Coronary calcium does not accurately predict near-term future coronary events in high-risk adults.

BACKGROUND: Prognostic risk models have had limited success in predicting coronary events in subjects with multiple risk factors. We and others have proposed an alternative approach using radiographically detectable coronary calcium. We evaluated and compared the predictive value of these 2 approaches for determining coronary event risk in asymptomatic adults with multiple coronary risk factors. In addition, we assessed the predictive value of a risk model that included calcium score and cardiac risk-factor data. METHODS AND RESULTS: We recruited 1196 asymptomatic high-coronary-risk subjects who then underwent risk-factor assessment and cardiac electron-beam CT (EBCT) scanning and were followed up for 41 months with a 99% success rate. We applied the Framingham model and our data-derived risk model to determine the 3-year likelihood of a coronary event. The mean age of our cohort was 66 years, and mean 3-year Framingham risk was 3.3+/-3.6%. Sixty-eight percent (818 subjects) had detectable coronary calcium. There were 17 coronary deaths (1.4%) and 29 nonfatal infarctions (2. 4%). The receiver operating characteristic (ROC) curve areas calculated from the Framingham model, our data-derived risk model, and the calcium score were 0.69+/-0.05, 0.68+/-0.05, and 0.64+/-0.05, respectively (P=NS). When calcium score was included as a variable in the data-derived model, the ROC area did not change significantly (0.68+/-0.05 to 0.71+/-0.04; P=NS). CONCLUSIONS: Neither risk-factor assessment nor EBCT calcium is an accurate event predictor in high-risk asymptomatic adults. EBCT calcium score does not add significant incremental information to risk factors, and its use in clinical screening is not justified at this time.

Nitrate-enhanced thallium 201 single-photon emission computed tomography imaging in hibernating myocardium.

OBJECTIVES: This study tested the usefulness of nitrate-enhanced thallium 201 imaging for detecting myocardial viability. BACKGROUND: Previous work suggests that nitrates enhance the ability of (201)Tl imaging to detect viable myocardium. METHODS: Eighteen patients with coronary artery disease underwent (201)Tl imaging at rest, after 4 hours of redistribution, and during intravenous nitroglycerine infusion (mean dose = 5.96 +/- 5.37 microgram/kg/min). Twelve patients had their echocardiograms repeated after revascularization. Perfusion and wall motion were scored from 0 to 2 (absent to normal). RESULTS: All the regions identified as viable by the rest/redistribution pair of scans were identified as viable by the rest/nitroglycerine pair of scans. Ninety-one percent of these regions were identified as viable by the single nitroglycerine scan alone. In patients who underwent revascularization, the total (201)Tl perfusion score improved from 193 to 214 after revascularization (P =.009). Wall motion score improved from 151 to 168 after revascularization (P =.09). Both the rest/nitroglycerine and rest/redistribution studies correctly predicted 14 (88%) of 16 regions that improved after revascularization. Most importantly, the rest/nitroglycerine and rest/redistribution studies were able to predict postrevascularization myocardial viability (absence of akinesis or dyskinesis after revascularization), with a sensitivity of 95% and 92%, respectively, and a predictive accuracy of 84.4%. CONCLUSIONS: Nitroglycerine infusion during (201)Tl imaging is a useful technique for detecting underperfused, viable myocardium, requires less time to perform than rest/redistribution imaging, and may allow detection of viable myocardium with a single (201)Tl single-photon emission computed tomographic study.

Comparison of exercise electron beam computed tomography and sestamibi in the evaluation of coronary artery disease.

