Catheter-related infection in gastrointestinal fistula patients.

AIM: To study the incidence, bacterial spectrum and drug sensitivity of catheter-related infection (CRI) in gastrointestinal fistula patients. METHODS: A total of 216 patients with gastrointestinal fistulae during January 1998 to April 2001 were studied retrospectively. Two hundred and sixteen catheters of the 358 central venous catheters used in 216 gastrointestinal fistula patients were sent for microbiology analysis. RESULTS: Ninety-five bacteria were cultivated in 88 catheters (24.6%). There were 54 Gram-negative bacteria (56.8%), 35 Gram-positive bacteria (36.8%), and 6 fungi (6.4%). During the treatment of CRI, 20 patients changed to use antibiotics or antifungal, and all patients were cured. The mean time of catheters used was 16.9+/-13.0 d. CONCLUSION: CRI is still the common complication during total parenteral nutrition (TPN) treatment in patients with gastrointestinal fistulae, and Gram-negative bacteria are the main pathogens, and bacterial translocation is considered the common reason for CRI.

Proliferative response of human prostate cancer cell to hormone inhibited by androgen receptor antisense RNA.

BACKGROUND: The failure of endocrine treatment for advanced prostate cancer might be related to aberrant activation of androgen receptor (AR). Prostate cancer cell line LNCaP contains AR that can be activated by androgen, estrogen and progesterone. This study was set to investigate the effects of antisense AR RNA on growth of LNCaP cultured in medium containing varied concentrations of R1881, 17beta-estradiol, and progesterone, respectively. METHODS: LNCaP cells transfected with antisense AR RNA retroviral vector pL-AR-SN were designated as LNCaPas-AR. LNCaP cells containing empty vector pLXSN served as LNCaPNeo. LNCaP and LNCaPNeo were taken as controls. In vitro cell growth assay, proliferative cells of LNCaP and tranfected LNCaPs were counted by typan staining when they cultured with synthetic androgen R1881, 17beta-estradiol, and progesterone, respectively. RESULTS: Growth of LNCaPas-AR was inhibited significantly (P < 0.05) compared with that of LNCaP and LNCaPNeo at 1 nmol/L R1881, 10 nmol/L 17beta-estradiol, and 1 nmol/L progesterone, respectively. No difference was seen between LNCaP and LNCaPNeo (P > 0.05). Microscopic observation showed that LNCaP and LNCaPNeo cells grew well, but only few LNCaPas-AR cells were alive. CONCLUSIONS: Our observations indicate that antisense AR RNA retroviral vector pL-AR-SN could change androgen-independent characteristics of LNCaP cells, which might shed some novel insights into the treatment of androgen-independent prostate cancer.

[Identification of a novel mutation of the SEDL gene in X-linked spondyloepiphyseal dysplasia tarda].

OBJECTIVE: To investigate further the genetic basis of hereditary X-linked spondyloepiphyseal dysplasia tarda (SEDL). METHODS: Single strand conformation polymorphism (SSCP) combined with polymerase chain reaction and denaturing polyacrylamide gel electrophoresis were used to detect the mutation for the coding exons of SEDL gene as well as their exon/intron boundaries in 5 unrelated Chinese boys clinically diagnosed as having SEDL. DNA sequencing analysis was further used to identify the mutation. RESULTS: The 13 bp deletion mutation consisting of IVS5-2-1delAG and 322-332del TTTTCAATGAA was identified in one of SEDL patients, but not detected in 30 chromosomes from 30 unrelated normal male individuals. CONCLUSION: This is a novel mutation cosegregated with the patient's skeletal disease. The intragenic deletion occurred in the acceptor splice site of the 3' region of intron 5 and the 5' coding region of exon 6, which may result in one or a combination of splicing defects. The results of this study expand the spectrum of SEDL mutations associated with SEDL, and this will help to elucidate further the role of this novel protein in the etiology of this form of osteochondrodysplasia.

