regulation of lipolysis--biochemical research on the fat cells of obese patients].

Subcutaneous adipose tissue was obtained from ten obese patients. Isolated fat cells were incubated with porcine beta-lipotropin but no stimulation of lipolysis was obtained. To avoid accumulation of lipolysis inhibiting substances, such as free fatty acids and adenosin, as well as protease activity, perifusion of isolated fat cells was carried out. Under these test conditions, which allow continuous determination of released glycerol, lipolytic activity could be shown for the peptide hormone beta-lipotropin in human adipocytes.

[Experiences with Weber's subcapital rotation osteotomy in recurrent shoulder dislocation].

In 23 patients suffering from recurrent posttraumatic dislocation of the shoulder joint the derotation of the humeral head was performed as described by Weber. A description of the operative technique is given. As recommended by Weber an angled 1/2-tubular plate with 7 holes was used for stable osteosynthesis 17 times. After movement treatment removal of the osteo-synthesis-material was possible 6 to 8 months later. Because of high absence of recidivity with repair of full mobility and full strength the derotation of the humeral head is a treatment of choice in cases of recurrent posttraumatic dislocation of the shoulder joint.

[Therapy for recent and old injuries of the fibular capsular ligament in the ankle joint].

Fresh injuries of the fibular capsule-ligament-complex in the age in which physical competition takes place must be treated surgically. To achieve optimal postoperative results, the different forms of ruptures of the fibular ligaments must be treated in a differentiated way. A reconstruction of the Lig. talofibulare anterius generally succeeds without trouble, whereas a reconstruction of the Lig. calcaneofibulare often requires a plastic replacement by a periostal flap. At old ruptures, however, the tendon plastic is the safest way of reconstructing the stability of the ankle. The distally stalked tendon graft has several advantages, especially when the graft is attached in accordance with the anatomy of the fibular ligaments.

[Cartilage injuries of the femoro-patellar joint in the computerized tomogram].

The conventional arthrographic methods for demonstration of the femoro-patellar joint are not sufficiently reliable. Through the use of CT-arthrography a cross-sectional image free of superimposition and possessing a high density resolution is available thus facilitating a direct demonstration of the joint cartilage. Traumatic and degenerative lesions of the cartilage can be clearly shown by CT-arthrography. Damage of cartilage in patients with chondromalacia patellae can be differentiated in its different stages. The shape of the patella and its relation to femoral condyles can be evaluated more accurate than by conventional axial x-rays.

[Gluing of fresh cartilaginous lesions of the talus using concentrated fibrinogen adhesive (author's transl)].

Lesions of the talar cartilage are extremely difficult to diagnose clinically or roentgenologically. Often they are masked by accompanying lesions of the bone or the capsular ligaments, so that arthrotomy will be necessary to obtain information on the condition of the cartilage. Reconstruction of the articular surface is a most important prerequisite für restoring the ability of the cartilage to cope with physiological load. Hence, the aim of the treatment is the replantation of abraded cartilaginous fragments. The use of concentrated fibrinogen as a biological tissue adhesive has been gaining in importance as an alternative to the methods of refixation practised so far. The actual process of gluing is technically simple. 7 patients in whom gluing of the cartilage at the talus had been performed, remained free from complaints for a follow-up period of three years.

[Patella dislocation with special reference to cartilage damage].

Dislocations of the patella are discussed in the light of their pathogenesis, their possible injury pattern and their consequences. An exemplary case is presented. Especially the injury to the retropatellar cartilage with following chondromalacia--sometimes initiating the development of patellofemoral osteoarthritis--renders the dislocation of the patella a severe injury of the knee region. In case of spontaneous reduction and uncertain history the diagnosis may be difficult. The dislocation of the patella can be suspected if a hemarthrosis is found without an instability of the knee ligaments. The possibilities of an operative repair are described with special regard to the cartilage injury. Importance was layed upon the shape variabilities of the patellofemoral joint predisposing to recurrent dislocation.

[Prospective comparison of ARTOSCAN-MRI and arthroscopy in knee injuries].

The results of magnetic resonance imaging (MRI) were compared with those of arthroscopy in a prospective series of 276 patients. A "dedicated system" for MRI of limbs and peripheral joints--the 0,2 Tesla ARTOSCAN (ESAOTE, Italy)--was used for imaging knee joint lesions. T1-weighted spin echo sagittal images, T2-weighted gradient-echo coronal images, and axial views for lesions of bone and the femoropatellar joint were acquired. If necessary paraxial sagittal and oblique coronal views were obtained for imaging of the cruciate ligaments. This protocol allowed excellent visualization of the cruciate ligaments, medial and lateral meniscus in almost all patients. Compared with arthroscopy performed within 48 hours after imaging, the sensitivity, specificity, and accuracy were respectively, 91, 92 and 91 per cent for tears of the medial meniscus; 80, 96, and 92 per cent for tears of the posterior meniscus; 100, 100, and 100 per cent for tears of the posterior cruciate ligament; 93, 98, and 99 per cent for tears of the anterior cruciate ligament; and 73, 100, and 92 per cent for full-thickness articular cartilage lesions. The examination can be performed within 30 to 45 minutes at a cost that is lower than that of diagnostic arthroscopy. ARTOSCAN imaging is a safe and valuable adjunct to the clinical examination of the knee and an aid to efficient preoperative planning.

[Fatigue fractures in sports. Personal experiences and literature review].

Stress fractures are bone fractures, in most cases at the lower extremities, caused by overload. Spondylolysis and -listhesis can be considered as stress fractures, too. During the past 10 years, 67 female and male athletes with stress fractures were treated and followed up until full ability to practice sports was achieved again. Bone scan (technetium 99) and nuclear resonance imaging MRI are devices for an early and exact diagnosis. Stress fractures mostly occur to runners. Conservative treatment includes a training-free interval with painlessness as the most important criteria. Only in rare cases immobilization or surgical treatment are necessary. As prophylaxis it is important to advice the athletes not to ignore overload pain.

[Sports fitness following surgical treatment of osteochondrosis dissecans].

A retrospective analysis of 29 patients with regard to their sportive activities after operative treatment of osteochondritis dissecans was carried out. The study included 21 males and 8 females with an average age of 18 years. In 4 cases patients' history revealed a trauma. Osteochondritis dissecans was located in 16 cases in the knee joint, in 12 cases in the ankle joint and in one case in the elbow joint. In 11 cases the articular cartilage was not disrupted, and therefore subchondral focus drilling and autologous cancellous tissue grafting were a sufficient therapy. 10 out of those 11 patients had no complaints during their sportive activities 3 years after this operation. In another 10 cases with an osteochondral fragment, reimplantation was possible. 5 of those 10 patients were free of complaints during sportive activities 3 years later. In 2 of those 10 patients the reimplanted fragments did not become incorporated and had to be removed. 2 out of 8 patients with removal of necrotic fragments and curretage of subchondral bone had no complaints 3 years postoperatively. Best results could be achieved in patients with intact articular cartilage and subchondral lesion. Good results showed patients after reimplantation of the osteochondral fragment. Poor results were found in cases with necrotic osteochondral fragments. Osteochondritis dissecans with an osteochondral fragment should not be further treated conservatively but gives an urgent indication for operation. MRI is very helpful for this decision.
