[Reversed sural neurovascular fascio-cutaneous flap for reconstruction of soft-tissue defects in ankle and foot].

OBJECTIVE: To evaluate clinical significance of reversed sural neurovascular fascio-cutaneous flap for reconstruction of soft-tissue defects in ankle and foot. METHODS: From July 1994 to December 2002, 52 cases of soft-tissue defects in the ankle and foot were reconstructed by use of reversed sural neurovascular fascio-cutaneous flap, including 47 cases of traumatic defects, 3 cases of chronic ulcer and 2 cases of tumors. The flap area ranged from 4 cm x 6 cm to 10 cm x 21 cm. RESULTS: The flaps survived in 48 cases; the distal part necrosed and secondary free-skin graft were further conducted in 4 cases. All soft-tissue defects were repaired and their accompanied bone and tendon exposure healed. Forty-six cases were followed-up for 5 months to 48 months, the color and texture of the flaps were excellent and 2-point discrimination was 11-17 mm (14 mm on average). The functions of ankle joints were good. CONCLUSION: The reversed sural neurovascular fascio-cutaneous flap is convenient in design and dissection. Its use can retained and replace vascular anostomosed flaps to certain degrees.

OBJECTIVE: To study the safety and immunogenicity of the Bilive combined hepatitis A and B vaccine produced by Sinovac Biotech Co., Ltd. METHODS: Samples were selected from first year students of a senior high school (adults group) and first to fifth grade 1-5 students of 3 primary schools (children group). Those who were susceptible to both hepatitis A virus (HAV) and hepatitis B virus (HBV), HAV only or HBV only were assigned to group AB, A and B respectively and were vaccinated with three doses (0, 1 and 6 month schedule) of Bilive combined hepatitis A and B vaccine, inactivated hepatitis A vaccine and recombined hepatitis B vaccine respectively. The dosage for adult group was 500 U hepatitis A antigen and/or 10 micro g hepatitis B surface antigen and the dosage for children group was half the dosage of adult group. The potential adverse effects were observed within 72 hours after vaccination. Serum samples were collected for testing anti-HAV and anti-HBs at month 2 and 7 after the initial dose. RESULTS: The rates of local adverse effects were 0.58% and 2.56% in children AB group and adults AB group and the general adverse effects rates were 9.88% and 5.45% respectively. Both local and general adverse effect rates were not significantly different to the control group. The sero-conversion rate of anti-HAV in children and adults AB group reached 100%, one month after 3 doses. The geometric mean titer (GMTs) reached 33,910 mIU/ml and 23,435 mIU/ml respectively, significant higher than that in control group (group A). The sero-conversion rates of anti-HBs were 97.30% and 96.63%, and GMTs were 103 mIU/ml and 102 mIU/ml in children and adults AB group respectively. No significant difference on sero-conversion and GMT was observed when compared with control group. CONCLUSION: The Bilive combined hepatitis A and B vaccine had good safety profile, and the immunogenicity both on anti-HAV and anti-HBs was similar to that of separated components.

[Expression of cytokeratin19, galectin-3 and HBME-1 in thyroid lesions and their differential diagnoses].

OBJECTIVE: To study immunohistochemical expression of cytokeratin19 (CK19), galectin-3 (Gal-3) and HBME-1 in thyroid lesions and to assess their usefulness as markers in the differential diagnoses of thyroid nodular lesions. METHODS: Immunohistochemical staining was performed on formalin-fixed paraffin-embedded tissue of 21 cases of nodular goiters, 14 cases of toxic goiters, 15 cases of follicular adenomas (FA), 13 cases of follicular carcinomas (FC), 13 cases of follicular variant papillary carcinomas (FVPC) and 48 cases of classic papillary carcinomas (CPC). RESULTS: All three markers were expressed in the cytoplasm with no or weak expression in benign lesions and diffuse and strong in malignant cases. Positive expressions of CK19, Gal-3 and HBME-1 were present in 11of 21, two of 21, four of 21 in nodular goiters, seven of 14, one of 14, one of 14 in toxic goiters, nine of 15, two of 15, two of 15 in FA, 10 of 13, eight of 13, seven of 13 in FC, 13 of 13, 11 of 13, 12 of 13 in FVPC, and 48 of 48, 45 of 48, 46 of 48 in CPC. The expression rates of the three markers between benign lesions (nodular goiters, toxic goiters and FA) and malignant lesions (FA, FVPC and CPC) were statistically significant. Among the three follicular lesions (FA, FC and FVPC), the differences were statistically significant as well. Nine, seven and six cases were negative for all three markers in nodular goiters, toxic goiters and FA, respectively. Only one case in FC was negative for all three markers, no case was all negative in FVPC and CPC; the rate of one case with two or more positive marker expression in nodular goiters, toxic goiters, FA, FC, FVPC and PC was 14.2% (3/21), 21.43% (3/14), 20.0% (3/15), 69.2% (9/13), 92.3% (12/13), 100.0% (48/48), the differences between benign lesions and malignant lesions and between FA, FC and FVPC were also statistically significant. CONCLUSIONS: Immunohistochemical stains of CK19, Gal-3 and HBME-1, especially when used in combination, can be an important adjunct to the histopathological diagnoses of thyroid lesions.