Aortic valve replacement for a patient with essential thrombocythemia: a case report.

A 51-year-old male patient with 3+ aortic insufficiency, hepatitis C, osteomyelitis right foot, and a preoperative platelet count 1.3 million/mm3 required cardiopulmonary bypass (CPB). Essential thrombocythemia is a relatively unknown entity with the utilization of CPB. After consulting with the surgeon, an anesthesiologist and another perfusionist, a team approach was used to discuss treatment for this patient during CPB. The treatment consisted of exchange transfusions, autotransfusion techniques, and current protocol for blood gas management. No incidence of hypercoagulability was observed during this procedure or postoperatively. Based on current knowledge of pathophysiology and review of the literature, plateletpheresis should be the current management of essential thrombocythemia.

[Immunomagnetic beads sorting and functional identification of human spermatogonial stem cells].

OBJECTIVE: The very nature of spermatogonial stem cells (SSC) is still poorly understood. The objective of this study is to explore the specific markers of human SSC and search for the suitable method for their isolation and functional identification. METHODS: Adults testicular cell suspensions were sorted by immunomagnetic beads method using alpha6,beta1 integrin and Thy-1 markers. The light-scattering properties and DNA Ploidy of the resultant subpopulations were analyzed by flow cytometry. The efficacies of the sorting on human SSC were evaluated by germ cell transplantation. RESULTS: (1) The alpha6+ Thy-1+ c-kit- and beta1+ Thy-1+ c-kit- cells were relatively uniform subpopulations in size and morphology, which represented about 2%-3% and 0.5%-1% of the unsorted testis cells, respectively. The analysis of light-scattering properties showed that both of the subpopulations had low side light-scattering properties. The DNA Ploidy analysis showed significant changes of these two cell subpopulations in DNA Ploidy. The percentage of diploid cells in alpha6+ Thy-1+ c-kit- cell subpopulation significantly increased to 51.2% and synthesis phase and tetraploid cells disappeared. (2) The functional evaluation showed that the SSC in the alpha6+ Thy-1+ c-kit- cells were enriched 40 times and the SSC in the beta1+ Thy-1+ c-kit- cells 20 times that of the unsorted cells. CONCLUSION: The alpha6,beta1 integrin and Thy-1 may be used for the SSC isolation as positive markers. The immunomagnetic beads sorting using alpha6,beta1 integrin and Thy-1 markers can result in significant enrichment of human SSC. It will open up a wide prospect for the researches on the biology of human SSC and the treatment of male sterility.

Complexation of uranium(VI) and samarium(III) with oxydiacetic acid: temperature effect and coordination modes.

The complexation of uranium(VI) and samarium(III) with oxydiacetate (ODA) in 1.05 mol kg(-1) NaClO(4) is studied at variable temperatures (25-70 degrees C). Three U(VI)/ODA complexes (UO(2)L, UO(2)L(2)(2-), and UO(2)HL(2)(-)) and three Sm(III)/ODA complexes (SmL(j)((3-2)(j)+) with j = 1, 2, 3) are identified in this temperature range. The formation constants and the molar enthalpies of complexation are determined by potentiometry and calorimetry. The complexation of uranium(VI) and samarium(III) with oxydiacetate becomes more endothermic at higher temperatures. However, the complexes become stronger due to increasingly more positive entropy of complexation at higher temperatures that exceeds the increase in the enthalpy of complexation. The values of the heat capacity of complexation (Delta C(p) degrees in J K(-1) mol(-1)) are 95 +/- 6, 297 +/- 14, and 162 +/- 19 for UO(2)L, UO(2)L(2)(2-), and UO(2)HL(2)(-), and 142 +/- 6, 198 +/- 14, and 157 +/- 19 for SmL(+), SmL(2)(-), and SmL(3)(3-), respectively. The thermodynamic parameters, in conjunction with the structural information from spectroscopy, help to identify the coordination modes in the uranium oxydiacetate complexes. The effect of temperature on the thermodynamics of the complexation is discussed in terms of the electrostatic model and the change in the solvent structure.

