[Repair the palmar soft tissue defect of the finger with cross-finger flap with cutaneous branch of the ulnar digital finger].

OBJECTIVE: To discuss the clinical effect of cross-finger flap with cutaneous branch of the ulnar digital finger on repairing the palmar soft tissue defect of the finger. METHODS: From October 1996 to June 2004, cross-finger flaps were used to repair the palmar soft tissue defect of the finger in 25 cases (32 fingers ) with tendon or bone exposed. There were 18 males and 7 females, and their ages ranged from 13 to 45 years. Among them, 6 cases were incised injury, 8 cases were impact and press injury, 11 cases were crush injury; and 2 cases were thumb, 8 cases were index, 5 cases were middle finger, 3 cases were ring finger, 2 cases were little finger, 2 cases were index and middle finger, 2 cases were middle and ring finger, and 1 cases were index, middle, ring and little finger. The time from injury to diagnosis was 30 min to 48 h, and the size of the tissue defect was 1.5 cm x 1.0 cm to 4.1 cm x 2.0 cm. All cases were treated with emergent operation, and the sense of the flap was recovered by anastomosing the cutaneous branch of the ulnar digital finger and the distal digital nerve of injured finger. The flap pedicle was dissected 3 weeks later. RESULTS: Follow-up was conducted for 6 to 26 months and it showed that the cross-finger flaps all survived with full digital fingertip, satisfactory appearance, good function, and normal sense. The discrimination of two points was 5-8 mm. CONCLUSION: As it is easy to operate and with satisfactory appearance and good function restoration, cross-finger flap with cutaneous branch of the ulnar digital finger is effective in repairing the palmar soft tissue defect of the finger.

[Impacts of steep pulsed electric fields on lymphatic capillaries in VX2 implanted breast cancer in rabbits].

BACKGROUND & OBJECTIVE: Electrochemotherapy mediated by electric pulse has become a multidisciplinary biomedical engineering technique in modern medical science. Its main mechanisms are enhancing the diffusion of chemotherapeutic drugs, antibodies, or genes into the inner part of tumor cells mediated by membrane-electropermeabilization caused by electric pulse. Our previous studies confirmed that steep pulsed electric field (SPEF) could irreversibly cause membrane electropermeabilization, and lead to death of tumor cells. This study was to explore the acute killing effects of SPEF on lymphatic capillaries in VX2 implanted breast cancer in rabbits. METHODS: Tumor model of VX2 implanted breast cancer was successfully established in rabbits. Isosulfan blue staining, 5'-AMP-ALPase enzymohistochemical double staining, and electron microscopy was used to observe the morphologic changes of local lymphatic capillaries around cancer tissues exposed to SPEF. RESULTS: After exposed to SPEF, no lymphatic vessels were found with isosulfan blue staining, only blurred structures were observed; enzymohistochemistry showed no positively stained lymphatic vessels, only fragmental structures around cancer tissues were observed; integrity and continuity of lymphatic endothelium were destroyed under transmission electron microscope. CONCLUSION: SPEF has the potential to destroy lymphatic capillaries around VX2 implanted breast cancer, and can decrease the possibility of post-treatment lymphatic metastasis.

[Effect of single dose irradiation to parotid gland on the structured and function changes of bilateral parotid glands in miniature pig].

OBJECTIVE: To evaluate the effects of a solitary megadose protocol of ionizing radiation (IR) to parotid gland on the structured and function changes of bilateral parotid glands in miniature pig. METHODS: Fourteen minipigs were subjected to either 15 or 20 Gy to one parotid gland with a linear accelerator, while another four minipigs served as non-IR controls. Salivary flow rates and salivary chemistries were measured pre-IR, and 4 and 16 weeks post-IR. A quantitative assessment of gland weight and acinar area, and detailed serum chemistry and hematological analyses, were also performed. RESULTS: Parotid gland weights were significantly decreased in the 15 and 20 Gy groups at 4 and 16 weeks post-IR. The acinar cell area in glands of both IR groups was significantly reduced. Parotid flow rates decreased by 60% with 15 Gy at 16 weeks post-IR. In the 20 Gy group, salivary flow rates were reduced by 80% at 16 weeks post-IR. Additionally, parotid flow rates significantly reduced in contralateral glands with 20 Gy at 16 weeks, while structure and weight did not changes in parotid glands. CONCLUSION: Structural changes in salivary gland parenchyma occurred relatively early after IR, while the alterations in salivary output were relatively delayed. Further, reductions in salivary flow were not proportional to acinar cell area loss. There isn't a significant structured change of contralateral glands, but significant reduction of parotid flow rate at this time.