[Gene delivery system based on low molecular weight polyethylenimine and its transfection activity in the skin tissue].

Low molecular weight polyethylenimine (LMW-PEI) was linked to an expressing plasmid contains a green fluorescence protein (GFP) reporter gene and effective gene transfer was observed in CM7221 cell line tested. We examined the relationship among the molecular weight, structure of PEI and their transfection activity and cytotoxicity on CM7221 cell line. We also examined the position and continuance time of the GFP reporter gene expressed in the skin tissue of mouse. Results showed that LMW-PEI/DNA complexes led to high levels of expression in the CM7221 cell line (about 55%). However, with the increasing of PEI molecular weight, the transfection activity of PEI was decreasing. There was an increasing cytotoxicity with the larger PEI molecules. Further research showed that LMW-PEI induced a significant and long-lasting (7 days) expression of the GFP reporter gene in the hair vesicle, sweat, gland, sebaceous gland in the mouse skin tissues. The LMW-PEI described here is a new, highly efficient and non-cytotoxic vector. It would be a useful non-viral vector for gene delivery technology, particular useful as simple skin-specific vehicles of therapeutic genes.

[Effects of lower limit of subsurface drip irrigation on tomato growth and its yield in plastic tunnel].

A subsurface drip irrigation experiment was conducted in a plastic tunnel to study the effects of different lower limits of subsurface drip irrigation on tomato growth and its yield. The lower limits of subsurface drip irrigation were expressed by soil water suctions detected by tensiometers placed in 30 cm soil layer. The designed values of soil water suction in this experiment were 10, 16, 25, 40 and 63 kPa, respectively. The results showed that the height and biomass of tomato plant decreased significantly with increasing lower limits of subsurface drip irrigation. Parabolic regression curves were suitable to simulate the relationships of stem diameter of tomato plant, yield, and water use efficiency (WUE) with lower limits of subsurface drip irrigation. The relationship between the ratio of stem diameter to plant height of tomato and lower limit could be simulated by three-dimensional multinomial regression curve. The growth of tomato root and shoot and their ratio R/S) varied under different lowerlimits of subsurface drip irrigation. The tomato plant grew well, RI/S was favorable, and the yield and WUE were higher when soil water suction was in the range of 25- 33 kPa. When this range was served as the criteria for subsurface drip irrigation, the soil water content at the beginning of irrigation should be lower, and the irrigation frequency should be fewer than that of conventional irrigation. This range of soil water suction was in favor of improving the WUE and decreasing labor cost for tomato cultivation in plastic tunnel.

[Characteristics and seasonal variations and influence factors on polycyclic aromatic hydrocarbons in Guangzhou city].

Weekly particulate and gas phase polycyclic aromatic hydrocarbons (PAHs) were sampled by a modified high-volume sampler in Guangzhou city from April 2001 to March 2002. The average total gas and particulate concentrations (sigma PAHs) were 312.9 ng/m3 and 23.7 ng/m3, respectively. The predominant PAHs in the air were 2 to approximately 4-ring compounds in vapor phase. Clear seasonal trends were observed, with higher concentration in summer and lower in winter for gas phase PAHs and adversely for particulate phase PAHs. Ambient temperature was predominating factor influencing the variation of gas phase PAHs concentrations and temperature together with wind speed and relative humidity were main meteorological parameters affecting the fluctuations of particle phase PAHs.

[Expression of pituitary tumor-transforming gene in endometrial carcinoma].

OBJECTIVE: To study the expression of pituitary tumor-transforming gene (PTTG) and its relationship with the expression of basic fibroblast growth factor (bFGF) protein and microvessel density (MVD) in endometrial carcinoma. METHODS: Expressions of PTTG mRNA and protein were assessed by semi-quantitive RT-PCR and immunohistochemistry methods respectively in 50 cases of endometrial carcinomas, 15 cases of hyperplasia endometria and 12 cases of normal endometrial tissue. Expressions of bFGF protein were detected by immunohistochemistry. Microvessels were highlighted by staining endothelial cells with CD(34) antigen, and MVDs were counted. RESULTS: The expression rate and average quantity of PTTG mRNA were detected in a significantly greater proportion endometrial carcinomas (96%, 0.84 +/- 0.08) than in hyperplasia endometria (60%, 0.78 +/- 0.06) and normal endometrial tissue (33%, 0.48 +/- 0.12, P < 0.01, respectively). The expression of PTTG protein in endometrial carcinomas (70%) was significantly higher than in hyperplasia endometria (40%) and normal endometrial tissue (17%, P < 0.01). The expression of PTTG was related to surgical-pathological stage, myometrial infiltration depth, lymphatic metastasis and pathological subtype (P < 0.05, respectively), but was irrelevant to patients' age and pathological grade (P > 0.05, respectively). The average quantity of PTTG mRNA and expression rate of PTTG protein in tissues with bFGF protein coexpression (0.86 +/- 0.07, 87%) were higher than in those without bFGF protein coexpression (0.80 +/- 0.06, 42%, P < 0.01, respectively). The MVD in tissues with PTTG protein expression (62 +/- 18) was higher than in those without PTTG protein expression (51 +/- 12, P < 0.05). CONCLUSIONS: PTTG may play an important role in carcinogenesis and development of endometrial carcinoma. PTTG induces an angiogenesis through bFGF which is a key determinant step in tumor progression and metastatic spread.