[Comparative experimental study on treatment of rat's injured sciatic nerve with electroacupuncture and Buyang Huanwu Decoction].

OBJECTIVE: To study the effect of the electroacupuncture and Buyang Huanwu Decoction (BYHWD) on the regeneration of the rat's injured sciatic nerve. METHODS: Rat's sciatic nerve injury model was made by operation. The rats were treated with electroacupuncture or BYHWD and their nerve electro-physiological indices and histological changes tested by HRP were observed. RESULTS: It was demonstrated that the recovery ratios of nerve conduction velocity and muscle evoked action potential of the group treated with electroacupuncture and the group treated with BYHWD were higher than those of the group treated with western medicine or the group without treatment. The differences were significant. The labeling cells in anterior horn of spinal marrow and spinal ganglion of the group treated with electroacupuncture and the group treated with BYHWD were more abundant than those of the group treated with western medicine or the group without treatment. The differences were significant. The effect of electroacupuncture was better than that of the BYHWD. CONCLUSION: It suggests that both electroacupuncture therapy and BYHWD can promote the recovery of the function in early stage of injured nerve, and they are effective methods for promoting the regeneration of injured peripheral nerve.

Role of NF-kappaB and cytokine in experimental cancer cachexia.

AIM: To assess the putative involvement of NF-kappaB and pro-inflammatory cytokines in the pathogenesis of cancer cachexia and the therapeutic efficacy of indomethacin (IND) on cachexia. METHODS: Thirty young male BABL/c mice were divided randomly into five groups: (a) control, (b) tumor-bearing plus saline, (c) tumor-bearing plus IND (0.25 mg x kg(-1)), (d) tumor-bearing plus IND (0.5 mg x kg(-1)), and (e) tumor-bearing plus IND (2 mg x kg(-1)). Colon 26 adenocarcinoma cells of murine were inoculated subcutaneously to induce cachexia. Saline and IND were given intraperitoneally daily for 7 days from the onset of cachexia to sacrifice. Food intake and body composition were documented, serum levels of TNF-alpha and IL-6 and activity of NF-kappaB in the spleen were investigated in all animals. RESULTS: Weight loss was observed in all tumor-bearing mice. By day 16, body weights of non-tumor mice were about 72 % of healthy controls (P<0.01), and the weight of gastrocnemius was decreased by 28.7 % (P<0.01). No difference was found between groups in food intake (P>0.05). Gastrocnemius weight was increased markedly (P<0.01) after treatment of IND (0.5 mg x kg(-1)), while the non-tumor body weights were not significantly elevated. Tumor-bearing caused a 2-3 fold increase in serum levels of both TNF-alpha and IL-6 (P<0.01). The concentration of TNF-alpha (P<0.05) and IL-6 (P<0.01) in tumor-bearing mice was reduced after administration of 0.5 mg x kg(-1) IND for 7 days. But the level of IL-6 was slightly elevated following treatment of IND 2.0 mg x kg(-1). NF-kappaB activation in the spleen was increased in tumor-bearing mice in comparison with controls in electrophoretic mobility shift assay (EMSA). NF-kappaB activity was reduced in mice treated with 0.5 mg x kg(-1) of IND, whereas a higher NF-kappaB activity was observed in mice treated with 2.0 mg x kg(-1) of IND. CONCLUSION: Colon 26 adenocarcinoma cells can induce severe cancer cachexia experimentally, and the mechanism may be partially due to the enhanced TNF-alpha and IL-6 in tumor-bearing animals, which is controlled by NF-kappaB. Low dose of indomethacin alleviates the cachexia, decreases the activation of NF-kappaB and the serum levels of TNF-alpha and IL-6, and prevents body weight loss and muscle atrophy, while no further effect is gained by a higher dosage.

