A traditional herbal medicine enhances bilirubin clearance by activating the nuclear receptor CAR.

Yin Zhi Huang, a decoction of Yin Chin (Artemisia capillaris) and three other herbs, is widely used in Asia to prevent and treat neonatal jaundice. We recently identified the constitutive androstane receptor (CAR, NR1I3) as a key regulator of bilirubin clearance in the liver. Here we show that treatment of WT and humanized CAR transgenic mice with Yin Zhi Huang for 3 days accelerates the clearance of intravenously infused bilirubin. This effect is absent in CAR knockout animals. Expression of bilirubin glucuronyl transferase and other components of the bilirubin metabolism pathway is induced by Yin Zhi Huang treatment of WT mice or mice expressing only human CAR, but not CAR knockout animals. 6,7-Dimethylesculetin, a compound present in Yin Chin, activates CAR in primary hepatocytes from both WT and humanized CAR mice and accelerates bilirubin clearance in vivo. We conclude that CAR mediates the effects of Yin Zhi Huang on bilirubin clearance and that 6,7-dimethylesculetin is an active component of this herbal medicine. CAR is a potential target for the development of new drugs to treat neonatal, genetic, or acquired forms of jaundice.

Size distributions of maternal and fetal DNA in maternal plasma.

BACKGROUND: The discovery of fetal DNA in maternal plasma has opened up an approach for noninvasive prenatal diagnosis. Despite the rapid expansion in clinical applications, the molecular characteristics of plasma DNA in pregnant women remain unclear. METHODS: We investigated the size distribution of plasma DNA in 34 nonpregnant women and 31 pregnant women, using a panel of quantitative PCR assays with different amplicon sizes targeting the leptin gene. We also determined the size distribution of fetal DNA in maternal plasma by targeting the SRY gene. RESULTS: The median percentages of plasma DNA with size >201 bp were 57% and 14% for pregnant and nonpregnant women, respectively (P <0.001, Mann-Whitney test). The median percentages of fetal-derived DNA with sizes >193 bp and >313 bp were 20% and 0%, respectively, in maternal plasma. CONCLUSION: Plasma DNA molecules are mainly short DNA fragments. The DNA fragments in the plasma of pregnant women are significantly longer than those in the plasma of nonpregnant women, and the maternal-derived DNA molecules are longer than the fetal-derived ones.

Cloning and expression of a novel CREB mRNA splice variant in human testis.

Identification of genes specifically expressed in adult and fetal testis is important in furthering our understanding of testis development and function. In this study, a novel human transcript, designated human testis cAMP-responsive element-binding protein (htCREB), was identified by hybridization of adult and fetal human testis cDNA probes with a human cDNA microarray containing 9216 clones. The htCREB transcript (GenBank Accession no. AY347527) was expressed at 2.35-fold higher levels in adult human testes than in fetal testes. Sequence and ntBLAST analyses against the human genome database indicated that htCREB was a novel splice variant of human CREB. RT-PCR-based tissue distribution experiments demonstrated that the htCREB transcript was highly expressed in adult human testis and in healthy sperm, but not in testes from patients with Sertoli cell-only syndrome. Taken together, these results suggest that the htCREB transcript is chiefly expressed in germ cells and is most likely involved in spermatogenesis.

Raising awareness of fall risk among Chinese older adults. Use of the Home Fall Hazards Assessment tool.

The purpose of this study was to evaluate the effectiveness of the Home Fall Hazards Assessment (HFHA) tool adjusted for Chinese homes to raise awareness of the dangers of falling among Chinese older adults. This descriptive study of 415 Chinese adults age 60 and older was completed in the older adults' homes in the community. The study was an assessment by nurses of hazards in the home setting using the HFHA. The hazards identified by nurses were compared with those reported by the elderly individuals. The study identified the most frequently found hazards in the homes and showed that a large discrepancy existed between the actual hazards assessed by trained nurses using the HFHA and the perceived hazards reported by the participants (p < .001). The study suggests that the HFHA can be an effective health education method and that the involvement of older adults in the assessment process will effectively raise their awareness of existing risk factors for falls in their everyday life.

[Significance of serological markers and virological marker for hepatitis E in rhesus monkey model].