This blinded, single center study prospectively compares exercise electron beam computed tomography (EBCT) with stress technetium-99m (Tc-99m) sestamibi single-photon emission computed tomography (SPECT) in 33 patients undergoing coronary angiography for evaluation of chest pain. Patients undergoing routine cardiac catheterization for the diagnosis of chest pain were imaged at rest using EBCT. Patients exercised on a semi-supine ergometer, and exercise EBCT was immediately followed by injection of Tc-99m sestamibi for assessment of myocardial ischemia. At peak exercise, Tc-99m SPECT, followed immediately by nonionic contrast material, was injected intravenously to directly compare these 2 imaging techniques. Patients were reimaged with Tc-99m SPECT at rest 24 to 48 hours after stress. Exercise EBCT, which was analyzed using a global ejection fraction measure, had a sensitivity of 81% and a specificity of 76%, compared with angiography. Using the development of a new regional wall motion abnormality as evidence of obstructive coronary artery disease (CAD), EBCT yielded a specificity of 100% and a sensitivity of 88%. Reversible perfusion defects identified by SPECT, as evidence of obstructive CAD, revealed a sensitivity of 75% and a specificity of 71%. The specificity of regional wall motion analysis by EBCT was significantly better than SPECT (p <0.01) in this population. This study demonstrates regional wall motion assessed by EBCT to be as sensitive and more specific than SPECT myocardial perfusion imaging in identifying obstructive CAD as defined by angiography.

Safety and efficacy of carvedilol in severe heart failure. The U.S. Carvedilol Heart Failure Study Group.

BACKGROUND: Many patients remain markedly symptomatic despite optimal current therapy for heart failure. Beta-blockers have often been viewed as contraindicated in this group because of their potential adverse short-term effects on cardiac function. METHODS AND RESULTS: One hundred thirty-one patients with severe congestive heart failure were enrolled into a double-blind, placebo-controlled study of the vasodilating beta-blocker carvedilol. All patients had symptomatic, advanced heart failure while on standard triple therapy, as evidenced by a mean ejection fraction of 0.22, marked reduction in distance traveled in a 6-minute corridor walk test, and severe impairment in quality of life measured by the Minnesota Living With Heart Failure Questionnaire. After a 2-week, open-label test of 6.25 mg twice daily carvedilol, 105 patients were randomized (2:1) to receive either carvedilol (up to 25 mg twice daily, n = 70) or matching placebo (n = 35) for 6 months while background therapy with digoxin, diuretics, and an angiotensin-converting enzyme inhibitor remained constant. Ten patients (8%) did not complete the open-label period because of adverse events and 11.4% in both the carvedilol and placebo groups dropped out in the double-blind phase. The study was terminated early by the Data Safety and Monitoring Board and follow-up evaluation was therefore aborted before the projected number of patients and follow-up time was achieved. Quality of life, which was the primary endpoint, improved similarly in the carvedilol and placebo groups, whereas the global assessment by the physicians and the patient exhibited a better response to carvedilol (P < .05). Hospitalization and mortality rate were too low to evaluate a difference, and exercise time and New York Heart Association classification did not change significantly in response to the drug. Left ventricular ejection fraction rose significantly (+0.09) in the carvedilol group compared with the placebo group (+0.02, P = .004). CONCLUSION: The beta-blocker carvedilol can be safely employed in patients with severe heart failure. Improved left ventricular function with a trend for some improvement in symptoms combined with the experience with the drug in the larger population of less severe patients in this multicenter trial suggests that carvedilol may have a favorable long-term effect in heart failure of diverse severity.

Carvedilol produces dose-related improvements in left ventricular function and survival in subjects with chronic heart failure. MOCHA Investigators.