[Impacts of farmland consolidation on farmland landscape].

Farmland consolidation is the act of regulating, improving, and comprehensively renovating the structure, quality, and layout of field, water, road, forestry, and village in the countryside in a certain area by means of administration, economy, law, and engineering techniques according to the goal and usage defined by land use planning, so as to improve farmland use rate and its output rate, to increase farmland area, and to achieve better productive, living, and ecological environment. Recently, farmland consolidation has been carried out all over the country, especially in its economy-developed regions. But, unscientific planning and unsuitable farmland consolidation engineering have negative effects on field ecological system. In this paper, based on the technology of GIS and RS, the basic theories and methods of landscape ecology and a compositive grading method were applied to analysis the dynamics of farmland landscape fragmentation in Tongxiang county. The results showed that the farmland landscape fragmentation in this county was strongly affected by consolidation. More attention should be paid to the protection of farmland landscape during consolidation, and to avoid or decrease the negative effects resulted from unscientific planning and unsuitable farmland consolidation engineering.

Repair and reconstruction of severe leg injuries: retrospective review of eighty-five patients.

OBJECTIVE: To explore a good way of the reconstruction of severe tibial shaft fractures by using different flaps and external fixators. METHODS: Eighty-five patients of Type IIIC tibial shaft fractures with average age of 42.5 years were treated in our hospital from 1990 to 2005. Injuries were caused by motorcycle accidents in 66 patients, by machine accidents in 16 patients, and by stone bruise in 3 patients. The management procedures consisted of administration of antibiotics, serial debridment, bone grafting if needed, application of different flaps, such as free thoracoumbilical flaps, fasciocutaneous flaps, saphenous neurocutaneous vascular flaps, sural neurocutaneous vascular flaps and gastrocnemius muscular flaps, and different external fixations, for instance, half-ring fixators, unilateral axial dynamic fixators, AO fixators, Weifang fixators, and Hybrid fixators. The average follow up was 6.3 years. RESULTS: All flaps survived. Eighty-three cases had bone healed. The average bone healing time of different external fixations was 5.5 months in 47 cases with half-ring fixators, 9.2 months in 4 cases treated with unilateral axial dynamic fixators, 8.5 months in 6 cases with AO fixators, 10.7 months in 16 cases with Weifang fixators, and 7.8 months in 10 cases with assembly fixators. Except half-ring fixation, other fixations all needed necessary bone graft. Two cases treated with unilateral axial dynamic fixators had nonunion of bone and developed osteomyelitis. The wounds healed after the removal of the fixators and immobilization by plaster. The last follow up examination showed ankle and knee motion was normal and no pain was noted. CONCLUSIONS: The combination of half-ring external fixators with various flaps provides good results for Type IIIC tibial shaft fractures.

[Simultaneous determination of twelve sulfonyl urea herbicide residues in rice by high performance liquid chromatography with solid-phase extraction].

A high performance liquid chromatographic (HPLC) method with solid-phase extraction was developed for screening of 12 sulfonyl urea herbicides in rice. The sample was cleaned up and extracted with ENVI-18 (C18) and ENVI-Carb (GCB) columns. The separation was performed on a Symmetryshield RP8 column (4.6 mm i.d. x 150 mm)with a linear gradient elution (acetonitrile and 5 mmol/L acetic acid as mobile phase), and the wavelength of an ultraviolet detector was set to 240 nm for the detection. The linear range was 0.1 - 10 mg/L, and the correlation coefficients were 0.9983-0.9999. Recoveries from rice samples spiked with 12 sulfonyl urea herbicides at spiking levels ranging from 0.01-0.50 microg/g were from 72.2% to 106.5%, with relative standard deviations from 0.6% to 6.4%. The lowest detection limits (S/N = 3) were 0.01-0.02 microg/g. The results indicate that the method is easier, faster, sensitive and has better purification effect. It also demonstrates that this multiresidue analytical method can meet the requirements for simultaneous determination of sulfonylurea herbicides in rice.

Evaluation of efficacy and safety of diacerein in knee osteoarthritis in Chinese patients.

