The utility of benzoyl peroxide in hydrophase base (Brevoxyl) in the treatment of acne vulgaris.
Available for more than 5 decades, benzoyl peroxide has been a "workhorse" of acne therapy. The benefits of this agent include reduction in Propionibacterium acnes (P. acnes) with decrease in inflammatory lesions, efficacy as both "leave on" and cleanser formulations and reduced emergence of antibiotic-resistant P. acnes strains. As the effect of benzoyl peroxide on P. acnes is a direct toxic effect rather than as a "true" antibiotic, resistance to benzoyl peroxide does not occur and has never been reported. Benzoyl peroxide in hydrophase base (Brevoxyl Creamy Washes and Gels) has shown significant efficacy in the treatment of acne, with lower irritancy than other benzoyl peroxide preparations. It is felt that the low irritancy of this product is related to a unique delivery vehicle containing dimethyl isosorbide, which dissolves benzoyl peroxide crystals on the skin. Clinical studies demonstrating the efficacy and safety of benzoyl peroxide in hydrophase base will be reviewed.

Advances in therapy for psoriasis: an overview of infliximab, etanercept, efalizumab, alefacept, adalimumab, tazarotene, and pimecrolimus.
Psoriasis is a chronic skin disorder that affects approximately 2% of the US and European population. Over the last several years, one of the major focuses in psoriasis research has been the development of biologic therapies for this disease. The aim of these therapies is to provide selective, immunologically directed intervention with fewer side effects than traditional therapies. The goal of this article is to review the progress of the biologic agents which are available, or under investigation for clinical use: infliximab, etanercept, efalizumab, alefacept, and adalimumab. In addition, two other investigational therapies, oral tazarotene and oral pimecrolimus will be discussed. Clinical data for these agents, including the most recent phase II and/or III study results, will be discussed, as well as the most recent safety data.

The anti-inflammatory effects of tetracyclines.
Tetracyclines commonly are used in medicine and dentistry because of their efficacy and safety as bacteriostatic antimicrobial agents. As such, tetracyclines function by inhibiting bacterial multiplication and growth. In addition to these effects, tetracyclines have anti-inflammatory properties and have been shown to exhibit several intracellular and extracellular biologic effects that are not related to antibiotic activity. These activities correlate with anti-inflammatory and anticollagenolytic properties. This article will review the anti-inflammatory properties of the tetracyclines and the various clinical applications related to these actions. In addition, the use of subantimicrobial dose doxycycline 20 mg twice a day (SDD) will be discussed because of its anti-inflammatory effects.

Therapeutic options for herpes labialis: experimental and natural therapies.
Herpes labialis, a common condition characterized by recurrent vesicular eruptions primarily on the lips and perioral skin, causes pain and discomfort for millions of adults each year. Over the past several years, the major focus of herpes research has been on the treatment of genital herpes. However, several studies have been conducted to evaluate the efficacy of therapies specifically for herpes labialis. Last year in Cutis, we reviewed oral and topical therapies for herpes labialis. In this final part of the series, we review experimental and natural treatments that are available for herpes labialis and its associated symptoms.

Partial remission of psoriasis following rituximab therapy for non-Hodgkin lymphoma.
Rituximab, a human-mouse, chimeric, monoclonal antibody that targets the B-cell CD20 antigen and causes rapid and specific B-cell depletion, is indicated for the treatment of low-grade or follicular, relapsed or refractory, CD20+ B-cell, non-Hodgkin lymphoma (NHL). We report the case of a middle-aged woman with psoriasis who experienced a partial sustained remission of her psoriasis after treatment with rituximab for NHL and discuss potential pathophysiologic mechanisms for this unexpected effect in a condition known to be mediated by T cells.