[Fertility alteration of male sterile rice line annong S-1 and the expression of fertility related aprt gene].

The thermo-sensitive point and the thermo-sensitive stage for fertility alteration in the thermo-sensitive genic male sterile (TGMS) line Annong S-1 were studied. In nature environment and green house, the leaf, root and young panicle were treated by low and high temperature respectively. In total, 8 treatments were carried out. Results indicated that during the thermo-sensitive stage of fertility alteration, in high temperature environmental conditions, the low temperature treatment in the root did not significantly induce Annong S-1 to be fertile, while the low temperature treatment in young panicle obviously induced Annong S-1 to be fertile. Therefore, the thermo-sensitive point of Annong S-1 is the young panicle. The stage of fertility alteration was also investigated. Results indicated that alteration occurred in the phase from the formation of pollen mother cell to the tetrad of miosis. aprt gene is related the male sterility. The expression of aprt gene in root, young panicle and leaf were investigated by RT-PCR respectively. In young panicle, the expression of aprt gene was greatly down regulated by high temperature, while in root and leaf the expressions have no great changes. These data show that the young panicle is sensitive to temperature change. The result also supported that the young panicle is the thermo-sensitive point of Annong S-1 in the fertility alteration. These conclusions can be used to direct in hybrid rice seed production.

[CK20 mRNA expression in peripheral blood of patients with gastrointestinal carcinoma and its clinical significance].

BACKGROUND & OBJECTIVE: Detecting expression levels of cytokeratin 19 (CK19), CK20, MUC1, and carcinoembryonic antigen (CEA) in peripheral blood, lymph node, and bone marrow is a major method for diagnosing micro-metastasis in patients with gastrointestinal carcinoma of early stages. This study was to examine the expression, before and after operation, of CK20 mRNA in peripheral blood of patients with gastrointestinal carcinoma, to further explore hematogenous-spread micro-metastasis status of these patients, and significance of CK20 mRNA detection in treatment during perioperation period. METHODS: Peripheral blood samples were collected before, and 2 weeks after operation from 62 patients with gastrointestinal carcinoma, and 22 controls (12 patients with benign gastrointestinal disease, and 10 healthy volunteers). CK20 mRNA expression in the blood samples was specifically detected by nested reverse transcriptase-polymerase chain reaction (RT-PCR). Pathologic examination of all cases was done by conventional methods. RESULTS: Of 62 patients with gastrointestinal carcinoma, 34 (54.3%) have positive CK20 mRNA expression before operation, and so did 31 (50.0%) postoperatively, but no CK20 mRNA expressed in 22 controls. In patients of stageI-IV, CK20 mRNA positive rates were 37.5% (3/8), 36.3% (8/22), 66.7% (18/27), and 100% (5/5), respectively, difference between stage I+II (36.9%) and stage III+IV (83.3%) was significant (P< 0.05). Among 35 patients with local lymph node metastasis, 27 (77.1%) were positive for CK20 mRNA; among 27 patients without lymph node metastasis, 7 (25.9%) have positive CK20 mRNA expression (P< 0.01). CK20 mRNA expression was not correlated with tumor cell differentiation, and cancer embolus existance. There was no significant difference in CK20 mRNA expression before and after operation (P >0.05). CONCLUSIONS: CK20 mRNA may be a sensitive marker for detecting micro-metastasis,and recurrence of patients with gastrointestinal carcinoma. It may be helpful for the correct clinical staging, and reasonable treatment for patients.

The first metatarsal web space: its applied anatomy and usage in tracing the first dorsal metatarsal artery in thumb reconstruction.

OBJECTIVE: To clarify the anatomical relationship of the structures in the first toe webbing space for better dissection of toes in thumb reconstruction. METHODS: The first dorsal metatarsal artery, the first deep transverse metatarsal ligament and the extensor expansion were observed on 42 adult cadaveric lower extremities. Clinically the method of tracing the first dorsal metatarsal artery around the space of the extensor expansion was used in 36 cases of thumb reconstruction. RESULTS: The distal segments of the first dorsal metatarsal artery of Gilbert types I and II were located superficially to the extensor expansion. The harvesting time of a toe was shortened from 90 minutes to 50 minutes with 100% survival of reconstructed fingers. CONCLUSIONS: The distal segment of the first dorsal metatarsal artery lies constantly at the superficial layer of the extensor expansion. Most of the first metatarsal arteries of Gilbert types I and II can be easily located via the combined sequential and reverse dissection around the space of the extensor expansion.