BACKGROUND: We conducted a multicenter, placebo-controlled trial designed to establish the efficacy and safety of carvedilol, a "third-generation" beta -blocking agent with vasodilator properties, in chronic heart failure. METHODS AND RESULTS: Three hundred forty-five subjects with mild to moderate, stable chronic heart failure were randomized to receive treatment with placebo, 6.25 mg BID carvedilol (low-dose group), 12.5 mg BID carvedilol (medium-dose group), or 25 mg BID carvedilol (high-dose group). After a 2- to 4-week up-titration period, subjects remained on study medication for a period of 6 months. The primary efficacy parameter was submaximal exercise measured by two different techniques, the 6-minute corridor walk test and the 9-minute self-powered treadmill test. Carvedilol had no detectable effect on submaximal exercise as measured by either technique. However, carvedilol was associated with dose-related improvements in LV function (by 5, 6, and 8 ejection fraction [EF] units in the low-, medium-, and high-dose carvedilol groups, respectively, compared with 2 EF units with placebo, P < .001 for linear dose response) and survival (respective crude mortality rates of 6.0%, 6.7%, and 1.1% with increasing doses of carvedilol compared with 15.5% in the placebo group, P < .001). When the three carvedilol groups were combined, the all-cause actuarial mortality risk was lowered by 73% in carvedilol-treated subjects (P < .001). Carvedilol also lowered the hospitalization rate (by 58% to 64%, P = .01) and was generally well tolerated. CONCLUSIONS: In subjects with mild to moderate heart failure from systolic dysfunction, carvedilol produced dose-related improvements in LV function and dose-related reductions in mortality and hospitalization rate.

Prognostic significance of cardiac cinefluoroscopy for coronary calcific deposits in asymptomatic high risk subjects.

OBJECTIVES: This research investigated the prognostic significance of radiographically detectable coronary calcific deposits. BACKGROUND: Coronary calcific deposits are almost always associated with coronary atherosclerosis. We investigated the association between fluoroscopically determined coronary calcium and coronary heart disease end points at 1 year of follow-up. METHODS: This prospective population-based cohort study was conducted in the suburbs of Los Angeles. Fourteen hundred sixty-one asymptomatic adults with an estimated > or = 10% risk of having a coronary heart disease event within 8 years underwent cardiac cinefluoroscopy for assessment of coronary calcium at initiation of the study. Clinical status including angina, documented myocardial infarction, myocardial revascularization and death from coronary heart disease were determined after 1 year. RESULTS: The prevalence of calcific deposits was high (47%). A follow-up examination at 1 year was successfully completed in 99.9% of subjects. Six subjects (0.4%) had died from coronary heart disease and 9 (0.6%) had had a nonfatal myocardial infarction. Thirty-seven subjects (2.5%) reported angina pectoris, and 13 (0.9%) had undergone myocardial revascularization. Fifty-three subjects had at least one event during the 1-year period. Radiographically detectable calcium was associated with the presence of at least one of these end points, with a risk ratio of 2.7 (confidence limits 1.4, 4.6). The presence of coronary calcium was an independent predictor of at least one end point when controlling for age, gender and risk factors. However, three deaths due to coronary heart disease and two nonfatal myocardial infarctions occurred in subjects without detectable coronary calcium. CONCLUSIONS: The presence of coronary calcific deposits incurs an increased risk of coronary heart disease events in asymptomatic high risk subjects at 1 year. This increased risk is independent of that incurred by standard risk factors.

Spontaneous variability of ventricular function in patients with chronic heart failure. The Western Enoximone Study Group and the REFLECT Investigators.