OBJECTIVE: To evaluate the efficacy and safety of diacerein in patients with knee osteoarthritis (OA). METHODS: A total of 223 patients satisfying the American College of Rheumatology criteria for knee OA were chosen for this 17-week, randomized, double-dummy, diclofenac sodium-controlled trial, with diacerein dosage of 100 mg/d and diclofenac sodium of 75mg/d. Efficacy and safety of both drugs were evaluated. RESULTS: Totally 106 patients in the diacerein group and 107 patients in the diclofenac group were considered qualified for the evaluation. After 12 weeks of treatment, the total effective rates of patients/physicians' overall assessment in diacerein and diclofenac groups were 65.4%/61.6% and 61.2%/61.2%, respectively (P > 0.05). The primary efficacy parameter [visual analog scale (VAS) assessment of pain on 20 metres walking] and the secondary efficacy parameters [tenderness on palpation, Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), and 36-item Short-Form (SF-36) Health Survey] significantly improved compared with baseline in both groups (P < 0.05). In the follow-up period, there were no obvious changes in above parameters in diacerein group. However, in diclofenac group, pain on 20 metres walking, tenderness on palpation, and WOMAC became aggravated after withdrawing the drug for 4 weeks (P < 0.05). Moreover, the consumption of paracetamol was significantly lower in diacerein group than in diclofenac group during follow-up (P < 0. 001). The incidences of related adverse events were 35.7% in diacerein and 45.1% in diclofenac group, respectively. Mild-to-moderate gastrointestinal disorders were the most frequent adverse events. CONCLUSIONS: Diacerein is as effective as diclofenac sodium in treating patients with knee OA. Furthermore, it has better extended effect and a good safety profile. It is generally well tolerated and has no severe adverse effect.

[Analysis of HLA-B locus gene polymorphism in Sichuan Yi ethnic group and Xinjiang Uygur ethnic group].

The polymorphism of HLA-B alleles in Sichuan Yi and Xinjiang Uygur population was investigated using the PCR-SSP method. Twenty one alleles were detected in HLA-B loci in 106 Sichuan unrelated Yi healthy subjects. Of them, B*40, B*15 and B*51 were the most common alleles with an allele frequency of 0.1981, 0.1368, 0.1274, respectively; while B*47, B*44, B*18, B*57 and B*78 were the rare alleles with an allele frequency of 0.0189, 0.0142, 0.0094, 0.0047 and 0.0047, respectively. The distribution of HLA-B allele frequencies in Sichuan Yis was between Southern Han and Northern Han. In 110 Xinjiang unrelated healthy Uygur subjects, 27 alleles were detected in HLA-B loci. Of them, B*35 and B*51 were the most common alleles with an allele frequency of 0.1136 and 0.1136, respectively; while B*41, B*56 and B*78 were the rare alleles with a frequency of 0.0045, 0.0045 and 0.0091, respectively. Frequencies of "Caucasoid origin" HLA alleles such as B*08, B*35 and B*50 in Xinjiang Uygurs were higher than other ethnic groups in China. The result of chi2 tests showed that the distributions of HLA-B alleles in Yi and Uygur ethnic groups were in Hardy-Weinberg equilibrium. Heterozygosity (H), discrimination power (DP) and probability of paternity exclusion (EP) of HLA-B locus from Sichuan Yi ethnic group were computed to be 0.8977, 0.9661 and 0.8009; and those from Xinjiang Uygur ethnic group were 0.9372, 0.9857 and 0.8732. The data obtained in this study on the distributions of HLA-B alleles in the Sichuan Yi and Xinjiang Uygur population provide important group genetics information for forensic and paternity tests to estimate the frequency of a DNA profile in these two populations, and can be used in transplant matching, anthropological and disease association studies.

[Transfer of neurovascular island flap from the same finger for repairing pulp defect].

OBJECTIVE: To observe the clinical effects of neurovascular island flap from the same finger for repairing pulp defect. METHODS: From November 2003 to February 2005, 32 pulp defects in 30 cases were covered with neurovascular island flap from the same finger. There were 25 males and 5 females. The age ranged from 18 to 56 years. The operation was performed after debridement and 2-8 days antibiotics therapy. The defect area ranged from 1.5 cm x 1.2 cm to 3.5 cm x 2.1 cm. The flap was harvested on the dorsal part of the finger ulnarly or radially. The distal end of the flap should be more than 5 mm away from the nail base to avoid nail injury. The ventral and dorsal cut should not exceed the middle line respectively. The flap size ranged from 2.0 cm x 1.5 cm to 4.0 cm x 2.5 cm. The donor site was covered with flap of subdermal vascular plexus from the medial side of the upper arm. RESULTS: All 32 transferred flaps survived after operation. There was no vascular crisis. Twenty-five cases were followed up from 2 to 8 months. The flaps had good appearance and texture and blood circulation. Two-point discrimination was 7-10 mm. The function of finger motion was returned to normal. CONCLUSION: Transfer of neurovascular island flap from the same finger offered a sensational skin flap for reconstruction of pulp defect. The technique was simple, and the clinical result was satisfactory. It is an ideal method for reconstruction of thumb or finger pulp defects.
