Heterogeneity in regional notification patterns and its impact on aggregate national case notification data: the example of measles in Italy.

BACKGROUND: A monthly time series of measles case notifications exists for Italy from 1949 onwards, although its usefulness is seriously undermined by extensive under-reporting which varies strikingly between regions, giving rise to the possibility of significant distortions in epidemic patterns seen in aggregated national data. RESULTS: A corrected national time series is calculated using an algorithm based upon the approximate equality between births and measles cases; under-reporting estimates are presented for each Italian region, and poor levels of reporting in Southern Italy are confirmed. CONCLUSION: Although an order of magnitude larger, despite great heterogeneity between regions in under-reporting and in epidemic patterns, the shape of the corrected national time series remains close to that of the aggregated uncorrected data. This suggests such aggregate data may be quite robust to great heterogeneity in reporting and epidemic patterns at the regional level. The corrected data set maintains an epidemic pattern distinct from that of England and Wales.

Studies toward the synthesis of the shark repellent pavoninin-5.

Sharks are the most dangerous predators of people in the sea, resulting in people being mauled and killed each year. A shark repellent could help to diminish this danger. The aglycone of the shark repellent pavoninin-5, (25R)-cholest-5-en-3beta,15alpha,26-triol (5a), was synthesized from diosgenin (9). Removing mercury from the Clemmensen reduction of 9 gave a higher yield of (25R)-cholest-5-en-3beta,16beta,26-triol, 10a, and was also more environmentally friendly. Attempted methods for the transposition of the C-16beta hydroxyl to the 15alpha position are described. A successful method for this transposition via the 15alpha-hydroxy-16-ketone, 8a, using the Barton deoxygenation reaction on the 16-alcohol 14b, is reported.

Synthesis of (25R)-26-hydroxycholesterol.

We describe the synthesis of (25R)-cholest-5-en-3beta,26-diol ((25R)-26-hydroxycholesterol) from diosgenin in four steps in 58% overall, yield via a modified Clemmensen reduction followed by a Barton deoxygenation reaction.

Supercritical fluids and their applications in biotechnology and related areas.

This article serves as an overview, introducing the currently popular area of supercritical fluids (SFs) and their uses in biotechnology and related areas. It covers the fundamentals of supercritical science and moves on to the biotechnological and associated applications of these fluids. Subject areas covered include pure substances as supercritical fluids, the properties of supercritical fluids, organic cosolvents, solubility, and the following applications: extraction, chromatography, reactions, particle production, deposition, and the drying of biological specimens. Within each application, and where possible, the basic principles of the technique are given, as well as a description of the history, instrumentation, methodology, uses, problems encountered, and advantages over the traditional, nonsupercritical methods.