PURPOSE: The spontaneous variation of the left and right ventricular ejection fractions (LVEF and RVEF) was evaluated in patients with chronic heart failure receiving only digoxin and diuretics over a 12-week interval. PATIENTS AND METHODS: Fifty-one patients with stable heart failure were studied with radionuclide angiography. A baseline evaluation and a 12-week follow-up study were performed. Heart failure therapy consisted of digoxin and diuretics alone during this time. RESULTS: The mean baseline LVEF (n = 51) was 27.2 +/- 9.5 (range: 7 to 50) and the LVEF after 12 weeks was 27.6 +/- 9.7 (range: 11 to 53; p = NS versus baseline). Mean RVEF (n = 19) was 31.9 +/- 11.3 at baseline and 30.3 +/- 11.3 (range: 14 to 50; p = NS versus baseline) after 12 weeks. Although there was no significant change in mean LVEF or RVEF in this group of patients, individual patients demonstrated considerable spontaneous variation. Individual LVEF values changed from 0 to 26 ejection fraction percentage points (mean of individual changes = 5.6 +/- 5.5). Individual RVEF determinations over the 12-week period varied by 0 to 15 percentage units (mean = 5.6 +/- 4.9). Thirty-five percent of patients had an absolute change in LVEF greater than 5 and 37% of patients had an absolute change of RVEF greater than 5. Even after deletion of the two worst outliers from the LVEF and RVEF data, a change in LVEF greater than 13 and a change in RVEF greater than 11% units were necessary to exclude spontaneous variation as a likely cause for the observed changes (95% confidence limits). No relationship between a change in the individual patient's LVEF or RVEF was found when these values were compared with exercise time, systolic or diastolic blood pressure, heart rate, or degree of baseline left or right ventricular dysfunction. CONCLUSION: In patients with heart failure, large (greater than 5) spontaneous changes in LVEF and RVEF may be seen in over one third of patients during a 12-week period. This variability should be considered when the ejection fraction is used as an index of improved or worsened cardiac function. The use of the LVEF and RVEF to assess interventions or therapy for heart failure should be interpreted with caution.

Double-blind, placebo-controlled study of the efficacy of flosequinan in patients with chronic heart failure. Principal Investigators of the REFLECT Study.

OBJECTIVES: The aim of this study was to assess the efficacy of flosequinan in chronic heart failure. BACKGROUND: Flosequinan is a new vasodilator drug that acts by interfering with the inositol-triphosphate/protein kinase C pathway, an important mechanism of vasoconstriction. The drug dilates both peripheral arteries and veins, is orally active and has a long duration of action that permits once-daily dosing. Previous studies have shown that flosequinan produces sustained hemodynamic benefits in heart failure, but large scale studies evaluating its clinical efficacy have not been reported. METHODS: One hundred ninety-three patients with chronic heart failure (New York Heart Association functional class II or III and left ventricular ejection fraction < 40%) receiving digoxin and diuretic drugs were randomly assigned (double-blind) to the addition of flosequinan (100 mg once daily, n = 93) or placebo (n = 100) for 3 months. The clinical status and exercise tolerance of each patient was evaluated at the start of the study and every 2 to 4 weeks during the trial while background therapy remained constant. RESULTS: After 12 weeks, maximal treadmill exercise time increased by 96 s in the flosequinan group but by only 47 s in the placebo group (p = 0.022 for the difference between groups). Maximal oxygen consumption increased by 1.7 ml/kg per min in the flosequinan group (n = 17) but by only 0.6 ml/kg per min in the placebo group (n = 23), p = 0.05 between the groups. Symptomatically, 55% of patients receiving flosequinan but only 36% of patients receiving placebo benefited from treatment (p = 0.018). In addition, fewer patients treated with flosequinan had sufficiently severe worsening of heart failure to require a change in medication or withdrawal from the study (p = 0.07). By intention to treat, seven patients in the flosequinan group and two patients in the placebo group died. CONCLUSIONS: These findings indicate that flosequinan is an effective drug for patients with chronic heart failure who remain symptomatic despite treatment with digoxin and diuretic drugs. The effect of the drug on survival remains to be determined.

Assessment of myocardial perfusion defect size after early and delayed SPECT imaging with technetium-99m-hexakis 2-methoxyisobutyl isonitrile after stress.