[Transmission disequilibrium test for congenital dislocation of the hip and HOXB9 gene or COL1AI gene].

OBJECTIVE: To detect the correlation between the congenital dislocation of the hip (CDH) and HOXB9 gene or COL1AI gene. METHODS: A microsatellite DNA marker D17S1820 was chosen in the region of chromosome 17q21 where exists the HOXB9 gene which regulates the embryonic limb development and exists the COL1AI gene. The genotypes of 303 members in 101 CDH nuclear family trios were analyzed by the techniques of polymerase chain reaction(PCR) and denaturing polyacrylamide gel electrophoresis. Then transmission disequilibrium test (TDT) was used to test the data of genotypes. RESULTS: There exist 12 alleles at this polymorphic locus. Transmission disequilibrium was found between CDH and the fourth allele of D17S1820 (chi-square=6.025,P=0.014). CONCLUSION: CDH is associated with the region of chromosome 17q21. HOXB9 gene and/or COL1AI gene may be susceptibility genes of CDH.

Polymorphism of E. coli F18 receptor gene in different pig breeds.

The genetic variation of E. coli F18 receptor(ECF18R) loci in 867 pigs including Large Yorkshire, Landrace, Duroc, Ningxiang pig, Shaziling pig and Daweizi pig was detected by PCR-RFLP with Hin6 I. The results showed as follows: The three exotic breeds including Large Yorkshire, Landrace and Duroc were proved to be polymorphic at the Hin6 I-RFLP site. The average susceptibility genotype frequency of AG and GG in the exotic breeds was 0.94, G allelic frequency was 0.76. The C-->A mutation frequency at M307 site was very low in exotic breeds. The resistance genotype frequency of AA was 0.06. But the three native breeds including Ningxiang pig, Shaziling pig and Daweizi pig presented only GG genotype and no variation at the Hin6 I-RFLP site. The chi 2 value of ECF18R genotype distribution at the Hin6I-RFLP site in different pigs indicated that ECF18R genotype distribution of every exotic pig breed had significant or highly significant differences with three native pig breeds'. The ECF18R genotype distribution of three native pig breeds was the same completely. The others had no difference except that Landrace ECF18R genotype distribution had significant differences with Duroc's in the exotic breeds of pig.

Retroperitoneoscopic renal pedicle lymphatic disconnection for chyluria.

OBJECTIVE: To report our experiences in retroperitoneoscopic renal pedicle lymphatic stripping for chyluria. METHODS: Six cases of filarial chyluria were admitted to our hospital from November 2001 to June 2002. Of these cases, 4 were men and 2 women, with age ranging from 34 to 52 years (mean, 42 years). Diagnosis was made by using urine test for the presence of chyle and fat globule, cystoscopy, excretory urogram and retrograde pyelography. Chyluria was found on the left renal unit in 2 cases and on the right side in 4 cases. The technique of retroperitoneoscopic management of chyluria consisted of nephrolympholysis, hilar vessel stripping and ureterolympholysis. RESULTS: Operative time ranged from 69 to 120 minutes (mean, 95 minutes). Intraoperative blood loss was 50-180 ml (mean, 85 ml). Chyluria disappeared in all patients immediately after operation. Mild hematuria occurred in 4 cases within 12 hours and disappeared at 24 hours. Subcutaneous emphysema around the lesions was found in 2 cases and was spontaneously absorbed 3 days after the treatment. There was no lymphatic leak at the lesions. The patients were discharged from the hospital 5-9 days after the treatment. All patients gained weight and their haemoglobin and serum protein increased by 13.5 g/L and 3.66 g/L respectively. No chyluria recurrence was reported during 1-1.6 years follow-up. CONCLUSIONS: Retroperitoneoscopic renal pedicle lymphatic disconnection for chyluria is a safe, effective and efficient surgical procedure with minimal invasion, less pain, lower morbidity, short hospital stay and rapid recovery.

