Single case study. A novel psychiatric adjunct to cancer therapy.

Confronted with the hazards of a patient unable to cooperate predictably with a 3-day radium implantation procedure as a part of cancer therapy, the authors describe in a case report their solution to an unusual clinical dilemma. The patient displayed adverse responses to psychotropics and nursing care was markedly restricted by radiation exposure limits. Behavioral management of the patient through the interval of radium therapy was successfully effected by the use of closed-circuit television monitoring combined with brief nursing interventions. This novel psychiatric adjunct to cancer therapy averted the threat posed by displacement of the radium implants.

Primary uterine tumors and multiple endocrine adenomatosis, type I.

Multiple endocrine adenomatosis, Type I was initially diagnosed in a 35-year-old woman with primary chief cell hyperplasia of the parathyroids. Approximately 5 years later, vaginal bleeding developed and a well-differentiated endometrial adenocarcinoma was recognized. An adenomatoid tumor of the uterus was discovered in addition to a nonfunctional islet cell tumor of the pancreas. Multiple endocrine adenomatosis is reviewed in relation to possible gynecologic neoplasms.

The Brenner tumor: a clinicopathologic review.

The Brenner tumor of the ovary has been the topic of voluminous clinical and pathologic literature since its original description by Macnaughton-Jones in 1898. One of the major problems confronting most investigators of this uncommon neoplasm has been the question of histogenesis. There is general agreement that the tumor is derived from surface (coelomic) epithelium, as are the serous and mucinous cystadenomas. Most Brenner tumors are asymptomatic except for some which have features suggestive of estrogen production. The major pathologic variants are the proliferating and the malignant Brenner tumors; a poor prognosis is associated with the latter neoplasm. Approximately 30% of all benign Brenner tumors have a second histologic type of tumor in the ipsilateral or contralateral ovary, a serous or mucinous cystadenoma in most cases. Brenner tumors have also been reported in the broad ligament and in the testis on rare occasions.

Pelvic exenteration: complications of urinary diversion.

We report complications directly related to urinary diversion among 32 patients who underwent pelvic exenteration for gynecologic malignancy. Eight patients (25 per cent) sustained 10 major complications, including 8 urinary fistulas and 2 fistulas from the entero-enterostomy sites. The frequency of these complications was greater among patients who had had total exenteration than among those who had had anterior exenteration. We attempt to identify factors responsible for these complications and discuss methods for reducing the morbidity related to urinary diversion in these patients.