It is thought that the distribution of 99mTc-sestamibi undergoes minimal change during the first 4 hr after injection. Thus [99mTc] sestamibi unlike 201Tl should not demonstrate significant redistribution in ischemic myocardium. We tested this assumption by quantifying perfusion defect size in early and delayed clinical SPECT images obtained after exercise stress or after dipyridamole infusion using an automated algorithm. Twenty patients with coronary artery disease aged 55 +/- 10 yr (13 male and 7 female) underwent stress imaging. Technetium-99m-sestamibi was injected at peak exercise stress in 12 patients and after an intravenous infusion of dipyridamole in 8 patients; SPECT images were obtained 1 and 4 hr later. All patients underwent rest imaging with a second dose of 99mTc-sestamibi 1-3 days after the stress studies. Total left ventricular mass and left ventricular defect mass were quantified with an automated algorithm previously validated in animal and patient studies. Estimates of total left ventricular mass from studies obtained 1 hr after stress (341 +/- 90 g) were comparable to values obtained after 4 hr (336 +/- 89 g), with a correlation of r = 0.97; p < 0.0001. The left ventricular defect size 1 hr after exercise or dipyridamole infusion (149 +/- 74 g) was similar to that observed after 4 hr (151 +/- 73 g). The two measurements of hypoperfusion with stress were highly correlated (r = 0.91; p < 0.0001) and were significantly larger than the defect size at rest (121 +/- 70 g). These observations support the conclusion that 99mTc-sestamibi does not redistribute significantly in ischemic myocardium between 1 and 4 hr after injection.

Effects of exercise test results on physician perceptions of coronary artery disease probability using a threshold analysis.

The incremental diagnostic value of exercise electrocardiographic testing compared with clinical data alone in the diagnosis of coronary artery disease (CAD) was found to be of limited value in a previous study. That study used a computer algorithm for diagnosing disease. Thus, the strict use of the results apply only to computerized disease diagnosis rather than physicians' diagnoses. The aim of the present study was to determine whether exercise test data improve a physician's ability to detect the presence or absence of CAD and 3-vessel or left main CAD, and whether the data effect the decision to perform angiography. The study sample comprised a data base of 312 patients whose clinical data, exercise test results and coronary anatomy were known. Individual cases were presented to 8 cardiologists with and without exercise data. The cardiologists provided estimates of disease probability and were asked whether they would request coronary angiography. Receiver-operating characteristic curves and goodness-of-fit analysis showed better discrimination and calibration of the estimates for the presence of CAD and 3-vessel/left main CAD after exercise test results were known. Individualized probability thresholds for deciding whether to request angiography were determined for each physician. Forty-three percent of patients crossed greater than or equal to 1 threshold, with 64% of the crossings in the correct direction. For estimating the presence of any CAD, 84% of crossings from below to above a threshold were correct, whereas only 36% from above to below were correct. For 3-vessel/left main CAD, 36% of crossings from below to above a threshold were correct, whereas 88% from above to below were correct.(ABSTRACT TRUNCATED AT 250 WORDS)

Hemodynamic effects of bepridil in patients with coronary artery disease.

Bepridil is a calcium antagonist with a unique chemical structure and properties that differ from other calcium antagonists (e.g., a long half-life, sodium channel inhibition). In patients with normal left ventricular function, intravenous bepridil produces modest and short-lived reductions in heart rate and blood pressure. A transient negative inotropic effect is associated with higher doses (3-4 mg/kg). At the highest recommended oral dosage (400 mg/day), bepridil decreases resting heart rate and blood pressure; maximal exercise double product is not significantly reduced. In patients with impaired left ventricular function who are receiving oral bepridil 400 mg/day, virtually no distinguishable effects on heart rate, mean arterial pressure, pulmonary artery diastolic pressure or systemic vascular resistance were observed compared with placebo treatment. In patients with coronary disease, bepridil improves exercise hemodynamics (i.e., stroke volume index, cardiac index, ejection fraction) and reduces the frequency of wall motion abnormalities compared with placebo. Scintigraphic studies performed during stress suggest that compared with placebo, bepridil improves myocardial perfusion.

Bepridil hydrochloride compared with placebo in patients with stable angina pectoris.