An efficient synthesis of 4-(phenylsulfonyl)-4H-furo[3,4-b]indoles.

The fused heterocycle 4-(phenylsulfonyl)-4H-furo[3,4-b]indole, which is an indole-2,3-quinodimethane synthetic analogue, is prepared in five steps from indole in 46% yield. A similar sequence is used to synthesize C-3 derivatives (3-methyl, 3-phenyl, and 3-heptyl). Thus, indole-3-carbaldehyde (1) is protected as the N-phenylsulfonyl derivative 2 and converted to the ethylene acetal 6. Lithiation at C-2 followed by treatment with an aldehyde affords the expected hydroxy acetals 7 and 8. Exposure to acid effects cyclization to the furoindoles 5 and 9. Furthermore, C-1 lithiation of furo[3,4-b]indole 9c followed by treatment with methyl iodide affords disubstituted furo[3,4-b]indole 10.

[Radiofrequency catheter ablation autonomic nerve injury]

OBJECTIVE: To investigate the correlation between RFCA catheter cumulative energy and autonomic nerve injury. METHODS: Forty-one patients with paroxysmal supraventricular tachycardia were enrolled, Patients were excluded if they had Diabetes, Hypertension, Congestive Heart Failure or other organic heart disease. HRV and biochemical markers were measured before and after the RFCA. RESULTS: Compared with pre-ablation values, there was significantly decrease in post-ablation low frequency (LF) and high frequency (HF). This was noted in both the septal group (AVNRT and septal pathway) and free wall group (free wall accessory pathway).Post-procedure,the sensitivity of cardiac troponin I(cTnI) for myocardial injury detection was 58.3%, AST was 41.7%. This was significantly higher than other markers(CK:4.2%, CK-MB:10.4%, LDH:20.8%). The post-ablation sensitivity of cTnI was 54.2%, 6.3% and 52.1%at 1 hour, 12 hours, and 24 hours respectively. A significant correlation between cumulative energy and delta HF(r=0.688,P=0.01) or delta LF (r=0.462, P<0.05).was noted in free wall group.(delta HF=pre-ablation HF-post-ablation HF/pre-ablation HF x 100%). There was no significant correlation between biochemical markers and either delta HF or delta LF. CONCLUSION: RFCA induced injury on cardiac autonomic nerves related to both cumulative energy and ablation site, but not size of myocardial injury as determined by cTnI measurement. cTnI is an excellent biochemical marker of myocardial injury.

Rehabilitation therapy for short bowel syndrome.

OBJECTIVE: To investigate the effect of rehabilitation therapy for short bowel syndrome on patient nutritional status and intestinal adaptation. METHODS: The rehabilitation therapy included enteral or parenteral nutrition, glutamine, recombinant human growth hormone and rehabilitative diet. From January 1997 to July 2000, twenty - seven patients with short bowel syndrome received the treatment. The average age of the patients was 38.5 +/- 19.3 years, and the length of residual small intestine ranged from 15 to 80 cm, with an average of 46.8 +/- 23.4 cm. The ileocecal valve was preserved in 14 cases, and the average time between the onset of short bowel syndrome and the rehabilitation therapy was 86 +/- 105 days. RESULTS: After the treatment, nutritional status of the patients improved markedly, and intestinal absorptive capacity improved. Eight patients were followed up for more than 2 years, among whom 4 (50%) were weaned from total parenteral nutrition. Thirteen patients were followed up for more than 1 year, and 10 patients (76.9%) were weaned from total parenteral nutrition. CONCLUSIONS: Rehabilitation therapy for short bowel syndrome can improve patient nutritional status effectively and promote intestinal adaptation, providing a new hope for these patients. The therapeutic effects are related to the length of the residual small intestine, patients age and duration between massive intestinal resection and start of the treatment. Early initiation of rehabilitation therapy promotes intestinal adaptation and increases patients ability to wean from total parenteral nutrition.