OBJECTIVE: To evaluate the serological markers and biological marker in the diagnosis of hepatitis E infection in a rhesus monkey model. METHODS: 86 rhesus monkeys had been infected with different doses of HEV. Hence, they were taken sequential blood samples at intervals up to 86 weeks for 4 hepatitis E virus (HEV) specific antibody assays (E2-IgM, E2-IgG, GL-IgG, and YES-IgG), and nucleic acid assay. RESULTS: All the animals produced E2-IgG and all but one also produced E2-IgM and excreted the virus in stool, whereas positive rate of GL-IgG and YES IgG were low and correlated with virus level. Hepatitis occurred over a period of 4 weeks (between 3 an 7 weeks) after infection. Virological marker occurred mainly during incubation period and declined rapidly after onset of hepatitis. Seroconversion of E2-IgM occurred before onset of hepatitis in 70% monkeys and declined rapidly up to 50% of peak value after 4 weeks. E2-IgM seroconversion was closely paralleled by E2-IgG; however, E2-IgG persisted in all animals for the entire duration of experiment of up to 86 weeks. Production of GL-IgG and YES-IgG was delayed by one week after the E2 antibodies, these antibodies showed a transient occurrence and seroprevalence declined to 50% of the peak value over a period of 12 weeks. CONCLUSION: E2-IgM might be used as a suitable acute hepatitis E marker, and E2-IgG as a suitable epidemiological marker. The seroconversion or titer elevation of GL-IgG and YES-IgG antibodies probably used to confirm the infection. The viral markers are optional for early diagnosis.

[Investigation of sub-clinical infection of hepatitis E virus in blood donors].

OBJECTIVE: To understand the prevalence of sub-clinical infection of hepatitis E virus (HEV) among blood donors. METHODS: A cluster sampling strategy was used to sample all blood donors from July to August in 2002 in Beijing. Their blood was tested for IgM and IgG antibody against HEV. RESULTS: The prevalence of anti-HEV IgM among blood donors in Beijing was 1.74%. The rate of abnormal alanine aminotransferase (ALT) in anti-HEV IgM positive donors is significantly higher than anti-HEV IgM negative donors. Among all ALT abnormal donors, 2.68% can be associated with HEV sub-clinical infection. The percentage is similar with HBV but higher than HCV. CONCLUSION: There are sub-clinical infection of HEV among blood donors, which is one of the cause of abnormal ALT in the donors.

[Detection of hepatitis E virus on a blood donor and its infectivity to rhesus monkey].

OBJECTIVE: To understand the infectivity and pathogenicity of the plasma of hepatitis E virus (HEV) viremia to primate animals. METHODS: RNA fragment of HEV genotype IV was detected on one healthy donor who was positive for anti-HEV IgM and negative for anti-HEV IgG. Then 10 ml plasma from above donor was transfused to rhesus monkey to observe its infectivity and pathogenicity. RESULTS: Acute hepatitis E was developed in rhesus monkey who accept HEV RNA positive plasma. It was confirmed by virological, immunological, biochemical and histopathological data. CONCLUSION: Acute hepatitis E can be induced by plasma transfusion of HEV viremia, which indicate the possibility of transfusion transmitted hepatitis E

[Evaluation of the reliability of three different hepatitis E diagnosis reagents on the clinical acute hepatitis E].

OBJECTIVE: To evaluate the reliability of different hepatitis E diagnosis reagent tests on the acute hepatitis E. METHODS: Three acute hepatitis E diagnosis tests, E2-IgM (Wantai, China), GL-IgM and GL-IgG (Genelabs, Singapore) were compared for their reliability in a sera panel composed by 273 healthy individuals and 525 hepatitis. RESULTS: The specificity of E2-IgM on the diagnosis of acute hepatitis E was 100.0%, it was significantly higher than GL-IgM (96.7%) and GL-IgG (85.4%). The sensitivity of E2-IgM and GL-IgG were 97.9% and 93.8% respectively, both significantly higher than GL-IgM (72.9%). Among 65 acute hepatitis cases being positive on GL-IgM test but negative on E2-IgM, 58 (89.2%) cases were found to be positive with anti-hepatitis A virus IgM, it indicated that the GL-IgM test might be interfered by other IgM antibodies on serum. CONCLUSION: E2-IgM is a good test for the diagnosis of acute hepatitis E.