Bepridil is a calcium antagonist with a unique chemical composition and a long elimination half-life (42 hours). We evaluated the efficacy of bepridil 300 mg once/day in a crossover comparison with placebo in 45 patients with angina. Patients had an average of 7.6 anginal episodes/week during the placebo baseline phase of the trial. After 4 weeks of bepridil therapy, anginal frequency decreased to 2.9 episodes/week (p less than 0.05). Likewise, mean nitroglycerin consumption declined from 7.4 tablets/week during the placebo baseline phase to 4.0 tablets/week during bepridil therapy (p less than 0.05). Statistically significant increases over the previous period (placebo baseline or double-blind placebo) were seen in total exercise time, time to angina, and total work (p less than 0.05). During bepridil therapy, 13 of 45 patients (29%) no longer experienced angina as an exercise end point despite the increase in work and exercise time. Bepridil significantly prolonged both the QT and corrected QT (QTc) intervals; the mean increases were 10.0% and 5.6%, respectively. Side effects were reported with equal frequency in the placebo and bepridil arms of the trial, and no serious side effects were reported. In an intermediate fixed dose of 300 mg/day, bepridil relieved anginal symptoms with few side effects. Bepridil appears to be a safe and effective treatment for stable angina.

Oral enoximone therapy in chronic heart failure: a placebo-controlled randomized trial. The Western Enoximone Study Group.

In a parallel study design, 164 patients with New York Heart Association Functional class II or III heart failure were randomized to receive either enoximone given as 50 mg three times a day, or 100 mg three times a day, or a matching placebo. All patients were receiving digitalis and/or diuretics and had left ventricular ejection fractions less than or equal to 45. Exercise tests were performed after 1, 4, 8, and 12 weeks of treatment. Enoximone produced significantly greater increases in exercise time than placebo treatment at weeks 4 and 8 (p = 0.012, p = 0.029, respectively) but not after 12 weeks. Left ventricular ejection fraction increased significantly after the first dose of enoximone but not after 12 weeks of long-term therapy. Heart failure symptoms and the physicians' evaluations of cardiac status were significantly improved in both enoximone therapy groups during the first 4 weeks of evaluation when compared with evaluations of cardiac status in the placebo group. Diuretic doses were increased more frequently for patients who were receiving a placebo. Adverse events were reported with similar frequency in the placebo and 50 mg enoximone treatment groups; 100 mg enoximone resulted in a significantly greater incidence of adverse events. Mean heart rate and ventricular ectopic activity were not significantly different among the three treatment limbs. Enoximone appears to improve exercise tolerance, ventricular function, and symptoms of heart failure for 4 to 8 weeks. Comparison of thallium-201 and technetium-99m hexakis 2-methoxyisobutyl isonitrile single-photon emission computed tomography for estimating the extent of myocardial ischemia and infarction in coronary artery disease.

Single-photon emission computed tomography (SPECT) using thallium-201 (Tl-201) was compared with technetium-99m hexakis 2-methoxyisobutyl isonitrile (Tc-99m MIBI) in 24 patients with coronary artery diseaes. Patients exercised to the same work load as each isotope was studied. Normal and hypoperfused left ventricular mass was determined with an automated method. Estimated total left ventricular mass was similar for both stress/redistribution Tl-201 and stress/rest Tc-99m MIBI images. The mean estimated defect size in the redistribution Tl-201 images was 32 +/- 34.7 vs 33 +/- 38.4 g in the resting Tc-99m MIBI studies (difference not significant). The individual determinations of defect mass were highly correlated (r = 0.93; p less than 0.0001). Estimated defect size in the stress Tl-201 images (52 +/- 46.2 g) was significantly larger than the exercise Tc-99m MIBI estimates of defect mass (42 +/- 39.9 g; p less than 0.05). A linear correlation existed between stress thallium and technetium estimates of defect size (r = 0.85) but 15 of 24 Tc-99m MIBI defects were smaller than the Tl-201 defects. Partial redistribution of Tc-99m MIBI could explain the discordance. Stress Tc-99m MIBI SPECT defect size determined by visual interpretation or by the use of isocount analysis may be smaller than what is seen with stress Tl-201 SPECT.

Double-blind comparison of once daily betaxolol versus propranolol four times daily in stable angina pectoris. Betaxolol Investigators Group.

Betaxolol is a new, highly cardioselective, once-a-day beta blocker with a long half-life (mean 16 hours). The antianginal efficacy of 2 doses of betaxolol (20 and 40 mg) given once daily was evaluated and compared with propranolol (40 or 80 mg) 4 times daily. Ninety-two patients completed the 10-week double-blind trial. The resting and exercise heart rate, blood pressure and double product were similar for all treatment arms of the study during placebo treatment. Significant decreases in these measures occurred during active drug treatment when compared with placebo. No significant intergroup differences were noted at rest. Maximal exercise heart rate and double product were significantly lower during treatment with betaxolol 40 mg daily than in the propranolol 40 mg 4 times/day treatment group (p less than 0.05). All patients had chest pain and greater than or equal to 1 mm of ST-segment depression during the baseline placebo exercise test. After 10 weeks of active treatment, 55% of the patients were free of chest pain during maximal exercise (difference not significant between treatments). In the betaxolol 40 mg/day group, fewer (6 of 19; 32%) of the patients developed ST-segment depression with exercise (p less than 0.05) compared with propranolol 80 mg 4 times daily (21 of 26; 81%). Betaxolol appears to be a useful once-a-day cardioselective beta blocker for the therapy of angina pectoris.

Simultaneous assessment of left ventricular wall motion and myocardial perfusion with technetium-99m-methoxy isobutyl isonitrile at stress and rest in patients with angina: comparison with thallium-201 SPECT.

The newly developed technetium-99m (99mTc) isonitriles can be used for the simultaneous evaluation of ventricular function and myocardial perfusion. We compared technetium-99m hexakis-2-methoxy isobutyl isonitrile [( 99mTc] MIBI) derived first-pass left ventricular wall motion at stress and rest with simultaneous myocardial perfusion defined by [99mTc]MIBI SPECT. These results were then compared with 201TI SPECT. We examined 28 patients with coronary artery disease; 25 had a previous myocardial infarction. We found concordance between segmental wall motion and myocardial perfusion imaging in defining normal, ischemic, and infarcted myocardium in 68% and 69% of segments using [99mTc]MIBI and 201TI respectively. The best agreement between wall motion and myocardial perfusion was seen in the inferior wall, while most of the discrepancies were found at the apex. Agreement between [99mTc]MIBI and 201TI SPECT myocardial perfusion was seen in 93% of segments. Technetium-99m-MIBI appears to be an ideal radiopharmaceutical for the simultaneous evaluation of ventricular function and myocardial perfusion during stress and at rest.

Comparison of atenolol and nifedipine in chronic stable angina pectoris.

The antiangina effects of atenolol, 50 to 200 mg once daily, or nifedipine, 10 to 30 mg 3 times daily, were evaluated in a multicenter, randomized, double-blind, parallel study of 39 patients with known symptomatic coronary artery disease. Treatment was titrated to produce at least a 30% increase in treadmill exercise duration over placebo baseline and then maintained at that dosage for an additional 3 weeks. Both treatments produced significant (p less than 0.001) increases in duration of exercise, total work and exercise capacity when compared with placebo baseline. These improvements in exercise performance were obtained with significant (p less than 0.001) reductions in both ST-segment depression and rate-pressure product for atenolol compared with nifedipine. Furthermore, the total angina attack rate and rate at rest were significantly (p less than 0.01) less frequent with atenolol than with nifedipine. Hence, the antiischemic effects of atenolol exceeded those of nifedipine, based on ST-segment depression and rate-pressure product at comparable workloads.