[Effects of high-volume hemofiltration on serum levels of tumor necrosis factor and its receptors in patients with multiple organ dysfunction syndromes].

OBJECTIVE: To investigate whether serum tumor necrosis factor-alpha (TNF-alpha) and its receptors can be removed by high-volume hemofiltration (HVHF) or continuous veno-venous hemofiltration (CVVH). METHODS: The study was performed in 12 multiple organ dysfunstion syndrome (MODS) patients with acute renal failure (ARF). They were randomized to receive either CVVH (n=10) or HVHF (n=8). TNF-alpha and soluble tumor necrosis factor-receptor (sTNF-R1 and sTNF-R2) concentrations were measured in serum by enzyme-linked immunoadsorbent assay (ELISA). RESULTS: Compared with that before the therapy, the average concentrations of plasma creatinine and urea were decreased significantly 8 hours after HVHF or CVVH in MODS patients with ARF (P<0.001). In patients on HVHF, the serum TNF-alpha concentrations were significantly lower 8 hours after treatment (P<0.01) compared with that before treatment, 1 hour and 4 hours after treatment. There were not significant changes in the serum TNF-alpha concentrations in patients on CVVH and the serum sTNF-R1 and sTNF-R2 concentrations in patients on CVVH or HVHF. CONCLUSION: In MODS patients with ARF undergoing HVHF, the serum TNF-alpha concentrations dropped significantly, but the serum sTNF-R1 and sTNF-R2 concentrations do not change significantly. Our study suggests that HVHF may be the better option for the treatment of MODS patients.

[Expression and bioactivity analysis of the fusion protein GFP-V(L) of the neutralizing monoclonal antibody MA18/7 against HBV].

AIM: To express the fusion protein of enhanced green fluorescent protein (EGFP) with the light chain variable domain of the neutralizing monoclonal antibody MA18/7 (mAb) against hepatitis B virus in E.coli, and determine its bioactivity. METHODS: The EGFP gene was cloned into vector pTO-T7 to construct an expression vector. And then according to ORF gene, MA18/7-V(L) was inserted into the 5' terminal of EGFP gene free of terminal code TAA to construct expression vector of fusion protein. The fusion protein was expressed in E.coli and its bioactivity was detected with ELISA and relative fluorescence intensity. RESULTS: The expression vector EGFP-V(L) was constructed. SDS-PAGE analysis showed that expressed fusion protein was mainly in the form of inclusion body. The fusion protein retained the property of EGFP and it could bind to V(H) to form Fv which had binding activity to pre-S1. CONCLUSION: The obtained fusion protein had good bioactivity and could be applied to further studies.

[Effects of chronic alcohol administration on the activities of the key glycolytic enzymes of rats].

In order to study the effects of chronic alcohol administration on the activities of the key glycolytic enzymes, a oral administration of alcohol solution in different concentrations was given to rats for 9 weeks. The activities of glucokinase (GK), phosphofructokinase (PFK) and hexokinase (HK) in liver were determined at the end of the study. The results showed that the activities of GK and PFK in the groups administrated with 30% and 50% alcohol solution decreased obviously in comparison with that in control group (P < 0.05). But the activity of HK had no significant differences in all five groups. Correlation analysis indicated that the activities of GK and PFK were correlated with the dose of alcohol significantly (r = -0.99(-)-0.97, P < 0.05). These results demonstrated that chronic alcohol administration can suppress the glycolytic pathway. They can also be one of the mechanisms of diabetes mellitus relating to alcohol because of the high blood sugar and the disturbed blood sugar regulation.

[Physiological mechanism of rice seedlings in low temperature-and damping-off disease resistance induced by calcium application into soil].

Applying calcium oxide into soil could improve the ability of rice seedlings in low temperature-and damping-off disease resistance. After treated with calcium, the active oxygen eliminating enzyme activities of rice seedlings increased significantly. Compared with control, the SOD activity in seedling's root and shoot increased. The POD activity in rice seedling's root also increased, but decreased in shoot. The CAT activities decreased at the early stage, but increased at late stage. The soluble protein content in seedling's root and shoot was higher than that of the control. In the PAGE pattern of isoperoxidase, the band number of shoot increased, but that of root decreased. AU these changes were part of reasons that applying calcium oxide into soil improved the resistance of rice seedlings to cold and damping-off disease.
